1.Study on Human Body Bioavailability of Troxipide Capsules and Tablets

OBJECTIVE:To study human body bioavailability of trosipide capsules and tablets.METHODS:18healthy men volunteers were administered randomly with cross single dose of capsules or tablets of trosipide.The blood concentrations of which were determined by HPLC and the pharmacokinetic parameters and the relative bioequivalency were calculated with3p97program.RESULTS:The human body concentration-time curves of capsules and tablets of trosipide were in line with two compartment model.The C max of capsules and tablets of trosipide were(1.01?0.26)?(0.99?0.35)?g/ml respectively;the T max were(3.39?1.04)?(3.33?1.19)h respectively;the AUC 0~48 were(10.16?2.40)?(10.46?2.85)(?g?h)/ml respective_ ly and the t 1/2ke were(5.60?0.22)?(6.50?2.35)h respectively.The relative bioavailability of troxipide capsules was(96.85?24.1)%.CONCLUSION:Troxipide capsules and tablets were bioequivalent.
2.Determination of Acipimox in Human Serum by HPLC

OBJECTIVE: To establish HPLC method for the determination of acipimox in human serum. METHODS: The determination was performed on C18 column under room temperature, the mobile phase consisted of methanol -water(15:85) with a flow rate of 0.8ml/ min and detection wavelength of 260nm, the quantitation was conducted by external reference method,the serum was subjected to protein sediment by methanol,the supernate fluid obtained thereafter was undergone sample injection. RESULTS: The linear range of acipimox was 1.0-16.0?g/ml (r = 0.9 987),the minimum detection concentration was 0.5?g/ ml; the average recovery of method was 96.05% . CONCLUSION: This method is simple, fast, sensitive and accurate, and which can be applied for blood concentration monitoring and pharmacokinetic study of acipimox.
3.Pharmacoeconomics of the oral antidiabetic drugs in National Essential Medicine

To compare the economic efficacies of the oral antidiabetic drugs in National Essential Medicine for treating type 2 diabetes mellitus.200 diabetic patients with body mass indices between 19-27 kg/m2 were assigned into 5 groups:group A received glibenclamide,group B glipizide,group C metformin,group D glibenclamide +metformin,and group E glipizide + metformin.Pharmacoeconomic evaluation was performed by cost-effectiveness analysis( CEA ).Fasting glucose level in patients treated with these 5 drugs all decreased significantly,as well as HbA1c.Glibenclamide was more in line with the principles of pharmacoeconomics,but should be used carefully for its serious and prolonged hypoglycemia,especially in elderly patients.According to the method of cost-effectiveness analysis,it was more economical to use metformin to control fasting glucose level while it is more reasonable to use glipizide to control the postprandial glucose whereas controlling of postprandial blood glucose is considered as a priority.Glipizide+mefformin combination may be recommended to the patients whose blood glucose level is poorly controlled by a single drug alone.
4.Effect of liraglutide combined with insulin on HbA1c, T lymphocyte subsets and islet function in patients with type 1 diabetes

Objective To investigate the effect of liraglutide combined with insulin on the HbA1c, T lymphocyte subsets and islet function in patients with type 1 diabetes.Methods 46 cases from endocrinology department of Tangshan Traditional Chinese Medicine Hospital with type 1 diabetes were selected and randomly divided into two groups each group had 23 patients.Control group were given recombinant human insulin to control blood sugar according to the blood glucose level, and experimental group were given liraglutide on the basis of control group.HbA1c, T lymphocyte subsets, C-Peptide ( CP) and islet function were compared after treatment.ResuIts Compared with control group after treatment, HBAlc level in experimental group was lower (P<0.05); CD4 +T was higher, CD8 +T was lower, and CD4 +/CD8 +T was higher (P<0.05); the fasting plasma glucose (FPG) level was lower (P<0.05);2-hour postprandial blood glucose (2 hPG) was lower (P<0.05); CP and 2-hour postprandial C-Peptide (2 hCP) were higher (P<0.05).There was no significant difference in liver and kidney function between two groups.ConcIusion Liraglutide combined with insulin could significantly reduce serum glycated hemoglobin level, increase CD4 +T level and reduce CD8 +T level, improve islet function in patients with type 1 diabetic.

