1.The effect of comprehensive intervention on the quality of life of esophageal cancer patients treated with radiotherapy

Objective To discuss the effect of comprehensive intervention on the quality of life of esophageal cancer patients receiving radiotherapy.Methods 100 patients with esophageal carcinoma treated with radiotherapy for the first timewere selected between October 2012 and December 2014 in Nantong Tumor Hospital in Jiangsu Province and randomly divided into control group and observation group (n =50 in each group) with a random number table.In the control group,routine dietary instruction,fluid infusion support and symptomatic intervention were used;comprehensive intervention was added in the observation group.Scores of core quality of life questionnaire (QLQ-C30) and nutritional indexesbefore and after radiotherapy in the two groups were compared.Results There were no statistically significant differences in QLQ-C30 scores and nutritional indexes between the two groups before radiotherapy (all P>0.05).After radiotherapy,the physical [(75.6 ±13.1) vs.(63.8 ± 12.4)] and emotional function [(61.9 ± 14.3) vs.(52.5 ± 13.7)],fatigue [(36.6 ± 13.2) vs.(45.8±15.0)],pain [(34.8±16.1) vs.(44.3±17.0)],insomnia [(49.2±15.7) vs.(57.2±14.3)],loss of appetite [(50.2 ± 16.2) vs.(59.0 ± 15.8)],nausea and vomiting [(21.5 ± 10.3) vs.(29.9 ±11.3)],general health status [(68.8 ± 13.4) vs.(58.2 ± 12.8)] inthe observation group were significantly better than in the control group (all P <0.05).Innutritional status,body weight [(59.3 ± 8.5) kg vs.(54.4 ± 7.3) kg],body mass index [(21.9 ±2.1) kg/m2 vs.(18.4 ±2.8) kg/m2],hemoglobin [(125.9 ± 8.9) g/Lvs.(107.3±9.5) g/L],albumin [(35.1±6.9) g/Lvs.(29.0±5.3) g/L],and prealbumin [(213.54 ±37.47) mg/L vs.(174.56 ±36.26) mg/L] in the observation group were all higher than thosein the control group after radiotherapy (all P < 0.01).Conclusion Comprehensive intervention for esophageal cancer patients receiving radiotherapymay improve nutritional status,reduce esophagitis,thirsty,fatigue,loss of appetite and other symptoms,improve the quality of life,ensure the completion of radiotherapy.
2.The relationship between the macular edema and the thickness of macular subfoveal choroid in diabetic retinopathy patients

Objective To evaluate the relationship between the existence of DME and the SFCT in DR pa-tients. Methods We collected 70 cases which were clinical diabetic involving 122 eyes. They are 36 men with 64 eyes and 34 women with 58 eyes. The average age is 56.7 ± 9.7 year-old. All patients had been examined by oph-thalmologic examination. Grouping: patients without diabetic retinopathy (NDR) group; mild-to-moderate NPDR group; severe NPDR group; PDR group. The last three groups were divided into two subgroup groupsrespectively , which were cases with and without significant macular edema (CSME + / CSME). EDI was used to measure the SFCT. SPSS 17.0 software was used for statistical analysis. Results The average SFCT of NDR group, mild-to-moderate NPDR group, severe NPDR group and the PDR group respectively were (282.94 ± 104.21)μm, (313.62 ± 94.40)μm, (382.44 ± 76.91)μm, (335.00 ± 73.82)μm. Compared with the NDR group, SFCT was thicker than the other three groups, and difference was statistically significant (F = 2.786, P = 2.786). There were no statisti-cally significant difference of SFCT between the mild-to-moderate NPDR/CSME - group and mild-to-moderate NPDR/CSME + group, severe NPDR/CSME - group and severe NPDR/CSME + group PDR/CSME-group, PDR/CSME + group. Conclusions The SFCT of DR patients could potentially be thickenedas disease became serious. When considered the same degree DR, there is no obvious difference of SFCT between patients with CSME and without CSME.
3.History and Research Progress of Asari Radix et Rhizoma Processing

