1.Effect of liposomal transfection of UCH-L1 siRNA on proliferation and apoptosis of lung cancer cell line H157.

BACKGROUND AND OBJECTIVESeveral reports demonstrated that the ubiquitin C terminal hydrolase-L1 (UCH-L1) has been found to be an oncogene in malignant tumors such as esophageal carcinoma, lung cancer and breast cancer. The aim of this study is to explore the effects of liposomal transfection of UCH-L1 siRNA on the proliferation and apoptosis of lung adenocarcinoma cell lines H157.
METHODSUCH-L1 siRNA was synthesized and transfected into H157 cell by liposome. Cell morphological change was observed with microscope, and cell proliferation and apoptosis index detected by flow cytometry, UCH-L1 mRNA expression was determined by RT-PCR and protein level of UCH-L1 was determined by Western blot.
RESULTSFor the H157 cell transfected with siRNA, cell proliferation was inhibited significantly, cell apoptosis appeared obviously, the expression of UCH-L1 mRNA and protein level of UCH-L1 significantly decreased.
CONCLUSIONUCH-L1 siRNA is able to inhibit the proliferation of lung adenocarcinoma cell lines H157 and induce the apoptosis. UCH-L1 might become a new target for lung carcinoma gene therapy.

2.Effects of electroacupuncture on the elderly's postoperation cognitive dysfunction after general anesthesia

Objective To study the effects of electroacupuncture (EA) on the elderly's postoperation cognitive dysfunction (POCD) after general anesthesia.Methods Patients who need to have surgical operation done for lumbar spinal stenosis were randomly divided into an observation group (electroacupuncture and routine intravenous anesthesia) and a control group (only routine intravenous anesthesia),and EA was administered to those in the electroacupuncture group for 30 minutes at an interval of 10 minutes,beginning at 30 minutes prior to routine intravenous anesthesia and lasted till the end of the surgery.The differences regarding intraoperative propofol and remifentanil dosage,cognitive function,the levels of serum interleukin-6(IL-6),interleukin-10 (IL-10) and S100-β were compared.Results The intraoperative propofol and remifentanil dosages in observation group were (6.54 ± 3.27) μg/(kg· h) and (3.67 ± 2.53) mg/(kg· h),respectively,while those in control group were (9.70 ± 4.82) μg/kg and (5.62 ± 1.73) mg/kg.The intraoperative propofol and remifentanil dosages in observation group were significantly lower than those in control group (P < 0.05).At 72 hours post-operation the incidence of POCD in observation group was significantly lower than that in control group (1 case,2.5% versus 8 cases,20%),while the MMSE score was higher in observation group than that in control group (25.7 ± 1.9 versus 23.2 ± 2.1).The post-operation level of serum IL-6,IL-10 and S100-β in observation group were significantly lower than that in control group (P < 0.05).Conclusion Electroacupuncture could significantly improve the elderly's postoperation cognitive dysfunction after general anesthesia.It could inhibit the excessive expressions of IL-6,IL-10 and S100-β,alleviate the brain damage and reduce the incidence of POCD.
3.Influence of New Labor Standards on the Indications in the Birth Process and the Prognosis of Mothers and Infants

Objective:To explore the influence of new labor standards on the indications in the birth process and the prognosis of mothers and infants.Methods:186 cases treated in our hospital from January,2015 to January,2016 were divided into the observation group (85 cases) and the control group (101 cases),the observation group received new labor standards,the control group adopt Friedman labor standards.The clinical indications,pregnant complications,pregnant outcome,neonatal-perinatal outcome were compared between two groups.Results:The cesarean delivery rate,number of using oxytocin,forceps delivery rate of observation group were significantly lower than those of the control group(P<0.05);the duration time of both first and second stage of labor were obviously longer than those of the control group (P<0.05);the duration time of active phase,bleeding volume in birth process in both groups showed no statistical difference (P>0.05);there was no adverse maternal and infant events in both groups;the incidence rate of pregnancy complications,fetal distress in uterus,asphyxia neonatorum and neonatal body weight were of no statistical difference (P>0.05).Conclusion:The new labor standards prolong the duration time of birth and give women fully trial opportunities,could effectively reduce the rate of cesarean section,reduce the over intervention production.
4.The intervention study of berberine about the imbalance of NE/a1-AT in mice with NASH

