1.Development and service of information resources in medical library of CDC in health information era

After the development and service of information resources in medical library of CDC were described in aspects of its characteristics and service target, the development and service strategies of information resources in medical library of CDC in health information era were analyzed with suggestions put forward for meeting the demands of scientific workers, such as construction of the service platform for sharing digital medical and health literature resources in Hubei Province, development of big data-oriented innovative medical information resources, construction of mobile equipments-based service platform for medical information resources, enforcement of team construction, optimization of staff allocation.
2.Content Determination of Menthanol in Qiangli Pipa Syrup by Headspace GC

OBJECTIVE:To establish a method for the content determination of menthanol in Qinagli pipa syrup. METHODS:Headspace GC was performed on the column of HP-INNOWax capillary column,column temperature was 80 ℃,temperature inlet sample was 150 ℃,the detector was hydrogen flame ionization detector with temperature of 250 ℃,the flow rate of nitrogen was 0.8 ml/min with constant flow and no split,the volume injection was 1 ml,the equilibrium temperature of headspace bottle was 80 ℃,and the equilibrium time was 20 min. RESULTS:The linear range of menthanol was 2.486-24.86 μg/ml(r=0.9999),RS-Ds of precision,stability and reproducibility tests were lower than 2%,recovery was 95.4%-99.8%(RSD=1.5%,n=9). CON-CLUSIONS:The method is simple and accurate,and can be used for the content determination of menthanol in Qinagli pipa syrup.
3.Pharmacokinetics of intravenously infused domestic alfentanil in patients undergoing general anesthesia

Objective To investigate the pharmacokinetics of intravenously infused domestic alfentanil in the patients undergoing general anesthesia.Methods Twelve American Society of Anesthesiologists physical status Ⅰ or Ⅱ patients of both sexes,aged 18-47 yr,weighing 49-86 kg,scheduled for elective subtotal thyroidectomy under general anesthesia,were enrolled in the study.Anesthesia was induced with iv midazolam 0.02 mg/kg,alfentanil 25 μg/kg,propofol 1.5 mg/kg and rocuronium 0.8 mg/kg.The patients were endotracheally intubated and mechanically ventilated.Anesthesia was maintained with inhalation of 0.8%-2.0% sevoflurane,iv infusion of alfentanil 1 μg · kg-1 · min-1,and intermittent iv boluses of rocuronium 10-20 mg,bispectral index value was maintained at 40-60,and infusion was stopped at 10 min before the end of surgery.Before induction of anesthesia,at 1,3,5,8,10,14,20,35,65,95 and 125 min ofalfentanil infusion,and at 5,15,30,60,120,180,240,300 and 360 min after termination of alfentanil infusion,venous blood samples were collected for determination of the plasma concentration of alfentanil (by high performance liquid chromatography-mass spectrometry/mass spectrometry).DAS 3.0 software was used to analyze the pharmacokinetic parameters of alfentanil.Results The pharmacokinetics of domestic alfentanil was best described by a two-compartment pharmacokinetic model.The distribution half-life was (1.8 ± 0.8) min,and the elimination half-life was (91±22) min.The volume of distribution at steady state was (0.38±0.12) L/kg,and the clearance was (4.3± 1.6) ml · kg-1 · min-1.The elimination of domestic alfentanil followed the first-order kinetics.Conclusion The distribution of intravenously infused domestic alfentanil is fitted to a two-compartment pharmacokinetic model with elimination following the first-order kinetics in the patients undergoing general anesthesia.
4.Tetracycline inhibits Staphylococcus aureus growth in bacterial prostatic tissues of rats

Objective To probe the methods of experimental research for the drug activities of the rat prostates. Methods With androgen and Staphylococcus aureus injected into the prostates, the bacterial prostatitis (BP) models and benign prostatic hyperplasia with bacterial prostatitis (BHP-BP) models were developed. The animals were fed with tetracycline and then the drug activities in the serum and the prostatic tissues, the quantities of the bacteria and the histopathlogical changes in the prostateic tissues were separately detected in different time. ResultsAfter ten days, the histopathlogical changes of the benign hyperplasia and mesenchyma were found in the prostatic tissue from BHP animals and acute suppurative inflammation were dicegnosed in prostatic tissue from the BP group after infected for two days. The drug activities of 22.44 mg/g in BP group and 20.26 mg/g in BPH-BP group were found until 3.5~4 hours and the values were higher than MBC of the tetracycline and the antibacterial activity of the sera at the same time. After 8 to 11 days of treating, the bacteria would not be isolated from the prostates of the animals with BP or BHP-BP. Stopping treatment a week 11 days after treating acute or chronic inflammation histopathlogical change did not be found in the prostates of animals with BP. 30 CFU of bacteria could be found in per gram tissue of prostate and both of the acute and chronic suppurative inflammation could be found in the BP group without treatment at the 28 days afterinfected.Conclusion The tetracycline activities in the prostates of rats with BP or BPH-BPis same as or even higher than that of serum and the MBC of the drug. The quantity of sensitive bacteria in prostates will be diminished and disappeared during treatment.
5.The impact of chemotherapy on coagulation function in patients with gastrointestinal cancer

