1.Design of Medical Insurance Management System in No.1 Military Medical Project

Objective To design a suit of medical insurance management system that is in accordance with local medical insurance requirements and hospital management of medical insurance needs.Methods Combining the analysis of the needs of local medical insurance and hospital medical insurance management with the actual flow control of the medical insurance patient in hospital,on the basis of No.1 Military Medical Project,the MMS was designed by using Visual Basic 2005 tool,VB.NET and C/S mode.Results By the successful development,the managers of hospital can be carried out integrated management and dynamic tracing to hospitalized patients with medical insurance and the whole microcomputer operation of medical insurance management can become true.Conclusion The modular design,scalability and simple operation can improve medical insurance efficiency,management capacity and practical value.
2.Validation of MRI-or CT-based gross tumor volume delineation in head and neck cancer: a comparative study

Objective To validate the gross tumor volume (GTV) delineation in head and neck cancer based on magnetic resonance imaging (MRI) or computed tomography (CT) by cross-sectional autopsy,and to provide a basis for clinical evaluation of the two imaging methods.Methods Ten New Zealand rabbits were selected for transplantation of VX2 carcinoma cells,and a head and neck cancer model was successfully established in six rabbits.Each rabbit was fixed and received MRI scan and CT scan in the same body position.Then,they were sacrificed and fixed in gelatin solution (-70℃) for 72 h; all cryopreserved rabbits underwent cross-sectional autopsy using a jig saw,with the same position and sectional thickness as in MRI scan and CT scan,and cross-sectional autopsy images were obtained using a digital single-lens reflex camera.GTVs were separately delineated based on CT,MRI,and cross-sectional autopsy images.The GTVMRI,GTVCT,GTVSA,and volume difference ratios (VDRs) were calculated;two-way classification ANOVA and paired t-test were used for difference analyses.Results The mean values of GTVMRI,GTVCT,and GTVSA were 8.20 ± 2.56,8.40 ± 2.20,and 8.11 ± 2.88 cm3,respectively,without significant differences among them (F =0.06,P =0.943).The mean values of VDRMRI-SA and VDRCT-SA were 0.180 ± 0.060 and 0.309 ± 0.091,respectively,with a significant difference between them (t =7.49,P =0.001).Conclusion The GTV delineation based on MRI is more accurate than that based on CT in head and neck cancer.
3.Vibramycin inhibits the expression of MMP-2 and the invasiveness of PC-3 cells in vitro

Objective To study the inhibition of vibramycin on the expression of matrix metalloproteinase 2 (MMP 2) and the invasiveness of androgen independent prostatic carcinoma cell line PC 3 in vitro. Methods Immunohistochemistry stain and transwell chamber were used to investigate the expression of MMP 2 in different concentration of vibramycin treated PC 3 cells and the invasive ability of different concentration of vibramycin treated PC 3 cell. Results The positive rate of MMP 2 in PC 3 cells was decreased at a concentration of 5 mg/L of vibramycin and decreased dramatically at the concentration of 10 mg/L.The cells moved through the membrane was (82.0?4.6)/field in the control group,while decreased to (26.1?3.6)?(7.2?2.2) and (3.3?0.7)/field in 5?10 and 20 mg/L vibramycin treated PC 3 cell respectively. Conclusions Vibramycin can inhibit the invasiveness and metastasis of PC 3 cells, the mechanism of which is related to the inhibition of MMP 2 in PC 3 cell.
4.Promoting effects of Guavenoic acid (GA)on proliferation,insulin synthesis and secretion in INS-1 cells and their mechanism

