1.Risk factors analyse in children with nosocomial infection of pediatric department

Objective To analyze risk factor in children with nosocomial infection of pediatric department. Methods 3 250 children were collected as observation group in recent five yesrs.The infective characteristic were analyzed retrospectively in observation group.Results 73 hospital infective cases were found in the observation group.50 cases of pulmonary infection,10 cases of digestive tract infection,10 cases of urinary system infection,and 3 cases of oral infection.Age(χ2 =5.76,P <0.05 ),duration of hospitalization(χ2 =6.05,P <0.05 ),invasive operation(χ2 =7.75,P <0.05)and preventive antibiotics use(χ2 =7.86,P <0.05)were correlated with nosocomial infection.Staphylococcus aureus and Staphylococcus haemolyticus of G + strain was most,staphlococcus epidemidis and klebsiella of G -strain was most.Conclusion The risk factors should be paied attention in paediatric cases,as well as strengthen the strict aseptic operation and apply with bacterial culture of secretion,which is of significance to guide clinical medication.
2.Investigation of Water Sources Contaminated by Volatile Organic Compounds in Jiangsu Province

Objective To investigate the water source contamination in Jiangsu Province. Methods Using blowing and arresting device and GC-MS, 90 water samples collected from 15 city drinking water sources in Jiangsu Province were analyzed. Results The main detected compounds were 1, 2-dichloroethane, chloroform, benzene, trichloroethene, toluene, tetrachloroethene, chlorobenzene, m-xylene, p-xylene, o-xylene, 1, 4-dichlorobenzene, 1, 2-dichlorobenzene, 1, 2, 4-trichlorobenzene and hexachlorobutadiene, while 1, 1-dichloroethene, trans-1, 2-dichloroethene, cis-1, 2-dichloroethene, 1, 1, 1-trichloroethane, carbon tetrachloride, bromodichloromethane, dibromochloromethane, bromoform, isopropylbenzene, 1, 2, 3-trichlorobenzene and styrene were not found. The highest concentration of 1, 2-dichloroethane was 27.79 ?g/L, which was close to the limit of surface water. It should be noticed that the detected concentrations of chlorobenzene in water source of district 3 and water source of district 4 were a little higher. Compared with the others, in water source of district 1, water source of district 9 and water source district of 14 a higher concentration of toluene and xylene were detected and the concentration of benzene in water source of district 1, water source of district 2, water source of district 3 and water source district 4 was higher. Only in water source of district 13, none of 25 volatile organic compounds was detected. Conclusion Some drinking water sources have been contaminated by volatile organic compounds in Jiangsu Province.
3.Value of ultrasonic diagnosis of congenital prepyloric diaphragm

Objective To evaluate the value of ultrasonic diagnosis of congenital prepyloric diaphragm. Methods Twelve cases were collected for the analysis of the characteristics of ultrasonography in diagnosis of congenital prepyloric diaphragm. Results Of the 12 cases of congenital prepyloric diaphragm, valvula in pyloric antrum could be observed in 5 cases, inclusive of 2 cases of complicating pachyntic stenosis of pyloric muscular layer. Other valvula in pyloric canal could not be found with ultrasonography. No pyloric muscular layer pachynsis was observed, but signs of pyloric obstruction such as gastric retention, slow or difficult transfer of gastric contents, gastro-esophageal reflux were found. Prepyloric diaphragm was confirmed surgically in all cases. Conclusion Ultrasonic diagnosis of congenital prepyloric diaphragm is of practical value in clinical practice and can provide reliable imaging data for the option of preoperative approaches.
4.The study on the relationship between multiple lacunar infarction and outcomes of mild Cognitive impairment

Objective To evaluate the impact of multiple lacunar infarction (MLI) on the outcomes of mild cognitive impairment (MCI). Methods MCI patients were selected in outpatients and hospitalized patients using a common international standard MCI screen. Magnetic resonance imaging (MRI) and computer tomography (CT) were used to determine the presence of MLI. MCI patients were followed up for 6~24 months and their cognitive status and the impact of MLI on MCI were evaluated using Kaplan-Meier survival analysis. Results One hundred twenty patients were diagnosed as having MCI based on clinical evaluation and neuropsychological assessment. Among them, 43 (35.8%) patients had MLI and the rest 77 (64.1%) patients did not have. Twenty-seven MCI patients with MLI (62.8%) developed dementia, while 15 MCI patients without MLI (19.4%) developed dementia during 6~24 months follow-up. Kaplan-Meier analysis indicated that the incidence of dementia was significantly higher in patients with MLI than in patients without MLI did (P<0.05). Conclusions MCI patients with MLI are more prone to dementia. These results suggest that early interventions aimed at controlling the modifiable risks are warranted for dementia prevention in MCI patients with MLI.
5.Hearing restoration in a deaf animal model with transplantation of adipose-derived mesenchymal stem cells from guinea pigs

