ObjectiveTo study the influences of fetal umbilical cord around neck on foetus、newborn infants and the choices of delivery mode during delivery.Methods220 cases of cephalic delivery pregnant women with fetus who had fetal umbilical cord around neck symptom and 220 cases of cephalic delivery pregnant women with fetus who didn't have fetal umbilical cord around neck symptom,were selected to make a contrast observation.ResultsThe foetus who had fetal umbilical cord around neck symptom had higher rate of fetal distress and neonatal asphyxia.The maternal cesarean section would be much higher,too.The rate is 18.5％,8.6％,49.6％ which were higher than the foetus who didn't have such kind of symptom,the rate is 5.4％,1.8 ％,26.4％ ( all P ＜ 0.05 ).The rate of fetal distress and neonatal asphyxia was proportional to the number of circles around neck.One circle around neck is 12.6％,1.4％ ;two circles around neck is 27.1％,17.0％ ;three and more than 3 circles around neck is 69.2％,53.8％.ConclusionWe should enhance perinatal B-mode ultrasonic examination and fetal monitoring circle around neck,pay close attention to the factor of umbilical cord around neck and select suitable delivery mode so that we can reduce the incidence of fetal distress and neonatal asphyxia.When B-mode ultrasonics displays that the number of the umbilical cord around the fetus' neck is three and more than 3 circles,the incidence of neonatal asphyxia is high and the cesarean section is the best choice under such circumstances.

Objective To investigate the therapeutic effects of retinal angiomatosis in differentclinical stages.To discuss the indication of vitrectomy for retinal hemangioblastoma. Methods TheclinicaI data of 22 cases(33 eyes)were retrospectively analyzed.The retinal hemangiomas were dividedinto 5 stages according to their degrees of development from simple angioma without vessel dilation tofeeder vessel dilation and intra-retinal exudates.local retinal detachment,massive retinal detachment andcomplication occurrence in proper order.The methods of treatment were laser photocoagulation,trans-scleral cryotherapy and vitrectomy. 13 eyes were treated with laser photocoagulation, 5 eyes withcryotherapy combined with laser and 11 eyes with vitrectomy.Tumor resection and silicone oil tamponadewas performed in 3 eyes during vitrectomy.The patients were followed up for 46 months on average.Visual acuity(VA),the condition of the hemangioma and retina was compared pre～and post-operationrespectively. Results In a11 13 eyes treated with laser phot.coagulation the hemangiomas regressed andthe retina remained attached.VA improved in 2 eyes,and remained unchanged in 11 eyes.Cryotherapycombined with laser photocoagulation was performed on 5 eyes. In this group,4 eyes'hemangiomasregressed and no new hemangiomas occurred,proliferative vitreous retinopathy and vitreous hemorrhagewas observed in 1 eye which vitrectomy was performed later.VA improved in 2 eyes,unchanged in 2 eyesand decreased in 1 eye.In the 11 eyes treated with vitreoretinal surgery,new hemangiomas was found in 1eye,exudative retinal detachment was caused by hemangiomas in 2 eyes,proliferative vitreous retinopathywas observed in 2 eyes.and the retina remained attached in 8 eyes.VA improved in 3 eyes,unimproved in3 eyes,and decreased in 5 eyes.In the 3 eyes with surgical resection of retinal hemangioma during vitrectomy,2 eyes'retina remained attached,1 eye had exudative retinal detachment and no new hemangiomas occurred.VA improved in 2 eyes and decreased in 1 eye. Conclusions Laser photocoagulation or combined with cryotherapy is effective in treating the hemangiomas in early stage.Vitreetomy is advisable for late stage of retinal angiomatosis,especially with vitreous hemorrhage,epiretinal membrane,proliferation and large scale of retinal detachment.Surgical resection of isolated large retinal hemangioblastoma may be useful for selected patients.

