1.Discussion on English translation of traditional Chinese medicine with the intercultural communication perspective

Analysis of differences between traditional Chinese medicine (TCM) philosophy with malnstream western medicine was done with reference to intercultural communication theory. It was found that the differences are malnly manifested through holism philosophy of TCM and dualism of Western medicine, intuition thinking mode of TCM and mechanical thinking mode of western medicine, and ideographic Chinese language and objective description of the Western medicine. The translation principle was also discussed with the perspective of intercultural communication. It is of great significance to respect the recipient’s privilege, to create intimate text, and to understand the bilingual differences if TCM translation is to be promoted.
2.Pregnancy outcome after cervical conization

Objective To investigate the effect of cervical knife conization (CKC) or loop electrical excision procedure (LEEP)on the outcome of subsequent pregnancies and mode of deliveries. Methods A retrospective case-control study including 228 women after treatment with LEEP or CKC for cervical intraepithelial neoplasia (CIN) Ⅱ -Ⅲ who gave birth in the First Affiliated Hospital of Sun Yat-sen University and He-xian Memorial Hospital of Pangyu from January 2004 to January 2010 was performed.Patients (n = 228) without cervical surgical history were randomly extracted from the respective hospitals birth registries as controls and were matched by age, gestation,parity and income.The information including gestational age, premature rupture of membranes (PROM), type of deliveries and birth weight of the two groups were collected.Results The gestational age of women treated with conization was (268.3±26.2) d, longer than that of the women without surgery (279.4±25.3) d (t=4.60, P<0.01). The incidence of preterm birth was 18.0%(41/228) and 4.4% (10/228) (x2 = 21.22, P< 0. 05). The incidence of PROM was higher in conizationgroup (10.1%, 23/228) than that (1.3%, 3/228) in control group (x2=16.32, P<0. 05). Risk for PROM was almost eight fold (OR=8. 42, 95%CI: 2.49-28.44) higher in conization group. Cesarean section rate was higher in conization group (69.3 % ) than in control group (39.0 % )(x2=42.06, P<0. 01). The gestational age of women treated with LEEP was longer than those treated with CKC[(269.8±24.6) d vs (260.2± 26.5) d, t= 4. 01, P<0.01]. The incidence of preterm birth was 13. 1% (22/168) and 31.6% (19/60) (x2 = 10. 34, P<0. 05). The mean birth weight of women with LEEP was heavier than that with CKC[(3358.5 ±812.2) g vs (3295.9 ±832.6) g, t=3.08, P<0. 01]. The incidence of PROM (7.1%, 12/168) of woman with CKC was higher than that (1.3%, 11/60) of women with LEEP (x2 =6.10, P<0.05). Conclusions Conization might increase the incidence of preterm delivery and preterm PROM. LEEP showed less adverse effect onthe outcome of subsequentpregnanciesthan CKC,and waspreferredfor primigravida, and the risk of treatment should be informed in advance.
3.Analysis of the situation of pre-pregnancy health check in rural areas of Sichuan province

Objective To investigate the situation of pre-pregnancy health check in rural areas of Sichuan province and get the key difficult points during the process of carrying out the work.Methods According to the economic situation in Sichuan province and geographic distribution,random eight areas in Sichuan province (Chengdu,Neijiang,Nanchong,Ziyang,Meishan,Zigong,Les-han and Suining),investigate pregnant women in maternal and child health care institutions and family planning service institutions at all all levels in the areas above.Results Only 334 people (33.26%)underwent the pre-pregnancy health check during the 974 pregnant women in rural areas.The rate of planned pregnancy is low;the rate of unintended pregnancy is close to 40.00%.55.29%of the planned pregnancy women didn′t undergo the pre-pregnancy health check.And the factors that affect their behavior of under-going pre-pregnancy health check are whether planned pregnancy and personal attitude towards pre-pregnancy health check.Conclu-sion The rate of pre-pregnancy health check is low in the pregnant women from rural areas of Sichuan province,the health sector needs to make efforts to promote and enhance the awareness of the rural pregnant women to prevent birth defects and the initiative in participation.
4.Femoral compression variation in osteoporosis rats after intervention with different drugs

BACKGROUND:The mechanical index is an important method for the evaluation of the therapeutic efficiency of drug treatment for osteoporosis animal models. OBJECTIVE: To explore the effects of various drug treatments on osteoporosis through a mechanical performance test about the femoral compression of rats. METHODS: Thirty-six Wistar female rats were randomized into six groups: normal control group, model group, Dan Qiparticles group, alpha-D3 group, premarin group, ipriflavone group, with six rats in each group. Osteoporosis models were made in al groups except for the normal control group, and after modeling, the rats in different groups were treated withDan Qi particles, alpha-D3 group, premarin and ipriflavone, respectively. After 15 weeks, the rats were kiled by abdominal aortic bloodletting to take out the left and right femurs that were placed on a universal testing machine to perform a compressive test at a speed of 5 mm/min. RESULTS AND CONCLUSION:The maximum load, maximum stress, maximum displacement, maximum strain, and elastic modulus were significantly lower in the model group than the other four groups (P < 0.05). There was no difference in different mechanical parameters between alpha-D3 group and model group as wel as between Dan Qi particles group and normal control group (P> 0.05). These findings indicate that osteoporosis leads to the variation of compression mechanical properties of the femur. There are good compression mechanical properties of the femur after treatment with premarin and ipriflavone, andDan Qi particles has the best effect.
5.Comparison of three anti-aquaporin 4 antibody detection methods in neuromylitis optica