Objective To investigate the chemotherapy on coagulation function in patients with gastrointestinal cancer.Methods One hundred eight-one cases of gastrointestinal cancer patients who were taken chemotherapy in our hospital were collected from January 2009 to May 2012.According to tumor metastasis,they were divided into distant metastasis group ( n=68) and no distant metastasis group ( n=113 ),and then 270 cases of healthy persons were matched as control group.Plasma prothrombin time ( PT),activated partial thromboplastin enzyme time ( APTT),plasma fibrinogen ( FIB ),the total thrombin time ( TT),and D-dimer (DD) level were observed and compared between these groups.And the level of coagulation indicators in patients with gastrointestinal cancer was compared before and after chemotherapy.Results There were significant differences among three groups on levels of PT,APTT,FIB,TT and D-D (F=4.443,4.791,5.795,3.671,10.564,respectively,P<0.05) before chemotherapy.The PT[(11.31 ±0.98) s and (11.20 ±0.95) s vs (11.99±0.89)s] and APTT[(29.01 ±4.52)s and (28.25 ±3.98)s vs (30.45 ±4.95)s] and TT [(19.35 ± 2.09) s and (18.68 ± 1.98 ) s vs (19.98 ± 1.89 ) s] in the tumor without distant metastasis and tumor distant metastasis groups were significantly lower than those in the healthy control group ( P<0.05 ),however FIB[(3.05 ±0.68) g/L and (3.89 ± 1.01 ) g/L vs (2.29 ±0.38) g/L] and D-D[(98.88 ± 15.94) μg/L and (227.31 ± 35.12 ) μg/L vs (35.41 ± 3.43 ) μg/L] were significantly higher than those in the healthy control group (P<0.05);FIB[(3.89 ± 1.01 ) g,/L vs (3.05±0.68) g/L],DD[(227.31 ± 35.12) μg/L vs (98.88 ± 15.94) μg/L] in distant metastasis group were also significantly higher than those in no distant metastasis group( P<0.05 ).On comparison before and after chemotherapy,there was no significant difference on PT,APTT,D-D and TT between distant metastasis group and no distant metastasis group ( P > 0.05 ),but FIB decreased significantly in two groups after chemotherapy [Distant metastasis group:( 3.25 ± 0.78 ) g/L vs (3.89 ± 1.01) g/L;No distant metastasis group:( 2.58 ± 0.75 ) g/L vs ( 3.05 ± 0.68 ) g/L;P<0.05 )] Conclusion The patients with gastrointestinal cancer are on the hypercoagulable state.In addition,hypercoagulable state could be increased with the emergence of metastases.Chemotherapy may be a transient increasing in the risk of thrombosis.Clinicians need to recognize the hypercoagulable state in cancer patients before and after chemotherapy,that will provide help for clinical treatments.
6.The relationship between the macular edema and the thickness of macular subfoveal choroid in diabetic retinopathy patients

Objective To evaluate the relationship between the existence of DME and the SFCT in DR pa-tients. Methods We collected 70 cases which were clinical diabetic involving 122 eyes. They are 36 men with 64 eyes and 34 women with 58 eyes. The average age is 56.7 ± 9.7 year-old. All patients had been examined by oph-thalmologic examination. Grouping: patients without diabetic retinopathy (NDR) group; mild-to-moderate NPDR group; severe NPDR group; PDR group. The last three groups were divided into two subgroup groupsrespectively , which were cases with and without significant macular edema (CSME + / CSME). EDI was used to measure the SFCT. SPSS 17.0 software was used for statistical analysis. Results The average SFCT of NDR group, mild-to-moderate NPDR group, severe NPDR group and the PDR group respectively were (282.94 ± 104.21)μm, (313.62 ± 94.40)μm, (382.44 ± 76.91)μm, (335.00 ± 73.82)μm. Compared with the NDR group, SFCT was thicker than the other three groups, and difference was statistically significant (F = 2.786, P = 2.786). There were no statisti-cally significant difference of SFCT between the mild-to-moderate NPDR/CSME - group and mild-to-moderate NPDR/CSME + group, severe NPDR/CSME - group and severe NPDR/CSME + group PDR/CSME-group, PDR/CSME + group. Conclusions The SFCT of DR patients could potentially be thickenedas disease became serious. When considered the same degree DR, there is no obvious difference of SFCT between patients with CSME and without CSME.
7.Nursing of patients with primary open angle glaucoma underging implantation of EX-PRESS device

