3.Evaluation of risks of skin sparing breast conserving surgery for early stage breast cancer

Objective To evaluate the oncologic risks of skin sparing breast conserving surgery for carcinoma of breast at early stage.Methods Serial sections of skin overlying the tumor were done for pathological examination in 50 cases of early stage breast cancer after skin sparing breast conserving operation.Results In 50 cases of breast conserving surgery,the extent of skin removal was 0.5-1.0cm in 21 cases,1.0-1.5cm in 25 cases,and 1.5-2.0cm in 4cases.Skin involvement was found in one patient with stage Ⅱ breast cancer,and the rate of skin involvement was 2%.Skin involvement was closely related with the changes in skin overlying the tumor,but not with the tumor size,pathologic types or axillary lymph node metastasis.Conclusion The rate of skin involvement in early stage breast cancer is rare,therefore if there are no abnormal clinical manifestations in the skin,skin sparing breast conserving surgery is safe.
4.Comparison of on -line hemodiafiltration and high -flux hemodialysis for correcting calcium and phosphate metabolic disorder in uremia patients

Objective To compare the effect on correcting calcium and phosphate metabolic disorder and the impact on quality of life in uremia patients between on -line hemodiafiltration and high -flux hemodialysis.Methods 90 patients with uremia,digital watches were randomly divided into QL -HDF group and HFHD group,Ca2 +,P3 +, serum parathyroid hormone(PTH)level were measured before and after dialysis blood.The survival quality of patients were evaluated by the KDQOL -SFTM1.3 scale.The incidence of adverse reactions were collected.The similarities and differences of two groups were compared.Results The comparison of Ca2 + levels in the two groups before and after dialysis showed no significant difference(P >0.05);Of the QL -HDF group after treatment,P3 + was (1.82 ± 0.19)mmol/L,PTH was (401.6 ±16.7)pg/L,which were significantly lower than those of QL -HDF group[P3 +(2.14 ±0.22)mmol/L,PTH(425.0 ±17.2)pg/L](t =24.256,21.059,all P <0.05)in addition to the burden of kidney disease;life(BKD),the other ten areas scores of the QL -HDF group were higher than those of the HFHD group;The symptoms and discomfort(SPL)and the effect of nephropathy in life(EKD),cognitive function(CF), quality(QSI),social function(SEXF)and sleep(SLEEP),patient satisfaction(PS)had significant differences in areas such as score(all P <0.05),working condition(WS)and social support(SOS),dialysis staff support(DSF) and other domain scores had no significant difference (all P >0.05 );6 cases of adverse reactions was in the QL -HDF group (13.33%),14 cases of adverse reactions was in the HFHD group(31.11%),there was significant difference between the two groups(χ2 =5.361,P <0.05).Conclusion The two methods can improve the metabolic disorders of calcium and phosphorus in patients with uremia,and QL -HDF showed stronger ability to remove the ability,and the quality of life is higher,it is worth promoting in clinical practice.
5.Retention Behavior of Acid Drugs in Microemulsion Liquid Chromatography

The retention mechanism of acid drugs such as naproxen, ibuprofen, diclofenac, phenylalanine and 2,4-dichlorophenoxy acetic acid in microemulsion liquid chromatography has been discussed.The reten tion modeling between capacity factors (k') of acid drugs in microemulsion liquid chromatography and concen tration of surfactant, cosurfactant and lipophilic organic solvent and pH value was set.The results showed the effect of change of concentration of surfactant, cosurfactant and lipophilic organic solvent and pH value on capacity factors of acid drugs in microemulsion liquid chromatography was fully consistent with the theoretical modeling.The retention modeling can better reflect the effect of composition of microemulsion solution and pH value on retention behavior of acid drugs in microemulsion liquid chromatography.
6.The Experimental Study of the Effects of Different Swimming Load Training on Rat Brain Morphology

Under the different load of training stimulating, the changes in center nervous system of rat were observed.This experimental training intensities included normal -load and over-load using a swimming pool with circular water flow in order to investigate the effects on the neuron size and the density of dendritic spine of the large pyramidal cells in layer Ⅴ in motor areas of cerebral cortex.This study lead to the following conclusions:the size of nucleus and nucleolus and the density of spines of apical dendrites, insidal dendrites and basal dendrites of large pyramidal cells in layer Ⅴ in motor areas of cerebral cortex of normal-load and over-load group were higher than those of control group.It improve the capacity of brain in information processing.
7.The value of ultrasound in thyroid nodules management: analysis of ultrasonographic features in 123 patients with thyroid nodules

Objective To evaluate whether ultrasonographic features of thyroid nodules are associated with histological malignancy, and to set up helpful criteria in identifying those who should accept subsequent fine-needle aspiration biopsy (FNAB). Methods 146 thyroid nodules of 123 patients accepted ultrasonographic evaluation and FNAB. All patients but one with suspicious or malignant cytology were operated. Results Out of total 146 nodules, 43 were histologically malignant, 102 were cytologically benign, and 1 nodule with suspicious cytology was not followed. Hypoechoic pattern, blurred margins, irregular shape, microcalcification, and type Ⅲ vascularity were significantly more frequent in malignant than benign nodules (P < 0. 05). No one of the ultrasonographic features could independently predict malignant nodules. A better compromise is probably to apply FNAB in nodules with at least one or two malignant ultrasonographic signs. Conclusion No single parameter could satisfactorily identify a subset of patients to be selectively examined by FNAB. A cost-effective approach to FNAB should depend on combination of specific ultrasonographic features.
8.The effects of compound of paeonol and PNS on expressions of collagenⅠandⅢprotein and mRNA in rat model of acute myocardial infarction