Setting Li Dai Zhong Yao Pao Zhi Zi Liao Ji Yao as clue, this article searched each edition of China Pharmacopoeia, National Processing Standard of Traditional Chinese Medicine issued in 1988, and relevant materials of local processing standards and modern processing study, and concluded and combed the contents about Asari Radix et Rhizoma processing in ancient and modern literature. There are records about Asari Radix et Rhizoma processing methods in history, including net processing, cutting processing, heating processing and processing excipient. However, Asari Radix et Rhizoma processing methods in modern time are relatively simple. In addition to the version of the Processing Standard of Traditional Chinese Medicines in Shanghai issued in 2008, containing the honey Asari Radix et Rhizoma, other editions of China Pharmacopoeia and processing standards only recorded net processing and cutting processing. Recent research showed that processing methods of modern stir frying processing, alkali processing, wine processing, low temperature ultrafine comminution processing and so on, can effectively reduce the toxicity of Asari Radix et Rhizoma and enhance the safety of clinical application. This article summarized the history and research progress of Asari Radix et Rhizoma processing, and provided a reference for the study on modern processing.
4.The expression of glutamate receptor in hippocampus of neonatal rats with hypoxic-ischemic brain damage

Objective To study the expression of Ca-A/K channel-related molecules glutamate receptor 2 and 1(GluR2/1) in hippocampus tissues of neonatal rats with hypoxic-ischemic brain damage (HIBD). Methods A total of 60 7-day-old Sprague-Dawley rats were randomly divided into sham operation group and HIBD group. Hippocampal tissues were obtained at 0 h, 1 h, 6 h, 24 h, 48 h and 72 h after HIBD. The expression of GluR2, GluR1 and autophagy marker protein Beclin-1, LC3 were detected by Western blot assay. Results Edema and focal softening and necrosis were observed 6 h after HIBD in the brains of neonatal rats. Compared with Con group, at each time point, the expression levels of GluR2 were lower while the levels of GluR1, Beclin-1 and LC3 were higher significantly in HIBD group (P<0.05). The protein levels of LC3, Beclin-1, GluR1 and GluR2 in hippocampus tissues of HIBD group were significantly different among different time points after the estab-lishment of HIBD model (F=10.65~701.14, P<0.01). The protein level of GluR2 was decreased from 1 h to 24 h after HIBD and reached the lowest level at 24 h. The levels of GluR1, Beclin-1 and LC3 were increased at 6 h, plateaued at 24 h and remained there until 48 h. The levels of these proteins returned back to the initial level at 72 h. Conclusions Ca-A/K channel-related mol-ecules GluR2 and GluR1 play important roles in the autophagic cell death of hippocampus tissues in neonatal rats with hypoxic-ischemic brain damage.
5.A multi-center clinical trial of natural calcined bone repair material in repairing bone defect after tooth extraction

Objective:To study the efficacy and safety of natural calcined bone repair material(NCBM)in repairing bone defect af-ter tooth extraction.Methods:A randemized,double-blinded,parallel,positive control(Bio-Oss)and multi-center clinical trial was employed.Imaging examination was used as the main efficacy evaluation index,surgical wound healing,rejection reaction,bone me-tabolic changes,bone infection signs were the subordinate efficacy evaluation indexes,the incidence of adverse reactions was observed for safety evaluation.Results:280 cases were included,269 cases completed the trial.In NCBMand Bio-Oss group the effective rate of imaging examination was 93.08% and 93.70%(P >0.05)respectively.The wound healing time of the 2 groups was less than 7 days,no rejection reaction,bone metabolic change and bone infection sign were observed.The incidence of adverse events in NCBM and the Bio-Oss group was 0.72% and 2.14%(P >0.05)respectively.Conclusion:The efficacy and safety between natural cal-cined bone repair material is not inferior to Bio-Oss in repairing bone defect after tooth extraction.
6.Clinical analysis of 25 children with mild encephalitis/encephalopathy with a reversible splenial lesion