Objective To explore the effect of NE/α1-AT on the pathogenesis of NASH and the adjustment function of berberine to the imbalance of NE/α1-A and the therapeutic action of berberine to NAFLD.Methods 8-week-old male C57BL/6JApoE/ mice (26),according to the weight assigned to the high-fat-high-cholesterol group (18) and standard chow group (8).HFHC group fed with high fat high cholesterol diet and normal water;the SC group fed with normal diet and drank normal water.After 8 weeks,the HFHC group divided into HFHC group and BBR group according to the weight,Berberine group with 200mg/kg/d berberine,the other two groups fed with the same volume of distilled water.12 weeks all mice were sacrificed.We detected body weight,live weight,live function and lipid metabolism.HE and oil red O staining were used to evaluate pathological changes.ELISA method was used to detect NE and α1-AT expression in mice live tissue;RT-PCR method was used to detect protein α1-AT mRNA expression in live tissue; Immunohistochemistry and Western Blotting were applied to determine expression of NE.Results Compared with the SC group at the end of 12 weeks,ALT,AST were significantly increased in HFHC group,TG was significantly higher than the SC group.Body weight and live weight of ApoE-/-mice fed with HFHC diet significantly higher than the SC group.HE staining showed obvious steatosis and inflammatory foci in HFHC group.The NAS score at the end of model establishment was 5-8 points,which reached the diagnostic criterion for NASH.The concentration and activity of NE in the SC group were significantly increased,and the level of α1-AT was decreased,which lead to ratio NE/α1-AT was significantly higher than the SC group.BBR in mice treated with HE staining showed inflammation ballooning lower than HFHC group.At week 12,ALT,AST of BBR group increased compared with the SC group,and significantly lower compared with the HFHC group.NE/α1-AT was significantly higher than the HFHC group.Conclusions 12 weeks of high fat and high cholesterol feeding,ApoE-/-mice can successfully establish the NASH mouse model;a marked change and contact exist between NE and α1-AT in the process of development of NASH; Berberine can reduce liver lipid deposition and the degree of inflammation in NASH mice,and can improve the NE/ alpha 1-AT imbalance.
5.Role of glucose transporter 2 and 4 in insulin resistance in spontaneous hyperuricemia mice

Objective To investigate the role of glucose transporter ( GLUT ) 2 and 4 in hyperuricemia-induced insulin resistance. Methods Male uric acid oxidase gene knock-out spontaneous hyperuricemia mice ( KO) and wild-type mice ( WT) were fed with high-fat diet to establish an insulin resistance model. Then, some of KO mice were treated with allopurinol for lowering uric acid. Uric acid, fasting plasma glucose (FPG), and fasting insulin ( FINS) were detected. Intraperitoneal glucose tolerance test ( IPGTT ) and insulin tolerance test ( ITT ) were performed. Finally, the expression levels of Slc2a4 and Slc2a2 mRNA in tissues were determined by real-time PCR, while those of GLUT2 and GLUT4 proteins in tissues were analyzed by Western blot. Results There was no significant difference in FPG among various groups. The level of FINS in KO group was significantly higher than that in WT group [(0.636± 0.07) vs (0.456 ± 0.03) ng/ml, P<0.01], with decreased insulin sensitivity and impaired glucose tolerance. The uric acid level in the KO group remained at a high level [ ( 549. 68 ± 48. 7 ) vs ( 216. 61 ± 27. 5 )μmol/L] . After uric acid level in KO mice was reduced by allopurinol, insulin sensitivity and glucose metabolism were improved. Compared with WT group, the expression levels of Slc2a4 and GLUT4 in the gastrocnemius muscle were decreased while the expression levels of Slc2a2 and GLTU2 in liver were increased in KO group, which were reversed by allopurinol-mediated uric acid reduction. Conclusion Uric acid may induce insulin resistance via decreasing Slc2a4/GLUT4 expressions in skeletal muscle, and increasing Slc2a2/GLTU2 expressions in liver.
6.Clinical use of 'Full right-Full left’ split liver transplantation: a report of 4 cases

