1.Antagonistic effect of captopril on activation and injury of vascular endothelial cells induced by lipopolysaccharide

AIM: To investigate the antagonistic action of Captopril (Cap) on the activation and injury of human umbilical vascular endothelial cells (HUVECs) induced by lipopolysaccharide(LPS) and the possible mechanisms. METHODS: After 18 h exposure of the cultured HUVECs to LPS (1 mg/L), or LPS (1 mg/L) plus Cap at the concentration of 10~(-7)mol/L, 10~(-5)mol/L and 10~(-3)mol/L, the expression of vWF protein in the conditioned media was tested by enzyme-linked immunosorbent assay (ELISA), the expression of ICAM-1 protein in HUVECs was determined by indirect immunofluorescence technique with flow cytometry as well. In addition, the expression of TNF? mRNA was determined by in situ hybridization. 2.Primary hypertension treated with acupuncture combined with auricular point sticking: a randomized controlled trial.

OBJECTIVETo compare the differences of clinical effects on primary hypertension between acupuncture combined with auricular point sticking and western medicine.
METHODSNinety patients with primary hypertension were randomly divided into a medication group and a combination group, 45 cases in each group. In the medication group, captopril tablets were treated with oral administration, 12.5 mg each time, twice a day; in the combination group, acupuncture and auricular point sticking were used, and acupuncture was applied at Renying (ST 9), Fengchi (GB 20) and Baihui (GV 20), while auricular point sticking was adopted at Shenmen (TF4), Xin (CO15), Jiangyagou and Jiaogan (AH6a), once a day for continuous 4 weeks. After treatment, the changes of 24 h ambulatory blood pressure, angiotensin II (Ang II) level, creatinine level and syndrome scores were observed and the clinical efficacy was assessed in the two groups.
RESULTSAfter 4 weeks' treatment, the 24 h ambulatory blood pressure was both improved compared with that before treatment in the two groups (P<0.01, P<0.05). The reducing of 24 h ambulatory blood pressure in the combination group was superior to that in the medication group (P<0.05). There were reducing tendencies about coefficients of variation (CVs) of the two groups, and in the combination group, the CVs of 24 h ambulatory blood pressure and systolic pressure acquired more obvious reducing range compared with the medication group (both P<0.05). Serum Ang II level and plasma creatinine level of the patients in the two groups after treatment were both decreased (both P<0.05) and the reducing of Ang II level and creatinine level of the combination group was more obvious than that of the medication group (both P<0.05). The syndrome scores of the two groups after treatment were both remarkably decreased (both P<0.05), and the reducing of the syndrome scores of the combination group was more marked than that of the medication group (P<0.05). The total effective rate of the combination group was 95.6% (43/45), which was superior to 71.1% (32/45) of the medication group (P<0.05).
CONCLUSIONAcupuncture combined with auricular point sticking for primary hypertension has reliable effect. It is better than captopril for the improvement of 24 h ambulatory blood pressure, AngII level and creatinine level, and can improve dizzy, palpitation and other clinical symptoms.

3.Clinical curative effect of Wangbi tablet combined with joint mobilization in the treatment of periarthritis of shoulder

Objective To observe the clinical effect of Wangbi tablet oral therapy combined with joint mobilization in the treatment of periarthritis of shoulder, to provide reference for the treatment of periarthritis of shoulder.Methods A total of 136 patients with scapulohumeral periarthritis in our hospital from January 2015 to June 2016 were selected as the research object, according to the order of hospitalized patients were randomly divided into observation group and control group,with 68 cases in each group, the observation group was treated with Wangbi tablet combined with joint mobilization,the control group was treated with single joint mobilization.After treatment, the clinical curative effect,complications or adverse reactions were comparedbetween the two groups, visual analogue scale(VAS)was used to compare the degree of pain of shoulder joint of patients before and after treatment, the shoulder joint function score were compared by the Japanese Orthopedic Association(JOA)'s shoulder disease score, the patients were followed up for a half a year and the Barthel index score was used to compare the daily life ability score of dynamic contrast.Results (1)The total effective rate of the observation group was(64/68), the control group was 82.35%(56/68), the observation group was significantly better than the control group(P<0.05).(2)The incidence of adverse reactions in the observation group was(8/68), the control group was 7.35%(5/68), and the safety of the two groups was good.(3)the VAS score of the observation group was(3.09±0.98), the control group was(4.53±1.02), and the pain of the observation group was significantly lower than that of the control group(P<0.05).(4)After treatment, the JOA score of the observation group was(16.54±2.36), and the control group was(12.97±2.71).The observation group was significantly higher than the control group(P<0.05).(5)The observation group treatment and follow-up of the Barthel index score was(73.11±6.21),(83.89±7.95)points, the control group was(68.41±5.65),(73.88±6.80), the activities of daily living of the observation group were higher than that of the control group(P<0.05).Conclusion There is a significant effect of Wangbi tablet therapy combined with joint mobilization for the treatment of patients with periarthritis of shoulder, the safety of the treatment is high, can reduce the pain of patients, improve the activity of shoulder joint function and activity of daily living, the comprehensive curative effect, with a high clinical value to promote.
4.Comprehensive geriatric assessment and its clinical impact in oncology

