1.CT and MRI Diagnosis of Nasopharyngeal Angiofibroma

Objective To investigate the value of CT and MRI in diagnosing nasopharyngeal angiofibromas.Methods 18 cases of nasopharyngeal angiofibromas were retrospectively analyzed including the location, density ,signal intensity, extension of tumors and changes of surrounding bones.Results 16 cases located in the area around sphenopalatine foramen. 6 cases exhibited homogeneous density on CT scans(6/11 cases) and 12 cases displayed inhomogeneous signal intensity on MR scans(12/14 cases). All the Fourteen MRI examinations revealed vice signal by vessels, inhomogeneous enhancement and extensive invasion along the foramina or fissures.Conclusion MRI not only determines the location and traces out the extension of nasopharyngeal angiofibroma more exactly, but also displayes its diagnostic signal fashion. Namely MRI has greater value in diagnosing this tumor than CT.
2.Research on UKF control of epileptic-form spikes in neural mass models.

Neural mass models are able to produce epileptic electroencephalogram (EEG) signals in different stages of seizures. The models play important roles in studying the mechanism analysis and control of epileptic seizures. In this study, the closed-loop feedback control was used to suppress the epileptic-form spikes in the neural mass models. It was expected to provide certain theory basis for the choice of stimulus position and parameter in the clinical treatment. With the influence of measurement noise taken into account, an unscented Kalman filter (UKF) was added to the feedback loop to estimate the system state and an UKF controller was constructed via the estimated state. The control action was imposed on the hyper-excitable population and all populations respectively in simulations. It was shown that both UKF control schemes suppressed the epileptic-form spikes in the model. However, the control energy needed in the latter scheme was less than that needed in the former one.

3.Analysis of the complication of transvaginal hysterectomy of patients with non-prolapsed uterus

Objective To explore the advantages and disadvantages of transvaginal hysterectomy for patients with non-prolapsed uterus,and explore the cause and preventive measurements of the complications of transvaginal hysterectomy.Methods The complications of transvaginal hysterectomy ( group TVH ) in 110 patients with non-prolapsed uterus and of transabdominal hysterectomy ( group TAH ) in 120 patients were compared with each other.These patients were chosen randomly from June 2006 to April 2010.Results In the TVH group,rectum hurt occurred in one patient,prolapse of fallopian tube in one patient and celiac internal bleeding in one patient.The preoperative diagnosis were not consistent with the postoperative diagnosis in four patients.Two patients encountered with postoperative residual end bleeding and four patients with vaginal residual end polyps.In the TAH group,three patients had postoperative diagnosis inconsistent with their preoperative diagnosis.In addition,there were four abdominal wall incision liquefaction ( including two phase suture in two patients),three postoperative bronchial lung infection and two residual end polyps.No celiac internal bleeding and visceral injury occurred in this group.Conclusion TVH has a higher risk of visceral injury and postoperative bleeding than TAH.These complications should be avoided when treating the patients with TVH technique.
4.Efficiency analysis of the first time platelet transfusion in patients with blood diseases

Objective To study the efficiency of the first time platelet transfusion in patients with blood diseases.Methods Investigate the effect of platelet transfusion in 87 patients with blood diseases.All the patients were transfused 10U platelet (platelet≥2.5×1011 ).Observe CCI after platelet transfusion and analyse the influence factors to the efficiency of the first time platelet transfusion such as the kind of diseases,splenomegaly,fever,age and sex.Results After platelet transfusion,66 patients (75.9 %) were effective.The efficancy rates were 80.0 %,78.1%,65.2 %,82.3 % in AA,ITP,AL,MDS (x2=2.88,P>0.05).In 31 patients with a fever and infaction the efficancy rate was 58.1%,but it was 85.7% in 56 patients without a fever and infaction (x2=8.3308,P<0.01).The efficancy rate in 59 patients with non-splenomegaly was 86.4 %.It was higher than that in 28 patients with splenomegaly which was 53.6%(x2=11.2033,P<0.01).The efficancy rate was 76.0% in male patients and it was 75.7 % in female patients (x2=- 0.0012,P>0.05).The efficancy rate was 74.5 % in young group (<60 years old)and 77.8 % in old group (≥60 years old)(x2=0.1231,P>0.05).Conclusion Fever and splenomegaly are the influence factors to the efficiency of the first time platelet transfusion.
5.CT differential diagnosis of fnngal ball in paranasal sinus caused by different mycotic pathogenic agents