OBJECTIVETo study the dispersivity of baicalin solid dispersion by Raman spectrometry and infrared spectrometry in order to obtain a new and simple method in examining the dispersivity of solid dispersion.
METHODBaicalin solid dispersion was prepared by solvent method. The Raman spectra, infrared spectra and X-ray powder diffraction patterns of crude baicalin, PVP K30, physical mixture, solid dispersion were acquired by Raman and Fourier transform infrared spectrometers and X-ray powder diffractometry. Then the spectra were compared with each other in order to find out the differences between baicalin solid dispersion and other materials.
RESULTThe result of Raman spectrometry was that amorphous baicalin presented in the solid dispersion, which was in agreement with that of X-ray powder diffractometry.
CONCLUSIONRaman spectrometry which is fast, direct and non-destructive is an ideal method for examining the dispersivity of solid dispersion.

Objective To compare the difference of cutpoint and clinical significance of HbA1C for the diagnosis of abnormal glucose metabolism in two population groups with different ages.Methods According to oral glucose tolerance test(OGTT),the cutpoint and clinical significance of HbA1C for the diagnosis of type 2 diabetes and impaired glucose regulation(IGR)were investigated in the two population groups.Results The mean HbA1C of 1 064 young subjects in an academy and 1 671 aged subjects in a community were 5.31% ±0.41% and 5.79% ±0.71%,respectively.The cutpoints of HbA1C for diagnosis of diabetes were 5.7%(specificity 86.7%,sensitivity 66.7%)and 5.9%(specificity 73.8%,sensitivity 80.1%)in the two population groups,and 5.6% for diagnosis of IGR (specificity 82.8%,sensitivity 55.8%)and 5.7%(specificity 60.9%,sensitivity 64.3%),respectively.87.8%,78.7%,and 38.5% were diagnosed diabetes by current OGTT criteria at HbA1C levels of ≥5.7%,≥5.9%,and≥6.5%,IGR being 61.6%,39.6%,and 4.1%,and normal glucose tolerance being 24.4%,10.0%,and 0.4%.Conclusion The cutpoints of HbA1C for diagnosis of diabetes and IGR are different in populations with different ages and HbA1C levels.As one of diagnostic criteria for diabetes,HbA1C 6.5% with relatively higher specificity and lower sensitivity must be combined with fasting blood glucose,random blood glucose,and OGTT.

Cancer toxin is the key pathogenesis of malignant tumors. The basic principle of cancer treatment is “dispelling pathogen and resolving toxins, reinforcing healthy qi and reinforcing the foundation”. As one of the “eight methods of anticancer and detoxification”, the counteracting toxin with toxin therapy is a commonly used clinical treatment of malignant tumors. This paper discussed the method of counteracting toxin with toxin and its application in the prevention and treatment of malignant tumors from the aspects of history tracing, academic connotation, application principles and clinical application. Toxic Chinese medicinals with anticancer function are required to eliminate cancer toxins based on the principles of excessive cancer toxicity and plentiful healthy qi, as well as in accordance with the various stages and classifications of tumors, thereby improving the theoretical connotation of the method of counteracting toxin with toxin, and promoting the popularization and application of the pathogenesis theory of cancer toxin in the prevention and treatment of malignant tumors.
10.Clinical analysis and prognosis of children with neuroblastoma with pancreatic infiltration

