1.Research on ECG Telemonitoring System Based on Virtual Instrument

Objective To develop an ECG telemonitoring system which can meet many needs of cardiopaths with enjoying medical care out of hospitals.Methods The signals of collection and pretreatment in ECG105 that were transmitted to the computer through serial port,the computer's powerful functions in analyzing and processing signal were utilized fully to design ECG telemonitoring system based on virtual instrument by LabVIEW.Results The system can redisplay and analyze the stored signal and judge whether the heart rate is abnormal,and implement ECG data remote transmission.Conclusion The display,storage,redisplay,diagnosis and tekecommunications of ECG can be achieved conveniently and provide convenience for many cardiopaths with its powerful functions and low cost.
2.Investigation and analysis of medical and nursing interns' differences and similarities of role expectation on clinical nurses

Objective To investigate medical and nursing interns' differences and similarities of role expectation on clinical nurses,and provide the basis for further carrying out physician-nurse cooperative education.Methods Using self-designed questionnaires of role expectation on clinical nurses to investigate medical and nursing interns of two degrees in Zhengzhou University who already had full ten months' internship,among them,154 medical students,145 nursing students.Results Medical and nursing students' differences were mainly on “physician-nurse collaboration and communication.Nursing students were more active than medical students.Both medical and nursing students had the lowest scores in item 15 (reverse entry)“The nurses just need to perform doctors' orders,without providing comments or suggestions”.Conclusions Colleges and clinical teachers should encourage nursing students to learn more medical knowledge,strengthen their clinical practice capacity,improve their professional competence and professionalism,thereby enhancing the status of nursing and nurses.Launching physician-nurse collaboration education in nursing and medical students,and help medical and nursing students realize nurses' different roles,improve the trust between doctors and nurses,promote the development of physician-nurse relationship.
3.Hyperhomocysteinemia and cardiovascular diseases

Many data show that hyperhomocysteinemia is correlated with the increased rate of cardiovascular diseases.Yet,there is controversy about the relationship between mild hyperhomocysteinemia and cardiovascular diseases.Lots of study suggest that the concentration of plasma homocysteine was reduced by using folic acid in patients with cardiovascular diseases.But,the effect of folic aicd can not be identified from other factors reducing the concentration of plasma homocysteine.
4.Imaging characteristics of spontaneous intracranial hypotension and the effect of epidural infusion treatment (report of a case)

Objective Study the imaging characteristics of spontaneous intracranial hypotension(SIH) and the effect of epidural infusion treatment.Methods Clinical data of one patient with SIH was retrospective analysis.Results The patient presented with symptoms of postural headache,nausea,vomiting;pressure of cerebrospinal fluid(CSF) couldn't be measured.Cranial MRI found typical subdural fluid collections,diffuse thickening of the pachymeninges with enhancement,engorgement of venous sinuses,enlargement of the pituitary gland,and downward displacement of the brain.High-resolution CT myelography precisely located the site of CSF leak.Epidural infusion of normal saline relieved immediately the symptom of intracranial hypotension.Conclusions The imaging characteristics of SIH includes engorgement of venous sinuses,diffuse thickening of the pachymeninges with Gd enhancement,subdural fluid collections.The treatment with epidural infusion for SIH is very effectual.
5.Ethical Analysis of All-Free Health Care in Shenmu County

The "all-free health care" system is being implemented in Shenmu County in Shaanxi Province,whose ethical orientation is during the transitional process from "basic medical insurance" to "meeting residents′ actual healthcare demand".The "all-free health care" system safeguards human health rights,embodies the principle of giving priority to social benefits,and is in accordance with the principle of effectiveness.However,due to the lack of scientific management,the capital flows violate the principle of prevention in the system.
6.Bibliometric Analysis of Hospital Archive Information in China

This paper analyzes the status quo of the information of hospital archives in China from the perspective of the publication year, the area where the first author is located, the distribution of the journals and the content of the literature, and so on through reviewing the related literatures in CNKI.It was concluded that great progress has been made in the depth and breadth of the research on the information of hospital archives in our country.However, it is necessary to promote the digitization of archives resources, electronization of increment and internetization of the archive use, innovate management model of archive information, speed up the integration with the information society under the guidance of the 13th Five-Year Plan of National Archive Development, to promote the level of the modern archive management with the information as the core.
7.Diagnosis value of thyroid imaging reporting and data system in thyroid nodules.

OBJECTIVE:
To explore the diagnostic value of the proposed thyroid imaging reporting and data system (TI-RADS) classification in thyroid nodules, and to investigate interobserver variability among different observers using TI-RADS classification for ultrasound.
METHOD:
We reviewed 667 thyroid nodules conventional ultrasound image data from 532 patients who were confirmed by Pathological diagnosis. Those ultrasound images were reviewed by 4 experienced sonographers who can independently assessed the sonographic characteristics and analyzed according to the TI-RADS classification. It is to assess the diagnostic sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of the TI-RADS classification. The multirater kappa statistics were used to assess the interobserver agreement among different observers.
RESULT:
The overall sensitivity, specificity, negative predictive value (NPV) and Accuracy were 94%, 79%, 97% and 81%, respectively. Positive predictive values (PPVs) for categories 4 and 5 were 55% and 92%. There was substantial interobserver agreement for categories 3, 4B and 5 (Kappa = 0.62, 95% CI, 0.58-0.65), and was moderate agreement for category 4A (Kappa = 0.57) and 4B(Kappa = 0.60).
CONCLUSION
The diagnostic criteria of TI-RADS for differentiating between benign and malignant thyroid nodules have a high diagnostic value. There was substantial interobserver agreement in different experienced sonographers. The TI-RADS diagnostic criteria have a high diagnostic value and has great practical value in making a proper and further treatment plan.

Objective Discuss how to develop nurse arrange in groups mode in basic level hospital, resolve more effectively the problem of nurse delayed on duty to ensure nursing care quality and patients' safety.Methods Divide nurses of different level, different working ability and work experience into several groups to carry on arranges in groups.According to the quantity of patient in ward, the nursing work load, nurse establishment and so on, arrange groups with different number of nurse to attend duty.Results Realized nurse level management, the senior nurse's sense of responsibility strengthened, the work value feeling promoted, nurse delayed on duty rate reduced 89.5%;has realized three satisfactions, namely doctor, nurse, the patient were satisfied.Conclusion Basic level hospital, according with practical work characteristics, develop arranges in groups pattern continuously, and thus has safeguarded the noon and at night patient's safety well, has manifested truly take the patient as the center service idea;reduced times of handover, effectively save time of work, raises the working efficiency, reduces the safe hidden danger;realized nurse level management, enhanced nurse the value feeling.
10.Research of Assistant Diagnosis System for Arrhythmias

Objective: Develop an assistant diagnosis system for arrhythinias which can reduce doctors' workload and improve their veracity of diagnosing ECG. Methods: Firstly, this paper achieves filter and detection of waves using wavelet transform and extracts parameters; Secondly, it reduces parameters using rough set and identifies arrhythmias according to relevant rules; At last, it gets the membership of abnormal heart beat by fuzzy neural network. Results: The main modules such as filter, detection of waves and identification of arrhythmias are achieved well, and a complete system is formed. Conclusions: This system can identify nineteen arrhythmaias and get their membership and position. It could assist doctors in making the proper diagnosis on ECG potentially.
