1.Effect of donor's activating KIR on prognosis after haemopoietic stem cell transplantation

Objective To investigate the effect of donors' activating killer cell immunoglobulin-like receptor (aKIR) on recipients prognosis post haemopoietic stem cell transplantation (HSCT).Methods Fifty-nine cases of related HLA full matched HSCT were studied. Sequence specific primer polymerase chain reaction (SSP-PCR) was used to type donors' KIR. Virus,bacterium,and fungi infection as well as bleeding,relapse,survival were observed in the recipients post transplantation. Relationship between aKIR and clinical indexes mentioned above were analyzed.Results Donors' aKIR showed no beneficial effects on recipients' virus and bacterium infection. Furthermore,the incidence of fungi infection was increased in HSCT if donors expressed KIR3DS1 (?2= 4.804 ,P= 0.028 ). No significant difference was found in survival curve between donor aKIR positive HSCT or negative one (KIR2DS1+/-: P= 0.651 ; KIR2DS2+/-: P= 0.847 ; KIR3DS1+/-: P= 0.341 ). No effect of donor's aKIR on relapse was observed,too.Conclusion In Guangdong Han,so far as related HLA full matched HSCT is concerned,no improvement in recipient prognosis due to donor's aKIR is verified.
2.Immune response of neutrophil against candida infection

Neutrophil ( PMN) is the main immune effect cell of killing candida yeast cells .It can recognize different cellular components of candida through a variety of surface and intracellular pattern recognition receptors , phagocytose and/or form extracellular traps to capture candida yeast and mycelia , then kill them with oxidation or non-oxidation mechanism .It can also adjust innate immunity and adaptive immune response in candida infection by interaction with other immune cells .This article reviews recent progress of the multiple subsets of neutrophil , the mechanism of its recognizing and killing candida , and the interaction with other immune cells , to provide new perspectives for clinical therapy of candida infections .
3.Multiple organ parenchymal cell apoptosis and its induction early after ischemia-reperfusion in rats and mice

AIM: To determine the evolutionary pattern of parenchymal cell apoptosis in multiple organs early after intestinal ischemia-reperfusion(I/R) and its induction mechanisms and the role of apoptosis in triggering SIRS/MODS. METHODS: An I/R model was reproduced by clipping and releasing the superior mesenteric artery in rats and mice. Flow cytometry, electron microscope, DNA agarose gel electrophoresis, TUNEL method, fluorescent and Gomori's silver-HE staining were used to detect apoptosis. Distribution features of apoptotic parenchymal cells in multiple organs were observed. Immunohistochemical staining of HSP 70 and Bcl-2 were performd to study the induction mechanisms of apoptosis.RESULTS and CONCLUSION: 1. Damage of the liver, lung, gut and kidney was appeared in early phase of I/R. The percentages of apoptosis in parenchyma organs increased progressively. The percentages of cell necrosis increased with the prolonged I/R duration. 2. Percentages of apoptosis were much higher near the central veins of liver lobules, in the outer medulla of the kidney, and the antimescenteric border of intestinal mucosal epithelium because of ischemia. 3. The expression of HSP 70 increased and Bcl-2 reduced in the areas mentioned above because of hypoperfusion. 4. Apoptosis of I/R hepatocytes, splenocytes and thymocytes was obviously increased after Kupffer cell blockage with GdCl3, showing the functional state of Kupffer cells may play an important role in SIRS/MODS.
4.Analysis of the levels of psychological distress and its influence factors in patients with lung cancer

Objective To understand the levels of psychological distress in patients with lung cancer,and analyze the reason and influential factors. Methods The general information of the patients and the distress thermometer(DT) were used to carry out the cross-sectional study in 117 patients with lung cancer received treatments from September 2013 to January 2014. Results The score of psychological distress in patients with lung cancer was (3.10±1.70) points,and 56 patients (47.9%) had the significant psychological distress (DT score≥4 points). The main cause of lung cancer psychological distress ranking in the top 10, in turn, was fear, pain, fatigue, sleep problems, fear, tension, economic problems, eating, breathing, constipation. Mutiple linear regression analysis showed that the number of chemotherapy, age, vocation could affect the total score of distress. Conclusions The patients with lung cancer have obvious psychological distress. Emotional and physical problems of the patients are the main reason of psychological distress of patients with lung cancer.The number of chemotherapy,age,vocation of the patients are related to the psychological distress of patients with lung cancer.
5.Prevalence and related factors of post-traumatic stress disorder in nurses two years after Wenchuan earthquake

Objective To explore the prevalence and related factors of post-traumatic stress disorder (FTSD) among nurses who took part in rescue of 5.12 Wenchuan earthquake after two years.Methods The PTSD Checklist-Civilian Version (PCL-C) was used to diagnose PTSD among nurses from Mianzhu,one of the city where the earthquake happened.Results Totally 223 subjects finished the evaluation.The incidence rate of PTSD among the nurses was 1.7%(PCL-C>50),and 8.2% people had different degree of post-traumatic stress disorder.Comparing the three symptom clusters,hyper-arousal was the most prominent one.Different age,professional position,marriage,health condition significantly influenced the prevalence of PTSD.The risk factors of PTSD was bad health condition and being divorced or widowed.Conclusions The nurses still showed some symptoms of PTSD two years after the earthquake,individualized psychological intervention should be applied to them.