Objective To investigate the effects of Hep-A and He p-B on vascular endothelial growth factor (VEGF)-induced breakdown of blood-r etinal barrier. Methods The mice were subcutaneously injected vehicle, Hep-A or Hep-B 10 mg/kg twice a day for 5 days. Then, 1 ?l of 10 -6 mol/L VEGF were intravitreous injected. After 6 hours, 13.7?10 4 Bq /g 3H-mannital were injected intraperitoneally. The mice were sacrificed and the retinas, lungs, kidneys were removed and examined for radioactivity. The result were analyzed using SPSS software to calculate and compare retina/lung and et ina/kidney leakage ratio among groups of different treatment. Objective To investigate the expression and role of Skp2 and P21 in the process of breast hyperplasia cancerizing.Methods The expression of Skp2 and P21 protein in normal breast tissue,adenosis of breast,breast atypical hyperplasia,breast hyperplasia cancerization,and breast cancer were detected by immunohistochemical staining SP method.Results(1) The expression of Skp2 and P21 in normal breast tissue and adenosis of breast was negative,the positive staining rate of Skp2 and P21 in breast atypical hyperplasia was 25.71% and 37.14%,in breast hyperplasia cancerization 63.16% and 73.68%,breast cancer,75% and 100% respectively.The contradistinction between breast atypical hyperplasia and breast hyperplasia cancerization was of significant statistically.But there was no statistical significance between breast hyperplasia cancerization and breast cancer.(2) The expression of P21 could lead to accelerated expression of Skp2.(?2= 10.024,P=0.002).There was a significant positive correlation between Skp2 and P21(r=0.563,P=0.000).Conclusion Skp2 and P21 are involed in the early stage of breast cancer and contribute to the maligant phenotype without affecting infiltration.Both of them are danger factors for breast hyperplasia cancerizing and contribute to malignant process from normal breast tissue and atypical hyperplasia to breast cancer.They are effective indicators for prognosis of breast hyperplasia.

Aim To evaluate the antitumor activity of ruthenium polypyridyl complexes and the underlying mechanism. Methods The right complexes 2b were filtered with highest activity and lowest toxicity by MTT assay. The change of cell cycle was detected by flow cytometry . The expression of p53 and p21 was detected by Western blot. The in vivo antitumor activity of 2b was evaluated by the assay of tumor bearing nude mice. Results 2b potentially inhibited proliferation of a variety of hepatoma cell lines, among which Hep 3B cell was the most significant ( IC50 was 12. 1 μmol · L-1 ) . The apoptosis of Hep 3 B cell was induced by 2b, as evidenced by DNA fragmentation, chromatin condensation and appearance of subG1 peak. The ac-tivities of caspase-9 and caspase-3 were activated by 2b. The phosphorylation of p53 was induced by 2b. The expression of p53 and p21 was also up-regulated by 2b. The growth of tumor of nude mice was signifi-cantly inhibited by 2b in vivo experiment. Conclusion 2b has good in vitro and vivo antitumor activities, and it can inhibite growth of Hep 3 B cells by inducing apoptosis.

Objective To explore the effect of two different enema methods in congenital megacolon children. Methods 70 cases of congenital megacolon children were randomly divided into the experimental group and the control group with 35 cases in each group respectively. The control group used traditional reflux-enema; The experimental group was implemented systemic and modified back reflux-enema (negative pressure drainage vessel to reflux-enema plus the padded-fixation of anal canals, cooperated with manual massage of lower abdomen along the direction of colon, if necessary, changed body position). The average enema time, the postoperative hospital stay and the effect of cleaning the intestine in the two groups were observed. Results Compared with the control group, the average preoperative enema time and hospitalization time was significantly shorter, and intestine cleaning effect was better in the experimental group. Conclusions Modified reflux-enema could increase preoperative enema time and postoperative hospitalization time, and had good effect on cleaning intestine.