Objective To summarize the clinical data and characteristics of neuroblastoma (NB) with pancreatic infiltration and to assess the clinical features and the prognosis of NB.Methods According to NB protocol at Beijing Children's Hospital,Capital Medical University(BCH-NB-2007),based on Hong Kong NB protocol,the patients were divided into 3 groups of low-risk (LR) group,medium risk (MR) group and high-risk (HR) group.All children were followed up till March 31,2017.Diagnosis of pancreatic infiltration of NB was made by abdominal enhancement of CT,enhanced magnetic resonance imaging (MRI) or 18-fluorodeoxyglucose-positron emission tomography-computed tomography(18F-FDG-PET/CT),any of which could suggest NB pancreatic infiltration or postoperative pathology prompted NB to infiltrate the pancreas.Retrospective summary and analysis of indicators were performed,which included the initial diagnosis of primary tumor and metastatic tumor site,tumor markers,clinical stage,risk group,imaging features and treatment.Results (1) Totally 50 eligible patients were included:27 females,23 males,median age of 33 months (7-129 months),10 cases ≤ 18 months,40 cases > 18 months;3 cases were of International Neuroblastoma Staging System(INSS)-Ⅲ,47 cases of INSS-Ⅳ;2 caes of LR,3 cases of MR,45 cases of HR;28 cases had a fever,27 cases with abdominal mass,14 cases with abdominal pain,9 cases with limb pain,5 cases with vomiting,4 cases with diarrhea,and 1 case with jaundice.Forty-nine cases of primary tumor were located in the retroperitoneal adrenal gland,and 1 case in the pelvic cavity.Thirty-two cases had tumor diameter≥ 10 cm.(2)Tumor markers and imaging features:the median serum lactate dehydrogenase (LDH) value in 50 cases was 669 U/L (263-6 762 U/L),of them 19 cases > 1 000 U/L.A total of 80% cases had neuron specific enolase (NSE) > 0.15 ng/L.Nine cases had elevated amylase (AMY),and 7 cases had elevated lippase (LPS),and all the levels were elevated in 5 cases.A total of 41 cases had pancreas infiltration by abdominal ultrasound,44 cases had pancreas infiltration by abdominal enhancement computed tomography (CT),100% (14/14 cases)of pancreas infiltration was confirmed by abdominal reconstruction enhancement nuclear imaging MRI,and NB pancreas infiltration was proved in 41.3% (19/46 cases) by 18F-FDG-PET/CT.Comparison of the above 4 imaging studies:one imaging examination index was positive in 7 cases,accounting for 14.0%,2 positive in 26 cases,accounting for 52.0%,3 positive in 15 cases,accounting for 30.0%,and 4 positive in 2 cases,accounting for 4.0%.(3) Treatment outcomes:totally 50 cases received treatment,including 2 cases of LR,all cases were of INSS-Ⅲ,and 1 case with complete remission (CR).Three cases of MR belonging to INSS-Ⅳ had complete resection of the tumor,1 case had recurrence and died,and the other two were stable.Forty-five cases with HR,median follow-up lasting for 15 (4-53) months,16 cases had occurrence (35.6%),3 cases were relapsed after stopping treatment for 2,3,18 months,respectively;tumor progressed in 12 patients during treatment,and 1 case got severe intracranial infection and gave up treatment before death.Kaplan-Meier analysis showed the expected 3-year event free survival(EFS) rate was 22.1%,and 3-year overall survival(OS) rate was 38.5%.Conclusions Preliminary results show that 90% with pancreatic infiltration of NB belong to Ⅳ HR group of children,and almost primary tumor is almost located in the retroperitoneal ragion.NB with pancreatic infiltration clinical manifestations is hidden and nonspecific.More than half of the children have no obvious abdominal pain or vomiting,and so imaging examination is needed to determine the situation of pancreatic metastasis further.Abdominal reconstruction enhancement MRI has a high sensitivity and specificity for pancreatic metastatic lesions,which can be used as the basis for early diagnosis.The overall prognosis is poor.The expected 3-year EFS rate can be 22.1%,3-year OS rate was 38.5%.