Aim To investigate the effects of Guaveno-ic acid (GA)on proliferation,insulin synthesis and secretion in INS-1 cells and their possible mechanism. Methods INS-1 βcells were cultured in vitro.Control group,medium group,model group,drug groups and positive group were set.INS-1 cells were treated with GA (0.3,1 ,3,1 0,30 nmol·L -1 )for 48 h.The cell proliferation was tested by MTT assay.Acid-alco-hol was used to extract the insulin in the cells and the amount of insulin synthesis of INS-1 cells was tested by RIA.5.6,1 6.7 mmol·L -1 glucose was used to chal-lenge INS-1 cells for 1 h to the insulin secretion model (BIS and GSIS)was tested,and the insulin secretion of INS-1 cells was tested via RIA.The mRNA expres-sion of insulin gene,PDX-1 and MafA was tested by q-PCR.Results Compared with medium group,GA could promote the proliferation of INS-1 cells signifi-cantly (P <0.01 )and promote the synthesis of insulin in INS-1 cells significantly (P <0.01 ).GA(0.3 ~30 nmol· L -1 )could promote the BIS,GSIS of INS-1 cells significantly (P <0.05 or P <0.01 ).GA (3,30 nmol·L -1 )could up-regulate the mRNA expression of insulin gene,PDX-1 ,MafA in INS-1 cells signifi-cantly (P <0.01 ).Conclusions GA could signifi-cantly improve the proliferation of INS-1 cells and pro-mote the insulin synthesis and secretion of INS-1 cells, which may be associated with up-regulation of insulin gene,PDX-1 ,MafA mRNA expression.

6.Expression and adjuvant effects of the fusion peptide TBP5.

Thymopentin (TP5) and bursopentin (BP5) are both immunopotentiators. To explore whether the TP5-BP5 fusion peptide (TBP5) has adjuvant activity or not, we cloned the TBP5 gene and confirmed that the TBP5 gene in a recombinant prokaryotic expression plasmid was successfully expressed in Escherichia coli BL21. TBP5 significantly promoted the proliferation of thymic and splenic lymphocytes of mice. The potential adjuvant activity of the TBP5 was examined in mice by coinjecting TBP5 and H9N2 avian influenza virus (AIV) inactivated vaccine. HI antibody titers, HA antibodies and cytokines levels (IL-4 and IFN-γ) were determined. We found that TBP5 markedly elevated serum HI titers and HA antibody levels, induced the secretion of both IL-4 and IFN-γ cytokines. Furthermore, virus challenge experiments confirmed that TBP5 contributed to inhibition replication of the virus [H9N2 AIV (A/chicken/Jiangsu/NJ07/05)] from mouse lungs. Altogether, these findings suggest that TBP5 may be an effective adjuvant for avian vaccine and that this study provides a reference for further research on new vaccine adjuvants.

7.Mid- and long-term follow-up for the graft patency after coronary artery bypass graft surgery

Objective To analyze the mid- and long-term results after coronary artery bypass graft surgery (CABG), to evaluate the accuracy of multi-detector CT coronary angiography (MDCT) on the graft patency status and to compare the patency for different grafts. Methods One hundred and one cases underwent CABG from June 1992 to March 2008 were followed up by searching the database of MDCT (42 cases) and selective coronary angiography (SCA, 59 cases). The mean following up period was (66.79 ±44.27) months. Three hundreds and 10 grafts including 115 arterial and 195 venous were analyzed.A comparison of the patency rate between the arterial and venous grafts was also carried out. At the mean time, 2 groups were divided according to the paft lesions( patency of not). Uunivariate analysis and Logistic regression analysis were made for statistics. Results At the follow-up of (53.93 ± 36.80) months, the patent rate of LIMA, RA, SV and CV examined by MDCT was 94.7%, 92.0%, 85.9% and 60.0%, respectively. The patent rate for MDCT patients with angina was 83.5%, and 95.2% for angina-free group ( P = 0.046). At the follow-up of (75.95 ±47.09) months, the patent rate of LIMA, RA, SV and CV examined by SCA was 87.1%, 81.0%, 53.6% and 57. 1%, respectively. In SCA patients the patent rate was 62.0% for angina group and 100% for angina-free group ( P = 0.025 ). According to logistic regression analysis, only the postoperative period was statistically related to the graft lesion. Conclusion MDCT could find out the occlusions and obvious stenoses of the grafts after CABG. It might be served as a noninvasive method to evaluate the grafts patency postoperatively. The mid- and long-term patency of arterial grafts is better than venous grafts. Graft lesion is an important factor leading to postoperative recurrent angina pectoris. The severity of the graft lesion has a close relation to the lenth of postoperative period.
8.Expression and clinical significance of survivin and PTEN in bladder transitional cell carcinoma

