1.Study on the efficacy and safety of dezocine applicated in postoperative analgesia for elderly patients

Objective To investigate the efficacy and safety of dezocine applicated in postoperative analgesia for elderly patients.Methods 50 elderly patients with elective cholecystectomy were selected as the research object,and they were randomly divided into the study group and control group.The control group patients took sufentanil + tropisetron treatment,while the study group took sufentanil + dezocine + tropisetron treatment.The postoperative analgesia sedation score,adverse reaction of the two groups were observed and recorded,and the contrast analysis was carried on.Results There was no significant difference in comparison of postoperative analgesia sedation score of the two groups (P > 0.05) ; The incidence of adverse reaction of the study group was 12%,which was significantly lower than 40% of the control group(x2 =20.37,P < 0.01).Conclusion Dezocine for postoperative elderly patients has obvious effect,postoperative analgesia and sedation,and high safety,which is worthy of reference.
2.Pharmacokinetics of Single Cefoperazone Sodium and Cefoperazone Plus Tazobactam in Dogs

OBJECTIVE:To study the pharmacokinetics of Cefoperazone in dogs and compare the pharmacokinetic differ?ence between single Cefoperazone and Cefoperazone plus Tazobactam.METHODS:Six healthy dogs were given intravenously a single dose of Cefoperazone Sodium and Cefoperazone-Tazobactam Sodium.Levels of Cefoperazone in dogs'serum were de?termined by RP-HPLC method.The pharmacokinetic parameters were obtained and compared between Cefoperazone Sodium and Cefoperazone-Tazobactam Sodium.RESULTS:The serum drug concentration-time course in dogs was conformed to the two-compartment model after a single dose of Cefoperazone Sodium and Cefoperazone-Tazobactam Sodium.Area under the concentration-time curve(AUC 0~∞ )was(486.17?117.98)、(469.74?71.49)(mg?h)/L,half-time(T 1/2? )was(0.96? 0.13)、(0.85?0.17)h,respectively.There were no significant differences between two groups.CONCLUSION:Taking AUC 0~∞ as index,two preparations are bioequivalent.
3.Analysis of CT antigen expression and humoral immunogenicity of NY-ESO-1 protein in gastric carcinoma

Objective:To study the expression of 11 cancer-testis (CT) genes and autologous humoral immune response to NY-ESO-1 in gastric carcinoma in China to provide some data for developing a vaccine against this cancer. Methods: Pathology tests were conducted in 101 gastric carcinoma samples,including tumor tissue and paired adjacent normal non-tumor tissue. Reverse transcription-polymerase chain reaction (RT-PCR) was used to test for 11 CT antigen gene expression and ELISA for autologous humoral immune response to NY-ESO-1.The antigen of NY-ESO-1 was also detected in tumor tissues by mAb E978 in IHC.Results: About 74.3% of the samples expressed at least one CT antigen,and the most frequently expressed one was MAGE-3 (41.6%). Twelve out of the 101 samples (11.9%) were found to be NY-ESO-1 mRNA positive. Simultaneously, autologous humoral immune response to NY-ESO-1 was detected in 6 of 12 NY-ESO-1 mRNA positive patients. Conclusion: Multiple CT genes were expressed in gastric cancer samples. MAGE-3,SSX-1,SSX-4, NY-ESO-1/LAGE-1, and MAGE-1 can be chosen as candidate antigens for cancer vaccines due to their higher expression rate. The potential of a multivalent CT antigen vaccine, including NY-ESO-1 and MAGE-3 along with other CT antigens, should be evaluated in patients with gastric cancer.
4.Effects of minimally invasive puncture and drainage of intracranial hematoma on the blood-brain barrier in patients with cerebral hemorrhage.

The effects of minimally invasive surgery on the blood-brain barrier (BBB) of 30 patients with cerebral hemorrhage were investigated. Difference of the BBB index and serum MBP concentration were assessed in 15 cases of conservative treatment group and 15 cases of minimally invasive surgery group. The BBB index in minimally invasive surgery group was significantly lower than in conservative treatment group (P<0.05), and the BBB index in the two treatment groups was significantly higher than in control group (P<0.01). Serum MBP concentration in minimally invasive surgery group was significantly lower than in conservative treatment group (P<0.05), and that in the two treatment groups was significantly higher than in control group (P<0.01). It was suggested the permeability of BBB in patients with cerebral hemorrhage was increased, and BBB index and serum MBP concentration in patients with cerebral hemorrhage were increased. Minimally invasive surgery can reduce the lesion of cytotoxicity to BBB and cerebral edema.
5.Analysis of nine cases of acute thallium poisoning.

