Objective: To observe the relationship among blood pressure, left ventricular hypertrophy (LVH) and sinus heart rate turbulence (HRT) in patients with essential hypertension (EH) and its clinical significance. Methods: A total of 125 EH patients received 24h ambulatory blood pressure monitoring, dynamic electrocardiogram and echocardiography examination in order to measure 24h mean blood pressure, turbulence onset (TO), turbulence slope (TS) and left ventricular mass index (LVMI). According to 24h mean blood pressure, patients were divided into hypertension stage 1 group (n=46), stage 2 group (n=41) and stage 3 group (n=38); according to LVMI, patients were divided into non- LVH group (n=74) and LVH group (n=51). TO and TS were compared among all groups. Results: TO: Compared with hypertension stage 1 and stage 2 group, there was significant increase in TO [(-2.48±1.75) % vs. (-1.86±1.19) % vs. (-0.87±1.14) %] in hypertension stage 3 group, and that of stage 2 group was significantly higher than that of stage 1 group（P<0.05～<0.01）; compared with non- LVH group, there was significant increase in TO [(-0.15±0.45) % vs. (1.08±1.19) %] in LVH group, P<0.01. TS: compared with hypertension stage 1 and stage 2 group, there was significant decrease in TS [(3.76±1.87) ms/RR interval vs. (2.33±1.48) ms/RR interval vs. (1.55±1.06) ms/RR interval] in hypertension stage 3 group, and that of stage 2 group was significantly lower than that of stage 1 group （P<0.05～<0.01）; compared with non- LVH group, there was significant decrease in TS [(4.76±1.75) ms/RR interval vs. (3.02±1.08) ms/RR interval] in LVH group, P<0.01. Conclusion: The higher the blood pressure is, the more abnormality the heart rate turbulence is; the heart rate turbulence abnormality of patients complicated with LVH is more severe than that of non- LVH patients in patients with essential hypertension, so heart rate turbulence is help to judge prognosis and guide treatment.

Objective:To analyze one case of diabetes allergic to insulin preparations and investigate the role of clinical pharmacist in the treatment. Methods:Different insulin preparations were tried out after the patient was allergic to one kind of insulin preparation. However, the allergic symptoms were not improved significantly. Clinical pharmacist recommended to use glargine combined with oral hypoglycemic drugs instead of insulin containing protamine for the patient. Results:The allergic symptoms of the patient were improved significantly after the drug adjustment in the hypoglycemic program by clinical pharmacist. The blood glucose was controlled and aller-gic symptoms didn't show again during the hospitalization. Conclusion:Clinical pharmacist can participate in the treatment of diabetes allergic to insulin preparations through identifying allergens accurately and improving individual hypoglycemic program using pharmaco-logical knowledge. It is important to improve the clinical diagnosis and treatment, and ensure and promote the safety of medical treat-ment and drug use.

OBJECTIVE To analyze the correlation between number of tissue eosinophils(Eos) and blood Eos levels in patients with chronic rhinosinusitis with nasal polyps(CRSwNP), and to investigate if tissue and blood eosinophilia predicts disease severity and prognosis in CRSwNP. METHODS A retrospective analysis of 102 patients, who diagnosed with CRSwNP was carried out in this study. Surgical specimens were evaluated for degree of tissue Eos, which was compared with blood Eos. Patients were divided into two groups according to the results of endoscopy one year after surgery: recurrent and non-recurrent group. Statistical analysis was performed to compare tissue and blood Eos and CT Lund-Mackay scores. RESULTS Blood and tissue Eos in recurrent group was significantly higher than that in non-recurrent group(P=0.032, 0.033, 0.017). There was not a statistical difference of CT scores between two groups(P =0.089). There was a significant correlation between tissue Eos and absolute number of blood Eos(r=0.637, P=0.000) as well as between tissue Eos and percentage of blood Eos(r=0.542, P=0.000). However Eos and CT scores were not correlated statistically(P>0.05). CONCLUSION Blood Eos levels can predict the degree of Eos infiltration in nasal polyps. Blood and tissue Eos correlate with prognosis.

