Chinese Journal of Anesthesiology 1994;0(04):-
To compare the clinical effect and side-effect of intravenous PCA(PCIA) with epidural PCA (PCEA). Method: One hundred and eighty postoperative patients, who were randomly divided into three groups: group PCIA, group PCEA and control, were observed for 3 days after operation. Result: Overall patients in two PCA groups were satisfied with the postoperative analgesia. The incidence of urinary retention in group PCIA were significantly lower than that in group PCEA (P0.05). 10% in group PCIA were in medium sedation whereas no eases in group PCEA were in sedation. At 6th hour after operation, serum cortisol level of control patients was much higher than that of PCA patients. Conclusion=Both PCIA and PCEA have excellent analgesia and reduce stress response after operation. PClA has lower incidence of urinary retention and is performed easily, but inhibits gastrointestinal motility much more and has higher sedative incidence compared with PCEA.

Medical Journal of Chinese People's Liberation Army 1982;0(01):-
To assess the clinical effect of HA regimen on newly diagnosed patients in chronic phase of chronic myeloid leukemia (CML). Ninety four cases of CML patients were grouped in accordance with the requirements on the basis of treatment regimens and Sokal risk index, and the clinical effects of HA regimen wer evaluated. We found that HA regimen showed satisfactory immediate effect on CML in chronic phase. CR rates achieved in high risk and medium risk groups(77 4%,20 0%) with HA were higher than that with Hu regimen(35 0%,0%). However, HA regimen was incapable of extending the duration of CML. So HA should not be used as first line treatment in the treatment on newly diagnosed patients in chronic phase of CML, except for high risk group whose symptoms could not be controlled with other regimens.

Aim To explore the protective effect of ac-tive components of Xiaoxuming decoction ( XXMD ) on focal cerebral ischemia/reperfusion injury in rats dur-ing early recovery period .Methods The ischemia and reperfusion of middle cerebral artery model rats were established by nylon wire with 2 hours ischemic time on healthy male SD rats .The models of MCAO were eval-uated by Zea-Longa′s standard score .The model rats were randomly divided into sham operation group , the ischemia/reperfusion model group , the active compo-nents group of Xiaoxuming decoction and the positive group (extract of ginkgo biloba leaves EGb 761).Rats were orally administrated with different drugs 24 h after operation for up to 14 days, once a day.The effect of active components of Xiaoxuming decoction on behavior changes of MCAO rats in different recovery period was evaluated by a series of behavioral assessent methods such as modified neurological severity scores and cor-ner test. The infarct volume was observed by TTC stai-ning.Moreover, the contents of MDA and the activities of GSH-Px, SOD and NOS in the penumbra and core tissues of rat brain were detected by spectrophotometric method. Results Compared with the I/R model group, the active components group of XXMD could significantly alleviate the neurological deficit scores with prolonged administration . The motion function tended to be normal , stayed longer on the balance beam, sensory function gradually restored sensitivity , reduced the radio of turning to the right .Among them , compared with model group , the active components of XXMD could effectively improve the neurological dys-function after five days of administration ( P<0.05, P<0.01).Meanwhile, the active components of Xiaox-uming decoction could significantly reduce the infarct volume percentage in the cerebral tissue on post opera-tion day 5 and 14(P<0.01).In addition, the active components of XXMD could reduce the content of MDA and the activity of NOS , increase the activity of SOD and GSH-Px.Conclusion The active components of XXMD can generate neuroprotective effect in early stage of recovery and may play a major role in regula-ting the level of oxidative stress in rat brain .

Objective To investigate the protective effect of melatonin and its possible mechanism for repairing in the immature white matter damage due to brain hypoxia-ischemia (HI).Methods Forty-eight three-day SD rats after birth were randomly divided into 3 groups:sham-operated(SHAM) group,HI group and melatonin treatment(MT) group.Periventricular white matter damage (PWMD) to animal models were estabished according to Rice modeling.MT group was treated with melatonin pre-operatively,immediately postoperation,1 hour postoperation and 24 hours postoperation via intraperitoneal injection, and the other groups were injected with the same volume of dissolvent. The rats were executed by decollation after 2 days and 14 days. The histological changes in periventricular white matter were observed by HE staining and immunohistochemistry. Results For the 3 groups, the structure in ope-ration side of the white matter in the peripheral ventricles of the brain 2 days postoperation were significant different (P <0.05).The O4 positive cells decreased one by one/greatest in the SHAM group[(75.548 ± 7.333)/hpf] followed by MT group [(59.971 ± 3.635)/hpf],and HI group [(40.511 ± 2.848)/hpf] (P < 0.05).The expression of Casepase-3 increased in the SHAM group (107.724 ± 10.266),MT group (132.289 ± 8.537),and HI group (202.168 ± 14.367),and the difference was statically significant (P < 0.05).Ventricular index was greater in operation side of the white matter in the peripheral ventricles of the 14-day-brain in the SHAM group(0.928 ±0.063),MT group (1.813 ± 0.110),HI group (2.752 ± 0.201),increasingly,while absorbance value of myelin basic protein decreased one by one in sequence(39.504 ± 1.673,21.729 ± 1.614,11.344 ± 1.118).Conclusions MT plays a role in protecting the periventricular white matter via inhibiting the apoptosis of oligodendrocyte progenitor cell, and thus benefits the PWMD.

Objective To investigate the effect of pretreatment with parecoxib for postoperative analgesia in patients undergoing laparoscopic hernia repair. Methods Sixty patients undergoing laparoscopic hernia repair were assigned in two groups by random digits table with 30 cases each. The patients in pretreating group received parecoxib 40 mg intravenously before anesthesia, and in control group with the same capacity of normal saline. All patients were anesthetized with combined spinal epidural blockage and tranquilized with pethidine, droperidol and diazepam. Postoperative pain of the abdomen incision was assessed by visual analogue scale (VAS) and the pain of shoulder or back was judged by a 4 grade scale. Side-effects, supplement analgesic, passage of gas by anus 24 h after surgery were observed as well. Results The abdomen incision VAS at 4,8,12,24 h after surgery in pretreating group were lower than those in control group (P < 0.05). And the incidence of pain of shoulder or back was lower in pretreating group (13.3% ,4/30) than that in control group (33.3% ,10/30)(P <0.05). While the cases needing supplement analgesic also decreased in pretreating group (13.3% ,4/30) compared with control group (40.0%, 12/30)(P < 0.05). And the side effects and passage of gas by anus 24 h after surgery had no significant difference in two groups. Conclusion Pretreatment with parecoxib in patients undergoing laparoscopic hernia repair may relieve the postoperative incision pain and the pain of shoulder or back while reducing the analgesic supplement without side effects.
