1.Expression of pancreatic ATP-sensitive K~+ channels in rats with chronic pancreatitis and drug interventions

Objective To observe the expression of pancreatic K_(ATP) channels (Kir6.2/SUR1) in rats with chronic pancreatitis and explore the intervention of nateglinide on the changes.Methods Wistar rats were induced to suffer from chronic pancreatitis and then randomized into model group, nateglinide group and control group.Then OGTT of them were observed.RT-PCR was used to detect the expression of Kir6.2 and SUR1 mRNA, and western blot to detect the expression of Kir6.2 and SUR1 proteins.Results Model rats displayed impaired glucose tolerance (IGT).The expression of Kir6.2 and SUR1 in model group decreased significantly(P<0.05), and nateglinide displayed up-reg-ulation to the expression in some degree.Conclusion The expression of pancreatic K_(ATP) channels in rats with chronic pancreatitis diminished, which might be the important mechanism of the development of pancreatogenic diabetes.Nateglinide can up-regulate the expression in some degree, which indicates that it may have latent effect of ameliorating the prognosis of patients with chronic pancreatitis.
2.Investigation of geriatric syndromes in very elderly people in Beijing community

Objective To investigate the geriatric syndromes in the very elderly people in Beijing community,in order to provide the basis for management of geriatric syndromes and rational use of medical resources.Methods All community-dwelling residents aged 95 years and over living in Haidian district and some residents aged 85 years and over living in Haidian,Chaoyang and Xicheng districts were investigated in household in Beijing.The survey included the general condition,functional status and geriatric syndromes such as falls,senile urinary incontinence,chronic pain syndrome,chronic constipation and malnutrition occurrence.Results 336 seniors with the mean age of (96.0± 3.0) years were investigated.The percentage of people who had falls within 1 year,chronic pain,and chronic constipation was 27.2% (88/324),37.3% (122/327) and 47.6% (160/336) respectively.The incidence of urinary incontinence was 37.1% (118/318),with 33.3 % (46/138) in males and 40.0% (72/180) in females,and there was no significant gender difference in the incidence of urinary incontinence (P=0.220).The incidences of malnutrition and malnutrition risks was 10.4% (31/299) and 50.5% (151/299).37.2% of subjects could walk outdoor with a better physical function.The self-rated health satisfaction was lower in elderly people with malnutrition and urinary incontinence than in subjects without those diseases (40.0% vs.70.4%,34.0% vs.64.6%,x2 =25.20,25.30,both P<0.001).26% of them had never heard of geriatrics.Conclusions Geriatric syndromes are common in community-dwelling older seniors.It is urgent to spread the geriatric knowledge and set a reasonable goal for diagnosis and treatment in the seniors.
3.Recombinant adenovirus-mediated hypoxia inducible factor 1alpha mutant promotes proliferation of adipose-derived mesenchymal stem cells

BACKGROUND:To improve the survival rate of transplanted tissue, most scholars focus on cel therapy, particularly cel-assisted fat grafting.
OBJECTIVE:To analyze the effect of recombinant adenovirus-mediated hypoxia inducible factor 1alpha (HIF1α) mutant on survival rate of transplanted fat particles through transfection of adipose-derived mesenchymal stem cels.
METHODS:Recombinant adenovirus-mediatedtriple-mutantHIF1αwas inserted into an adenovirus pAdEasy-1 system, folowed by viral packaging and titer determination. Human adipose-derived mesenchymal stem cels were cultured, passaged and identified, and subsequently transfected with three kinds of viruses and blank vector (experimental group with transfection of the triple mutantof HIF1α;positive control group; negative control group; blank control group). Transfection efficiency was determined using enhanced green fluorescent protein labeling. Additionaly, MTT assay was used to detect cel proliferation.
RESULTS AND CONCLUSION:The recombinant adenovirus was successfuly constructed and packaged in line with transfection requirements. Moreover, adipose-derived mesenchymal stem cels were successfuly identified by adipogenic, osteogenic and chondrogenic induction and could be used asseed cels for subsequent experiments. RT-PCR results showed that HIF1α mRNA expression in the experimental group and positive control group was significantly higher than that in the other two groups (P< 0.05). Western blot analysis showed that the relative absorbance value in the experimental group was significantly higher than that in the other three groups (P< 0.05). A significant increase in the cel proliferation was found in the experimental group, significantly different from the other three groups (P<0.05). Therefore, our findings indicate that transfection of adenovirus-mediated triple-mutant HIF1α not only can sustain the expression of target protein in transfected adipose-derived mesenchymal stem cels under normoxic conditions, but also can promote the proliferation of transfected adipose-derived mesenchymal stem cels.
4.Role of AGXT2 in ADMA metabolism and the development of cardiovascular and cerebrovascular diseases

Cardiovascular and cerebrovascular diseases have be-come a great health threat.Increasing evidence has shown that both asymmetric dimethylarginine (ADMA)and symmetric dim-ethylarginine (SDMA)play important and independent roles in the development of cardiovascular and cerebrovascular diseases as well as in the prediction of cardiovascular and cerebrovascular events.Alanine-glyoxylate aminotransferase 2 (AGXT2 )is re-cently observed to be involved in ADMA metabolism.Deficiency in the expression and activity of AGXT2 may thus play a role in the development of cardiovascular and cerebrovascular diseases by affecting ADMA levels in vivo.Several single-nucleotide poly-morphisms at AGXT2 locus are observed to be associated with plasma SDMA level.This review summarizes recent advances in AGXT2 and its role in ADMA metabolism and the clinical rele-vance.
5.Effect of lidocaine combined with ropivacaine in penile dorsal nerve block for pediatric circumcision

