1.The Current Situation of Hypertension Prevalent Rate and the Rates of Illness-awareness,Medication and Blood Pressure Control in Wenzhou

Objective To know the prevalence rate of hypertension and the rates of illness-awareness,medication and blood pressure control in Wenzhou. Method With random cluster sampling,1 034 residents aged≥15 yrs were selected from the communities of Wenzhou,and investigated with the standard questionnaires. Results The prevalence rate of hypertension in Wenzhou was 38.97% and the standardized rate was 22.27%. Among the patients with hypertension,51.12% of them were aware,and only 40.69% of them took medication and 9.18% of them had their blood pressure controlled. Conclusion The prevalent rate of hypertension is higher,but the awareness rate,medication rate and controlling rate are lower in Wenzhou city. The comprehensive interventions are needed for the prevention and treatment of hypertension.
2.The role of expression of perinuclear anti-neutrophil cytoplasmic antibody and anti-Saccharomyces cerevisiae antibody in the diagnosis of inflammatory bowel diseases

Objective To analyze the diagnostic significance of the expression of perinuclear anti-neutrophil cytoplasmic antibody(pANCA) and anti-Saccharomyces cerevisiae antibody(ASCA) in the patients with inflammatory bowel disease(IBD).Methods Serum samples were collected from 97 patients with IBD,including 86 patients with ulcerative colitis(UC) and 11 with Crohn′s disease(CD).Another group of 36 persons in whom colonoscopic examination was negative served as control.Determinations of ASCA and pANCA were performed using indirect immunofluorescence test(IIFT).Results The positive rate of pANCA in group UC,CD and control was 27.9%,0% and 0%,respectively.Obviously,the rate in group UC was higher than that in the other two groups(P0.05).Four of 11 CD patients(36.4%) were severe cases,among them 3 showed ASCA+;7 were moderate cases(63.6%),and among them 1 case showed ASCA+.The positive ratio of ASCA was significantly different between severe and moderate CD cases(P
3.Imatinib(Gleevec)in preparation for allogeneic hematopoietic stem cell transplantation in patients with Philadelphia-positive leukemia

Objective To retrospectively analyse the impact of imatinib mesylate(Gleevec)on HSCT outcome in patients with Ph(+)leukemias.Methods From June 2001 to June 2005,31 CML-BC/AP patients and 8 refractory Ph(+)ALL patients were treated with imatinib mesylate to induce remission,followed by allogeneic transplantation,and we evaluated its influence on engraftment,graft versus host disease(GVHD),overall survival(OS),disease free survival(DFS),relapse rate(RR)and transplant-related mortality(TRM).Results Eighteen of 39 patients achieved complete hematologic remission(CHR),9 patients bone marrow remission(BMR)and 4 patients partial response(PR),the overall response rate being 70.96%.After HSCT,all patients achieved complete allogeneic engraftment with a median of 14 and 13.5 days for neutrophil and platelet recovery,respectively.The cumulative incidence rates of Ⅱ~Ⅳ? and Ⅲ~Ⅳ? aGVHD were 61.53% and 15.38%.The probability of OS and DFS was(73.51?9.61)% and(61.28?12.37)%,when HSCT was performed in CR,compared with(36.36?14.50)% and(31.25?13.98)% in NR.The 3-year cumulative relapse rate(RR)was 20.41% vs 75.00%.Four cases died of transplant-related complications.Conclusion Pre-transplant imatinib mesylate seems effective and safe for refractory Ph(+)leukemias;it allows transplantation to be in a more favorable status,and leads to better outcome.
4.The early rehabilitating effects of mild hypothermia for patients with herpes simplex viral encephalitis

