1.Effect of short-term continuous subcutaneous insulin infusion treatment on beta-cell function, blood fat and C-reactive protein of newly diagnosed type 2 diabetes mellitus patients

Objective To investigate the effect on short-term continuous subcutaneous insulin infusion (CSII) treatment on beta-cell function,serum triglyceride(TG),C-reactive protein(CRP) and serum insulin like growth factor-1 (IGF-1) of newly diagnosed type 2 diabetes mellitus(T2DM) patients.Methods Thirty-two newly diagnosed T2DM patients with fasting plasma glucose(FPG) > 11.1 mmol/L were treated with CSII for 2 weeks.FPG,2 h postprandial blood glucose (2hPG),fasting insulin (FINS),2 h postprandial insulin (PINS),fasting C peptide (FC-P),2 h postprandial C peptide (PC-P),glycosylated hemoglobin (HbA1 c),Homa-β,Homa-R,TG,CRP and IGF-1 were measured and compared before and after CSII.Results (1) Compared with that of before treatment,the levels of FPG,2hPG,HbA1c,PINS and PC-P in patients after treatment were dramatically improved(t =12.04,11.57,7.61,20.77,6.16; P < 0.05),and Homa-β increased from (23.0 ±16.2) to (71.0±23.8)(t=9.46,P<0.05),while Homa-IR significantly decreased from (6.8 ±2.1) to (3.9 ± 2.6) (t =4.81,P < 0.05).(2) The levels of TG and CRP significantly decreased from (1.50 ± 0.32) mmol/L,(3.14±1.14) to (1.29±0.31) mmol/L,(1.95 ±0.52) mg/L(t =26.73,23.62;P <0.05).The levels of IGF-1 significantly increased from (341 ± 16) μg/L to (471 ± 17) μg/L (t =8.65,P < 0.05).Conclusion The excellent glycemic control,improvement of lipid metabolism,anti-inflammatory and improvement of beta-cell function can be achieved with short-term CSII intensive therapy in newly diagnosed T2DM patients with severe hyperglycemia.
2.Expression and significance of estrogen receptor- β and tyrosine kinase receptor- B in endometriosis associated ovarian cancer

Malignant transformation of ovarian endometriosis is known as endometriosis associated ovarian cancer (EAOC). However, the carcinogenic pathways by which EAOC develops remained poorly understood, and numerous studies found the risk factors of malignant transformation. Recent studies have provided evidence that estrogen receptor-β(ER-β) can influence the proliferation, motility and apoptosis of ovarian cancer cells. The signal pathway of tyrosine kinase receptor-B (TrkB)/brain-derived neurotrophic factor (BDNF) has a direct relation with the endometriosis, and its anti-anoikis plays a prerequisite role in proliferation of cancer. In the nervous system, estradiol and estrogen receptor can be combined through a variety of ways to promote BDNF/TrkB high expression and activity enhancement. Therefore,the relationship between high-risk factors of malignant transformation and ER-β expression and ER-β and TrkB/BDNF signal pathways need to be explored in EAOC.
3.Effects of GLP-1 treatment on protection of B cells in Otsuka Long-Evans Tokushima fatty rats

Objective: To investigate the effect of GLP1 on the blood glucose in type 2 diabetic rats,and its protective effects on the islet B cells.Methods: Thirty rats were divided into three groups: spontaneous type 2 diabetes animal model OLETF rats,GLP-1 [from the twelfth week 56 ?g/(kg?d),sc] therapy group and LETO rats as control.In the 14th and 20th weeks,standard OGTT including fast and 2 h-plasma glucose were measured respectively.In the 14th weeks,3 rats from each group were killed randomly,and the rest of the rats were killed until the 20th week.Immunostaining with the marker of PCNA,TUNEL,and insulin assessed metabolic changes in the islet. 4.Observation on sterile effect of sterile adhesive tape on opened infusion bottle mouth

