Abstract: Objective To summarize the clinical characteristics of 35 patients with human monkeypox in Chengdu City, in order to provide theoretical basis for prevention and control of monkeypox epidemic in China. Methods A total of 35 patients diagnosed with monkeypox infection by Chengdu CDC from July 1 to July 23, 2023 were included in our study. The results of general clinical data, blood laboratory tests, lymph node ultrasound and chest CT results were collected in order to analyze the clinical features of human monkeypox patients in Chengdu City. Results All 35 monkeypox patients were young adult males, and there were no serious or fatal cases. Among them, 32 cases (91.4%) were men who have sex with men (MSM), and 30 cases had engaged in male-to-male sexual behavior within 21 days prior to the onset of the disease, of which 13 cases had taken protective measures. Fever symptoms were observed in 26 cases (74.3%) of the patients, with 19 cases experiencing fever within 1-6 days after the appearance of rash. The initial rash commonly occurred in the male external genitalia. Color ultrasound examinations indicated that all patients had swollen inguinal lymph nodes. C-reactive protein was elevated in 26 cases (74.3%) of patients, and 19 cases showed CD3+CD4+T/CD3+CD8+T< 1.0. 15 cases (42.8%) of the patients were infected with both monkeypox virus and HIV, 28.5% (10/35) of patients had concomitant skin infections and anorectal proctitis, respectively. The mean time from rash onset to the shedding of rash scabs was 14.8 days. Conclusions The MSM population in sexually active age group is the main infection object of human monkeypox virus. In monkeypox patients in Chengdu City, the rash starting at genital areas and rash occurring before systemic symptoms were common. Swollen inguinal lymph nodes are especially common in monkeypox patients. Skin infection and anorectal proctitis are the most common complications in monkeypox patients in Chengdu City. The abnormal cellular immune function in monkeypox patients is mainly reflected in the inverted ratio of CD3+CD4+T/CD3+CD8+T. Currently, there is no evidence to suggest that protective measures during male-to-male sexual behavior can reduce the risk of human monkeypox infection.

Objective:To search and obtain the best evidence of aerobic exercise for patients with chronic heart failure (CHF) to provide a basis for clinically formulating scientific and standardized aerobic exercise programs for CHF patients. Methods:BMJ Best Practice, Joanna Briggs Institute (JBI) Evidence-Based Health Care Center Database, Guideline International Network (GIN) , UpToDate, National Guideline Clearinghouse (NGC) , National Institute for Health and Clinical Excellence (NICE) , Scottish Intercollegiate Guidelines Network (SIGN) , CINAHL, Embase, Cochrane Library, Chinese Guidelines Network, PubMed, China Biomedical Literature Database (CBM) , WHO and cardiology professional websites were searched to collect all evidence for aerobic exercise for CHF patients. The search time limit was from the establishment of the database to May 31, 2020.Results:A total of 14 articles were included, including 6 guidelines, 1 expert consensus and 7 systematic reviews. Finally, a total of 17 best evidence for 4 aspects including the necessity of aerobic exercise, preparation before aerobic exercise, 4 elements of aerobic exercise and the mode of aerobic exercise were summarized. Conclusions:Aerobic exercise is safe and effective for patients with heart failure with decreased ejection fraction. The setting of exercise intensity should be based on the results of a symptom-restricted cardiopulmonary exercise test. Exercise for more than 6 months is effective for CHF patients. Intermittent aerobic training assisted resistance exercise can improve peak oxygen uptake, cardiorespiratory fitness and muscle strength of patients, so as to improve cardiac function and quality of life, which should be regarded as an important way of exercise for patients with stable CHF.

Impaired tumor necrosis factor receptor-1 (TNFR-1) signaling has been found in some malignant tumors with poor prognosis. However, the exact role of TNFR-1 signaling in fibrosarcoma remains unclear. Here, we explored the question by comparing the growth of TNFR-1 deficient (Tnfr1 (-)) and TNFR-1 competent (Tnfr1 (+)) fibrosarcoma FB61 cells (FB61-m and FB61-R1) in mice. TNFR-1 expression on fibrosarcoma cells delayed their growth in vivo but not in vitro. Moreover, reduced FB61-R1 tumor growth was also obtained in TNFR-1 knockout mice. The mechanism relies mainly on the TNFR-1-mediated downregulation of vascular endothelial growth factor (VEGF) production by tumor cells. Importantly, treatment of FB61-m tumors with melphalan resulted in a short delay of tumor growth, followed by a quick remission. However, when FB61-R1 tumors were treated with melphalan, tumor growth was similarly delayed at first and then completely rejected. Our results reveal evidence for TNFR-1 on tumor cells as a prerequisite in chemotherapy for fibrosarcoma, and provide novel insight into the therapeutic approach against some types of tumors using TNFR-1 angonist.