Objective To observe the early rehabilitation effect of mild hypothermia on patients with herpes simplex viral encephalitis (HSE). Methods A total of 58 patients with HSE were randomized into two groups, a mild hypothermia therapy group (30 cases) and a normothermia control group (28 cases). Their rectal temperatures were controlled to (34±1)℃ and (37.0±0.5)℃ respectively. Serum levels of neuron specific enolase (NSE) were determined through radio-immunoassay (RIA). Soluble intercellular adhesion molecule-1 (sICAM-1) was measured with ELISA before and 1, 3, 5, and 7 d after treatment. The outcome was evaluated using the Glasgow Outcome Scale (GOS) 30 days after treatment. Results Compared with the normothermia control group, the mild hypothermia group's serum levels of NSE and sICAM-1 decreased quickly and significantly during the early stage of treatment and remained better 30 d later. Conclusion Mild hypothermia therapy can dramatically reduce inflamma-tion and facilitate the rehabilitation of damaged neurons, provide protective effects and improve the outcome for pa-tients with SHE.
5.Pharmaceutical Care on a Patient with Cerebral Venous Sinus Thrombosis and Epilepsy

Objective To investigate the role of clinical pharmacist in medical therapy for a patient with cerebrovascular disease and epilepsy.Methods One patient was hospitalized due to headache and twitch.Clinical diagnosis was cerebral venous sinus thrombosis and epilepsy.During the treatment,the clinical pharmacist gave suggestion on drug selection of anti-infection and anti-epileptic therapy,assessed drug interaction between omeprazole-escitalopram and mannitoldexamethasone,monitored the adverse drug reaction of valproate,and provided medication education to the patient.Results Physician adopted clinical pharmacist's suggestion.The patient discharged with stable condition.Conclusion The participation of clinical pharmacists in the medication therapy of patients with cerebrovascular disease can reduce the risk of drug use and promote the efficacy and safety of medication.
6.Curcumin downregulation of MGMT expression to enhance the chemosensitivity of temozolomide on maligant glioblastoma

Objective To explore the roles of curcumin in regulating high level O6-methylguanine-DNA methyltransferase (MGMT) expression and the chemosensitivity of gliomas cells to temozolomide (TMZ) in malignant gliomas.Methods The expressions of MGMT were detected in C6 and U87 cell lines and primary malignant gliomas cell by qRT-PCR.The cell proliferation was analyzed after added in signal curcumin,signal TMZ,curcumin combined with TMZ in cells by CCK-8 assay.The cell apoptosis rate was detected by flow cytometry.Results Compared with single curcumin (C6:0.64 ±0.03;U87:0.63 ±0.06;primary cell:0.51 ±0.07) and single TMZ (C6:0.53 ±0.06;U87:0.51 ±0.04;primary cell:0.79 ±0.03),curcumin combined with TMZ (C6:0.14 ±0.01;U87:0.12 ±0.03;primary cell:0.29 ±0.02) could significantly reduce the expression of MGMT in C6,U87 cell lines and primary cell line (C6:F=23.675,P=0.006;U87:F=29.021,P=0.001;primary cell:F=25.534,P=0.001).The combination of curcumin and TMZ could significantly inhibit the cells proliferation,decrease the halfinhibition concentration (IC5o) value (C6:F=6.731,P=0.012;U87:F=17.321,P=0.008;primary cell:F=18.857,P =0.007).The apoptosis rate of the cells was significantly increased after the combined action of curcumin and TMZ (C5:F=25.871,P=0.001;U87:F=6.847,P=0.009;primary cell:F=36.641,P=0.000).Conclusion Synergistic effect of curcumin and TMZ can reduce the expression of MGMT and increase the chemosensitivity of TMZ to malignant glioma,and provide new strategy and methods for the treatment of malignant glioma.
7.A case report of successful management of middle ear foreign body following hearing aid fitting with ossicular chains integrity.

A case of ear mold impression middle ear foreign body was reported. In the exploratory operation we found the ear mold impression surrounded the ossicular chains. During the process of removal the middle ear foreign body, the incudostapedial joint was protected from obvious external force and ossicular chains were kept integrity. Tympanoplasty was completed after management of foreign body. The patient recovered well postoperatively and was discharged as scheduled.