Objective To study the effect of paeonol (PAE) and panax notoginseny saponins (PNS) on the expressions of collagenⅠandⅢprotein and mRNA in rats with acute myocardial infarction (AMI), and to explore the molecular mecha?nism of improving myocardial fibrosis. Methods The rat model of AMI was made using the left anterior descending coro?nary branch ligation.According to the intervention rats were divided into model group, PAE (8 mg/kg) group, PNS (40 mg/kg) group, PAE (4 mg/kg)+PNS (20 mg/kg) group, PAE (8 mg/kg)+PNS (40 mg/kg) group and captopril positive control group (10 mg/kg). Sham operation group, only wear line without ligation. The left ventricular mass index (LVMI) was detected after treatment for 28 d. Masson staining was used to observe changes of myocardial fibrosis. Western blot assay and RT-PCR technique were used to detect protein and mRNA expression levels of collagenⅠandⅢ. Results The values of LVMI were increased in model group compared with those of sham operation group and treatment groups. Compared with PAE group and PNS group, values of LVMI were significantly decreased in PAE (4 mg/kg)+PNS (20 mg/kg) group and PAE (8 mg/kg)+PNS (40 mg/kg) group. There was a more significant decrease in formula high dose group (P < 0.01). The model group showed pathological change. There were different degrees of improvement in pathological structure in all treatment groups, more sig?nificant improvement was found in formula low dose group, formula high dose group and captopril positive control group. There were different degrees of increase in expressions of collagenⅠandⅢprotein and mRNA in model group compared with those of sham operation group and treatment groups. Compared with PAE group and PNS group, the protein and mRNA expression levels of collagenⅠandⅢwere significantly decreased in formula low dose group and formula high dose group,more significant decreased was found in formula high dose group (P<0.05). Conclusion Compound of paeonol and PNS can improve myocardial fibrosis in myocardial infarction rats, which may be related with reduced expressions of collagenⅠandⅢprotein and mRNA.
9.The effects of combination of paeonol and PNS on TGF-β/Smad pathway in ventricular remodeling after myocardial infarction in rats

Objective To study the effect of paeonol (PAE) and PNS on the expression of transforming growth factor (TGF)- beta 1/ Smad2/3 pathway in rats with acute myocardial infarction (AMI), and the possible molecular mechanism thereof. Methods Model of AMI was made using left anterior descending coronary branch ligation. According to the inter?vention methods rats were divided into model group, PAE group (8 mg·kg-1), PNS group (40 mg·kg-1), PAE (4 mg·kg-1)+PNS (20 mg·kg-1) low dose group, PAE (8 mg·kg-1)+PNS (40 mg·kg-1) high dose group and captopril positive control group (10 mg · kg-1). Rats without ligation were used as Sham operation group. Left ventricular systolic blood pressure (LVSP), left ventricular diastolic pressure (LVEDP) and the maximum rise and fall rate (/dtmax DP) were detected after 28-day treat?ment. HE staining was used to observe changes of myocardial tissue. The protein expression levels of TGF-β1 and Smad2/3 were detected by Western blot assay. Results There were significant differences in parameters used for detecting treatment group and model group, formula group and single drug group, formula high dose group and formula low dose group (P <0.01). The model group showed pathological changes. All treatment groups showed different degrees of pathological improve?ment. There was the most significant improvement in formulae group and captopril group. Compared with the model group, TGF-β1 and Smad2/3 protein expressions were decreased in treatment group. The expression levels of TGF-β1 and Smad2/3 were significantly decreased in formula group than those of PAE group and PNS group, and lower levels in formula high dose group than those of formula low dose group (P<0.05). Conclusion Paeonol and PNS can inhibit the expressions of TGF-β/Smad 2/3 protein in rats with AMI, by blocking TGF-β/Smad pathway.
10.Establishment of a rat model with pulmonary arterial hypertension and its functioning mechanism

Objective To study the alteration of hepatocyte growth factor and c-met in the lungs of rats with pulmonary arterial hypertension and its possible underlying mechanisms.Methods Left pneumonectomy plus monocrotaline injection was used for rat PAH model.Rats were executed after measurement of their pulmonary arterial pressure at 1,2, 3 and 4 weeks after MCT injection.Then the right lung and heart were harvested for further analyses.Expression of HGF and c-met was determined by RT-PCR and Western blot.Expression of eNOS and caspase-3 was analyzed immunohistochemically and intrapulmonary TGF-β and ET-1 was analyzed with ELISA.Results Compared with the normal controls, manifestations of right heart hypertrophy and failure, well-elevated pulmonary arterial pressure were observed 28 days after left lung resection and MCT injection.Pulmonary vascular remodeling (i.e., pulmonary fibrosis, hyperplasia of pulmonary arterial endothelial cells and vascular smooth muscle cells in tunica media, as well as decease of vascular density) was observed.Intrapulmonary HGF expression decreased in a time-dependent manner at both mRNA and protein levels 4 weeks after MCT injection, while c-met stayed unchanged.Immunohistochemically, expression of eNOS was reduced and caspase-3 strengthened.On the contrary, ET-1 and TGF-β were obviously up-regulated(P < 0.01).Conclusion Intrapulmonary HGF expression was obviously downregulated in PAH rats, accompanying with reduction of NO and increase of ET-1 and TGF-β expression, while c-met still unchanged.