Objective:To review our experience in the use of "Full right-Full left" split liver transplantation in adult-to adult or adult-to adult-size child.Methods:The clinical data of liver donors to 4 recipients of full right-full left split liver transplantation performed at Beijing Friendship Hospital of Capital Medical University from January to December 2019 were reviewed. The surgical methods of split liver transplantation, cold ischemia time, operation time, intraoperative blood transfusion, and postoperative complications and prognosis were analyzed.Results:The 4 recipients of complete right hepatic-left hepatic split liver transplantation included 3 adults and 1 heavy child (45 kg). Their ages ranged from 14 to 48 years, and body weight ranged from 45 to 61 kg. The end-stage liver disease model score were 21, 12, 41, and 30 points. The ratios of graft mass to recipient's body mass ranged from 0.85% to 1.35%. The cold ischemia time was 457-650 min, and the operation time was 460-575 min. Early liver function recovered smoothly in all the 4 patients after transplantation, and no small liver syndrome occurred. Patients were followed up to 6 months after operation. One patient developed anastomotic biliary leak, which was cured by endoscopic retrograde cholangiopancreatographic treatment. Another patient developed biliary stricture presenting with repeated biliary tract infection despite percutaneous transhepatic puncture biliary drainage. A third patient died six months from lung infection.Conclusion:In properly selected patients, using full right-full left hemiliver by split liver transplantation increased organ utilization and provided patients with increased treatment opportunities.
7.Lowering the incidence of complications of portal vein through vascular graft interposition during pediatric transplantation

Objective:To explore the feasibility of using vascular graft interposition for lowering the complications of portal vein during pediatric liver transplantation.Methods:From June 1, 2013 to May 31, 2018, clinical data were collected for 297 children undergoing liver transplantation, including basic demographics, general preoperative status, preoperative tests, imaging findings, graft related profiles, surgical procedures and postoperative follow-ups, etc. Then the authors analyzed the effect of using interposition vessels upon lowering postoperative complications of portal vein reconstruction.Results:With a median age of 12 months, there were 153 boys (51.5%) and 144 girls (48.5%). The primary disease was mostly biliary atresia ( n=222, 74.7%). The median diameter of portal vein was 5 mm. There were 19 cases (6.4%) using vascular interposition. Among 20 cases of portal vein complications, there were portal vein stenosis ( n=17, 5.7%) and portal vein thrombosis ( n=3, 1.0%). After univariate analysis, binary Logistic regression analysis revealed that diameter of recipient's portal vein was an independent risk factor for the occurrence of portal vein complications after liver transplantation. Statistical analysis of children with portal vein diameter <4 mm ( n=90) was carried on and the results showed that there was no inter-group statistical difference ( χ2=3.061, P=0.080)on the occurrence of portal vein complications. Conclusions:Diameter of portal vein is an important factor affecting the strategic choice of portal vein reconstruction during pediatric liver transplantation and an independent risk factor for portal vein complications after liver transplantation. When the diameter of portal vein is ≤4 mm, using interposition vascular anastomosis shows no significant difference with other conventional modes.
8.Relationship between factors of gross tumor volume and locoregional failure of radical intensity-modulated radiation therapy for esophageal carcinoma

Objective Investigate the relationship between gross tumor volume (GTV)-related factors including GTV-T volume,the maximum thickness of the esophageal lesion plane and GTV-T volume/length(GTV-T volume divided by the length of the lesion calculated by the number of GTV-T layers) and the locoregional failure of radical intensity-modulated radiation therapy (IMRT) for esophageal carcinoma.Methods A total of 133 patients with esophageal cancer undergoing radical IMRT were enrolled.The factors related to GTV-T including GTV-T volume,the maximum thickness of the esophageal lesions,GTV-T volume/length were calculated.The relationship between GTV-T related factors and local recurrence of tumors was retrospectively analyzed.Results There was positively linear association between the locoregional failure rate of GTV-T and the volume of GTV-T.The volume of GTV-T tumor was 36 cm3,the maximum wall thickness was 2.5 cm,and the GTV-T volume/length was calculated as 5.3 cm2.These critical values could be utilized to predict the risk of locoregional failure of IMRT for esophageal carcinoma.Conclusions The GTV-T factors can be adopted to predict the local control and the risk of locoregional failure of radical IMRT for esophageal carcinoma to certain extent.
9. Application of Internet + micro course platform in training specialist nurses in Department of Cardiology

Objective:
To explore the new ideas and methods for training nurses, and improve the training effect of specialist nurses through the Internet + micro-course platform training model.
Methods:
Sixty-four nurses who participated in the training of specialist nurses in Department of Cardiology for 2017-2018 years were divided into two groups with 32 cases each according to the year. The control group received the traditional training mode, the experimental group applied the Internet + micro curriculum platform training mode, and after the training, two groups of students were investigated for nursing operation examination, theoretical examination, critical thinking scale, and feedback surveys were conducted to analyze the Internet + micro course platform training.
Results:
Before training, the theoretical examination, the operation examination, critical thinking scale were (85.78 ± 2.59), (85.66 ± 2.60), (347.50 ± 6.61) points in the experimental group and (86.75 ± 2.63), (86.38 ± 2.59), (348.81 ± 5.28) points in the control group, the difference was not statistically significant (