Objective To discuss the sterile effect of sterile adhesive tape on opened infusion bottle mouth. Methods From February to May, 2009, 400 infusion bottles were chosen and divided into the observation group and the control group with 200 bottles in each. The bottle mouth in the observation group was covered with sterile adhesive tape, while the control group used iodophor. The incidence of infusion reaction, disposable materials and time consuming of nurses were compared. Results No infusion reaction occurred in both groups. But disposable materials and time consuming was less in the observation group compared with the control group. Conclusions Application of sterile adhesive tape in sterilization of opened infusion bottle mouth is safe, reliable, simple and economic, it can save time and increase work efficiency of nurses.
5.Relation Analysis of Synovial Anti-Citrullinated Epitope Peptide Expression and Peptidyl Arginine Deiminase 4 Gene in the Rheumatoid Arthritis Patient

Objective To investigate the relation of synovial anti-citrullinated epitope peptide(CCP)expression and peptidyl arginine deiminase 4 (PADI4)gene in the rheumatoid arthritis patient.Methods From February 2013 to 2015 in Dongfeng General Hospital Affiliated to Hubei University,selected 110 patients with rheumatoid arthritis as the observation group, and selected 110 healthy people at the same period in this hospital for medical examination as the control group,both groups were given the synovial CCP expression positive rate and PADI4 gene expression detecting and correlation analysis.Results The synovial anti-CCP expression positive rates in the observation group and the control group were 70.9% and 9.1% re-spectively,and the observation group were significantly higher than the control group (P<0.05).The PADI4-104 genotype expression frequency compared between the observation group and control group,the difference were statistically significant (P<0.05).The G/G expression was more in the observation group,while the control group was more wit the C/G expres-sion.Pearson correlation analysis showed that in the observation group,synovial CCP positive expression were significant correlated to the expression of genotype PADI4-104 (r=0.344,P<0.05).Logistic regression analysis showed PADI4-104 genotype frequency were the main factors for the synovial CCP epitope expression (P<0.05).Conclusion The rheumatoid arthritis was more with synovial anti-CCP positive expression.There were also PADI4 polymorphism disorder expression, and clear correlation between the two.Thus impact the incidence of rheumatoid arthritis.
6.Outcomes of very low birth weight infants and extremely low birth weight infants fed either with human donors' milk or formula

Objective To evaluate the safety and effects of feeding very low birth weight infants and extremely low birth weight infants with human donors' milk in China.Methods One hundred and ninety-nine very low birth weight infants and extremely low birth weight infants (VLBWI/ELBWI) admitted to NICU in Nanjing Maternity and Child Health Care Hospital were analyzed retrospectively (from August 1,2013 to July 31,2014) one year before and one year after the establishment of human donors' milk bank.Only 1 infant's parents(1/199 cases) refused to use donor milk,and 198 infants were fed with donors' milk since their own mothers' milk was unavailable.One hundred and forty-eight infants admitted to NICU from August 1,2012 to July 31,2013 were chosen as the control group,who were fed with formula milk since their own mothers' milk was unavailable.The mortality,incidence of necrotizing enterocolitis (NEC) and sepsis,time to full enteral feeding and hospital stay between 2 groups were analyzed.Results One year before the establishment of breast milk bank,there were 148 cases of very low or extremely low birth weight infants receiving mixed feeding,and 16 cases of them had neonatal necrotizing enterocolitis,the incidence rate was 10.81%.Since the establishment of human milk bank,187 women donated breast milk in the first year and 260 555 mL of human milk were collected and delivered.Only 1 infant (1/199 cases) refused to use donor milk,and 198 infants were fed with donors' milk since their own mothers' milk was unavailable.Nine babies with VLBWI/ELBWI were diagnosed as NEC in donor milk feeding group,and 16 cases were diagnosed as NEC in formula-feeding group.The incidence rate of NEC was lower in the donor-milk-feeding group (4.52% vs 10.81%,x2 =5.02,P < 0.05).Donor-milk-feeding group reached full enteral feeding[(16.4 ± 5.3) d vs (18.2 ± 6.2) d,t =2.84,P < 0.05] and had shorter period of hospital stay[(35.1 ± 9.5) d vs (37.3 ± 10.4) d,t =2.05,P < 0.05] compared with formula feeding group.The mortality and incidence of sepsis had no difference between 2 groups (all P > 0.05).Conclusions Donors' breast milk is associated with a lower risk of NEC,sooner full enteral feeding and shorter hospital stay.Further research is needed to confirm these findings and measure the growth and long term neurodevelopment effects of donors' breast milk.
7.Prevention of mother-to-child transmission of hepatitis B virus:controversies and focuses of current strategies

