1.Effect of Plan. Do-Check-Act on homogenization of nursing processes in ICU

Objective To explore the effect of Plan. Do-Check-Act on homogenization of nursing processes in ICU. Methods A total of 36 nurses were selected to implement Plan. Do-Check-Act in ICU according to evidence-based medicine. Their nursing was inspected by a senior nurse. The differences of the following items before and after Plan. Do-Check-Act were compared: nursing quality comprehensive quality control index and specialized quality control index. Results After the implementation of Plan. Do-Check-Act, comprehensive quality control index of clinical nursing quality score by (89.28 ± 2.36) points up to (97.45±1.38) points, and there was significant difference(t=-7.310, P<0.01). The specialized quality control indicators including the incidence of ventilator associated pneumonia, the incidence of catheter-related bloodstream infection, the incidence of catheter-associated urinary tract infections, the incidence of accidental extubation, the incidence of pressure sores, the incidence of glycemic out of control, the incidence of sedation out of control and retention time were 26.2%(53/202), 17.8%(36/202), 18.8%(38/202), 11.9%(24/202), 2.5%(5/202), 18.3%(37/202), 15.3%(31/202), (168.0 ± 3.3) h before the implementation and 7.1%(14/196), 3.1%(6/196), 4.1%(8/196), 0.5%(1/196), 0, 3.6%(7/196), 3.1%(6/196), (96.0±4.2) h after the implementation. There were significant differences (χ2=4.913-25.907, t=66.195, all P<0.01). Conclusions The Plan. Do-Check-Act is an effective way to improve nursing quality of ICU to make it homogeneous, reduce adverse nursing and iatrogenic complications.

3.Correlation analysis of the incidence and head position in posterior semicircular canal benign paroxysmal positional vertigo

OBJECTIVE To verify the relationship between the incidence and habitual head position in bed in posterior semicircular canal benign paroxysmal positional vertigo(PSC-BPPV), and to provide practical information for the prevention and treatment of BPPV.METHODS There were 208 patients with PSC-BPPV were included in this study. The relationship of affected semicircular canals in PSC-BPPV patients and the habitual head positions in bed were analyzed.RESULTS There were 125 patients who had always taken the same position (right side lying in 85 cases, left side lying in 40 cases), and 83 cases in different positions. The affected PSC-BPPV were consonant with the head positions (Kappa=0.401). With Spearman correlation analysis, we can find that there was correlation between the affected PSC-BPPV and the head position(r=0.203) and age(r=-0.179,P<0.05), but there were no significant correlation with gender.CONCLUSION Habitual head position causes otoliths deposit in PSC at the lowest level during bedrest, and was one of the main factors leading to the ipsilateral PSC-BPPV.
4.Analysis of inferior colliculus region typical proteins in auditory pathway

OBJECTIVE To study the proteome of inferior colliculus and determinate the region-typical proteins which may be the candidate cause of the Central Auditory Processing Disorders. METHODS The telencephalon was taken as reference, and then identified and quantified the proteome of IC of adult rats with iTRAQ. Those with higher abundance in inferior colliculus than the other three regions were considered as IC-Region typical proteins,which may lead to functional specializations. RESULTS We identified 1937 cytomembrane proteins in total, among which there are 53 IC-Region typical proteins, which may lead to functional specializations of inferior colliculus.We used GO and KEGG pathway to analyze these proteins and then found that these proteins mainly take part in the regulation of neurons development and information integrations. CONCLUSION Our quantitative comparison of inferior colliculus has revealed two candidate proteins, including CaMKII and SV2A, which may play important roles in maintaining the balance of excitatory and inhibitory transmitters release. These proteins may be the candidate proteins for Central Auditory Processing Disorders.
5.Clinical application of helical CT 3D reconstruction for the dental orthopaedics

Objective To evaluate the clinical application of helical CT 3D reconstruction technique in the dental orthopaedics. Methods The helical CT was performed with 3.0 mm slice thickness and 1.0 pitch in 41 patients with dental orthopaedics. The 3D reconstructions, including maximum intensity projection (MIP), surface shaded display (SSD), and multiplanar reconstructions (MPR), were made for all the cases. Results Thirty-seven of the 41 patients showed malalignment, tilt, rotation, overlap of the teeth and the different space between the longitudinal axes of the teeth. Twenty-five cases of them have shown 36 buried teeth in all. The axial images covered all the information. SSD demonstrated the external contours and entire morphologies of the teeth and the mandible with the relationship of the teeth alignment and the mandible. MIP clearly manifested the full view and the longitudinal alignment of the teeth. Among the 36 buried teeth, there were 29 palatally and 7 labially presented teeth, and they were morphologically delineated on MIP through various angles. Conclusion The helical CT 3D reconstruction is a new technique to display the stereoscopic configuration of teeth. The combination of axial images and MIP, SSD, and MPR provides valuable anatomic and diagnostic information helpful for the surgeons to structure and determine the treatment protocol for the dental orthopaedics.
6.Advances in biological predictors of radioactive esophagitis

Radioactive esophagitis is one of the most common complications in thoracic tumor radiotherapy,biological factors such as single nucleotide polymorphisms (SNPs),miRNAs,and HIV infection may play key roles in the occurrence and development of radioactive esophagitis,and they have become an active field in protection areas of radiotherapy.We can identify the patients who may cause radioactive esophageal in high dose radiotherapy as early as possible,and modify the treatment plan to protect the esophagus.Therefore the biological factors of radioactive esophagitis are of important clinical significance.
7.Isolation, Purification and Structural Identification of Polysaccharide from Cordyceps Sinensis