Objective To investigate the expression of Vacuolar-H +-ATPase (V-ATPase) and P-glyeoprotein (P-gp) protein in colon carcinoma tissues,the correlation between the expression of V-ATPase and P-gp and their clinicopathological significance.Methods In samples from 80 cases of colon cancer,20 cases of colon adenoma and 10 cases of normal colonic mucosa tissues,the expression of V-ATPase and P-gp protein were detected by immunohistochemical method,their relationship was analyzed,the clinicopathological features and prognosis were evaluated.Results In colon cancer,V-ATPase and P-gp protein expression was 72％ and 80％,higher than that in colon adenomas (40％,35％),and in normal colon mucosa (20％,20％),the difference was statistically significant (P ＜ 0.05).There was a positive correlation between the expression of V-ATPase and that of P-gp (r =0.567,P ＜0.01).V-ATPase and P-gp protein expression in colon cancer was associated with TNM stage,lymph node metastasis and tumor differentiation (P ＜ 0.05).Patients with high V-ATPase expression had lower 5-year survival rate than those with low V-ATPase expression (P =0.023),and 5-year recurrence rate was higher than those with low expression (P =0.024).Conclusions The expression of V-ATPase is up-regulated in colon cancer,there is a positive correlation with colon cancer progress and metastasis,and high V-ATPase protein expression predicts poor prognosis.

Objective To investigate and analyze clinical obstetrics and gynecology clinicians' mastery of basic skills, and then find the way to help clinicians master basic clinical skills. Methods A self-designed questionnaire survey was conducted among 310 clinicians working in 1-3 grade hospital from Chongqing, Sichuan, Guizhou, Yunnan provinces anonymously by spot field investigation and letter-investigations from February 1, 2015 and to March 31, 2015. The data were analyzed by descriptive analysis, using SPSS 19.0 software package for rank sum test analysis. Result Among 310 valid questionnaires, 31.6% (98) clinicians considered having inadequate clinical skills, 26% (81) average and 42.3%(131) expert. All the basic skills obtained had a relationship with the number of operations, working years and professional title. Most of the basic clinical skills were from working in hospitals 85% (270), journal 82% (254), academic conference 58% (174); 94.2% (293) willing to participate in training classes like clinical teaching and training in moulds. Conclusion The gynecology and obstet-rics basic clinical skills have a great relationship with basic skills training. Providing opportunities for the basic clinical skill trainings, academic conferences, in-service education and establishing regional medical source sharing platform can enhance their clinical skills.

Objective To investigate the effect of omeprazole combined with FOLFOX scheme as an adjuvant therapy for stage Ⅱ or Ⅲ colon cancer patients after a radical resection.Methods 98 stage Ⅱ or Ⅲ colon cancer patients in our hospital from January 2008 to December 2009 were randomly divided into study group (48 cases) receiving regimen of omeprazole combined with FOLFOX and control group (50 cases) treated with FOLFOX chemotherapy after radical colectomy.Surgical specimens were examined for expression of V-ATPase protein.Chemotherapy period was 6 months,8-12 courses.We observed results of follow-up curative effect,comparing the side effects and postoperative 2 year,3 year and 5 year disease-free survival rate (DFS) difference using statistical analysis.Results Study was completed in all 93 cases,5 cases were lost to follow-up.The baseline data distribution in the two groups were balanced basically.In study group the gastrointestinal side effects of chemotherapy was lower than the control group (x2 =4.924 6,P =0.026).In the two groups,the 2-year,3-year and 5-year DFS were 73％ vs 60％ (x2 =1.743 7,P =0.187),62％ vs50％ (x2 =1.4075,P=0.235),49％ vs40％ (x2 =0.8159,P=0.366) (P＞0.05).V-ATPase protein expression was 71％ (70/98) in all samples.The 2-year and 3-year DFS of patients for V-ATPase protein positive expression in the two groups were 75％ vs 51％ (x2 =3.970 8,P =0.046),66％ vs 40％ (x2 =4.399 5,P =0.036).Compared with the control group,the 2-year,3-year DFS increased in the study group (P ＜ 0.05).In stage Ⅲ colon cancer patients,the 2-year DFS was 73％ vs 47％ (x2 =4.504 5,P =0.034).Conclusions PPI combined with FOLFOX in V-ATPase protein positive expression or Ⅲ stage colon cancer patients after radical colectemy improves long-term survival,as well as reduces the gastrointestinal side effects of chemotherapy.

Objective To study the optimum technological conditions of macroporous resin separation and purification of polyphenol composition. Methods Fingerprint of Ilex paraguariensis was established by HPLC method. The best resin, loading conditions and elution requirements were screened with chlorogenic acid and elution rate of total active constituents served as the index. Results The optimum technological conditions were as follows:the separation carrier was D101 macroporous resin,the concentration of the sample liquid was 0.10 g?mL-1 ,the resin adsorption quantity of crude drugs was 1.5 g?g-1 ,the impurity was removed by four times of amount of water at first,and then eluted with four times the amount of 70％ ethanol in a gradient manner.After purification,the elution rate of chlorogenic acid was up to 82.83％,and the elution rate of total effective component was about 72. 48％. The resemblance of HPLC fingerprint was over 0. 98, and the components were balancedly recoveried. Conclusion The purification process of total polyphenols in Ilex paraguariensis by D101 macroporous resin was simple,reliable,and can be used for large-scale production.