In this study nine cases of thallium poisoning in a series of homicidal poisoning were analyzed in order to provide more information concerning thallium poisoning. It was found that the most common clinical feature of thallium poisoning was peripheral neuropathy and paraesthesia was more common than amyasthenia. Understanding of these clinical characteristics of thallium poisoning was helpful to early identification and differential diagnosis. Since the early administration of Prussian Blue, as a specific antidote for thallium poisoning, can substantially improve the prognosis, it is of great importance to establish a correct and early diagnosis.
6.Dependent effects of monoamine transmitters on the cardiac injury in cerebral hemorrhage rats

BACKGROUND: Incidence of cardiac pathological changes is high under acute cerebral damage, which affects its prognosis. The massive increase of plasma monoamine transmitters might be one of the key reasons of the dependent effects of the heart after cerebral apoplexy.OBJECTIVE: To investigate the effects of monoamine transmitters on the cardiac injury in cerebral hemorrhage in rats for the exploration of the mechanism for the generation and development of cerebral-cardiac syndrome(CCS).DESIGN: A prospective, randomized and controlled study using experimental animals as subjects.MATERIALS: Study was conducted in the Laboratory of Neurology of Daping Hospital. Seventy healthy Wistar rats in either gender with a body mass between 250 g and 300 g were obtained from the animal center of Daping Hospital affiliated to Third Military Medical University of Chinese PLA.INTERVENTIONS: 1 μL of normal saline containing collagenase(l μ/μL) and heparin(7 μ/μL) was injected into the right caudate nucleus of the rats to establish cerebral hemorrhage animal model, while 1 μL of normal saline was injected into the rats of control group.MAIN OUTCOME MEASURES: Plasma noradrenaline (NE) content was analyzed by high-potent electrochemical method, changes of serous creatine kinase-MB (CK-MB) were analyzed by enzyme reaction rate method.RESULTS: Both plasma NE and serous CK-MB significantly elevated at 6 hours since the formation of hematoma in the brain of the rat, which were (2. 782 ± 0. 244) μg/L and(3. 504 ± 0. 742) μkat/L respectively. Plasma NE and serous CK-MB most significantly increased at 24 hours, the peak of hematoma after cerebral hemorrhage, which were(3. 517 ± 0. 200) μg/L and(5. 474 ± 1. 540) μkat/L respectively. NE and CK-MB gradually decreased afterwards, and plasma NE returned to preoperative level at 72 hours [ (1. 663 ±0. 094) μg/L] while serous CK-MB still higher than preoperative level [ (2. 713 ± 0. 526) μkat/L].CONCLUSION: Peripheral NE content elevation might participate the generation and development process of CCS.
7.Minimally invasive treatment for 48 patients with lower extremities venous ulcers

Objective To study the efficacy of minimally invasive treatment for lower extremities venous ulcers.Methods A total of 48 patients with lower extremities venous ulcers(56 hmbs)wereexamined with venography to identify the primary lesion that resulted ulcer. According to the type and degree of hemedynamie abnormality,high ligation of the great saphenous vein combined with endovenous laser treatment,subfascial endoscopic perforator surgery,and external banding valvuloplasty of the superficial femoral vein were done in 8 eases(8 limbs),high ligafion of the great saphenous vein combined with endovenous laser treatment and subfascial endoscopic perforator surgery were carried out in 24 cases(28 limbs),and high figation of the great saphenous vein combined with endovenous laser treatment was performed on 16 cases(20 linbs).In all cases,the ulcers were eircumferentially ligated at the same time.Results After the operations,superficial varicosis,pigmentations,and eczematous relieved remarkably in the 56 limbs.In 51 limbs,heavy feeling,ache,and edema disappeared 14-18 days after operation.The ulcers of the 56 limbs healed l6-48 days after operation.During 4 to 46 months follow up,ulcers recurred in 4 limbs(7.1%).Conclusions It is effective to correct the incompetence of the superficial,perforating,and deep veins and manage the surface of ulcers for lower extremities venous ulcers.Endovenous Iaser treatment and subfascial endoscopic perforator surgery are easily performing,minimally invasive,and safe operations.The recurrence of leg ulcers can be reduced by external banding valvuloplasty of the superficial femoral vein.
8.Effect of Heat Reinforcing Acupuncture Manipulation on Articular Synovium LDH, SDH and CCO in Rheumatoid Arthritis ;Rabbits