The paper analyzed legally the following predicaments of legitimization of rural doctors in China:no legitimacy protection for their practice,vague criteria for practicing medicine,conflicts between legal regulations for medical practice and rural realities,and lack of a liability insurance system.The authors,based on an exploration of the legislative framework for rural doctors,made four corresponding legislative proposals as follows:to clearly define the legal scope and identity of rural doctors,to establish a qualification and employment system for rural doctors,to develop practicing norms applicable for rural doctors,and to set standards for their income.
7.Detection of plasma protein S and growth-arrest specific protein 6 and their clinical significance in systemic lupus erythematosus

Objective To investigate the significance of plasma protein S and growth-arrest specific protein 6 (Gas6) in systemic lupus erythematosus (SLE).Methods Sandwich enzyme-linked immunosorbent assay (ELISA) was used to measure the protein S and Gas6 in the plasma of 103 SLE patients and 40 healthy controls.Mann-Whitney U-test,x2-test and Spearman's rank correlation were used for statistical analysis.Results The plasma concentrations of protein S and Gas6 were both significantly [protein S:30.6 (27.2,33.6) μg/ml,37.8(35.4,46.7) μg/ml,Z=6.04,P<0.01; Gas6:402.6(239.2,757.8) pg/ml,913.6(765.0,1 290.6) pg/ml,Z=4.26,P<0.01] decreased in plasma of SLE patients than that in healthy controls.There was a positive correlation between levels of protein S and Gas6 (r=0.312,P=0.001).The level of protein S in SLE patients was positively correlated with hemoglobin (Hb),WBC,platelet blood platelet (PLT),C3 and C4 (r=0.209,0.264,0.264,0.362,0.280,P<0.01 or P<0.05).Plasma protein S level was also found to be negatively correlated with auto-antibodies such as anti-double strand DNA (anti-dsDNA) antibody,anticardiolipin (ACL),anti-nucleosome antibody (AnuA) or IgG (r=-0.197,-0.264,-0.226,-0.229,P<0.01or P<0.05).The plasma Gas6 level was positively associated with age,disease duration and C-reaction protein (CRP) (r=0.229,0.198,0.263,P<0.01 or P<0.05).Patients with decreased Gas6 level showed higher incidence of fever,rash and serositis.Patients with decreased WBC or positive for proteinuria also showed decreased Gas6 (P<0.05).Conclusion Protein S levels are significantly decreased in plasma of SLE patients and is associated with a series of severe disease manifestations such as hematological involvement,decreased complements and the presence of auto-antibodies.Decreased Gas6 levels in SLE patients are observed and they are correlated with age,disease duration and certain clinical characteristics such as rash,renal involvement and inflammatory response.
8.Study on the evaluation index system of the quality of health education among the elderly in the community

Objective To establish a scientific and feasible evaluation index system of the quality of health education among the elderly in the community.Methods Based on the results of literature review,theory analysis,cross-sectional study and interviews,a draft evaluation index system was formulated.Then Delphi method was conducted on 28 specialists around the country,the weight of each index was determined by hierarchy analysis.Results After three rounds of Delphi procedure,the response rates of the three round consultation were all above 85%,and the authority coefficient and coordination coefficients was 0.858,0.210,respectively.An evaluation index system which covered three first-level items,7 second-level items and 35 third-level items was formulated.Conclusions Based on survey of the elderly and interviews of health education staffs in the community,and using the method of Delphi to collect experts' suggestion,a strong maneuverability and high reliability quality evaluation index system was formulated,which provided guidance for community health education of the elderly,and promoted its development towards scientization and standardization.
9.The Recovery of Memory after Traumatic Brain Injury

Objective:To explore the time course of memory recovery during post-traumatic amnesia after brain injury.Methods: A patient (WGS, aged 34) with lesions in the left temporal lobe was compared with four matched control subjects on various cognitive tests. These tests included Galveston Orientation and Amnesia test, Wechsler Memory Scale-Revised Test, Semantic Memory, and Remote Memory Test. WGS was tested at 20th days and 33rd days after the brain injury. Results: During the period of post-traumatic amnesia, marked recovery was observed for orientation, semantic memory and remote memory (especially the recent and the earlier items). While the recovery of learning for new knowledge was slow, learning curve of association of unrelated items was flat.Conclusion: The differential time courses for recovery of cognitive functions should be well considered in rehabilitative training after traumatic brain injury.