ObjectiveTo evaluate CT characteristics of fungal ball in paranasal sinus caused by different fungi and to enhance differential diagnosis.MethodsCT results and clinical data of 74 patients with fungal ball arising from the paranasal sinuses proved by histopathology from 2007 to 2009 were analyzed retrospectively.The CT characteristics of fungal ball in paranasal sinus caused by different fungi were compared using x2 test with P < 0.05 considered statistically significant.Results Among 74 mycotic pathogenic agents,aspergillus was found in 58 cases (including 36 cases with aspergilhs flavus,15 cases with aspergillus fumigatus and 7 with aspergillus versicolor),the others including 5 cases with penicillium,6 cases with schizophyllum commune,and 5 cases with scedosporium apiospermum.There were significant differences in the number of sinus involved ( single sinus involvement was seen in 29 cases caused by aspergillus group and 2 cases caused by non-aspergillus-group,respectively,with x2 =7.245,P =0.007 ),the incidence of fungus ball in ethmoid sinus [ 39.7% ( 23/58 ) of cases caused by aspergillus group and 81.3 % ( 13/16 ) of cases caused by non-aspergillus-group,respectively,with x2 =8.685,P =0.003 ] and calcification (40 of 58 cases caused by aspergillus group and 5 of 16 cases caused by non-aspergillus-group,respectively,with x2 =7.485,P =0.006 ),the location of calcification ( 26 of 40 cases with central calcification and 14 of 40 cases with peripheral calcification in cases caused by aspergillus group,while all of 5 cases caused by non-aspergillus-group with peripheral calcification,x2 =7.697,P =0.006).However,there was no significant difference in the incidence of bilateral lesions ( x2 =1.002,P =0.317 ),maxillary sinus involvement ( x2 =0.020,P =0.888 ),sphenoidal sinus involvement ( x2 =0.704,P =0.401 ),frontal sinus involvement ( x2 =0.126,P =0.723 ),bony sclerosis ( x2 =2.024,P =0.155 ),lamellar calcification (x2 =2.045,P =0.153 ),complication of nasal polyps( x2 =0.018,P =0.893) and submucosal cyst( x2 =0.779,P =0.378 ).ConclusionsThe common CT characteristics of fungal ball in paranasal sinus are unilateral sinus involvement with inhomogeneous high-density soft tissue and lamellar calcification.The CT findings of fungal ball caused by non-aspergillus-group are ethmoid sinus involvement and calcification located on the periphery instead of the center of fungal ball.
6.Clinical Observation of Shangke Quyu Tablet in Treating Traumatic Swelling and Pain

[Objective] To investigate the effect of Shangke Quyu Tablet (a formula modified from Tao Hong Siwu Decoction) in treating acute traumatic swelling and pain of soft tissue. [Methods] Two hundred and forty-six cases of traumatic injury in four limbs, which were classified to Qi-stagnation and blood-stasis syndrome, were randomized to two groups. After given fracture reduction and dislocation fixation, group A (n = 164) was treated with self-prepared Shangke Quyu Tablet and group B with Tongxuekang Capsules (n = 82) . Therapeutic effect was evaluated after 10 days of treatment. [Results] In group A, 116 cases were markedly improved, 30 much improved, 14 improved and 4 ineffective; in group B, 44 cases were markedly improved, 18 much improved, 16 improved and 4 ineffective. 7.Expression of Shank1 and its clinical significance in renal cancer tissue

