1.Investigation on the risk factors of cesarean section maternal with hospital infection and preventive measures

Objective To observe the occurrence of nosocomial infection in maternal hospital cesarean deliv-ery situation,analyze risk factors of hospital infection,and to explore the corresponding prevention measures.Methods The clinical data of 943 lying -in women with cesarean section were retrospectively analyzed.The conditions of hospi-tal infection occurred were observed,the risk factors for nosocomial infection were analyzed.Results 73 cases occurred maternal hospital infection,the infection rate was 7.74%.The infection occurred at surgical site in 41 cases, 11 cases occurred in urogenital system,6 cases occurred in the respiratory system,10 cases occurred respectively in the digestive system,skin and nervous system.More than 2 times of cesarean delivery operation,too large age,with basic diseases,low medical staff operation proficiency,not using antibiotics,too long time of hospital stay,too long time of placing a urinary catheter were independent risk factors of hospital infection after cesarean section(OR =0.223, 0.670,1.850,6.062,0.302,2.190,3.583,all P <0.05).Conclusion The incidence rate of hospital cesarean sec-tion maternal hospital infection are rising year by year,so the medical personnel should aim at the risk of infection fac-tors,take corresponding prevention measures,so as to effectively reduce the incidence of infection.
2.Effect of dinoprostone suppositories and oxytocin in promoting cervical ripening and childbirth outcome

Objective To explore the effect of dinoprostone suppositories and oxytocin on cervical maturation and delivery outcome in induction of labor in late pregnancy.Methods A total of 174 pregnant women in the department of obstetrics and gynecology in our hospital for treatment in late pregnancy were chosen.They were randomly divided into two groups: the control group 85 cases and the study group 89 cases.The control group was treated with routine induction of intravenous infusion of oxytocin, and the study group was taken with dinoprostone suppositories.The effective rates of induction of labor and the incidence of adverse reactions after induction of labor induced of the two groups were observed.The clinical effect of patients was with Bishop score to determine cervical maturity.The induction of labor time, natural childbirth rate, cesarean section rate, maternal postpartum hemorrhage and neonatal asphyxia score were as the evaluation criteria of childbirth outcome.Results The cervical maturation level was (7.26 ±1.82) after drug for 24 hours in the study group, significantly higher than that of the control group (6.34 ±1.37) (P<0.05).The induction time, spontaneous delivery rate and cesarean section rate of the study group were (6.34 ±1.37)h, 86.52%, 13.48%, significantly better than those in the control group (P<0.05).The average amount of bleeding in the study group was (225.96 ±12.81) mL and the neonatal asphyxia score was (9.73 ±0.15), the results were lower than the control group (P<0.05).The successful rate of induction of labor in the study group was 96.63%) higher than that in the control group 85.88%.The incidence of side effects in the study group was 10.11%, significantly lower than in the control group 21.18% (P<0.05), which were with statistical significance.Conclusion Dinoprostosterone suppositories have the advantage of promoting cervical ripening for late pregnancy in pregnant women compared with oxytocin.It can shorten induction time and improve natural childbirth rate.And the maternal prognosis is good, can be used as a safe and effective clinical induction.
3.The Study on the Change of RI Value and the Relationship Between HA to Ocular Blood Flow in Posthepatitis Cirrhosis by CDFI

Objective To study the RI value in HA to ocular blood flow in posthepatitis cirrhosis by CDFI.Methods The RI value of HA,OA,PCA,CRA in 32 normal person,102 posthepatitis cirhosis patients were studied by CDFI(U.S.A.Sequoia 512).Results The value of RI between HA to OA,PCA and CRA was 0.88,0.88,0.93 respectively.Conclusion A direct relationship is found between RI value of HA to ocular vascular,especially in HA to CRA.
4.Evaluation of left ventricular diastolic function in patients with Kawasaki disease by pulmonary venous and mitral flow

Conclusions It is available and valuable to combine PVF and MVF to assess left ventricular diastolic function in KD.
5.EFFECT OF L-THP ON CONTRACTION OF ISOLATION GUINEA PIG TRACHEA INDUCED BY SEVERAL AGONISTS

The antagonistic effects of L-THP, Ver and Pap to contraction of isolated guinea pig trachea preparations induced by BaCl2 10-3mol/L ,KC1 3?10-2mol/L, 5-HT 2?10-5mol/L, Ach 2?10-5mol/L and His 2? 10-5mol/L were studied.The antagonistic effects of L-THP were different from Ver and Pap. The IC50 value of L-THP for contraction induced by BaCl2, KC1, 5-HT and His was 3 .5? 10-5mol/L , 4.5 ? 10-5mol/L , 2.5?10-3 mol/L and 8.9? 10-5mol/L respectively. It was showed that sensibility of several agonists to inhibition by L-THP was correlated with dependence of extracellular Ca2+. The potencies of L-THP on anti-KCl and anti-5-HT actions were similar. The results suggested that LTHP possesses anti-calcium and anti-5-HT action on guinea pig trachea.

