The iodine deficiency disorders surveillances were performed in different historical periods according to different surveillance plans in China as various monitoring purposes.Compared with other editions of surveillance plan,the biggest changes were to observe the urinary iodine level,the salt iodine level and the goiter rate in children and pregnant women in the county level in this National Iodine Deficiency Disorders Surveillance Plan (the 2016 edition).In this paper,the implementation details on this surveillance plan were further interrelated,which would play an instrumental role for the staff in iodine deficiency disorders prevention and control.
2.Clinical application of the expert type terminal of remote electronic fetal heart rate home monitoring system

Objective To investigate the clinical value of the expert type terminal of long distance electronic fetal heart rate home monitoring system in the application to self monitoring of pregnant woman at home in peripartum Methods All the pregnant women ( n =284) were divided into two groups Research group ( n = 134) contained 73 high risk gravida There were 78 high risk gravida in control group ( n =150) In the research group, self monitoring at home in 134 women was taken by the expert type terminal of long distance electronic fetal heart rate monitoring system through telephone and fetal movement counting The women were requested to auscultate and transfer fetal heart rate (FHR) to electronic FHR monitoring center in hospital Non stress test(NST) was made 1~2 times every week when fetal movement was active When women felt any abnormality of baby, she should immediately test NST and then the photograph of NST was send to FHR monitoring center by telephone Doctor would make a diagnosis and management in time according to the photograph of NST In the control group, the way of fetal monitoring was fetal movement counting and regular NST test in the outpatient clinic Results The incidence of abnormal NST was significantly higher in the research group than that in the control group (respectively 22 0% vs 13 5%, P 0 05) Conclusions The expert type terminal of long distance electronic FHR monitoring system through telephone is a new way of the FHR self monitoring at home The application of this method could ease mental press of the women, decrease significantly perinatal mortality, decrease incidence of neonatal asphyxia, and improve quality of obstetrics Whatever there are high risk factors in any pregnant women, the system should be appied to all late pregnant women
3.Clinical analysis of continuous electronic fetal heart rate monitoring for preterm small for gestational age during labor

ObjectiveTo investigate the clinical value of continuous electronic fetal heart rate (FHR) monitoring for preterm small for gestational age (PSGA) during labor. MethodsThree hundred and three pregnant women who had inevitable preterm labor at 32-36 weeks′ gestation because of premature rupture of membrane or unknown cause underwent continuous electronic FHR monitoring during labor from Jan 2002 to May 2004. In total, 78 newborns were preterm small for gestational age (PSGA ,PSGA group) and 225 newborns were preterm appropriate for gestational age (PAGA, PAGA group). Objective To evaluate whether the predictive value of admission hyperglycemia for mortality differs between diabetics and non-diabetics with acute coronary syndrome (ACS).Methods A total of 1534 consecutive patients admitted with ACS to Central Control Unit of Beijing Friendship Hospital were analyzed.Among these patients,1332 patients who had a venous plasma glucose record at admission were retrospectively enrolled and were stratified according to admission glucose levels with or without diabetes.The primary end point was in-hospital and 2-year all-cause mortality.Results In patients without a history of diabetes,649 patients had glucose level ≤7.8 mmol/L(group A),204 patients with 7.8-11.0 mmol/L (group B) and 142 patients > 11.0 mmol/L (group C).Of those with diabetes,124 patients were in group A,142 in group B and 126 in group C.By Cox regression analysis,admission hyperglycemia (group B,HR 1.48,P =0.05 ; group C,HR 2.78,P < 0.001) was an independent predictor of long-term all-cause mortality in patients without diabetes.In patients with diabetes,blood glucose > 11.0 mmol/L (group C,HR 2.68,P < 0.001) also independently predicted long-term all-cause mortality.Conclusions Compared with the diabetic patients,elevation of blood glucose at admission is common in ACS patients without definite history of diabetes.Admission hyperglycemia should be considered as a risk factor strongly correlated with in-hospital and 2-year all-cause mortality in patients with ACS.
5.Diagnostic value in the evaluation of mass-like lesions in dense breasts:digital breast tomosynthesis versus conventional mammography

