2.Netherton syndrome caused by serine protease inhibitor of Kazal type 5 gene mutation: a case report and literature review

Objective:To investigate the clinical and molecular genetic features of neonatal congenital Netherton syndrome (NS) caused by mutations in serine protease inhibitor of Kazal type 5 ( SPINK5) gene. Methods:This study retrospectively analyzed the clinical data of an NS neonate admitted to Shanghai Children's Hospital in November 2018. SPINK5 gene was analyzed using high-throughput sequencing and Sanger sequencing. Relevant articles were retrieved from various databases including China National Knowledge Infrastructure, Wanfang and PubMed, and the reported cases who were diagnosed as NS within two months after birth with SPINK5 gene sequencing results were reviewed. Clinical features, gene mutations, treatment and follow-up results of NS were summarized using descriptive statistical analysis. Results:The patient presented with diffuse erythema and desquamation, sparse hair and repeated infections shortly after birth. Laboratory tests revealed elevated IgE (111 IU/ml) and "invagination-like" change in the hair under optical microscope. SPINK5 gene analysis found that there were compound heterozygous mutations of c.2468dup (p.Lys824Glufs*4) and c.377_378del (p.Tys126*) in the child. The pedigree analysis found that the two mutations were respectively inherited from the father and the mother, which supported the diagnosis of NS caused by SPINK5 gene mutation. Though skin rash improved after comprehensive treatments including anti-infection therapy, gamma globulin injection and skincare, the patient suffered from recurrent infection and was discharged from the hospital after giving up treatment and died of infection at two months old. Eleven NS cases were retrieved from literature and altogether 12 cases were analyzed here. The most common clinical manifestations in the 12 patients were early skin diffuse erythema and desquamation (12/12), infection (8/12), dry hair (7/12), hypernatremia dehydration (7/12), high IgE (5/12), growth retardation (4/12), respiratory failure (3/12), atopic constitution (2/12), diarrhea (2/12), dysphagia (1/12), hypothermia (1/12), wheezing (1/12), hypertension (1/12), liver failure (1/12) and metabolic alkalosis (1/12). Conclusions:NS is caused by SPINK5 gene mutation with atypical manifestations in neonates. Neonates with diffuse erythema and desquamation of the skin, repeated infections, dry hair and especially with high blood IgE should be considered the possibility of NS. Genetic testing is conducive to early diagnosis, guiding treatment decisions and providing a basis for genetic counseling.
3.Study on the training of emergency nurses under the Trinitarian mode

Objective To explore the methods of training emergency nurses under the Trinitarian Mode.Methods 77 emergency nurses were distributed to different groups and received different training contents and assessment criteria stratifiedly as well as in phases according to their working ages.The training content contained emergency capability,emergency specialist skills,evidence based nursing,nursing scientific research,and clinical practice,etc.Results Through the on-the-job training,their emergency caring ability were overall improved,the human resource was effectively utilized,and the overall ability for coping with public health emergencies had made great progress.Conclusions Under the Trinitarian emergency treatment mode,a diversified on-the-job training for the emergency nurses will promote the development of emergency care,as well as help keep pace with the rapid development of emergency medical sciences.
4.Study on purchase management of medical equipment based on ternary linear regression model

Objective:To pursue a scientific approach for medical equipment purchase, set up a standard purchase model, and try to transform the extensive and empirical way in purchase management into a scientific way.Methods: Analyze and test the data using linear regression method on yearly revenue, equipment value, yearly volume of work and yearly maintenance and repairs fee for 25 kinds of equipment in one hospital.Results: Assuming other parameters do not change, for each additional 10,000 yuan in equipment value, the revenue of the hospital increases by 14,858 yuan; for each additional 10,000 times in the volume of work, the revenue increases by 2,214,407 yuan; for each additional 10,000 yuan in maintenance, the revenue decreases by 132,683 yuan.Conclusion: Based on the scientific approach on equipment purchase management, we should improve the efficiency of medical equipment usage, scientifically mange the maintenance and repairs of equipment, in order to provide more accurate information and data for equipment purchase in hospital and make the purchase management more scientific, objective and rational.
5.Changes and the clinical significance of serum 25-Hydroxyvitamin D level in patients of Xinjiang Uygur and Han nationalities with mild cognitive impairment

Objective To investigate changes and the clinical significance of serum 25-Hydroxyvitamin D level in patients of Xinjiang Uygur and Han nationalities with mild cognitive impairment.Methods On the base of early stage of epidemiological survey of Xinjiang Uygur and Han population over 60 years old,adopting multi-stage stratified random cluster sampling method and referencing DSM-Ⅳ,206 cases of MCI patients (study group)were selected and 412 healthy elderly subjects (control) matching for age,nationality and gender were recruited for case-control study.The serum level of 25(OH) D was measured by enzyme-linked immunosorbent assay (ELISA).Results The serum level of 25(OH)D was significantly lower in MCI group of two nationalities (7.89±0.29 μg/L) than in normal control group [(8.65± 0.21) μg/ml,P<0.05].The serum level of 25(OH)D was (8.41±0.34) μg/L in Han group,and (8.37±0.32)μg/L in Uygur group,with no significant difference (P>0.05).The 25 (OH)D level was significantly lower in female[(8.23± 0.32) μg/L] than in male [(8.70±0.42) μg/L,P<0.05].The serum level of 25(OH)D was lower in female MCI group [(7.56±0.34) μg/L] than in female control group [(8.41 ±0.31)μg/L],with significant difference (P<0.05).While there was no significant difference between male MCI and male control group (P>0.05).Multivariate Cox regression analysis showed that the risk factors of MCI might include low serum 25 (OH) D levels (OR =1.163,95 % CI:0.978-1.447),a decreased physical labor (OR=1.420,95% CI:1.10-1.860),low education degree (OR =1.504,95% CI:1.129-2.130),celibacy (OR=1.293,95%CI:0.845-1.440),hypertension (OR=1.954,95%CI:1.342-2.670),diabetes (OR =1.320,95% CI:1.145-1.710),smoking history (OR =1.350,95% CI:1.101-2.293),high BMI (OR=1.329,95%CI:1.068-1.781).Conclusions The serum 25(OH)D level is lower in MCI patients in Xinjiang Uygur and Han populations than in normal people,but there is no significant difference.The 25 (OH)D deficiency is associated with increased odds for cognitive impairment in the elderly Xinjiang population,which is more obvious in the female.The occurrence and development of MCI is affected by multiple risk factors.
6.Clinical Observation of Sertraline Hydrochloride Combined with Esomeprazole in the Treatment of Patients with Gastroesophageal Reflux Disease Complicating with Depression

