1.Clinical significance of MRD detection in ALL-B of children by FCM

Objective To evaluate the clinical significance of minimal residual disease (MRD)detecion in ALL-B of children by flow cytometric (FCM).Methods 52 cases of children with ALL-B were performed bone marrow MRD by FCM analisis after induction therapy,3 moths therapy,and 6 moths therapy.After that,MRD detection was performed every 6 months. According to disease risks, three group were categorized,standard risk (SR),imidiete risk (IR) and high risk(HR).Results After 6 months,SR groups MRD positive cases were 4/21(19 %),IR groups MRD position cases were 8/23 (35 %),HR groups MRD position cases were 5/8 (63 %).9 cases relapsed in all 52 patients.There were significant differrence in replased rate between the positive and negtive MRD (P<0.001). Conclution The dynamic detection of MRD by FCM can be used to evaluate the therapeutic effect and prognosis of children with ALL-B. It is also useful in adjusting treatment strategy and for following up in children with ALL.
2.The correlation research between three ultrasonic technologies for the measurement error in breast cancer and the expression of ER, PR and VEGF

Objective To explore the three ultrasonic technologies of two -dimensional ultrasound(2D -US),ultrasonic elastography(UE) and contrast -enhanced ultrasound(CEUS) for the measurement error in breast cancer and the correlation with the expression of ER,PR,VEGF.Methods 50 patients with breast cancer were meas-ured by 2D -US,UE,CEUS preoperatively,and the pathological specimen were measured postoperatively.Then used the immunohistochemistry to detect the expression of ER,PR,VEGF in tumor,and analyzed the correlation with the measurement errors.Results The results of differences between 2D -US,UE,CEUS and pathology were respectively as follows:( -0.59 ±-0.34)cm,( -0.20 ±-0.14)cm,( -0.40 ±-0.31)cm,and the differences were statistically significant(F =20.497,P <0.001).The positive expression rate of ER and PR was high if the difference between UE and 2D -US was less than or equal to 0.44cm.And the positive expression rate of VEGF was low if the difference between CEUS and 2D -US was less than or equal to 0.19cm.Three ultrasonic technologies in the measurement of breast cancer were different,the trend of difference between UE and 2D -US was smaller if the ER and PR were positively expression,and the trend of difference between CEUS and 2D -US was bigger if the VEGF was positively expression.Conclusion There is correlation between different immunohistochemical expression of breast cancer with measurement error in three different ultrasonic imaging technologies.The results suggest that the molecular pathology difference of breast cancer can impact on ultrasonic imaging,which contributes to know the reason and regulation of measurement error in different ultrasonic imaging technology.
3.Regulation Mechanism of Long Noncoding RNAs in Colon Cancer Development and Progression

Colorectal cancer (CRC) is the second most common cause of cancer-related death worldwide, and its high rates of relapse and metastasis are associated with a poor prognosis. Despite extensive research, the underlying regulatory mechanisms of CRC remain unclear. Long noncoding RNAs (lncRNAs) are a major type of noncoding RNAs that have received increasing attention in the past few years, and studies have shown that they play a role in many biological processes in CRC. Here, we summarize recent studies on lncRNAs associated with CRC and the signaling pathways and mechanisms underlying this association. We show that dysregulated lncRNAs may be new prognostic and diagnostic biomarkers or therapeutic targets for clinical application. This review contributes not only to our understanding of CRC, but also suggests novel signaling pathways associated with lncRNAs that can be targeted to block or eradicate CRC.

4.Research progress of pyroptosis in pediatric surgical diseases

Pyroptosis is a kind of programmed cell death accompanied by inflammatory responses, which is mainly induced by the Caspase-1/4/5/11 activated mediated via inflammasomes or lipopolysaccharide.Pyroptosis causes the cleavage of various Gasdermin family members, including Gasdermin D, in which a large number of pro-inflammatory factors are released to cause inflammatory responses in various diseases.At present, pyroptosis has not been extensively analyzed in pediatric surgical diseases.This review summarizes the research progress of pyroptosis in pediatric surgical diseases like Hirschsprung′s disease, biliary atresia, neonatal necrotizing enterocolitis, and tumors.It is found that pyroptosis is involved in the occurrence and development of pediatric surgical diseases.The research on the specific mechanism of pyroptosis in pediatric surgical disease and involved signaling pathways contributes to the early diagnosis and treatment of some pediatric surgical diseases.
5.Effects of semaglutide on glucolipid metabolism and bone metabolism in patients with type 2 diabetes mellitus

