Objective To investigate the clinical effect of Gonadotropin-releasing hormone agonist (GnRHa) combined with addback therapy after laparoscopic surgery on 153 cases with severe endometriosis patients. Methods 153 cases with severe endometriosis patients who underwent laparoscopic surgery from 2011 to 2013 were retrospectively analyzed in the Second Affiliated Hospital of Zhengzhou University. According to GnRHa or addback therapy three groups were divided:GnRHa group,GnRHa and addback therapy group, control group. The curative effects of different treatment group, adverse reactions, recurrence rates, and fertility were analyzed. The follow-up time was 6-24 months with an average time of 17.15 months. Results 51 patients suffered from obvious menopausal symptoms and received addback therapy (Progynova,0.5~1 mg/d). The VAS score and Kupperman score in GnRHa group,GnRHa and addback therapy group were significantly superior to the control group (P <0.05). And no statistical significance was found in the interval to relapse, recurrence and postoperative pregnancy rate (P>0.05). The VAS scores in the GnRHa group and addback therapy group were 2.14±2.02 and 1.73±2.17 , respectively (P > 0.05). The main side effect of GnRHa group is hot flashes, diaphores, insomnia, sexual dysfunction, etc. The incidences were higher in the GnRHa group than that in the addback therapy group. But the addback therapy can significantly relieve the symptoms of low estrogen introduced by the application of GnRHa (P<0.05). Conclusions Laparoscopic combined with GnRHa treatment can significantly improve clinical curative effect of endometriosis. Addback therapy could abate the side reaction of GnRHa and do not affect the clinical curative effect. The clinical curative effect in the GnRHa and addbacktherapy group were superior to the GnRHa group.

Objective To study the role of homeobox A2(HOXA2) gene in cell growth, cellcycle and apoptosis of hepatoma cells, and discuss the feasibility that HOXA2 gene would be a potential therapy target of hepatoma. Methods Real-time quantitative polymerase chain reaction and revers transcriptional polymerase chain reaction were performed to examine HOXA2 expression in tumorous and non-tumorous tissue of patients was matched with liver cancer. siRNAs were chemically synthesized to interference HOXA2 in PLC/PRF/5 and MHCC-97L cells. Growth curve and soft-agar colony formation assay were performed to evaluate cell growth, and cell cycle and apop-tosis were analyzed by flow cytometry. Results HOXA2 was upregulated in HCC samples compared with matched non-tumor tissues(P<0.05). Two siRNAs against HOXA2 gene were designed and synthesized to specially knock-down HOXA2 in PLC/PRF/5 and MHCC-97L cells. HOXA2 knockdown inhibited cellular growth and soft-agar colony formation in PLC/PRF/5 and MHCC-97L cells. And Fowcytometry analysis revealed that HOXA2 knock-down by RNA interference could result in G1 arrest and S decrease and promoted cellular apoptosis. ConclusionHOXA2 gene has an important role in cell growth of hepatoma cells.

Objective To evaluate the clinical efficacy and the surgical treatment of spinal tuber-culosis by debridement, fusion and internal fixation to restore stability of the spine. Methods One hundred and fifty-two patients with spinal tuberculosis were treated surgically between 1996 and 2002. There were 15 in cervical spine, 67 in thoracic spine, 17 in thoracolumbar spine and 53 in lumbar spine. The lesion was of 1-3 levels without jumping lesions. The surgical procedures included: 1) anterior debridement, fusion with autograft and internal fixation with plate. 2) lateral debridement with posterior fusion and pedicle screw fix-ation. 3) posterior debridement, fusion and pedicle screw fixation. 4) anterior debridement and posterior fu-sion and fixation. All patients wore a brace for 3-5 months and chemotherapy was given for 6-9 months postoperatively. The blood test and imaging evaluation were undergone. All patients were followed for 3 to 5 years postoperatively. Results 1) The average operation time and blood loss was 4.5 hours and 650 ml for combined anterior and posterior procedure, 3.5 hours and 450 ml for anterior procedure and 3 hours and 350 ml for the other two procedures. 2) In regard to complications: major blood vessel injury in one case, temporary sinus formation in 5 and loosening and breaking of the instrumentation in 3. 3) All patients had relief of the symptoms and ambulation 1-2 weeks postoperatively. Normal daily function was recovered at 6-8 weeks and solid fusion was observed in 4-6 months postoperatively. All patients were cured of tubercu-losis lesions in spinal column or on other region, and there was no recurrence. Conclusion Effective chemotherapy and radical debridement of the lesion are the basis of success of surgical treatment of spinal tuberculosis. Fusion and internal fixation were the keys for restoration of the stability of spinal column, solid fusion and correction of the kyphotic deformity.

Objectives To analyze the effect of glucocorticoids on acute respiratory distress syndrome (ARDS) in adult patients. Method English and Chinese literature about the glucocorticoids treatment for ARDS were electronically searched. Meta-analysis was performed by Rev Man 5.0 software after the data of qualified studies were included in consistence with the criteria of inclusion and exclusion. Results Eight studies including 679 cases(steroid group 366, control group 313) met the inclusion criteria. Resuhs of meta-analysis showed that there were significant differences( P <0.05), between the steroid group and the control group, in mortality in all cases, mortality in patients treated at early stage, mortality in patients treated with low dose steroids, and PaO_2/FiO_2;the relative risks(RRs)/standardized mean difference(SMDs) and 95% confidence intervals(CIs) are 0.55(0. 34 ~ 0.87), 0.49(0.28 ~ 0.86), 0.46(0.24 ~ 0.88) and 2.99(0.63 ~ 5.34). There were no significant differences in mortality in patients treated during late stage, mortality in patients treated with high dose steroids and number of nosocomial infections(P > 0.05). Conclusions Low-dose glucocorticoids or routine dose glucocorticoids given during early stage can reduce mortality in patients with ARDS; the oxygenation of patients is significantly improved after steroid therapy; incidence of infectious complications is neither increased nor decreased by steroid therapy.

Objective To explore the value of combining pulmonary rehabilitation with heat and magnetic vibration (HMV) therapy and tiotropium for patients with stable but severe chronic obstructive pulmonary disease (COPD). Methods This was a paralleled, controlled, randomized study. Thirty-seven patients with stable severe COPD were enrolled and divided into two groups at random. One was the tiotropium group (T group) , while the other combined tiotropium therapy with HMV (the T + HMV group). The time span was 4 weeks. The examinations were performed at week 0, week 2 and week 4. The examinations included pulmonary function tests, arterial blood gas analysis, the 6 minute walking test (6MWT) , Borg's score and St George's Respiratory Questionaire (SGRQ). Results Inspiratory capacity (IC) increased in both groups. Forced expiratory volume in one second ( FEV1.0) , percent predicted FEV1.0 and FEV1.0/forced vital capacity ( FVC) increased significantly only in the T + HMV group. The average parameters of the pulmonary function test in the T + HMV group were significantly higher than in the T group. In both groups, alveolar PO2 ( PaO2) improved but alveolar PCO2 ( PaCO2 ) did not change and in this there was no significant difference between the groups. The 6 minute walking distance increased and the average Borg score decreased in both groups, and there was no difference between the groups. SCRQ dropped more than 4 scores in both groups, but the decrease in the T + HMV group was significantly greater. Conclusions Tiotropium can play an important role in the rehabilitation of patients with stable severe COPD. The combination of tiotropium with HMV therapy is superior to tiotropium alone in pulmonary rehabilitation for stable but severe COPD patients.