8.Clinicopathological analysis of elderly Epstein-Barr virus-positive diffuse large B-cell lymphoma of Han and Uygur in Xinjiang Uygur Autonomous Region

Objective To analysis the clinical features of elderly diffuse large B-cell lymphoma (DLBCL) patients with Epstein-Barr virus (EBV) infection and their prognostic factors.Methods 250 cases of DLBCL were retrospectively studied by in situ hybridization (ISH) to detect the EBV and by immunohistochemical to evaluate the histological type and Ki-67 protein.Results 36 cases with EBVpositive included 28 elderly (aged ≥ 60 years), of which 21 cases were Han, 15 cases were Uygur, male/ female ratio was 2 : 1.There were 23 cases with nodal presentation only, 13 cases with extra-nodal presentation.Twenty-nine patients presented with advanced disease (Ann Arbor stage Ⅲ/Ⅳ), 30 patients were found with high lactate dehydrogenase (LDH), 22 patients with high IPI score (3-5).Histological observation showed a diffuse and polymorphic proliferation of large lymphoid cells with varying degrees of reactive components.These tumor cells were frequently characterized by a broad range of B-cell maturation, containingcentroblasts, immunoblasts, and Hodgkin-and Reed-Stemberg (HRS)-like giant cells.Immunohistochemical studies showed that tumor cells were positive for CD20 and (or) CD79a in almost cases, most of the cases had a high proliferative index.CD10, bcl-6, Mum-1 performed histological type, 31 cases were non-germinal center.Except the age and location (P < 0.05), no other significant differences were observed in Han and Uygur elderly EBV+ DLBCL (P > 0.05).Conclusion The incidence of elderly EBV+ DLBCL is low, it has some unique clinical and pathological features with poor prognosis.
9.Correlation between the levels of vascular endothelial growth factor and its receptor Flt-1 and KDR in human brain glioma

Objective To evaluate the serum changes in glioma patients, and in relations with clinicopathalogy parameters response to treatment and survival in patients with glioma via the examination of the significance of serum VEGF, Flt-1 and KDR levels. Methods Serum VEGF,Flt-1 and KDR levels were analyzed in patients of glioma before and after treatment, and patients with brain metastasis, as well as in healthy controls. Serum VEGF, Flt-1 and KDR levels were assessed by Enzyme-Linked Immunosorbent Assay(ELISA) and the data were processed by SPSS 11.5 for t-test and relevant analyses. Results Serum VEGF, Flt-1 and KDR levels were higher in patients with glioma than in healthy control (P =0.001, P =0.043 and P =0.045 respectively). Significant difference on levels of VEGF was found (P =0.032) comparing glioma group with brain metastasis group. The pretherapeutic serum levels of VEGF and Flt-1 were significantly different with disease recurrence or persistence after treatment (P =0.026 and P =0.038). 10.Progress on Electrocatalysts of Hydrogen Evolution Reaction Based on Carbon Fiber Materials

Hydrogen evolution from water electrolysis is one of the effective ways to obtain clean hydrogen energy in the future. Pt-based materials are the efficient catalysts in hydrogen evolution reaction, but it is expensive, difficult to recycle, which impedes its application in the development of hydrogen energy and economy. Therefore, it is the key trend to develop efficient non-noble metal electrocatalysts with the aim of providing cost-competitive hydrogen energy. In this review, we highlighted the recent research efforts toward the synthesis of noble metal-free electrocatalysts for the hydrogen evolution reaction ( HER) , mainly focusing on nanomaterial catalysts supported on carbon fiber materials. We reviewed several important kinds of heterogeneous non-noble metal electrocatalysts, including sulfides, selenides, carbides, phosphides, and oxides. In the discussion, emphasis was given to the synthetic methods of these HER electrocatalysts, and the strategies for performance improvement. In addition, this paper also briefly summarized the application of carbon fiber material as substrate in the field of electroanalytical chemistry.
