1.Research in influence of bed rest time on orthostatic hypotension of elderly patients after hemodialysis

Objective To study the influence of different bed rest time on orthostatic hypotension,and explore a feasible and appropriate bed rest time for elderly patients after hemodialysis.Methods 58 elderly patients with regular hemodialysis were selected from January 2011 to May 2012.By self-control,they were divided into three groups according to different bed rest time:T0 group who got up immediately after the hemodialysis,T10 group who got up after 10-minute bed rest,T20 group who got up after 20-minute bed rest.The incidence of orthostatic hypotension was observed and compared between three groups.Results Compared to T0 group,the incidence of orthostatic hypotension in T10 and T20 groups was significantly reduced.However,no difference was seen between T10 and T20 groups.The acceptability rate in T10 group was 67.24%,higher than 17.24% of the T20 group.Conclusions Elderly hemodialysis patients should continue to lie down for at least 10 minutes after hemodialysis,and then slowly get up,this measure can reduce the occurrence of orthostatic hypotension.

The management of the scientific research files is an important basic work at the college,which is of great significance to the research work in both school and the scientific research organizations.However,some researchers and managers are lack of file consciousness and the management system remains unadapted to the new condition.These reasons result in the use of the dossiers inadequate or even absent.Therefore,it seems necessary and important for the perfection of the scientific research file work to take essential measures:establishing a scientific and reasonable file system,improving the file management regulation,and increasing the utilization ratio of the dossiers.
3.Study of the relationship between morphological characteristics of tumor microvasculature and blood supply type of primary hepatocellular carcinoma observed under DSA

Objective To discuss the relationship between morphological characteristics of tumor microvasculature and blood supply type of primary hepatic cell carcinoma (HCC) through DSA. Methods From Jan. 1998 to Feb.2002, 32 cases, male 26,female 6,aged 28-76 (mean 50.56?10.52) years, underwent preoperative digital subtraction angiography (DSA) without therapeutic embolization in the 301 hospital and 307 hospital. The intervals between DSA and curative resections were 13-70 (mean 31.89?19.11) days. The diameters of the tumors were 2-23 (8.05?6.17) cm. The blood supply type of tumor, the tumor microvasculature density(MVD), type and diameter of the tumor microvessels, width of tumor trabecular were recorded for these cases. The outcomes were then statistically analysized. Results ① Blood supply of the tumor observed on DSA tended to be positively correlated with type of the tumor microvasculature. ②MVD of hypovascular tumors were lower than those in moderate and hypervascular tumors.③The ratio of diameters between tumor microvessel to the trabeculae of the tumor went higher with the increase of HCC blood supply on DSA. Conclusions ①The density, the configuration and the distribution of tumor microvasculature were not evenly distributed in HCC. ②The features of tumor microvasculature possessed distinctive characteristics in different stages of tumor growth, simultaneously with some definite developing rules. ③ Distinct differences in morphological characteristics of tumor microvasculature between different blood supply types of HCC.
4.Assoaation of IL-10 gene single nucleotide polymorphisms with the acute susceptibility to lymphocyte leukemia