Objective To explore the effect of extended care on behavior mental state of patients with Hirschsprung disease.Methods 40 cases of patients with Hirschsprung disease underwent surgery in our hospital from January 2009 to June 2010 were set as the control group.42 cases of patients with Hirschsprung disease underwent surgery in our hospital from July 2011 to June 2012 were set as the experimental group.The patients in the control group received conventional guidance after Hirschsprung disease surgery before discharge,which included dietary guidance,anus functional exercise,regular anal expansion,follow-up upon discomfort and regular re-visit.The patients in the experimental group not only received conventional guidance after Hirschsprung disease surgery before discharge,but also received systematic extended care plan.The nursing effect was compared between two groups.Results Conduct problems and hyperactivity index of patients in the experimental group were compared with those in the control group,the differences were significant.The scores of learning problems,psychosomatic disorders,impulsivity-hyperactivity and anxiety were lower,and compared with those in the control group,the differences were not significant.Conclusions Implementation of extended care for patients with Hirschsprung disease surgery can improve behavior mental state of them.

Objective　To study the analyzing method of oscillatory potentials (OPs) and its clinical application.Methods　OPs from 40 normal control eyes, 33 eyes (33 cases) with early (0～Ⅱ stage) diabetic retinopathy (DR), 9 eyes (9 cases) with central retinal vein occlusion (CRVO) and 9 eyes (9 cases) with branch retinal vein occlusion (BRVO) were tested. In each OPs waveform, the frequency spectra from 10 to 290 Hz were derived by Fast Fourier Transform (FFT). The sum of power and the maximum power were calculated. The OPs in time domain was re-constructed by counter FFT with the components from 110 to 250 Hz in the frequency spectrum. The latencies, the amplitudes and the sum of amplitudes of wavelets in the re-constructed OPs were measured by the computer.Results　Comparing with normal control eyes, in the OPs of early DR, the mean frequency spectra from 140 to 210 Hz were reduced; the sum of power and the maximum power were reduced; the latencies of the four wavelets were prolonged; the amplitudes of the four wavelets and the sum of the amplitudes were attenuated. In the OPs of CRVO, the mean frequency spectra from 150 to 180 Hz were reduced; the sum of power and the maximum power were reduced; the latencies from the first to the third wavelets were prolonged; the amplitudes from the first to the third wavelets and the sum of amplitudes were attenuated. In the OPs of BRVO, the sum of power and the maximum power were reduced; the latencies from the second to the third wavelets were prolonged; the amplitudes from the first to the third wavelets and the sum of amplitudes were attenuated.Conclusion　The analysis of OPs in the present study can provide effective and objective indices for diagnosis of DR, CRVO and BRVO.

Objective:Effect of the low molecular weight natural tumor suppressor (LMW NTS) of human fetal hepatocyte and that of embryo calf hepatocyte on the growth of murine S 180 cells was observed.Methods:LMW NTS was isolated respectively from human fetal hepatocyte and embryo calf hepatocyte.(1)In vitro two layers agar plate culture method was used to detect the suppressive effect of the human fetal hepatocyte LMW NTS and the emtryo calf hepatocyte LMW NTS on the growth of murine S 180 cells.(2)Surviral of the mice bearing S 180 cells and treated respectively with intraperitoneal injections of the human fetal hepatocyte LMW NTS or the embryo calf hepatocyte LMW NTS(0.5 mg/one mouse/day for 21 days)was surveyed.Results:Both the human fetal hepatocyte LMW NTS and the embryo calf hepatocyte LMW NTS can obviously inhibit the growth of murine S 180 cell,and the suppressive effect was enhanced while the concentration of LMW NTS was increased.Survival of the mice bearing tumor that treated with intraperitoneal injections of the human fetal hepatocyte LMW NTS or the embryo calf hepatocyte LMW NTS was remarkably prolonged.Conclusion:The LMW NTS of embryo calf hepatocyte is similar to that of human fetal hepatocyte,and it possesses marked anti tumor activity both in vitro and in vivo.