ObjectiveTo evaluate three methods of detecting anti-aquaporin 4 (AQP4) antibody in neuromylitis optica (NMO),including indirect immunofluorescence assay organization(IIF),cell immunofluorescence method (CBA) and ELISA.MethodsThe patients were divided into NMO group (n =29), multiple sclerosis (MS) group (n = 23),and healthy controls group (n = 50).IIF, CBA and ELISA were used in 3 groups to detect serum anti-AQP4 antibody.The sensitivity and specificity as well as the consistency of positive results were compared.ResultsIn the aspect of the sensitivity of the three antiAQP4 antibody to diagnosis NMO, CBA (72.4%) > IIF (62.1%) > ELISA (51.7%) ; in the aspect of specificity, CBA (100.0%) > ELISA (98.6%) > IIF (97.3%).Kappa testing and evaluation method showed that the three detection methods were all in good consistency, particular in CBA and ELISA (P <0.01).ConclusionsCBA method showed a highest specificity and sensitivity in all these three anti-AQP4 antibody detection methods.CBA and ELISA are in better consistency of positive results.
6.The immunoregulatory effect of thalidomide on the peripheral blood T-lymphocytes in systemic Iupus erythematosus patients

Objective To investigate the immunoregulatory effect of thalidomide on the peripheral blood T-iymphocytes in systemic lupus erythematosus patients in vitro. Methods T-lymphoeytes were treated by thalidomide with different concentrations, then the proliferation of these T-lymphocytes proliferation was deteted by MTT while apoptosis and lymphocyte activation marker were analyzed by flow cytometry. The mRNA expression of IL-6, IL-10 and TNF-α was measured by real-time RT-PCR, One-way ANOVA was used for statistical analysis. Results In vitro, thalidomide inhibited the expression of CD3~+CD28~+ [500 μg/ml group vs the control group, (48±9)% vs (57±9)% P<0.05]. The pro-portion of apoptotic T-lymphoeytes in the 500 μg/ml group was (36±8)%, which was significantly higher than that in the control group [(23±5)%,P<0.05 ]. The values of A_(570nm) T-lymphocytes were significantly lower in the 100 μg/ml group, 300 μg/ml groupand 500 μg/ml group compared with those of the control group ( 100 μg/ml group vs 300 μg/ml group vs 500μg/ml group vs the control group, 0.39±0.05 vs 0.34±0.04 vs 0.30±0.03 vs 0.51±0.07, P<0.05), while thalidomide promoted the expression of CD8~+CD152~+ [ 100 μg/ml group vs 500 μg/ml group vs the control group, (5.0±0.6)% vs (7.8±0.7)% vs (4.2±0.6)%, P<0.05 ]. 500 μg/ml thalidomide inhibited the mRNA expression of IL-6, 2.5~500 μg/ml thalidomide inhibited IL-10, TNF-α mRNA expression of T-lymphocytes.Conclusion Thalidomide can inhibit the proliferative activities and CD28 expression of T-lymphocytes,reduce mRNA expression of IL-6, IL-10, TNF-α, stimulate CD28 expression and apoptosis of T-lymphocytes. These effects may play an important role in it's immune-suppressive effects on systemic lupus erythematosus.
7.Control study in 0.45% sodium chloride solution and sterile water for injection in airway humidification of patients with mechanical ventilation

Objective To observe the two different wetting fluid in airway humidification of patients with mechanical ventilation.Methods 40 patients with mechanical ventilation after cardiac surgery in our hospital from January to April,2009,were divided randomly into group A and group B,group A was given 0.45% sodium chloride solution 100ml plus ambroxol 15mg as airway humidification fluid,group B was given sterile water for injection100 ml plus ambroxol 15mg as airway humidification liquid.The amount of sputum aspiration,color,viscosity,the body temperature of patients,the lungs auscultation and chest X-ray were observed.Results No statistical difference was seen in sputum volume,color,viscosity,the body temperature of patients,lungs auscultation as well as chest X ray after mechanical ventilation for 4 hours,8 hours,16 hours,24 hours,1~2 d,3~5 d,6~7 d.No statistical difference was also seen in auscultation of the lung after mechanical ventilation for 4 hours,16 hours,24 hours,1~2 d,3~5 d,6~7 d.But auscultation of the lung in group B was better than that of group A after mechanical ventilation for 8 hours.Conclusions No sufficient fact can prove that different effect exists between 100 ml 0.45% sodium chloride solution plus ambroxol 15mg and 100 ml sterile water for injection plus ambroxol 15mg as airway humidification fluid during mechanical ventilation.
8.Expression and significance of Th17 cells from peripheral blood of patients with rheumatoid arthritis