Objective To observe the relationship of IL-10 gene single nucleotide polymorphism and the susceptibility to ALL. Methods The bone marrow and peripheral blood samples from 115 ALL patients and 323 healthy controls were collected in Peking University First Hospital and Beijing Dao-pei Hospital from January 2007 to December 2009. The DNA were extracted from all samples. The primers of -819C/T and -592A/C in the promoter region of IL-10 gene were designed for the PCR. The restrictive fragment length polymorphism of IL-10 gene was analyzed by using restrictive enzyme Msl Ⅰ and HpyCH4 Ⅲ.Sequencing was done in part of these samples to confirm the results of PCR. The differences of genotypes and allele ratio of -819 and -592 sites were analyzed between the ALL patients and healthy controls. Real-time quantitative PCR was performed to detect the EB virus (EBV) infection and the expression of BCR/ABL fusion gene. The differences of genotypes and allele ratio of -819 and -592 sites were analyzed between the positive and negative group. Results The genotype ratios of -819CC, -819TT, - 819CT, -592AA,- 592CC and - 592AC were 14. 8% ( 17/115 ), 45.2% ( 52/115 ), 40. 0% ( 46/115 ), 43.5% ( 50/115 ),16. 5% ( 19/115 ), 40. 0% ( 46/115 ) in ALL patients, and were 9. 9% ( 32/323 ), 16. 4% ( 53/323 ),73.7% ( 238/323 ), 11.8% ( 38/323 ), 15.5% ( 50/323 ), 72. 8% ( 235/323 ) in the healthy controls,respectively. The genotypes of -819 and -592 sites had statistically significant differences between the two groups(x2 values were 46.000 and 54.550, all P < 0. 05 ). The allele ratio of -819T and -592A were (65.2%, 150/230) and (63.5%, 146/230) in ALL patients, while they were 53.5% (344/646) and 48. 1% (311/646)in the healthy controls. There were statistically significant differences between the two groups (x2 values were 9. 877 and 15.986, all P < 0. 05 ). The EBV DNA were detected in 42 ALL patients,among which 22 were positive and 20 were negative. The genotype ratios of -819CC, -819TT, -819CT,-592AA, - 592CC, - 592AC in EBV positive group were 9. 1% ( 2/22 ), 40. 9% ( 9/22 ), 50. 0%(11/22) ,31.8% ( 7/22 ), 13.6% ( 3/22 ), 54. 5% ( 12/22 ), while they were 35.0% ( 7/20 ), 45.0%(9/20) ,20. 0% (4/20) ,35.0% (7/20) ,45.0% (9/20) ,20. 0% (4/20) in the EBV negative group. The genotypes of -819 and -592 sites showed no statistical differences between the two groups( all P > 0. 05 ).The BCR/ABL fusion gene were detected in 36 ALL patients, among which 20 were positive and 16 were negative. The genotype ratios of - 819CC, - 819TT, - 819CT, - 592AA, - 592CC, - 592AC in BCR/ABL positive group were 0% (0/20) ,45.0% (9/20) ,55.0% ( 11/20), 45. 0% (9/20) ,5.0% (1/20) ,50. 0%( 10/20), while they were 18. 8% ( 3/16 ), 50. 0% ( 8/16), 31.3% ( 5/16 ), 50. 0% ( 8/16 ), 18. 8%(3/16), 31.3 % (5/16)in the BCR/ABL negative group. The genotypes of -819 and -592 sites showed no statistical differences between the two groups ( all P > 0. 05 ). Conclusion The population with - 819TT and - 592AA genotype of IL-10 gene shows susceptibility to ALL.
5.Nosocomial Infection in Patients with Chronic Kidney Disease:Its Characteristics and Risk Factors

OBJECTIVE To discuss the characteristics and risk factors of nosocomial infection inpatients with chronic kidney disease.METHODS The data from chronic kidney disease(CKD) patients retrospectively analyzed.RESULTS The nosocomial infection rate of CKD patients was 14.73%,urinary tract was the most comun site,The main-pathogens were Gram-negative bacteria,and then Gram-positive bacteria and fungii.The patients with diabetic nephropathy,lupus nephritis,aging,lower glomerular filtration rate,hypoproteinemia,anemia,and long time duration were easy to get nosocomial infection.CONCLUSIONS Nosocomial infection in CKD patients is related to underlying diseases,age,kidney function,serum albumin level,hemoglobin level,duration time in the hospital.
6.Dynamic changes in Th17/Treg balance among children with seasonal allergic rhinitis

Objective To investigate the levels and significance of Th17 cells and regulatory T cells (Treg) in peripheral blood of children with allergic rhinitis during pollen and non-pollen seasons.Methods Thirteen children with hay fever, 10 children with house dust mite(HDM)-allergic asthma and 10 healthy children were recruited into this study.Percentages of Th17 and Treg cells were detected by flow cytometry.Levels of IL-17, IL-10 and TGF-β in cell culture supernatants were measured by ELISA.Results (1) The percentages of Th17 cells in children with allergic rhinitis [(3.4±2.4)%] were significantly higher than those in HDM-allergic asthmatics [(2.1±1.6)%] and those in healthy children [(0.5±0.3)%] during pollen season (both P<0.05).The levels of Treg cells in allergic rhinitis group [(2.1±1.3)%] and in HDM-allergic asthma group [(3.6±1.9)%] were significantly lower than those in healthy control group [(5.5±2.8)%] (both P<0.05).The levels of Th17 cells [(3.0±1.9)% vs (3.4±2.4)%, P<0.05] and ratios of Th17/Treg cells [(1.4±1.0)% vs (1.7±1.5)%, P<0.05] in children with allergic rhinitis were significantly decreased during non-pollen season as compared with those during pollen season, but the levels of Treg cells were up-regulated [(2.4±1.6)% vs (2.1±1.3)%, P<0.05].(2) Correlation analysis revealed that the ratios of Th17/Treg cells were positively correlated with the concentrations of FeNO (fractional concentration of exhaled NO) (r=0.321, P<0.05) and the counts of circulating eosinophils (r=0.198, P<0.05) in children with allergic rhinitis during pollen season.Conclusion The imbalanced Th17 and Treg cells in children with allergic rhinitis during pollen season might play a vital role in the regulation of allergic airway inflammation.
7.Characteristics and influencing factors analysis of depressive symptoms among only-child-lost-persons