Mother-to-child transmission (MTCT)is an important way of hepatitis B virus (HBV)transmission.Blocking the HBV MTCT has a great significance for the prevention and treatment of hepatitis B.This article reviews the current blocking strategies implemented in the antepar-tum,peripartum,and postpartum stages,and summarizes the controversies existing in the blocking strategies in different stages.The significance of HBV occult infection and germ cell transmission in the HBV MTCT is analyzed.The results indicate that the current strategies for the prevention of hepatitis B MTCT need further improvement.Attentions should be focused on HBV occult infection and germ cell transmission.
8.Effectiveness of Microresection Combined with Radiofrequency Ablation under Laryngoscope and Rigid Telescope for Laryngeal Precancerosis

Objective To investigate the effectiveness of microresection combined with radiofrequency ablation under laryngoscope and rigid telescope for laryngeal precancerosis. Methods Fifty-one patients with laryngeal precancerosis, which was diagnosed pathologically, were treated with microresection combined with radiofrequency ablation under laryngoscope and rigid telescope. The patients were followed up for over 6 months. The Human Milk Banking Association of North America(HMBANA) is a professional association for supporters of non-profit donor human milk banking.HMBANA develops guidelines for donor human milk banking practices.The construction of China's human milk bank has just begun in few areas in 2013.To further promote the development and implementation of donor human milk banking in our country,this review introduce the guideline for establishment and operation of a donor human milk bank which drafted by HMBANA in 2013.The guideline can provide the basis for the development of human milk bank in our country.
10.Expression of neurotrophic tyrosine kinase receptor TrkB in eutopic and ectopic endometium of women with endometriosis

Objective To investigate the expression of neurotrophic tyresine kinase receptor TrkB in the eutopic and ectopic endometium of women with endometriosis and evaluate the role of TrkB in the pathogenesis of endometriosis. Methods Seventeen women with endometriosis and 9 controls were studied. Expression of TrkB was investigated using RT-PCR, immunohistochemistry and Western blot, respectively. Results TrkB mRNA was expressed in eutopic endometrium in secretory phase and ectopic endometium of women with endometriosis, (23.51±0.51)% vs (35.29±0.67) % (P<0.05), but TrkB mRNA expression was not detected in control endometrium and eutopic endometrium in proliferative stage of women with endometriosis by RT-PCR. TrkB was expressed both on membrane (glycosylated receptor) and in cytoplasm (non-glycosylated receptor) of the glandular epithelial cells of ectopic endometium, and more often observed in cytoplasm of glandular epithelial cells of eutopic endometrium in secretory phase of women with endometriosis by IHC. In eutopic endometrium in secretory phase and ectopic endometium of women with endometriosis, the expression of TrkB protein was (0.12±0.02)% vs (0.37±0.01)% (P<0.05) by Western blot. Conclusions There is an overexpression of TrkB in eutopic endometrium in secretory phase and ectopic endometium of women with endometriosis. Full-length (glycosylated receptor) TrkB more often is overexpressed in ectopic endometium of women with endometriosis, and TrkB may act as a pathogenic role in formation of endometriosis.