Objective To compare the diagnostic value of digital breast tomosynthesis (DBT) versus digital mammography (DM) in the evaluation mass like lesions in dense breasts. Methods Eight hundred and fifty eight patients, who had DBT and DM, with breast diseases confirmed by pathology were retrospectively reviewed. The breast density and the imaging features were classified according to the standard of American College of Radiology(2013) and breast imaging reporting and data system (BI?RADS) scores. Six hundred and thirty one patients with mass lesions in dense breasts were chosen to included. All 631 patients had DM of both breasts and DBT of affected breast prior to mastectomy. Pathological results were used as the golden standard. Detection rate and diagnostic accuracy of masses, sensitivity and specificity of diagnosis, false negative and recall rates, the margins and spicules and BI?RADS category were calculated. The detection rate and diagnostic accuracy, the margins and spicules of masses were compared with Chi?square test, the BI?RADS scores were analyzed with rank test. Results With DBT, average detection rate increased from 77.3%(488/631) to 84.3% (532/631), average diagnostic accuracy rate increased from 73.4%(463/631) to 82.3%(519/631),with statistical significant difference between groups (c2=27.191,36.890,P<0.01).Sensitivity for DBT and DM were 68.1%(205/301) and 58.8%(177/301), respectively. Specificity for DBT and DM were 95.2%(314/330)and 86.7%(286/330), respectively. Recall rate for DBT and DM were 3.6%(23/631)and 9.8%(62/631), respectively. One hundred and seventy two cases and 75 cases with circumscribed masses of benign cases were detected by DBT and DM, respectively. One hundred and eighty-two cases and 115 cases with spiculated masses of malignant cases were detected by DBT and DM, respectively, with statistical significant difference between groups (c2=70.471 and 21.707, P<0.01). Of 330 malignant cases, the numbers of BI?RADS category 0, 2, 3, 4A, 4B, 4C, 5 on DM images were 19,4,8,13,52,93,141, respectively, the numbers on DBT images were 3,1,7,5,52,83,179, respectively, with statistical significant difference(Z=-2.235,P=0.025). Of 301 benign cases, the number of BI?RADS category 0, 2, 3, 4A, 4B, 4C, 5 on DM images were 43,10,60,117,58,11,2, respectively, the number on DBT images were 20,4,64,150,55,7,1, respectively, with no statistical significant difference between groups(Z=-1.846,P=0.065). Conclusion DBT significantly improves the detection of mass?like lesions in dense breasts, display of mass margin and architectural distortion of surrounding tissue.
6.Expert consensus on early prediction and diagnosis of diabetic kidney disease

Diabetic kidney disease (DKD) is a type of chronic kidney disease (CKD) caused by diabetes. The clinical diagnosis of DKD is usually based on the presence of increased albuminuria and/or decreased estimated glomerular filtration rate (eGFR), and exclusion of other causes of CKD. The clinical features of DKD are proteinuria, gradual decline in renal function, and severe renal failure in the later stages, which is one of the main causes of death in patients with diabetes. Any single biomarker might be insufficient to evaluate renal injury; thus, multiple methods and markers are needed. In addition, diabetic patients should be paid more attention to the kidney, and kidney damage should be evaluated with standardized assessment aimed at strengthening the early prediction and diagnosis of DKD.
7.The specific T cell immunity changes and its significance in invasive pulmonary aspergillosis patients

Objective To investigate the changes and significance of early immune response in specific T cell with invasive pulmonary aspergillosis(IPA) patients. Methods Peripheral blood mononuclear cells (PBMCs) were separated from whole blood of 8 cases of healthy individuals (healthy group) and 24 cases of IPA patients (IPA group, including 6 cases of pathological diagnosis, 9 cases of clinical diagnosis and 9 cases of tentative diagnosis), and the heat-inactivated Aspergillus fumigatus spores (Conidia) was used as an antigen to stimulate PBMCs produce Aspergillus-specific T lymphocytes. Interferon-gamma (IFN-γ) secreation, type and ratio of cytokine synthesis was examined. Results In IPA group, dot enzyme linked immunosorbent assay(ELISPOT) showed that the positive rate of IFN-γin pathological diagnosis patients and clinical diagnosis patients (5/6,7/9) was higher than that in tentative diagnosis patients (3/9). The positive rate of IFN-γin IPA group was 62.5%(15/24), in healthy group was 0 (0/8), and there was significant difference (P<0.05). The levels of CD4+T and CD4+T/CD8+T in IPA group were 0.202 0±0.085 6 and 1.01±0.34, in healthy group were 0.3853±0.1265 and 1.55±0.41. The levels of CD4+T and CD4+T/CD8+T in IPA group were significantly lower than those in healthy group ( P<0.05 or<0.01). The level of CD 8+T in IPA group was 0.298 5±0.069 1, and in healthy group was 0.257 6±0.102 6. The level of CD8+T in IPA group was 05). Conclusion Conidia as antigen can induce the specific Th1-type immune response of IPA, and display the immune status of the IPA patients, and can provide new ideas and methods for the diagnosis and assessment of the disease.
8.Aging performance of dental zirconia sintered by difference method