Objective To explore the characteristics and influencing factors of depressive symptoms among the only-child-lost persons.Methods A total of 841 only-child-lost persons and 674 controls conforming to entering group condition were selected.The difference of SDS score between the two groups and the demographic characteristics distribution differences of depression severity in only-child-lost persons were compared and the influencing factors were analyzed.Results The SDS score had statistical difference between the only-child-lost persons group and control group(P<0.05),and was correlated to the only-child-lost years and no physical disease;the proportion of mild to moderate depression in the only-child-lost persons of the low cultural level,somatic diseases and middle income level groups was significantly higher than that in the other groups;the only-child-lost persons with low income level,age≤60 years old and high educational level had a higher proportion of severe depression;the Logistic regression analysis showed that the educational level had a significant influence on the depression onset among the only-child-lost persons,the difference was statistically significant(P<0.05).Conclusion The characteristics of depressive symptoms among the only-child-lost persons are related to their demographic characteristics.

Objective To investigate the relationship of T2DM with acute glucose fluctuations and IMT. Methods Forty-five cases of T2DM patients were selected from January 2013 to 2014 February in our hospital as T2DM group, 50 cases of T2DM patients with acute cerebral infarction were selected in our hospital on the same period with T2DM group as DMCI group,the carotid artery IMT of two groups were measured, the patients in the DMCI group were divided into IMT thickening group(n=29), IMT normal group(n=21), the data of every groups were compared and the correlation of IMT with other indicators were analyzed. Results ①SBP, DBP, LDL-C, TC, TG, HbA1C, IMT, MAGE of DMCI group were higher than T2DM group, the difference was significant(P<0.05). ②SBP, LDL-C, IMT, MAGE IMT of IMT thickening group was significantly higher than IMT normal group, the difference was significant (P <0.05). ③MAGE, LDL-C, SBP were the influence factors for IMT (P<0.05). Conclusion Patients with high blood sugar stable and effi-cient glucose lowering blood sugar fluctuations can reduce the risk of atherosclerosis, reduce the incidence of diabetic complications.
9.Comparative study on living quality and mental health status in losing-single-child family couples

Objective To study the living quality and mental health status in losing-single-child family couples .Methods A total of 841 losing-single-child family couples and 674 family couples with children alive conforming to the entering group condition were random sampled from the three districts of Chongqing main city .The on-site survey was performed by using the general condi-tion questionnaire ,90 Symptom Checklist (SCL-90) ,Self-rating Anxiety Scale (SAS ) ,Self- rating Depression Scale (SDS ) ,Social Support Rating Scale (SSRS) ,Memorial University of Newfoundland Scale of Happiness (MUNSH) and WHO Quality of Life Scale (WHOQOL-BREF) Chinese version .Results There were no differences in gender ,age and culture between the losing-single-child family couples group and family couples with children alive group (P>0 .05);the SCL-90 total score and each factor score , SAS and SDS score in the losing-single-child family couples group were significantly higher than those in family couples with chil-dren alive group(P<0 .05) ,the SSRS score ,MUNSH score and WHOQOL-BREF Chinese version evaluation score were signifi-cantly lower than those in the family couples with children alive group(P<0 .05);the multivariate regression analysis showed that the four regression equations in the social relation field ,environment field ,depression factors and total happiness had statistical sig-nificance (P<0 .05) .Conclusion The living quality ,mental health status and subjective well-being in the losing-single-child family couples are significantly affected ,timely psychological crisis intervention and persistent psychological support and mental health ed-ucation should be give them to improve their psychological health status ,increase the living quality and subjective well-being .
10.Risk Identification and Evaluation of Clinical Trials of I nnovative Drugs under New Policy Environment

OBJECTIVE:To identify and evaluate the risk factors of innovative drug clinical trials ,and to provide references for the development of risk management and control strategies. METHODS :Using the method of literature research ,the risk factors of innovative drug clinical trials were obtained ,classified and sorted out ,and the risk evaluation indicators were initially formed. On this basis ,the questionnaire was designed. By means of interview and questionnaire survey ,54 experts were invited from 4 tertiary hospitals and 1 contract research organization to score. SPSS 24.0 software was used to perform principal component analysis on risk event status data. RESULTS & CONCLUSIONS :The risk evaluation system included 9 first-class indicators such as policy and legal risk ,clinical trial institution risk and trial scheme design risk ,and 31 second-class indicators such as industrial policy,law and regulation changes ,intellectual property risk . According to the analysis ,the risk factors of innovative drug clinical trials mainly included drug and biological sample management risk ,trial scheme design risk ,clinical trial institution risk ,safety report management risk ,ethical review risk ,policy and legal risk ,and subject management risk. Relevant parties in clinical trials need to focus on monitoring various risk factors ,establish dynamic monitoring and control mechanism and implementation guarantee mechanism ,and effectively prevent and control risk ,to promote the smooth implementation of clinical trials.
