1.Clinical application of carbon nano-particle in curative resection of mid-low rectal cancer

Objective To investigate the clinical application and curative effect of Carbon Nano-particles in curative resection of mid-low rectal cancer .Methods 26 patients with mid-low rectal cancer were divided equally into carbon nano-particle group and routine group .The labeling group were injected with carbon nano-particles 20~24 hours before operation and treated with TME laparoscopic radical resection .The data of lymph nodes detected in the two groups were compared .Results (1)Total number of lymph nodes detected in the labeling group was higher than that of the routine group (287 vs .124 ,P<0 .05) .(2)The labeling group had higher average lymph nodes detected per patient (22 .12 ± 3 .50 vs .9 .56 ± 1 .50 ,P< 0 .05) ,as well as higher average lymph nodes smaller than 5mm per patient(9 .18 ± 1 .41 vs .3 .50 ± 0 .82 ,P<0 .05) .(3)The metastasis rate of lymph nodes detected in la-beling group lacked statistical significance as compared with that of the routine group (3 .10 ± 0 .37 vs .2 .62 ± 0 .25 ,P>0 .05) .Con-clusion Adoption of carbon nano-particle in curative resection of rectal cancer can increase the number of lymph nodes detected ,re-veal more accurately the metastasis situation of lymph nodes ,facilitate the resection of lymph nodes and rectal cancer during the op-eration ,and provide guidance for postoperative clinical staging and treatment .

Objective To explore the molecular defects of insulin signalling in polycystic ovary and in vitro effects of troglitazone, one of the insulin sensitizers thiazolidinediones on polycystic ovary syndrome(PCOS) Method The metabolic and mitogenic effects of insulin and insulin like growth factor 1 (IGF 1) were examined in cultured human ovarian luteinizing granulosa cells from PCOS ( n = 11) and normally ovulatory (as control, n = 33) women with vehicle or troglitazone (1 ?g/ml) Results Basal rates were similar, but there were significant decreases in insulin stimulated glucose incorporation into glycogen in PCOS cells, a metabolic action of insulin However, IGF 1 response was found to be about twice greater in PCOS cells at all experimental concentrations with respect to thymidine incorporation compared to control cells, a mitogenic action Troglitazone increased 2 3 fold the insulin induced glycogen synthesis, but reduced the IGF 1 augmented responses of DNA synthesis in PCOS cells to within the range of control granulosa cells As compared with control, PCOS granulosa cells had higher insulin receptor substrate 1 (IRS 1) expression, but lower IRS 2 expression IRS 2 protein levels were increased and IRS 1 levels were reduced by troglitazone treatment, with a greater extent in the former Conclusions There is a selective defect in insulin actions in PCOS granulosa cells, suggesting ovarian insulin resistance and this metabolic phenotype is associated with an enhanced IGF 1 mitogenic potential Troglitazone could divergently alter signal protein expressions and thus insulin actions, as an ovarian insulin sensitizer and mitogen/steroidogenic inhibitor in PCOS

This article was inspired by the paper that Clinical Research of Traditional Chinese Medicine NeedsA Breakthrough, which was written by Shang Hongcai. This article took the 11th Five-year Plan of the National Science and Technology Support Project and the National Public Welfare Projects of Chinese Medicine, which were undertaken by the Heilongjiang Gynecology , which is one of the National Trial Bases of Chinese Medicine as an example to illustrate the Chinese medicine clinical trial management , achievement producing , and realization of achievement transformation . The emphasis should be made on the optimization of Chinese medicine clinical trial from the topic selection , operation mode and quality control . A comprehensive and detailed management system should be worked out to ensure the smooth conduction of clinical trials. The clinical research results should be clarified to achieve high quantity and quality outputs . The article was aimed to provide references for the development and breakthrough of Chinese medicine clinical trials .
5.Research on Traditional Chinese Medicine Syndrome and Treatment Rules of Literature Information Data Based on Polycystic Ovary Syndrome

This study was aimed to mine correlation between the cause and traditional Chinese medicine ( TCM ) syndrome of polycystic ovary syndrome ( PCOS ) from different angles and at different levels . The study found
that the inherent laws of the cause and syndrome by the application of different statistical methods and data mining methods . The literature information database of PCOS was searched . And the Microsoft Office Excel and the Microsoft Office Access were used in the establishment o the database . And the SPSS17 . 0 was used as the statistical software . The Medisc-3S system was used as the mining tools to discover the rules between etiology and syndrome . The function of frequency statistics and complex networks were used to discover and summarize the TCM syndrome and treatment of PCOS . The TCM syndrome and treatment of PCOS were discov-ered and summarized according to the statistical results . And the results showed that three reasons of etiology and pathogenesis of PCOS contained kidney-deficiency , phlegm-dampness , and blood-stasis . The five types of TCM syndrome included spleen-kidney deficiency , kidney-deficiency and blood-stasis , spleen-deficiency and phlegm-dampness , kidney-deficiency and phlegm-dampness , kidney-deficiency and liver stagnation . It was concluded that the treatment should be mainly from kidney-tonification and assisted with blood-activation and phlegm-removing . The association rules and complex networks of data mining methods were applied in the lit-erature research of PCOS . The results provided literature references and statistical evidences for the phlegm-dampness accumulation in the uterus as the pathogenesis of PCOS , and the methods of kidney-tonification , blood-activation and phlegm-removing as the treatment principles .

Phlegm is an important etiopathogenisis of female reproductive dysfunction in TCM.ZHU Dan-xi put forward the famous constitution theory of "phlegm in fat people",and there was internal regularity in pathogenesis of wemen with phlegm-dampness constitution.His viewpoint guided the clinical treatment objectively and provided a solid theorectical basis of preventing and treating female reproductive dysfunction induced by phlegm-dapness.Therefore,this paper will discuss the relationship between phlegm-dampness constitution and female reproductive dysfunction from the characteristics of pglegm-dampness constitute,correlation of obesity and pglegm-dampness constitute,and so on.

Polycystic ovary syndrome(PCOS),one of the most common causes of anovulation-induced infertility,is largely dependent on the influence of obesity and metabolic alterations,including an insulinresistant state and the metabolic syndrome,which consistently affect most women with PCOS.The clinical phenotype and the pathophysiological interact between obesity and PCOS have also been changed obviously in these paitients.Obesity has profound effects on the reproductive function of PCOS women and,particularly the abdominal phenotype,may add to the chances of androgen excess,PCOS,insulin resistance and hyperinsulinaemia.Obesity plays an important role in the pathogenesis of PCOS and obesity-associated metabolic alterations have a long-term impact on the patients' health.

The polycystic ovray syndrome(PCOS) is a heterogeneous condition with genetic predisposition. It is characterized by a myriad of symptoms including oligomenorrhea or amenorrhea,anovulation or infertility,hirsutism or acne. Insulin resistance appears to be an important factor in PCOS though the lack of an etiology has led to symptom oriented therapy which includes lifestyle modification,the use of cyclical progestagens and antiandrogens. Therapeutic approaches to PCOS remain an ongoing source of debate. Insulin sensitizing agents may bring new hope in therapy. Future research is aimed at shedding light on the pathophysiology so as to optimize treatment of women with PCOS.