Objective:To detect the expression of Shank1 protein in renal cell carcinoma (RCC), to investigate its difference between the tumor and carcinoma adjacent tissue, and to analyze its correlation with RCC clinicopathological characteristics and prognosis. Methods:The renal carcinoma and carcinoma adjacent tissues of 120 patients were selected from Cangzhou Central Hospital and Ji'nan Central Hospital from May 2008 to December 2014. The expression level of Shank1 was determined by immunohistochemistry and Western blot. Statistical analysis was performed to determine the relationship between the expression of Shank1 and the clinicopathological features of RCC patients. Results:Results of immunohistochemistry showed that the expression level of Shank1 in renal cancer tissue was significantly higher than that in carcinoma adjacent tissue, and the difference was statistically significant (P<0.05). Western blot results showed that the expression level of Shank1 protein in renal cancer tissue was also significantly higher than in carcinoma adjacent tissue. Correlation analysis found that the high expression level of Shank1 in renal cancer tissue was not significantly related to gender, age, tumor size, and TNM stage, but was significantly associated with the histological differentiation of RCC (P<0.05). Conclusion:Shank1 is abnormal y expressed in RCC renal cancer tissues and is correlated with the histological differentiation of RCC.

Biliary complication (BC) after liver transplantation has attracted increased attention from a clinical and research perspective.BC,which affects the long-term outcome of the liver transplant,includes diseases of biliary stricture,biliary obstruction,biliary fistula,and many more that need an operation or interventional therapy.The occurrence of these diseases is related to the surgical operation,biliary tract variation,vessel injury,and quality of the donor graft.This article reviews BC and its etiology,diagnosis,and therapy in order to help future clinical and experimental studies.
9.The study on the expression and clinical significance of APRIL in urothelial carcinoma

Objective To investigate the expression of a proliferation-inducing ligand (APRIL) in urothelial carcinoma tissue and its clinical significance. Methods We investigated the expression of APRIL mR-NA in urothelial carcinoma tissue of 33 patients in urothelial carcinoma in Cangzhou Central Hospital from July 2013 to March 2015, Among of them, 23 cases of adjacent pericancerous tissues were detected by qRT-PCR. Immunohistochemistry and western blot were used to detect the APRIL protein expression level in urothelial carci-noma tissue and adjacent pericancerous tissues. We analyzed the relationship between APRIL expression and clinical pathology in patients with urothelial carcinoma by statistical methods. Results The qRT-PCR revealed that the expression of APRIL in urothelial carcinoma tissue was significantly higher than that in the adjacent peri-cancerous tissues (P < 0.05). Immunohistochemistry and western blot showed that the expression of APRIL in urothelial carcinoma tissue was higher than that in the adjacent pericancerous tissues. Correlation analysis found that the high expression of APRIL in urothelial carcinoma tissue was not significantly related to gender , age , size of tumor, lymph node metastasizing or not, and clinical stages (P > 0.05). Conclusion The APRIL is a high abnormal expression in urothelial carcinoma tissues , and maybe related to the occurrence and development of urothelial carcinoma.
10.Investigation and analysis on application situation of Hierarchical Diagnosis and Treatment Guidelines of Obstetrics in Sichuan Province (version 2016)

Objective To understand the implementation and application situation of Hierarchical Diagnosis and Treatment Guidelines of Obstetrics in Sichuan Province (version 2016).Methods The sampling survey was performed on the obstetric medical staffs from the various medical institutions in whole province by the questionnaire survey and on-spot investigation,and some tertiary hospital experts were interviewed.Results A total of 2015 valid questionnaires (93.85 %) were received.Among them,1 369 people (77.7 %) read the Guidelines;1 752 (95.0 %) people thought that the Guidelines had practical operability in their hospitals;the causes considering that it had no practical operability were mainly disagree with referral by pregnant women or their family members,cumbersome referral,beds tension in superior hospital,etc.Improper referral sequencing was improper diagnosis and treatment during the referral process,non-standardized referral process,improper referral timing,no referral indications and so on.Conclusion The Guidelines promote the progress of obstetric hierarchical diagnostic and treatment work.It is necessary to strengthen the training of grassroots medical personnel and strengthen the management,supervision and assessment of referral.
