1.Effect of dietary and physical activity interventions on reducing excessive weight gain during pregnancy, cesarean section and macrosomia :A Meta-analysis

Objective To systematically review the effect of dietary and physical activity interventions on weight management during pregnancy.Methods The PubMed (from Jan.1990 to Sep.2013),Springer (from Jan.1960 to Sep.2013),Willy (from Jan.1990 to Sep.2013),Web of Science (from Jan.1990 to Sep.2013),Wanfang data (from Jan.1995 to Sep.2013),China National Knowledge Infrastructure (CNKI) (from Jan.1995 to Sep.2013)were searched for relevant articles.All published randomized controlled trails (RCT) on dietary and physical activity interventions during pregnancy were considered.The quality of RCTs was appraised and data were extracted by 3 reviewers independently.Meta-analysis were conducted by RevMan 5.2 software.Results In total,eleven RCTs were included in this review.Meta-analysis revealed that dietary and physical activity intervention significantly reduced incidence of cesarean section and macrosomia;However,there were no statistically significant differences in the total gestational weight gain.Conclusions Interventions based on dietary and physical activity during pregnancy appears effective in reducing incidence of cesarean section and macrosomia.However,there are many limitation of the Meta analysis,therefore more multi-center,large sample with high quality RCTs are required for future verification.
2.Postoperative observation and nursing of double intervention therapy in patients with hepatocellular carcinoma accompanying hypersplenism

Objective To explore the postoperative observation and nursing of double intervention therapy in patients with hepatocellular carcinoma accompanying hypersplensim.Methods The nursing of 30 patients with hepatocellular carcinoma accompanying hypersplensim who received hepatic arterial embolization combined with partial splenic embolization was retrospectively analyzed.Adverse events and nursing experience were summarized.Results All patients had varying degree of fever,abdominal pain,nausea,vomiting and other side effects.The tumor size was reduced and white blood cells and platelets increased to varying degrees.Hypersplenism was relieved after treatment,and serious complications didn't occur.All 30 cases were discharged.Conclusions In double interventional therapy for hepatocellular carcinoma accompanying hypersplenism,nurses must closely observe the disease and take measures to treat the postoperative side effects and serious complications,and promote the rehabilitation of patients.
3.Serum alanine aminotransferase level in patients with type 2 diabetes mellitus and cardiovascular risk assessment

Objective To study the association of the serum alanine aminotransferase (ALT) level with cardiovascular risks in patients with type 2 diabetes mellitus (T2DM).Methods A total of 676 patients with T2DM who excluded liver disease due to chronic viral hepatitis,autoimmune,drugs,alcohol,and other hereditary were selected.According to the ALT level,676 patients were assigned into elevated ALT group (ALT >40 U/L,126 cases) and normal ALT group (ALT 5-40 U/L,550 cases).And normal ALT group were divided into 4 groups on the interquartile distance:group 1 (ALT 5-11 U/L,108 cases),group 2 (ALT 12-15 U/L,114 cases),group 3 (ALT 16-21 U/L,127 cases),group 4 (ALT 22-40 U/L,201cases).The age,duration of diabetes,body mass index (BMI),waist-to-hip ratio (WHR),blood pressure,triglyceride (TG),total cholesterol (TC),high density lipoprotein cholesferol (HDL-C),low density lipoprotein cholesferol (LDL-C),fasting plasma glucose (FPG),2 h postprandial blood glucose (2 hPG),glycosylated hemoglobin (HbA1c),uric acid (UA),fasting insulin and homeostasis model assessment insulin resistance index (HOMA-IR) was compared among these groups.Results The levels of BMI,WHR,diastolic blood pressure,FPG,2 hPG,HbA1c,fasting insulin,TG,HOMA-IR and UA in elevated ALT group were higher than those in normal ALT group [(26.80 ± 3.60) kg/m2 vs.(23.40 ± 3.50) kg/m2,0.95 ±0.05 vs.0.91 ± 0.05,(75.3 ± 9.3) mm Hg (1 mm Hg =0.133 kPa) vs.(69.2 ± 9.5)mm Hg,(9.24 ± 2.56)mmol/L vs.(8.53 ± 2.78) mmol/L,(15.25 ± 4.62) mmol/L vs.(14.37 ± 5.06) mmoUL,(8.31 ± 1.76)% vs.(7.78 ± 1.94)%,(10.76 ± 6.78) mU/L vs.(7.85 ± 7.04) mU/L,(2.73 ± 2.40) mmol/L vs.(2.20 ± 2.78)mmol/L,4.13 ± 2.76 vs.3.06 ± 2.63,(332.42 ± 95.12) μ mol/L vs.(285.14 ± 79.53) μ mol/L] (P < 0.05),and HDL-C was lower than that in normal ALT group [(1.06 ± 0.27) mmol/L vs.(1.15 ± 0.57) mmol/L] (P <0.05).Compared with that in normal ALT group,the age was younger,and the duration of diabetes was shorter in elevated ALT group [(49.6 ± 10.5) years vs.(57.5 ± 11.3) years,(37.9 ±42.0) months vs.(54.5 ± 57.8) months] (P < 0.05).The levels of FPG,2 hPG,HbA1c and UA in group 4 were higher than those in group 1,2,3 [(9.18 ± 3.01) mmol/L vs.(8.33 ± 2.57),(8.38 ± 2.52),(8.45 ± 2.73) mmol/L;(14.56 ± 4.80) mmol/L vs.(13.15 ± 5.72),(13.42 ± 4.56),(13.50 ± 5.17) mmol/L; (7.81 ± 1.98)% vs.(6.76 ± 1.84)%,(6.79 ± 2.10)%,(6.88 ± 2.43)%; (288.24 ± 78.26) μ mol/L vs.(271.15 ± 75.43),(273.27 ± 71.25),(275.56 ± 69.57) μ mol/L] (P < 0.05) ; HOMA-IR was lower than that in group 1,2,3(2.67 ± 2.65 vs.2.84 ± 2.53,2.98 ± 3.10,3.12 ± 2.57,P < 0.05).Conclusion TheincreaseofALTlevel is associated with the clustered cardiovascular risk factors and insulin resistance in patients with T2DM.
4.Study on the relationship between polymorphism site rs13146124 of IRF2 and systemic lupus erythematosus patients﹡