Objective:To explore the effects of semaglutide on glucolipid metabolism and bone metabolism in patients with type 2 diabetes mellitus (T2DM).Methods:A total of 96 T2DM patients treated in the Second People′s Hospital of Hefei from May 2022 to May 2023 were divided into the study group (48 cases) and the control group (48 cases) by treatment methods. The patients in the study group were treated with semaglutide, and the patients in the control group were treated with metformin, respectively, they were treated for 12 weeks. The levels of glucolipid metabolism, body mass index (BMI), bone metabolism, serum levels of adipocytokine 13 (apelin-13), feeding inhibitory factor 1 (nesfatin-1) and chemerin were compared between the two groups before and after treatment, and the incidence of adverse reactions was recorded.Results:After treatment, the improvement of glucolipid metabolism index in the study group were better than those in the control group ( P<0.05). After treatment, the BMI in the study group was lower than that in the control group: (24.26 ± 1.08) kg/m 2 vs. (25.40 ± 1.36) kg/m 2, there was statistical difference ( P<0.05). After treatment, the levels of total type Ⅰ collagen amino terminal lengthening peptide (TPINP) and bone gla protein (BGP) in the study group were higher than those in the control group and β C-terminal telopeptide of type Ⅰcollagen (β-CTX) was lower than that in the control group: (48.50 ± 9.27) μg/L vs. (44.85 ± 8.20) μg/L, (20.73 ± 6.34) μg/L vs. (18.05 ± 6.20) μg/L, (0.30 ± 0.10) μg/L vs.(0.36 ± 0.08) μg/L, there were statistical differences ( P< 0.05). After treatment, the levels of serum apelin-13 in the study group was higher than that in the control group, while the levels of nesfatin-1 and chemerin were lower than those in the control group: (1 500.43 ± 153.36) ng/L vs. (1 350.24 ± 142.30) ng/L, (3.06 ± 0.65) μg/L vs. (3.75 ± 0.78) μg/L, (9.85 ± 2.46) μg/L vs. (11.37 ± 2.34) μg/L, there were statistical differences ( P<0.05). The incidence of adverse reactions between the two groups had no statistical difference ( P>0.05). Conclusions:Semaglutide can significantly improve glucolipid metabolism, bone metabolism, decrease BMI, nesfatin-1 and chemerin levels, and increase apelin-13 level in T2DM patients.
6.Clinical efficacy of modified totally laparoscopic intra-gastric surgery for submucosal tumors adjacent to the gastric cardia or pylorus