Objective To study the expression and significance of Th17 cells from peripheral blood of patients with rheumatoid arthritis (RA). Methods Intracelluar flow cytomete detection of IL-17/IFN-γ and IL-17/IL-6 was established using anti-CD3/Anti-CD28/IL-23 as stimulators after isolation of untouched human CD4~+T cells from PBMC. There were three groups in the present study: ①healthy controls group; ② RA stable group; ③RA active group. Results The isolation of untouched human CD4~+T cells from PBMC was effective and its purity was over 90%. The percentage of intracelluar IL-17 in CD4~+ T cells from RA patients was increased significantly. Such percentage in active group (1.54±0.41) was higher than that of stable group (0.70±0.21, P<0.01) and both of them were higher than those of healthy controls (0.42±0.12, P<0.01). Under anti-CD3/Anti-CD28/IL-23 stimulation, the percentage of intracelluar IL-17 was also increased significantly(P<0.01). The porcentage of intracellular IFN-γ was similar to that of IL-17, while that of IL-6 was not significantly different. There is an correlation between IL-17 and IFN-γ or IL-6. Conclusion There is an abnormal expression of IL-17 and IFN-γ in human CD4~+T cells in RA patients, which is related to disease activity . Th17 cells may be used as a new marker for the assessment of RA activity.
9.The immunoregulatory effects of thalidomide on T-lymphocytes of rheumatoid arthritis patients

Objective To study the immunoregulatory effects of thalidomide on the peripheral blood T-lymphocytes of rheumatoid arthritis patients.Methods MTr was used to detect the effects of different thalidomide concentrations on the proliferation of T-cells.Flow eytometry was used to analyze T-cells early apoptosis and the T-cells subsets in different concentration of thalidomide.The mRNA expression of IL-6,IL- 10 and TNF-α was measured by RT-PCR method.Results The level of thalidomide at 500 μg/ml inhibited the proliferation of T-ceils and the CD3+CD28+ expression of T-cell subsets,but promoted the early apoptosis and the CD8+CD152+ expression of T-cell subsets.Thalidomine at any concentration could inhibit the mRNA expression of IL-6,TNF-α.However,the level of thalidomide that could promote the mRNA expression of IL- 10 was 100 μg/ml and 500 μg/ml.Conclusion Thalidomide can inhibit the proliferation of T lymphocytes and the expression of CD3+CD28+ on T-cell subsets.It can promote the early apoptosis and the CD8+CD152+ expression of T-cell subsets.Thalidomide inhibits the mRNA expression of IL-6 and TNF-α but promote the mRNA expression of IL-10.Thalidomide has immuno-regulatory effects on rheumatoid arthritis T-cells.
10.The correlation between DVH at CT-image based 192Ir intracavitary brachytherapy and effects or complications for patients with locally advanced cervical cancer

Objective To investigate the correlation between dose volume histogram(DVH)of tumor targets and organs at risk(OAR)at CT-image based 192Ir brachytherapy and effects and complications for patients with locally advanced cervical cancer. Methods Ten patients with FIGO stage ⅢB cervical cancer received CT image-based 192Ir intracavitary brachytherapy after 54 Gy of three-dimentional four-field pelvic external beam radiotherapy and concurrent weekly cisplatin chemotherapy. Before each brachytherapy,CT images were acquired with applicators in place. Gross tumor volume(GTV), clinical target volume (CTV)and OAR were contoured and inverse treatment planning was designed and optimized by using PLATO treatment planning system. Conventional two-dimensional plans were also designed for comparison.The total intracavitary brachytherapy dose was 30 -42 Gy in 5 -7 fractions. The patients were followed, and the local control and complications were analyzed. The biologically equivalent dose(BED)and biologically equivalent dose in 2 Gy fractions(BED2)for GTV, CTV and OAR were calculated. The minimum dose in the most irradiated tissue volume 2 cm3(D2 cm3)adjacent to the applicator of the sigmoid colon, rectum,bladder and small bowel was determined from the DVH. Results The 1-year local pelvic control rate was 90% and grade 1-2 late complication of sigmoid colon and rectum was 50%. No grade 3 or more complications developed. On CT-image based planning, the BED and BED2 to 90% of the CTV(D90)were 95.50 Gy ± 7. 81 Gy and 79. 73 Gy ± 6. 57 Gy. The BED and BED2 to 90% of the GTV(D90)were 101.86 Gy ± 7.27 Gy and 84. 95 Gy ± 6. 1 Gy. The volume enclosed by 90% of prescribed dose(V90)for GTV and CTV were 92% ±4% and 87% ±7% respectively. The D2cm3 for rectum and sigmoid colon were 74. 97 Gy ±1.64 Gy and 67. 93 Gy ± 4. 30 Gy(EQD2, α/β = 3). Comparing with 2D brachytherapy plans , CT - image based planning has improved D90 and V90 for GTV and CTV with similar dose at point A and rectum reference point. Conclusions Computer tomography-image based 192Ir brachytherapy has resulted in the better dose distribution to the tumor targets with excellent tumor control and acceptable toxicity.