BACKGROUND:Can adipose-derived mesenchymal stem cells be used to treat sensorineural deafness caused by hair celldegeneration and loss? OBJECTIVE:To investigate the effect of adipose-derived mesenchymal stem cells from guinea pigs transplanted via the scala tympani on the recovery from sensorineural hearing in an animal model. METHODS:Intraperitoneal injection of gentamicin was performed to prepare sensorineural deafness models in guinea pigs. Then, adipose-derived mesenchymal stem cells from guinea pig were transplanted via the scala tympani. After 1 and 3 weeks, auditory brainstem responses were detected to observe the hearing variation after adipose-derived mesenchymal stem celltransplantation. Meanwhile, the migration and distribution of EDU-labeled adipose-derived mesenchymal stem cells in the cochlea were traced. RESULTS AND CONCLUSION:After 1 and 3 weeks of implantation, the results from auditory brainstem response showed that the hearing was gradual y improved. At 1 week of implantation, most of the cells were distributed in the perilymph and some cells migrated to the Corti organ and attached to the basement membrane. At 3 weeks of implantation, cells migrated and attached to the basement membrane of Corti organ, furthermore, some cells migrated to the cochlear nerve. The longer the time of implantation, the less cellsurvived. The results indicate that adipose-derived mesenchymal stem cells from guinea pig that are transplanted via the scala tympani can directly migrate and survive ultimately to improve the hearing.

AIM:To investigate the effect and mechanism of liver ischemia/reperfusion(I/R)injury on the changes of cardiac energy metabolism and structure.METHODS:48 healthy Wistar male rats were randomly divided into 6 groups as follows(n=8 in each group):control group(CTL),simply ischemia for 30 min without reperfusion(I group);reperfusion following ischemia for 30 min(I/R group);2 h reperfusion following ischemia for 30 min(I/R 2 h group);4 h reperfusion following ischemia for 30 min(I/R 4 h group)and 6 h reperfusion following ischemia for 30 min(I/R 6 h group).The level of serum endotoxin was measured.The levels of insulin and insulin antibody in heart were detected by radioimmunoassay.The contents of MDA,MPO and lactic acid in heart were also determined.RESULTS:During the process of liver I/R injury,the level of endotoxin increased in I group and I/R group and declined gradually for long time during reperfusion,but was still longer than that in CTL group(P0.05).CONCLUSION:During the process of liver I/R injury,endotoxin is absorbed from intestine and impairment of liver detoxication leads to endotoxemia,which might play a role in the changes of the energy metabolism and structure in heart.

9.Clinical significance of cell factors in serum samples of hand-foot-mouth disease

Objective To study the clinical significance of cell factors in plasma samples of hand?foot?mouth disease. Methods Sixty?nine cases hand?foot?mouth diseases children who were treated in Fifth People’ s Hospital of Wuxi from July to December 2014 were selected as research objects,and divided into normal hand?foot?mouth disease group with 48 cases and severe hand?foot?mouth disease group with 21 cases. Twenty healthy children were selected as healthy control group. Serum samples were collected from 3 groups of subjects. The lev?el of IgA,IgG and IgM were detected by immune turbidity method. The level of IFN?γ, IL?10, IL?13 and IL?2 were detected by flow cytometry. Results The level of serum IgG,IgA of severe hand?foot?mouth disease group patients were (12. 33±8. 58) g/L,(0. 78±0. 55) g/L,significantly lower than control group((7. 09±6. 36) g/L,(0. 61±0. 36) g/L) and the normal hand?foot?mouth disease group((7. 41±5. 07) g/L,(0. 57±0. 41) g/L),the differences were significant(P<0. 05),while there was no significant difference between normal hand?foot?mouth disease group and control group( P>0. 05) ,and there was no significant difference in term of the level of IgM among the three groups( P>0. 05) . The level of serum IL?10 of normal hand?foot?mouth disease group was (2. 69±1. 33) ng/L,significant?ly higher than severe hand?foot?mouth disease group ( ( 1. 54 ± 1. 23 ) ng/L ) and control group ( ( 1. 35 ± 1. 12 ) ng/L) ,the differences were significant( P<0. 05) . Conclusion The level of serum cytokines can reflect the ge?netic profile status,understand the development trend of the disease,provide a reliable index for early clinical in?tervention.
10.Analysis on self-evaluation of Chinese physicians′ competency and its influencing factors

Objective:To understand the current situation of physicians′ competency in China and statistically analyze its influencing factors, hence providing referential evidences for promoting their competency.Methods:The evaluation scale for Chinese physicians′ competency was developed by referring to sophisticated overseas evaluation frameworks. Based on the electronic platform of the medical alliance, stratified sampling was conducted at 39 hospitals in 13 provinces, from which 2 156 physicians were surveyed, and statistical analysis was made on the current situation and influential factor of their competency in 6 dimensions(medical knowledge, diagnosis and treatment, scientific research and teaching, interpersonal communication, spirit of professionalism, population health)Results:The score range in multiple dimensions of physicians′ competency was between 2.85 and 3.89(5 in total), among which spirit of professionalism dimension scores the highest, and teaching and research dimension scores the lowest. Logistic regression analysis shows that their competency level in 6 dimensions is correlated to a range of factors, including gender, degree, title, supervisor qualifications, standardized training program of specialist physicians, and affiliation or not to teaching hospitals( P<0.05). Conclusions:Their overall competency is expected to be elevated. To name a few, they are lack of teaching and research abilities, and need to conduct clinical-oriented scientific research; they are expected to enrich knowledge structure, and need to embrace a wider concept of health; they need to improve their non-technical service abilities, and should further emphasize their humanistic quality; physicians′ practice competency depends upon their extent of medical education, and ensuring the quality of standardized training becomes the key measure.