With the aging of the population,management of cancer in the elderly aged people represents a priority for health care.However,fewer evidence-based data are available to promote the treatment of these patients.Comprehensive geriatric assessment (CGA) is developed recently as a multi-disciplinary in-depth evaluation to assess life expectancy and risk of morbidity from cancer in elderly patients,which is helpful for physicians to develop a coordinated plan for cancer treatment and to provide appropriate interventions to decrease the risk of morbidity and mortality.
5.Gene polymorphisms of prognostic factors in non-small cell lung cancer

Many studies show that excision repair cross-complementing gene 1 (ERCC1),ribonucleotide reductase subunit M1 (RRM1),p53,epidermal growth factor receptor (EGFR),and vascular endothelial growth factor (VEGF) are closely related to the curative effect and prognosis of non-small cell lung cancer (NSCLC).In recent years,many studies have investigated the association between polymorphisms of these factors and the curative effect and prognosis in patients with NSCLC.The polymorphisms of these factors may be potential biomarkers to predict the clinical outcome in patients with NSCLC.
6.Clinical Observation of Bevacizumab Combined with Chemotherapy in the Treatment of Advanced Gastric Cancer

OBJECTIVE:To investigate clinical efficacy and toxic and side effects of bevacizumab combined with chemothera-py in the treatment of advanced gastric cancer. METHODS:84 patients with advanced gastric cancer were randomly divided into combination group and chemotherapy group,with 42 cases in each group. Chemotherapy group received FOLFOX4 chemotherapy regimen(oxaliplatin+calcium folinate+fluorouracil);combination group was additionally given bevacizumab 7.5 mg/kg intravenous-ly,d1,on the basis of chemotherapy. A treatment course lasted for 21 d,and both groups received 3 courses of treatment. The short-term efficacy,the quality improvement of life,serum tumor marker before and after chemotherapy and the toxic and side ef-fects were compared between 2 groups. RESULTS:The effective rate in combination group(57.14%)was significantly higher than chemotherapy group(35.71%),improvement rate of life quality in combination group was higher than chemotherapy group,reduc-tion rate of life quality in combination group was lower than chemotherapy group,the differences were statistically significant(P<0.05);after chemotherapy,the serum CEA and CA199 levels were significantly decreased in both groups,the decrease of combina-tion group was more significant than that of chemotherapy,with statistical significance(P<0.05). The incidence of decrease in leu-cocytes,platelet and hemoglobin,periphery neurotoxicity,nausea and vomiting,liver and kidney function in combination group was higher than in chemotherapy,but there was no statistical significance (P>0.05). CONCLUSIONS:Bevacizumab combined with chemotherapy is effective in the treatment of advanced gastric cancer and don’t increase toxic and side effects.
7.A randomized controlled study of water exchange colonoscopy and air insufflation colonoscopy

Objective To investigate the clinical application prospect of water exchange colonoscopy.Methods A total of 300 outpatients who underwent unsedated colonoscopy were randomized to the WEC group (n =150)or the AIC group(n =150).The maximum pain score, insertion time, the cecal intubation rate and the polyp detection rate were reported by an assistant during the insertion phase and at the end of colonoscopy.Results The maximum abdominal pain scores were 1.77 ± 1.29 in the WEC group and 2.84 ± 1.55 in the AIC group with significant difference (P =0.000).The intubation time of WEC was longer than that of AIC (11.91 ± 3.58 VS 8.03 ± 3.19 ,P =0.000)with no significant difference.There was no significant difference in the cecal access rate between the two groups(97.3% VS 94.0% ,P =0.157).The polyp detection of WEC was higher than that of AIC (16.7% VS 10.0%, P =0.089).In the post abdominal operation subject of colonoscopy procedure, WEC had higher rate of cecal intubation than AIC ,the maximum abdominal pain score were 2.25 ± 1.63 in the WEC group,lower than 3.48 ± 1.45 in the AIC group.Conclusion Water exchange colonoscopy can alleviate patients' abdominal pain symptoms, but with longer insertion time.There are no significant difference in the cecal access rate and the polyp detection rate between two groups.For difficult colonoscopy, water exchange colonoscopy is helpful to increase the rate of cecal intubation.
8.Study on transformation of anti-nasopharyngeal carcinoma plasmid pFY and bacterial strains screening