Objective To explore the effect of lidocaine combined with ropivacaine in penile dorsal nerve block for pediatric circumcision.Methods 130 children with pediatric circumcision surgery were divided into observation group and control group by the digital meter method,65 cases in each group.The control group was taken inhaled sevoflurane composite static note ketamine surgery,the observation group was given ropivacaine combined with intravenous infusion of ketamine for penile dorsal nerve block anesthesia.The anesthesia effects of the two groups were recorded.Results In the observation group,10 min after surgery,the rhythm of the heart was (98.47±10.88)times/min,central arterial pressure was (65.13±7.78)mmHg.In the control group,10min after surgery,the rhythm of the heart was (121.78±15.23)times/min,central arterial pressure was (85.27±12.31)mmHg,the differences between the two groups were statistically significant(t=10.040,11.150,all P<0.05).In the observation group,the used ketamine,waking up time,pain score,incidence rate of pain and irritability were (41.38±11.28)mg,(5.54 ±1.03)min,(2.11±1.13)points,7.69%,respectively,which in the control group were (71.47±16.82)mg,(13.67±2.57)min,(3.89±1.79)points,26.15%,respectively,the differences between the two groups were statistically significant(t=11.978,23.673,6.779,χ2=7.878,all P<0.05).Conclusion The anesthesia effect of lidocaine combined with ropivacaine in penile dorsal nerve block for pediatric circumcision is stable,safe and reliable,it is worthy of popularization and application in clinic.
6.Long-term Outcomes of cN0 Papillary Thyroid Carcinoma without Elective Central Compartment Neck Dissection.

Objective To determine the long-term outcomes of cN0 papillary thyroid carcinoma without elective central compartment neck dissection. Methods The clinical data of 180 patients with clinically lymph node negative papillary thyroid carcinoma who were treated in our center between 2000 and 2005 were retrospectively analyzed. All of these patients did not receive elective central compartment neck dissection. Clinicopathological characteristics including gender,age,surgical range,pathologic type,tumor size,and extrathyroidal extension(ETE)or not were collected. Results After a median follow-up period of 90 months,only one patient died of stroke without tumor. Sixteen patients had tumor recurrence:seven patients had a recurrent disease in residual thyroid tissue,two in the thyroid bed,six in central compartment,eight in lateral cervical compartment,and one in lung. The 10-year overall survival,disease-specific survival,and recurrence-free survival was 99.4%,100%,and 87.9%,respectively. The 10-year accumulative lymph node recurrence rate in central compartment and lateral compartment was 7.8% and 7.0%,respectively. ETE was an independent risk factor for central compartment lymph node recurrence. Male gender(P=0.010)and ETE(P=0.028)were independent risk factors for lateral compartment lymph node recurrence. Conclusions The prognosis of patients with cN0 papillary thyroid carcinoma without elective central compartment neck dissection is good after ten years of follow-up. Male gender and ETE are independent risk factors for lateral compartment lymph node recurrence.
7.Analysis on sensitivity of contrast-enhanced ultrasound and ultrasonic elastography in detecting different sizes of thyroid papillary carcinoma

Objective To explore the sensitivity of contrast-enhanced ultrasound (CEUS) and ultrasonic elastography(UE) in detecting different sizes of thyroid papillary carcinoma.Methods 64 cases of thyroid papillary carcinoma were divided into two groups according to the diameter size(group 1,≤1 cm;group 2,> 1 cm) and examined by the contrast-enhanced ultrasound and elastography,then the features were analyzed and the sensitivity for detecting of the two methods in different groups were compared.Results The sensitivity for detecting in CEUS was higher than in UE,which had significant difference between two methods.CEUS and UE were valuable for detecting and they had no difference in sensitivity for detecting in group 1.There was a significant difference in sensitivity for detecting between two methods in group 2.Conclusions Both CEUS and UE were valuable in diagnosis of thyroid papillary carcinoma and the sensitivity for detecting in CEUS is higher than in UE in group 2.

This article introduces the definition and sample size estimation of three special tests (namely, non-inferiority test, equivalence test and superiority test) for qualitative data with the design of one factor with two levels having a binary response variable. Non-inferiority test refers to the research design of which the objective is to verify that the efficacy of the experimental drug is not clinically inferior to that of the positive control drug. Equivalence test refers to the research design of which the objective is to verify that the experimental drug and the control drug have clinically equivalent efficacy. Superiority test refers to the research design of which the objective is to verify that the efficacy of the experimental drug is clinically superior to that of the control drug. By specific examples, this article introduces formulas of sample size estimation for the three special tests, and their SAS realization in detail.

This article introduces methods of sample size estimation of difference test for data with the design of one factor with two levels, including sample size estimation formulas and realization based on the formulas and the POWER procedure of SAS software for quantitative data and qualitative data with the design of one factor with two levels. In addition, this article presents examples for analysis, which will play a leading role for researchers to implement the repetition principle during the research design phase.