Objective: To isolate and purify the polysaccharide from Cordyceps sinensis, and analyze its structure. Methods:Cordyceps sinensis was cultured by a liquid fermentation method. A water-extraction and alcohol-precipitation method was applied to ex-tract polysaccharide from Cordyceps sinensis fermentation liquor (EPS) and polysaccharide from Cordyceps sinensis mycelium (IPS). Sephadex gel chromatography was applied to isolate and purify the polysaccharide. The purity and relative molecular weight of the poly-saccharide were determined by a gel filtration method. The monosaccharide composition of the polysaccharide was identified by GC. The content of uronic acid was analyzed by sulfuric acid carbazole method. Results: The analysis results showed that the molecular weight of EPS was 78kDa. The content of polysaccharide and uronic acid was 94. 8% and 6. 0%, respectively. EPS was composed of mannose,glucose and galactose with the molar ratio of 4. 5∶8. 0∶1. 0. The molecular weight of IPS was 42kDa. The content of polysac-charide and uronic acid was 92. 5% and 4. 5%, respectively. IPS was composed of mannose,glucose and galactose with the molar ratio of 2. 8∶3. 0∶1. 0. Conclusion:Both polysaccharide EPS and IPS in Cordyceps sinensis are heteropolysaccharide.
8.Transient Evoked Otoacoustic Emission in China Rural Primary School Children Hearing Screening

Objective To investigate the clinical meaning and feasibility of transient evoked otoacoustic emission (TEOAE) in China rural primary school children hearing screening.Methods TEOAE was recorded with Madsen Celesta 503 in 317 (634 ears) primary school children,then the results were compared with gold standard test group,including otoscopy, pure tone audiometry and tympanogram.Results In 317 (634 ears) pupils, the presence of TEOAE was 94.64% (600/634); the presence of gold standard was 94.48% (599/634). The consistency rate was (96.69?0.71)%; sensitivity (Sen) was 0.69?0.078;specificity (Spe)=0.98?0.0052, the false positive rate (FPR), that was ?, was 0.31; ? = 0.02; Youden's index J = 0.67?0.078, LR_+=34.5, LR_-=0.32, PV_+=0.71(71%),PV_-=0.98(98.%),AZ=0.8.Conclusion The TEOAE test is simple, quick, harmless, sensitive and objective. It can be used in hearing screening program in rural primary school children.
9.Effects of lycium barbarum polysaccharides on retinal pathological change and expression of VEGF in retina of diabetic rats

Background The primary pathological basis of diabetic retinopathy (DR) is new blood formation due to anoxia and inflammation,which results in breakdown of blood-retinal barrier (BRB).Vascular endothelial growth factor (VEGF) is a key factor promoting neovascularization.Researches determined that lycium barbarum polysaccharides (LBP) can protect cells against oxidative damage.However,the study of LBP in ophthalmology is lack.Objective This study was to investigate the effects of LBP on the dynamic pathological change of retinal vessels and expression of VEGF in retina of diabetic rats.Methods One hundred and seventeen SPF male SD rats were randomly divided into the normal control group,the diabetic mellitus (DM) group and the LBP group according to random number table.Type 1 DM models were induced by intraperitoneal injection of streptozotocin (STZ,55 mg/kg) in the rats of the DM group and the LBP group,and then 250 mg/kg LBP was intragastically administered in the rats of the LBP group.The morphological change of retinal vessels was dynamically observed by retinal stretched preparation with Evans blue (EB) in 4,10 and 16 weeks after modeling.E B (45 mg/kg) was slowly injection via jugular vein,and 1% polyoxymethylene was infused into the left ventricule.The eyeballs were extracted and retina were isolated.EB content in the retinas (mg/g) was calculated using retinal stretched preparation method at the time points mentioned above.Expressions of VEGF protein and mRNA in the retinas were detected by immunohistochemistry and real-time quantitative PCR at various time points,respectively.Results Retinal stretched preparation with EB exhibited that the abnormal degree in the shape,diameter of vessels and leakage of the retinal blood vessels were significantly slighter in the LBP group than those of the DM group in 4,10,16 weeks after modeling.At 4,10,16 weeks,EB content in the retinas was (12.17±1.55),(16.46±1.60) and (19.55±1.49) mg/g,which was significantly lower than (15.76± 1.90),(21.61 ±2.05) and (26.30±2.28) mg/g of the DM group (P<0.05).Immunochemistry showed that the expression of VEGF protein primarily located at retinal ganglion cells (RGCs) layer.The staining intensity for VEGF protein was weaker in the LBP group than that of the DM group.The expression levels of VEGF protein (A value) in the LBP group were 0.234±0.011,0.331±0.023 and 0.536±0.031at various time points,with significant decline in comparison with 0.281±0.018,0.533±0.055 and 0.765±0.075 of the DM group (all at P<0.05).Real-time quantitative PCR revealed that the expression levels of VEGF mRNA were 0.157±0.013,0.505 ±0.114 and 1.577±0.074 in the LBP group at various time points,which were significantly lower than 0.235±0.209,1.043±0.084 and 2.446±0.061 of the DM group (all at P<0.05).Conclusions LBP can alleviate the DM-induced retinal vasculopathy,lessen the leakage of vessels well,and further protect the BRB.