Objective To discuss the clinical manifestations,imaging features and prognosis of children with mild encephalitis/encephalopathy with a reversible splenial lesion(MERS).Methods Twenty-five patients with MERS admitted to Beijing Children′s Hospital,Capital Medical University,between November 2013 and March 2016 were enrolled and their clinical and imaging data were retrospectively analyzed.Ages of onset of these 25 cases were from 6 months to 13 years old.Because of different clinical manifestations in different onset ages,these 25 cases were divided into 2 groups:≤6 years old group (20 cases),with the onset age of 6 months to 3 years and 9 months old(average 2 years and 2 months);>6 years old group(5 cases),with the onset age of 9 years 3 months to 13 years old (average 10 years and 10 months).Results Nineteen cases among the 25 patients had infection history before onset,including 10 cases of digestive tract infection(all were ≤6 years old children),9 cases of respiratory tract infection(6 children ≤6 years old and 3 children >6 years old).The main clinical manifestations included convulsion (18/25 cases,72.0%),fever (17/25 cases,68.0%),vomiting (11/25 cases,44.0%),and disturbance of consciousness (11/25 cases,44.0%).The main clinical manifestation of ≤6 years old group was convulsion (18/20 cases,90.0%),while the main clinical manifestations of the>6 years old group were fever(3/5 cases,60.0%),headache and dizziness(2/5 cases,40.0%),and none of the patients in >6 years old group had convulsion.Eight cases had liver function injury,myocardial enzymes increased in 10 cases,and hyponatremia occurred in 9 cases.Magnetic resonance imaging (MRI) showed 21 cases were type Ⅰ MERS(only involving corpus callosum),and 4 cases of type Ⅱ MERS which involved corpus callosum as well as deep brain white matter,subcortical white matter (centrum semiovale).MRI lesions disappeared after 8-56 days (average 16.5 days) of anti-infection and reducing intracranial pressure treatment.Conclusion MERS is more common in ≤6 years old children,and digestive tract infection is common in ≤6 years old children,while respiratory tract infection is common in >6 years old children.The symptoms in children are mainly manifested as fever,convulsion,vomiting,conscious disturbance,and so on.Infection and hyponatremia are the main causes of MERS in children.MRI is the first choice of imaging examination methods.

In the item bank construction of medical microbiology examination questions,the accurate difficulty coefficient is an important parameter to guarantee the quality of the bank.Thus,the accurate difficulty estimate plays an important role in the item bank construction of examination questions.Methods of estimating the difficulty of medical microbiology test questions were explored,based on the knowledge points,the type of test questions,the structure of test questions and the degree of students’ familiarity with the questions and so on.Finally,the feasibility of methods was confirmed through the simulated tests.
9.Analysis of invalid detections in nucleic acid detection systems

Objective To analyze the reasons of invalid detections occurred in using three kinds of nucleic acid detection systems in our laboratory since 2016.Methods Analyze the numbers and types of invalid detections,for Roche Cobas S201 system from January to December 2016,Procleix Tigris system from January to September 2016,and Procleix Panther system from September 2016 to March 2017,respectively.Results The invalidation rates of Cobas s201,Tigris,and Panther systems were 0.90% (402/44 838),4.01% (2 960/73 835),and 1.34% (1 093/81 741),respectively,and there were statistically significant differences between the three detection systems (P<0.05).Except for the differences between Roche Cobas s201 and Panther 1404,there were statistically significant differences between instruments (P<0.05).Failure of instruments,invalid detection of reagent calibrators,fault operation and sample quality are the causes of invalid detection.Conelusion The main reason of invalidation is instrument failure and reagent calibrator failmre.Invalid detections of nucleic acid screening is related on different detection systems.
10.Expression of goat follicle-stimulating hormone analogous gene in Pichia pastoris.

In order to obtain the long-acting FSH preparation, the single strand long-acting analogous gene FSHbeta-CTP-alpha was successfully constructed by the C-terminal peptide(CTP) of carboxyl-terminal region of human chorionic gonadotropin with the goat FSHalpha-subunit and beta-subunit genes, then it was inserted into pPIC9K vector. The recombinant plasmid pPIC9K FSHbeta-CTP-alpha was transformed into Pichia pastoris GS115 by electroporation. The multi-copy inserts His+Mut+ were gained by the screening of phenotype and hyper-resistance to G418. After methanol induction, the supernatant was analysised by SDS-Polyacrylamide Gen Electrophoresis and Western blot. The results show that the transformants of FSHbeta-CTP-alpha could express the objective protein successfully and the molecular weight is about 29 kD. The concentration of supernatant was detected by Radio-immunoassay and the average expression of multi-inserts is 91.849 mIU/mL and the low-inserts is 37.419 mIU/mL. The expression of multi-inserts is higher than the low-inserts significantly. This research lay the foundation for studying the structure of FSH and the production of long-acting FSH preparation.