Objective To investigate the expression and clinical significance of survivin and PTEN in bladder transitional cell carcinoma(TCC).Methods The expression of survivin and PTEN was studied by S-P immunohistochemisty in 10 cases of normal urinary bladder tissues and 62 cases of the bladder TCC. Results All the normal urinary bladder tissues showed positive staining pattern of PTEN protein, negative staining pattern of survivin protein, positive rate of PTEN in 62 cases of the bladder TCC is 46.8%,the difference of positive rate between grade Ⅰ and grade Ⅲ was significant(P<0.05),the difference of positive rate between Tis~T_1 and T_2~T_4 showed a trend of decreasing, but there was no statistically significant difference(P>0.05);positive rate of survivin in 62 cases of the bladder TCC is 56.5%,the difference of positive rate between grade Ⅰ and grade Ⅲ was significant(P<0.05),the difference of positive rate between Tis~T_1 and T_2~T_4 was significant too(P<0.05).The expression of PTEN protein and survivin protein was reversely correlated(P<0.05).Conclusion The abnormal expression of PTEN protein and survivin protein play an important role in the occurrence and progress of the bladder TCC.It can be regarded as a useful diagnostic marker in the bladder TCC.
9.The resection of internal hemi-pelvic tumor using computer navigation and the reconstruction assisted by finite element analysis

Objective To analyze the security and effectiveness of the accurate resection and reconstruction of pelvic tumor with computer navigation and to evaluate the feasibility of pelvic ring structure and mechanics reconstruction using finite element analysis.Methors From December 2008 to June 2010,tumor resections assisted by computer navigation technology and pelvic reconstructions with assembly prosthesis designed by finite element analysis were performed in 12 patients with internal hemi-pelvic malignant tumors.There were 8 male and 4 female patients,with the average age of 39.7 years(range,25-53).The malignant tumors involved chondrosarcoma(4 cases),osteosarcoma(2),Ewing sarcoma(2),malignant fibrohistiocytoma(1),and metastatic tumor(3,in which including renal cell carcinoma 1 case,thyroid carcinoma 1,and breast carcinoma 1).According to Enneking staging classification,there were 1 case in Ⅰ A,2 cases in ⅠB,2 cases in Ⅱ A.and 4 cases in Ⅱ B.The local recurrence of tumor and functional results were followed up postoperatively.Results There were no nerves,vessels and pelvic organs injuries intra-operatively.The postoperative X-rays showed that the excised regions were proper to the preoperative schedule and en bloc resections were performed in all patients.All assembly prostheses were in satisfactory position.and the screw fixation was accordance with the finite element biomechanical analysis.All the cases were followed up for average 18.2 months (range,8-26).There were 1 case with local recurrence and pulmonary metastasis,1 case with deep infection,and 1 case with deep vein thrombosis.No prosthesis dislocation,loosening,breaking,and limb length discrepancy occurred in all patients.The average MSTS93 functional score was 70%in all patients at 6 months,and 68 % in 6 patients at 20 months follow.up.Conclusion The resection and reconstruction of pelvic tumor assisted by computer navigation is a safe and effective procedure.The finite element analysis can provide accurate biomechanical guidance to the reconstruction of pelvic ring.
10.Therapeutic effects of acute normovolemic hemodilution replacing autologous blood donation used in coronary artery bypass graft surgery in elderly patients

Objective To investigate effects of acute normovolemic hemodilution (ANH) used in coronary artery bypass graft (CABG) surgery in elderly patients.Methods 60 elderly patients (aged ≥65 years) received CABG surgery under cardiopulmonary bypass,of which 30 patients using ANH techniques and 30 patients without ANH techniques.Results All patients were revascularized completely with stable surgical procedure,no serious complications and no operative mortality were found.Oxygen supplying was balanced with demanding,without tissue hypoxia.Indicators of the myocardial protective effect,coagulation,liver function,kidney function,glucose metabolism,negative nitrogen balance,and postoperative drainage volume were not significantly different between the two groups (P>0.05).Allogeneic blood transfusion volume was significantly lower in study group than in control group[(0.9±0.9)U vs.(2.5±1.2)U,t=2.704,P<0.01].Conclusions The ANH technique used in CABG operation in appropriate elderly patient is feasible and safe,with satisfactory clinical results.
