3.Advances in the treatment of primary central nervous system lymphoma

Primary central nervous system lymphoma (PCNSL) is a rare extranodal lymphoma. Its pathological type mainly includes dif-fuse large B cell lymphoma. In MRI images, the condition is usually manifested as solitary deep lesions in brain parenchyma, with vari-ous forms of reinforcement. To date, chemotherapy based on high-dose methotrexate administration is the first-line therapy of PCNSL. Combined radiotherapy and chemotherapy can prolong the overall survival of patients. However, this approach simultaneously causes higher incidence of neurotoxicity. High-dose chemotherapy followed by autologous stem cell transplantation is recommended for re-current and refractory PCNSL. Temozolomide and rituximab can be used as treatment options because of their low toxicity and good tolerance. The prognosis of PCNSL depends on several factors, such as serum LDH concentration, age, ECOG/KPS score、cerebro-spi-nal fluid (CSF) protein concentration and tumor location.

5.BCG HSP70 gene transfection and its effect on immunogenicity of murine lymphocytic leukemia

Objective To assess the effect of heat shock protein 70 of BCG (BCG HSP70) gene transfection on tumorigenicity and immunogenicity of murine lymphocytic leukemia cells (L1210). Methods BCG HSP70 gene was transfected onto the surface of murine lymphocytic leukemia cells (L1210) by lipofectamine 2000. And then the positive clone (L1210-HSP70) highly expressing HSP70 was selected as the tumor vaccine to study the tumorigenicity experiments in nude mice and syngeneic mice, the therapeutic experiments, and the immunoprotective effects. Results The expression of BCG HSP70 on the L1210 cells surface was detected, and the L1210-HSP70 cells had the same tumorigenicity as the parental L1210 cells did. Tumorigenicity experiments in syngeneic mice:In L1210-HSP70 group, tumor growth was slow or without the formation of tumor. As compared with L1210 group and L1210-neo group the mice survival time was signiifcantly prolonged, showing a marked stimulating effect on L1210 specific Th1 cells,. Tumor-bearing mice showed complete coagulation necrosis and abundant CD8+T lymphocyte inifltration (P<0.05). The tumor vaccine of L1210-HSP70 cells had the antitumor therapeutic efifcacy and immune protection effect, demonstrating that the tumor growth was signiifcantly inhibited, tumor diameter was markedly reduced and the survival time of tumor-bearing DBA/2 mice was further prolonged. Conclusions BCG HSP70 gene transfection could effectively improve the immunogenicity of tumor cells, activate speciifc T cells and enhance the anti-tumor immunity in vivo. Meanwhile, the host anti-tumor immunity could be enhanced.
6.Application of hand control high frequency continuous positive airway pressure on the neonatal transport of severe congenital diaphragmatic hernia

Objective To investigate the clinic application significance of hand control high frequency continuous positive airway pressure treatment with self-inflating bags(SIB) fitted with positive end expiratory pressure (PEEP) valves on the neonatal transport of severe neonatal congenital diaphragmatic hemia (CDH).Methods Clinical data of a total of 28 infants with severe CDH transported by the pre-hospital emergency care team of our department were analyzed.The 28 patients were divided into treatment group (n =16) and control group (n =12).Hand control high frequency continuous positive airway pressure of SIB and PEEP valve were administered in the treatment group,while only hand control SIB positive airway pressure were given in the control group during the transportation.The exacerbation,complication and blood gas indicators at different time before and after treatment in two groups were monitored and compared.Results In the treatment group,no case aggravated,no case complicated with gas leakage,pulmonary hemorrhage,persistent pulmonary hypertension,intracranial hemorrhage; 2 cases died for multiple organ failure before operation (12.5%),5 cases died after operation (31.3%).In the control group,8 cases aggravated (66.7%),cases complicated with gas leakage,pulmonary hemorrhage,persistent pulmonary hypertension,intracranial hemorrhage were 5 (41.7 %),3 (25.0%),1 (8.3 %),respectively; 4 cases died for multiple organ failure before operation (41.7%),5 cases died after operation (33.3%).The indexes of pH,PaO2,PaCO2 and PaO2/FiO2 at different time before and after treatment between the treatment group and the control group or in each separate group all showed statistical significance (P < 0.05).Conclusion Early accurate diagnosis during transportation with hand control high frequency continuous positive airway pressure of SIB fitted with PEEP valves for CDH is a safe and effective ventilation method which can reduce complications and increase successful transfer and survival rate.

Objective To investigate the detection of anti-cyclic citrullinated peptide(anti-CCP)antibody,alkaline phosphatase (ALP),glucose-6-phosphate isomerase(G6PI)and rheumatoid factor(RF)in the diagnosis of rheumatoid arthritis(RA).Methods Serum RF,anti-CCP body,ALP and G6PI were detected in 206 cases of RA,68 cases of other autoimmune diseases and 48 persons with the healthy physical examination,at the same time the erythrocyte sedimentation rate(ESR)was measured;the sensitivity and specificity of each indicator and the relationship between each index and RA′s diagnosis.Results The serum level of each index in the RA group was significantly higher than that in the other two groups(P <0.05).The sensitivity of RF,anti-CCP antibody and G6PI for diagnosing RA is similar;the specificity of anti-CCP body was highest(94.8%),RF was similar to G6PI;ALP with rela-tively lower sensitivity and specificity also reflected the potential of RA to some extent.Conclusion The simultaneous detection of serum RF,anti-CCP body,G6PI and ALP can improve the accuracy of early diagnosis of RA,reduce the missed diagnosis and guide the clinical treatment.