Objective To determine the nursing measures of patients with primary open angle glaucoma undergoing implantation of EX-PRESS device.Methods 12 glaucomatous patients with EX-PRESS miniature glaucoma device received appropriate hygienic-education preoperatively,intensive nursing services during and after surgery,as well as health instructions in the follow-up period.Results 12 patients finally recovered from the surgery.The main post-operative complications were low tension and shallow anterior chamber(2 eyes),obstruction of the device by iris (1 eye).The intraocular pressures of all these patients remained normal levels before discharge and the visual acuity were the same as those pre-operation.Conclusions The effective nursing services for patients with EX-PRESS implantation is beneficial to the rehabilitation by increasing the adherence to the follow-up schedule,contributing to better understanding the common sense of the disease and the choice of the surgery.
8.Femur positioning technologies of robot-assisted surgical system for total knee replacement

Robot-assisted surgery is becoming a widely popular technology and now entering total knee replacement. The development of total knee replacement is introduced in this paper with the emphasis on femur positioning technologies. The experiments and error analysis of robot-assisted surgical system for total knee replacement demonstrate that femur positioning technologies have a high precision and thus deserve to be popularized theoretically and practically.
9.High pressure level in human airwayenhances the expression of 3 cytokines related to airway remodeling

Objective To explore the relationship between different airway pressure and expression of related cytokines to airway remodeling.Methods Fourty-two chronic obstructive pulmonary disease cases (COPD group) and thirty-three control cases were collected.The above cases underwent mechanical ventilation in the period of general anesthesia.According to different levels of peak inspiratory pressure(PIP), above two groups, randomly and respectively, were divided into high PIP (HPIP, 24 cmH2O) group, moderate PIP (MPIP, 22 cmH2O) group, low PIP (LPIP, 20 cmH2O) group.All positive end expiratory pressure (PEEP) was set to 5 cmH2O.Then the collection of BALF was performed before and 3 hours after applying ventilator.The related factors to airway remodeling, such as fibroblast growth factor 2 (FGF-2), transforming growth factor β1 (TGF-β1) and matrix metalloproteinase-9 (MMP-9), were measured by enzyme-linked immunosorbent assay (ELISA) and Western blot.Results 1)Beforemechanical ventilation, the levels of FGF-2, TGF-β1 and MMP-9 in COPD group were higher than control group (P<0.01).2)3 hours after mechanical ventilation, we saw significant upregulated expression of FGF-2, TGF-β1, MMP-9 in HPIP subgroup in control group (P<0.05) and the 3 factors levels of COPD group were all increased (P<0.05).Moreover, HPIP subgroup was significantly higher than MPIP and LPIP subgroup in COPD group (P<0.05).3)The expression of FGF-2, TGF-β1, MMP-9 in BALF had a positive correlation with the airway pressure levels in COPD group(P<0.01).Conclusions Under mechanical ventilation, sustained high airway pressure may enhance the expression of FGF-2, TGF-β1 and MMP-9 which may result in airway remodeling by mechanosensitive cation channel in bronchial epithelial cells, especially in COPD patients.
10.Psychiatric disorders in pharyngitis patients and its associated factor analysis.

OBJECTIVE:
To investigate the characteristics of psychiatric comorbidity in pharyngitis patients.
METHOD:
The psychological investigation and evaluation were done in 100 pharyngitis patients by using self-reporting inventory 90 (SCL-90). All the results were compared with healthy Chinese population, including factors of gender, age, course of disease and education level, using rank-sum test analysis, SPSS 19.0.
RESULT:
Mild and moderate levels of psychosis were respectively noted in 65% and 14%, with high score of somatization (P < 0.05), obsession (P < 0.05), interpersonal sensitivity (P < 0.05), anxiety (P < 0.05) and other (sleep, diet, P < 0.05) compared with control group. The factor scores of obsession (P < 0.05), anxiety (P < 0.05) in male group were higher than female group. The factor scores of somatization (P < 0.05) and obsession (P < 0.05) in junior group were higher than senior group. The factor scores of somatization (P < 0.05), interpersonal sensitivity (P < 0.05) and anxiety (P < 0.05) in long course group were higher than short course group. The factor score of interpersonal sensitivity (P < 0.05) in low educated group was higher than highly educated group.
CONCLUSION
Psychiatric disorders are prevalent in pharyngitis patients, especially upon the factors of somatization, obsession, interpersonal sensitivity, anxiety and other (sleep, diet). Male, junior, long course patients and low educated are in high risk group.