Objective To screen the stable high-producing strains carrying anti-nasopharyngeal carcinoma(NPC)plasmid pFY.Methods Competent E.coli JM109 was prepared by the CaCl2 method and transformed with anti-NPC plasmid pFY.The bacterial colonies obtained from the agar plate were screened for selecting the single colony conforming to the standards as the bac-terial strain and conducting the stability test.The plasmid content was detected by the plasmid extraction reagent kit.Anti-NPC plasmid pFY was transfected into nasopharynegal carcinoma cell line CNE-2.The influence of transfection reagent and the plasmid vector on the cell proliferation was detected by MTT.Results The DNA concentration of plasmids in the culture solution of bacte-rial strain obtained by screening was 30 mg/mL.The proportion of supercoiled DNA was 92%.The identification of electrophoresis and restriction enzymes showed that the plasmids harbored in the 50th progeny of this strain were same as those in the primary. Plasmid pFY had the evident inhibiting effect on the growth of CNE-3 cell line.Conclusion The stable high-producing strains of E. coli carrying anti-NPC plasmid pFY is successfully screened out,which lays the foundation for large-scale preparation of plasmid pFY for clinical utility.
9.Effect of Isokinetic Training Concentrically and Eccentrically on Knee Peak Torque and Hamstring/Quadricep Ratio

Objective To analyze the data of concentric (c) and eccentric (e) peak torque of hamstring (H) and quadricep (Q) in differ-ent gender and angle after 4-week concentric and eccentric isokinetic exercise training on knee, in order to provide suggestions for the knee extensor and flexor in rehabilitation exercise prescription. Methods 28 healthy college students from May to June 2014 in our college partic-ipated in maximum concentric (n=14) or eccentric (n=14) resistance training of the right knee extensors and flexors 3 times per week for 4 weeks. The concentric strength was measured with BIODEX isokinetic dynamometer assessment system 4 at 30 °/s, 60 °/s, 90 °/s, and ec-centric strength was measured at 60 °/s, 120 °/s, 180 °/s. PT, H/Q (He/Qc, Hc/Qc) ratio and H/H (He/Hc) or Q/Q(Qe/Qc) ratio were ana-lyzed. Results Concentric isokinetic training could improve the Hc and He at 60 °/s and 90 °/s (P<0.05), and the Qe and Qc increased at 30 °/s, 60 °/s, 90 °/s respectively (P<0.01). There was no significant improvement after eccentric training (P>0.05). After concentric isokinet-ic training, the He/Qc ratio decreased at 90 °/s (P<0.05), the Hc/Qc ratio decreased at 30 °/s (P<0.05). However, there was no difference in He/Qc and Hc/Qc after eccentric contraction (P>0.05). The Qc/Qe ratio increased at 60 °/s and 90 °/s (P<0.05), and the Hc/He ratio de-creased at 90 °/s (P<0.05) after concentric training. There was no difference in Hc/He and Qc/Qe after eccentric contraction (P>0.05). There were significant differences in Hc, He, Qc, Qe and He/Qc ratio between different genders (P<0.05). Conclusion Concentric training and ec-centric training have different influences on the muscle strength after 4 weeks training. Eccentric training only can reduce the knee stability. Hc/He ratio and Qc/Qe ratio can be used to evaluate the knee muscle function. Gender should be considered into training.
10.Prognostic value of pre-operative NLR, d-NLR, PLR and LMR for predicting clinical outcome in surgical colorectal cancer patients

Objective:To evaluate the impact and predictive value of neutrophil-to-lymphocyte ratio( NLR) ,derived neutrophil-to-lymphocyte ratio( d-NLR) ,platelet-to-lymphocyte ratio( PLR) and lymphocyte-to-monocyte ratio( LMR) on the prognosis of patients with colorectal cancer with curative resection.Methods: Retrospective analysis of 205 cases of clinical data and returning data after patients with colorectal cancer surgery,ROC curve was applied to determine thresholds for four biomarkers and their prognostic values were assessed using Kaplan-Meier curve,univariate and multivariate COX regression models of patients with NLR,D-NLR,PLR and LMR and other clinicopathological factors and the relationship between the 5-year recurrent-free survival( RFS).Results:According to the ROC curve,LMR could not be used as a potential diagnostic biomarker for subsequently analysis,for its AUC was less than 0.50, The optimal cut-off values were 3.12 with the highest specificity and sensitivity ( 75% and 73.1%) for NLR, while in the univariate analysis show,NLR,D-NLR and PLR were all risk factors of poor recurrence-free survival ( P<0.05 ) , the multivariate regression analysis,NLR≥3.12 and colorectal cancer staging were independent risk factors of prognosis of colorectal cancer.Conclusion:NLR≥3.12 can provide improved accuracy for predicting clinical outcomes in surgical CRC patients under surgery resection.