Objective:To observe the effects of aging performance on the property of dental zirconia sintered by microwave furnace and conventional furnace.Methods:48 zirconia wafer specimens were fabricated and randomly divided equally into 2 groups.The samples were sintered in a microwave furnace(Mgroup)and a conventional furnace(C group)respectively.The samples from the 2 groups were divided into 2 sub-groups and treated by room temperature and dry condition(A group)and 1 34 ℃ with 0.2 MPa pres-sure(B group,accelerated aging)respectively.The m phase fraction of the samples was determined by X-ray diffraction analysis.The biaxial flexural strength was examined by universal testing machine.The fracture cross section was observed by SEM.Results:CB and MB groups showed higher m phase fraction than CA and MA groups(P <0.05).The grain size of zirconia sintered by microwave method was uniform.The fracture surface of zirconia showed no obvious crack and no grain size change after accelerated aging.The biaxial flexural strength of the samples showed no significant difference among groups(P >0.05).Conclusion:The aging perform-ance of zircinia sintered by microwave method were similar to that sintered by conventional method.
9.Protective Effect of Phyllanthus Compound Ⅱon Experimental Hepatic Damage in Mice

【Objective】To observe the protective effect of Phyllanthus Compound Ⅱ(PCⅡ) on experimental hepatic damage in mice.【Methods】Sixty NIH mice were randomized into 5 groups: normal control group,model group,high-,moderate-and low-dose PCⅡ(110,55,and 27.5 g/kg respectively) groups,bifendate(150 mg/kg) group.Mice models of acute hepatic damage were induced with concanavalin A(Con A) and D-galactosamine(D-GalN)respectively.Serum levels of alanine transferase(ALT) and aspartate transferase(AST),and superoxide dismustase(SOD) activity were examined.The pathological changes of hepatic tissue were also observed.【Results】 PCⅡmarkedly decreased serum ALT and AST levels,and decreased hepatic SOD activity(P0.05 compared with bifendate group).The hepatic pathological changes were improved in PCⅡ groups.【Conclusion】PCⅡ exerts a better protective effect on mice hepatic damage induced by Con A and D-GalN,and its protective mechanism may be related with the inhibition of lipoperoxidation damage.
10.Agmatine inhibits formalin-induced protein kinase C gamma,phosphorylated cAMP response element binding protein,c-Fos and c-Jun up-regulation in the spinal dorsal horn

Aim To investigate the changes in the expression of protein kinase C gamma (PKC?), phosphorylated cAMP response element binding protein(pCREB)and immediate-early gene(c-fos and c-jun) in the spinal cord in formalin-induced inflammatory pain and study the effect of agmatine on the changes of PKC? activation, phosphorylation of CREB and expression of c-fos and c-jun.Methods Rats were decapitated at 10, 20 min or 2 h after intraplantar injection of 50 ?l 5% formalin and L_4, 5 spinal cords were dissected. Immunohistochemistry and western blotting analyses were used to observe the expression of PKC?, pCREB, c-fos and c-jun in the spinal dorsal horn and the effect of agmatine on the changes of their expression. Results Unilateral peripheral inflammation induced PKC? activation and CREB phosphorylation bilaterally while c-fos and c-jun expression ipsilaterally in rat spinal cord. PKC activity increased in membrane fractions with unchanged levels in the cytosolic fractions. Pretreatment intraperitoneally with 160 mg?kg-1 agmatine 15 min before inflammation significantly inhibited the activation of PKC? in the membrane fraction, suppressed the phosphorylation of CREB and the expression of c-fos and c-jun. Conclusion The mechanism of the analgesic effect of agmatine may be associated with inhibiting PKC? activation in the plasma membrane, CREB phosphorylation, c-fos and c-jun up-regulation which play roles in the hyperalgesia with peripheral inflammation.