Objective To investigate the correlation between single nucleotide polymorphism rs13146124 of interferon regu‐latory factor2(IRF2) on type Ⅰ interferon pathway and systemic lupus erythematosus(SLE) in a population from Guizhou Prov‐ince .Methods The polymorphism IRF2(rs13146124) was detected by using Taqman‐PCR in 366 cases of patients with SLE and 218 healthy controls .The genotype and allele frequencies were calculated and analyzed .Results The genotype frequencies of AA , AG and GG in IRF2 rs13142164 site in patients with SLE were 0 .011 ,0 .246 and 0 .743 respectively ,compared with the control group ,the difference was not statistically significant (χ2 =0 .093 ,0 .205 ,0 .136 ;P=0 .761 ,0 .651 ,0 .712) .The allele frequencies of A and G in IRF2 rs13146124 site in SLE patients were 0 .13 ,0 .87 respectively ,compared with the control group ,the difference was not statistically significant(χ2 =0 .071 ,P=0 .790) .There was no significant difference between the allele frequencies of A and G in IRF2 rs13146124 site in SLE and ANA ,dsDNA and other specific antibodies .There was no correlation between the allele frequen‐cies of A and G in IRF2 rs13146124 site in SLE and clinical features such as arthritis ,kidney damage ,etc .Conclusion The poly‐morphism of rs13146124 in IRF2 may not be associated with SLE in the population from Guizhou Province .
5.Clinical Application Investigation of Dezocine Injection in 12847 Inpatients in a Hospital

Objective:To assess the clinical use of dezocine injection. Methods:The application of dezocine injection in the in-patients during December 2015 and November 2016 in a hospital was statistically analyzed and evaluated from indications, dosage, treatment course and combined drug use, etc. Results:A total of 12847 patients with the age range of 0-97 and the average age of (49 ± 15. 6) years old were treated with dezocine injection. The top three departments using dezocine injection were orthopaedics (12. 70%), hand surgery (10. 30%) and liver surgery (9. 39%). Totally 132 patients were with overdose(1. 03%), and mainly in cardiac surgery. The medication course of 1042 patients was more than one week(8. 11%), which was too long, while most of the pa-tients were with tumor. Conclusion:The clinical use of dezocine injection in the hospital is basically reasonable. However, clinicians still need more training to minimize the risks involved in the process of clinical medicine application.
6.Effects of thyroid hormone levels on carotid artery lesions in MHD patients and its evaluation value