OBJECTIVE:To observe clinical efficacy and safety of sertraline hydrochloride combined with esomeprazole in the treatment of patients with gastroesophageal reflux disease complicating with depression. METHODS:80 patients with gastroesopha-geal reflux disease complicating with depression selected from our hospital were divided into control group and observation group according to random number table,with 40 cases in each group. Control group received Esomeprazole magnesium enteric-coated tablets orally 40 mg,qd;observation was additionally given Sertraline hydrochloride tablets 50 mg,qd,on the basis of control group. Both groups received treatment for 1 month. Clinical efficacies of 2 groups were observed as well as clinical symptom score,gastroesophageal reflux disease diagnostic questionnaire (GERD Q scale) score,SDS score. The occurrence of ADR was compared between 2 groups. RESULTS:The total effective rate of observation group 92.50%,which was significantly higher than 77.50% of control group,with statistical significance (P<0.05). Before treatment,there was no statistical significance in clinical symptom score,GERD Q scale score and SDS score(P>0.05). After treatment,above scores of 2 groups were improved signifi-cantly,and the observation group was significantly better than the control group,with statistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Sertraline hydrochloride com-bined with esomeprazole shows significantly therapeutic efficacy in the treatment of patients with gastroesophageal reflux disease complicating with depression,and can effectively alleviate the clinical symptoms and improve depression with good safety.
7.Orthognathic surgery in the treatment of condylar osteochondroma.

OBJECTIVETo evaluate the effect of orthognathic surgery in the treatment of condylar osteochondroma.
METHODSA total of 12 cases of condylar osteochondroma were treated with Le Fort I osteotomy, intraoral vertical ramus osteotomy, sagittal split ramus osteotomy and genioplasty.
RESULTSNo recurrence of condylar osteochondroma was observed in all 12 cases two years after the treatments. Facial asymmetry was obviously corrected.
CONCLUSIONThe orthognathic surgery methods are advantageous in improving facial figure without scar development in the treatment of condylar osteochondroma.

8.Xinjiang Uygurs and Hans present situation analysis of cardio-cerebral vascular diseases in the middle-aged and elderly people in the southern border region

Objective To understand the status and distribution of the Xinjiang Uygur and Han middle-aged and elderly population who suffer from cardio-cerebral vascular disease.Methods From July 2004 to June 2007,multi-stage stratified random cluster sampling methods was adopted to collect the random samples older than 50-year-old suffering from cardiovascular and cerebrovascular disease in South Xinjiang.Results In the 3685 Uygur and Han ethnic elderly people,the prevalence of hypertension,diabetes,lipid disorders,cerebrovascular disease and dementia were 28.20%,6.70%,15.39%,12.46% and 4.10% respectively in the southern border regions of Xinjiang.Except hypertension,the prevalence rate of other cardiovascular and cerebrovascular diseases in Uygurs and hans had significant differences.The survey found that the prevalence rate of dyslipidemia and diabetes in Hans was higher than the Uygurs,while the Uygurs prevalence rate of cerebrovascular disease and dementia.Conclusion The Xinjiang Uygur and Han elderly are faced with a grim situation of cardiovascular and cerebrovascular diseases;there are significant differences for the nationality and gender.
9.Comparative studies on perioperative hemodynamics in coronary artery bypass with or without cardiopulmonary bypass

Objective To analyze the hemodynamic changes during coronary artery bypass grafting with or without cardiopulmonary bypass.Methods Thirty patients undergone coronary artery bypass grafting from January to June,2006 were retrospectively analyzed.Among them,12 patients were operated on with cardiopulmonary bypass(CCABG)and 18 without cardiopulmonary bypass(OPCAB).The stabilizator(type Octops III)was used to fix target vessels in OPCAB group.In CCBAG group,the extracorporeal circulation was used routinely,and internal mammary artery and saphenous vein were used to anastomose with target branches of coronary artery.The hemodynamic changes at different time points during and after operation were monitored,and the relevant parameters were also recorded.Results The anaesthesia time,intraoperative blood loss and fluid input,and ventilating time after operation in CCABG group were significantly higher than those in OPCAB group,and the postoperative haematocrit(HCT)was significantly lower in CCABG group than that in OPCAB group(P

70 years, history of stroke, and a tear of the pleura. Conclusions The incidence of AF in CABG and off-pump CABG were almost similar. Old age of the patient was the pathological basis for developing AF, and the history of stroke and a tear of the pleura were the predisposing factors.