Objective To investigate the regulating effect of heat reinforcing acupuncture manipulation on body energy metabolism enzymes in rheumatoid arthritis (RA) and preliminarily explain the mechanism of heat-producing action of heat reinforcing acupuncture manipulation. Method Forty chinchilla rabbits were randomized into normal, model, equal reinforcement and reduction, twirling reinforcement, and heat reinforcing acupuncture manipulation groups. A model of cold syndrome-type RA was made by ovalbumin induction and exposure to low temperature in the other four groups not including the normal group. From two days after successful model making, the normal and model groups were grabbed and fastened (bound) by the same way as for the acupuncture groups, 30 min once daily. The equal reinforcement and reduction group received even reinforcing-reducing method;the twirling reinforcement group, twirling reinforcement method;the heat reinforcing group, heat reinforcing acupuncture manipulation. The needle was manipulated for 1 min and retained for 30 min once daily, for a total of seven days. The RA rabbit knee joint circumference was measured and the inflammation score was recorded according to synovial histopathological sections before and after treatment. After the completion of intervention, the rabbits were sacrificed and the articular synovium was rapidly separated for frozen sections. Articular synovium lactate dehydrogenase (LDH), succinate dehydrogenase (SDH) and cytochrome oxidase (CCO) activities were measured by histochemical staining. Result After acupuncture intervention, the RA rabbit knee joint circumference was shortened in all the equal reinforcement and reduction, twirling reinforcement, and heat reinforcing acupuncture manipulation groups, but the shortening effect on the RA rabbit knee joint circumference was better in the heat reinforcing acupuncture manipulation group than in the equal reinforcement and reduction and twirling reinforcement method groups (P<0.05);the inflammation score recorded according to the RA rabbit synovial histopathological sections was also decreased more in the heat reinforcing acupuncture manipulation group than in the equal reinforcement and reduction and twirling reinforcement method groups (P<0.05). Synovial LDH integral optical density, total positive area and area percentage were significantly higher in the model group than in the normal group (P<0.05), and significantly lower in the equal reinforcement and reduction, twirling reinforcement, and heat reinforcing acupuncture manipulation groups than in the model group(P<0.05) and also in the heat reinforcing acupuncture manipulation group than in the equal reinforcement and reduction and twirling reinforcement method groups (P<0.05). Rabbit synovial SDH and CCO activities were significantly higher in the model group than in the normal group (P<0.05). Rabbit synovial SDH and CCO integral optical density, total positive area and area percentage were significantly higher in the equal reinforcement and reduction, twirling reinforcement, and heat reinforcing acupuncture manipulation groups than in the model group (P<0.05) and also in the heat reinforcing acupuncture manipulation group than in the equal reinforcement and reduction and twirling reinforcement method groups (P<0.05). Conclusion Heat reinforcing acupuncture manipulation has a definite therapeutic effect on RA and can increase SDH and CCO activities to enhance aerobic metabolism and produce more local energy in a rabbit model of RA. That may be the mechanism by which heat reinforcing acupuncture manipulation produces heat.
9.Construction of recombinant vector containing fusion gene NT_4-Ant-Shepherdin[79-87]

Objective To investigate survivin as an anticancer therapeutic target by use of shepherdin [79-87],a novel peptide carrying the survivin sequence from Lys-79 through Leu-87,we constructed an recombinant vector containing fusion gene NT4-Ant-Shepherdin [79-87].Methods The gene of Ant-Shepherdin [79-87] was obtained by PCR and T-vector method.After cloned and digested with restricted enzyme,Ant-shepherdin [79-87] was inserted in PBV220NT4 vector.The recombinant vector was transformed into the competent cell,E.coli DH5?.The fusion gene of NT4-Ant-Shepherdin [79-87] was identified by agarose gel electrophoresis (AGE).Results DNA sequencing results verified that the sequence of Ant-Shepherdin [79-87] was consistent with what we had designed.After transformed E.coli DH5?,a fragment of 321 bp was confirmed.Conclusion The recombinant vector containing fusion gene NT4-Ant-Shepherdin [79-87] was successfully constructed in this experiment by molecular biology techniques,which provides the basis of further research of survivin for cancer gene therapy.
10.The effect of modified early warning scores on nosocomial emergency transshipment in primary hospital

Objective To investigate the effect of modified early warning scores (MEWS)on nosocomial emergency transshipment in primary hospital.Methods From July 2013 to February 2014,192 patients were selected as control group,and from March 2014 to October 2014,192 patients were selected as observation group.The control group conducted assessment and transshipment by experience.The observation group adopted MEWS to evaluate dis-ease and applied targeted treatment.Then,we compared the incidence of unsafe events,accuracy of handover,family member and the related departments 'satisfaction.Results After the implementation of MEWS,the incidence of unsafe events was decreased from 13.0% to 5.2%(χ2 =5.121 ).The accuracy of handover was increased from 85.4% to 94.3% (χ2 =8.248).The family member and related departments 'satisfaction were improved from 89.1%,87.5% to 97.4% and 96.4%,respectively (χ2 =15.561,χ2 =10.141).The differences were statistically significant (P <0.05).Conclusion MEWS can evaluate and classify the safety of patients objectively and effective-ly during nosocomial emergency transshipment.MWES can effectively reduce the incidence of unsafe events,increase the accuracy of handover,improve family member and related departments'satisfaction.