7.Effect of surgical stress on plasma glucagon-like peptide 1 level after enternal or parenteral intake of glucose in surgical critically ill patients

Objective To observe the metabolism characters of plasma glucagon-like peptide 1 (GLP-1) in surgical critical ill patients.Methods Totally 64 selective surgical patients were prospectively enrolled and randomly divided into two groups:enteral nutrition (EN) group (50% glucose 100 ml orally administered or through nasogastric tube on the next day after operation) and parenteral nutrition (PN) group (50% glucose 100 ml intravenously infused on the next day after operation).Blood glucose and plasma GLP-1 were determined before operation,after operation,and 0 min,30 min,90 min,and 180 min after the 50% glucose was used.Then,the EN group was divided into stress hyperglycemia (SH) subgroup and non-stress hyperglycemia (non-SH) subgroup.In both subgroups,plasma tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) were detected before operation,after operation,and the next day after operation.Blood glucose,plasma insulin,C-peptide,and GLP-1 were measured before operation,after operation,and 0 min,30 min,90 min,and 180 min after 50% glucose was used.Results The plasma GLP-1 level (pmol/L) of EN group increased significantly after 50% glucose was used,and reached the peak at the time of 30 min.In contrast,it did not change in PN group,and were significantly lower than EN group at the time of 30 min [(5.69 ± 2.92) pmol/L vs (33.52 ± 12.32) pmol/L,P <0.01] and 90 min [(5.71 ±2.14) pmol/L vs (15.92 ±6.63) pmol/L,P =0.00].Plasma insulin level in SH subgroup were significantly higher than in non-SH group after operation [(18.17±6.18) mU/Lvs (12.10±3.60) mU/L,P=0.01] and30 min [(56.02±14.78) mU/Lvs (43.39±15.60) mU/L,P=0.03] and 90min [(114.58±25.11) mU/Lvs (90.45±25.91) mU/L,P=0.03] after 50% glucose was used.Plasma C-peptide level in SH subgroup was also significantly higher at the time of 30min [(6.86 ±1.15) μg/L vs (4.76 ± 1.06) μg/L,P=0.04] and 90 min[(12.17 ± 1.96) μg/L vs (9.81 ± 2.35) μg/L,P =0.03] after 50% glucose was used when compared with non-SH subgroup.Plasma GLP-1 level in SH subgroup was significantly lower than in non-SH group at the time of 30 min [28.76 ± 9.61) pmol/L vs (37.36 ± 10.51) pmol/L,P =0.03] and 90 min [(13.49±4.26) pmol/Lvs (17.33±5.57) pmoL/L,P=0.04] after 50% glucose was used.Conclusions Early post-operative PN increases plasma GLP-1 level and reduces the SH incidence.In SH patients,endogenous insulin expression increases while plasma GLP-1 level decreases.Post-surgery inflammatory reaction may result in low plasma GLP-1 level and high blood glucose level.
8.Investigation and analysis on status quo of health statistic personnel in Chongqing municipality

Objective To understand the status quo of the health statistics human resources in Chongqing municipality. Methods The status quo of the health statistic human resources in Chongqing municipality was investigated by the national health statistics reporting system.The age,seniority,qualifications,specialty,job title,distribution,etc.in the health personnel were ana-lyzed by the SPSS17.0 statistical software.Results 74.89% of health statistic personnel were part-time workers.In the vast ma-jority of medical institutions,the number of health statistician was 1 or 2.As to the education background,73.50% of the total stat-istician number only got the junior college degree even below.Only 216 statistic personnesls′major fitted to the staff positions.Ba-nan district in the major city,Tongnan county and Dazu district in the west area,Wushan county in the east area,Dianjiang county and other counties in central area,and the new urban development area were relatively lack of human resources of the health statistic personnel.Conclusion The health statistic team in Chongqing municipality has low stability,inadequate staff and poor professional quality.The distribution of statistic personnel human resources is uneven among the five functional areas and various districts and counties.
9.The computer-aided design for the restoration of unilateral orbital defect

Objective: To develop a computer-aided three-dimensional design software and laser scanning technique for the restoration of unilateral orbital defect. Method: By laser scanning apparatus plaster face model and the acquired primitive face point data were scanned; with the three-dimensional design software the point data were reconstructed to one smooth face surface image and the data of the defect of the orbital tissue were obtained from the normal contralateral tissue data by software. Results: The face-model data obtained by laser scanning apparatus was distinct and precise; the Digisurf retrograde engineer software succeeded to fulfill the computer-aided design for restoration of unilateral orbital defect. Conclusion: The laser scanning technique and three-dimensional design software can be used in the computer-aided design of the unilateral orbital defect restoration.