Objective To investigate the relationship between thyroid hormone levels and carotid artery disease in maintenance hemodialysis ( MHD ) patients and evaluate its value. Methods Clinical data of one hundred and twenty?one MHD patients during January 2014 to July 2016 in Yanan University Affiliated Hospital were collected. The patients were divided into the normal carotid artery group and the carotid artery lesion group according to whether there was plaque or stenosis in the carotid artery ultrasonography. Clinical data related to the carotid artery lesions was collected, and the single factors that may affect the carotid artery lesions were analyzed,multivariate logistic regression was used to analyze independent risk factors for carotid artery disease, and the greatest independent risk factor of OR value was used to establish a ROC curve for evaluating carotid atherosclerosis in patients with MHD. Results The levels of serum calcium, TC, TG, LDL?C, TSH, CRP and Hcy in the carotid artery group were higher than those in the normal group ( P<0. 05 ) . The levels of serum phosphorus and FT3 in the lesion group were lower than those in the normal group ( P<0. 05 ) . Multivariate logistic regression analysis of risk factors for carotid atherosclerosis showed that CRP ( OR=1. 620,95%CI1. 288-2. 038,P=0. 048),Hcy (OR=1. 976,95%CI 1. 135?3. 441,P=0. 021),FT3 (OR=2. 521,95%CI 1. 584-4. 012,P=0. 017) were independent risk factors for carotid artery disease. FT3 evaluation of the ROC curve showed AUC=0. 848,P=0. 000,the cut?off value was 3. 705pmol/L,the diagnostic sensitivity was 0. 670,and the specificity was 0. 879. CRP valuation of the ROC curve suggested that AUC=0. 619,P=0. 000,the cut?off value was 16. 215 mg/L,the diagnostic sensitivity was 0. 932 and the specificity was 0. 333. Hcy evaluation of the ROC curve showed AUC=0. 767,P=0. 000,the cut?off value was 12. 565μmol/L,the diagnostic sensitivity was 0. 784 and the specificity was 0. 881. Conclusion Low concentration of FT3,high concentration of Hcy and high concentration of CRP in MHD patients were independent risk factors for carotid artery disease. The use of FT3 to evaluation the carotid atherosclerosis has a certain value.
7.Effect of uremic serum on endothelial cell apoptosis and intervention of recombinant human erythropoietin in the process

OBJECTIVE:To develop a capillary electrophoresis method for determing the enantiomer of ceftriaxone Na.METHODS:A chiral resolving agent,?-cyclodextrin,was employed as chiral additive for ceftriaxone Na enantiomeric separation by capillary electrophoresis.In different electrophoresis polarity mode,the effect of pH of background electrolyte and the concentration of ?-cyclodextrin were investigated.RESULTS:The optimal conditions for enantiomeric separation were as follows:separation voltage:28kV,buffer solution:NaH2 PO4 50mmol/L,?-CD 0.04mmol/L,Tirs 3.0mmol/L,pH7.15.CONC_LUSION:The method is simple,sensitive,rapid and accurate,and can be used for the quality control of ceftriaxone Na enantiomers.There was significant difference in contents of enantiomers of ceftriaxone Na between products of two factories.We suggest that a quality control method for ceftriaxone Na enantiomer should be established,which will provide scientific basis for quality control of drug and clinical choice of effective antibioties.
9.The antidepressant and neuroprotective effect mediated by TREK-1 potassium channel

Objective Recently, the role of TREK-1 in anti-depression and neuroprotection was demonstrated definitely.In this paper,a systematic review about the functional structure of TREK-1 as well as the underlying mechanisms which involved in depression resistance and nerve protection was made for purpose of providing new ideas for the treatment of clinical diseases.Methods The electronic search of Medline/PubMed,CNKI,CBM and Wanfang Data were performed to collect the introduction of TREK-1 and the studies of depression and nerve protection in which TREK-1 participated from inception of each database to October 2015 ,besides the relevant references of the included studies were also retrieved.Results 30 studies were included in this paper.14 studies were about the interpretation of TREK-1's structure, distribution and the regulatory factors, and the number of researches regarding the involvement of TREK-1 in anti-depression and neuroprotection was 13 and 7 respectively.Based on these studies,it was easily to conclude that inhibition or blocking of TREK-1 channels can produce obvious antidepressant effect, however, activation of TREK-1 channels will induce the dilation of local cerebral vascular and silence hyperactive neurons.Conclusion Some achievements about the antidepressant and neuroprotective effect of TREK-1 channels in vivo have been made gratifyingly,thus future research should focus on exploring the best TREK-lactivation ratio in different brain regions to gain the best balance between anti-depression and neuroprotection.

Objective To investigate the current status of transitional care for the elderly with chronic disease in community,and analyze its influencing factors. Methods A total of 845 elderly patients with chronic disease in community of Ningbo were investigated with the general- case
questionnaire, activities of daily living questionnaire, a series of transitional care questionnaires. Results A total of 756 elderly need transitional care in community. The influencing factors of transitional care were age, marriage,education, chronic disease type, times of hospitalization, living situation, health care category, economic status, family doctor signing. family doctor signing, highly educated, high income were the protective factors. Conclusions The need of transitional care for the elderly with chronic disease in community was high. It is necessary to seek strong support from the government, develop transitional caring platform for elderly in community, to provide diversified services according to the different needs of elderly patients.