Objective To explore the clinical efficacy of modified totally laparoscopic intra-gastric surgery for the treatment of submucosal tumors adjacent to the cardia or pylorus.Methods The retrospective crosssectional study was conducted.The clinicopathological data of 48 patients with gastric submucosal tumors adjacent to the cardia or pylorus between September 2014 and March 2018 were collected.There were 22 males and 26 females,aged from 38 to 78 years,with an average age of 58 years.Patients were performed multi-port or singleport modified laparoscopic intra-gastric surgery.Observation indicators:(1) surgical treatments;(2) postoperative recovery;(3) results of postoperative pathological examination;(4) follow-up.Patients were followed up by outpatient examination and telephone interview to detect the postoperative complications and tumor metastasis and recurrence up to June 2018.Measurement data with normal distribution were expressed as Mean±SD and measurement data with skewed distribution were described as M (range).Count data were represented as absolute number or percentage.Results (1) Surgical treatments:48 patients underwent modified totally laparoscopic intra-gastric surgery successfully,including one patient combined with proximal gastrectomy,without conversion to open surgery.Of the 48 patients,43 underwent multi-port modified laparoscopic intra-gastric surgery and 5 underwent single-port modified laparoscopic intra-gastric surgery.The operation time and volume of intraoperative blood loss were 68 minutes (range,45-110 minutes) and 20 mL (range,5-100 mL).The oncologic evaluation of 48 patients:48 patients had complete resection of tumors,without tumor rupture.The tumor diameter and distance from margin to tumor were 32 mm (range,20-40 mm) and 6 mm (range,5-10 mm).(2) Postoperative recovery:the time for initial oral intake and duration of postoperative stay were 2.8 days (rang,1.0-5.0 days) and 5.3 days(range,3.0-11.0 days).There were 4,3,1 and 1 patients complicated with surgical infection,delayed gastric emptying,sub-phrenic hydrops and digestive leakage respectively in the 48 patients.(3) Results of postoperative pathological examination:the distance from tumor margin to gastric cardia or pylorus,tumor diameter,circumferential resection margin were 15 mm (range,0-30 mm),24 mm (range,10-65 mm),6 mm (range,5-10 mm),respectively.Growth patterns of cancer in the 48 patents included 27 of intraluminal type,12 of intermural type,9 of mixed type.Pathological types of 48 patients:there were 26 patients with leiomyoma,9 with gastrointestinal stromal tumor,4 with other rare tumors,2 with carcinoid,2 with mucosa associated lymphoma,2 with inflammatory fibrous polyps,2 with gastritis cystica profunda,1 with ectopic pancreas.(4) Follow-up:41 of the 48 patients were followed up for 3-48 months,with a median follow-up time of 22 months.No tumor recurrence was detected in 37 of 41 patients by 3 times of gastroscopy and no stenosis or dysfunction of cardia or pylorus was detected in 39 patients by 2 times of upper gastrointestinal imaging (one patient undergoing two examinations).During the follow-up,there was no surgery-related complication or tumorspecific death.Conclusion Modified totally laparoscopic intra-gastric surgery is safe and feasible for the treatment of gastric submucosal tumors adjacent to the cardia or pylorus.
7.Diagnostic value of acetic acid spray combined with narrow-band imaging for early gastric cancer and precancerous lesion

Objective To evaluate the diagnostic value of acetic acid spray combined with narrow-band imaging ( NBI) for early gastric cancer and precancerous lesion. Methods A total of 136 gastric cancers and precancerous lesions from 132 patients detected by screening endoscopy from November 2015 to November 2017 in Guangming Chinese Medicine Hospital of Pudong New Area, Shanghai were enrolled in this study, and were divided into NBI group, acetic acid spray group, and acetic acid spray combined with NBI group ( combination group) . The image clarity, microsurface pattern, microvascular pattern, demarcation line, and mucosal whitening time were observed, and value of the 3 methods in diagnosis of early gastric cancer and precancerous lesions was analyzed. Results Image clarity of micro glandular in combination group was significantly better than that in NBI group and acetic acid spray group (χ2=8. 766, P=0. 003;χ2=5. 273, P=0. 022) , and image clarity of microvascular in combination group was significantly better than that in NBI group (χ2=7. 457, P=0. 006) . The overall diagnostic coincidence rate with pathology of combination group, NBI group, and acetic acid spray group was 91. 9%(125/136), 85. 3%(116/136), and 89. 7%(122/136), respectively. The diagnostic sensitivity of irregular or missing microsurface, irregular or missing microvascular, obvious demarcation line, mucosal whitening time <30 s for carcinoma ( including high grade intraepithelial neoplasia, early carcinoma and infiltrating carcinoma ) were 92. 3%, 91. 3%, 92. 3% and 90. 4%, respectively, in the combination group, and the diagnostic specificity of above indicators were 93. 8%, 93. 8%, 96. 9% and 90. 6%, respectively, accuracy were 92. 6%, 91. 9%, 93. 4% and 90. 4%, respectively. For observation of microsurface, the combination group was superior to the NBI group (χ2 =7. 378, P=0. 007) , but there was no significant difference compared with the acetic acid spray group (χ2=0. 427, P=0. 513);the acetic acid spray group was superior to the NBI group (χ2=4. 405, P=0. 036) . For observation of microvascular, the combination group was not significantly better than the NBI group (χ2=2. 398, P=0. 122). For observation of demarcation line, the combination group was not significantly better than the NBI group (χ2=2. 722,P=0. 099) and the acetic acid spray group (χ2=0. 216, P=0. 642). There was no difference between the acetic acid spray group and the NBI group (χ2=1. 433, P=0. 231) in observation of demarcation line. Conclusion Acetic acid spray combined with NBI has a good consistency with pathological results with high diagnostic sensitivity and specificity, and can clearly show microsurface of early gastric cancers and precancerous lesions compared to single NBI.
8.Prevention and treatment of hemorrhagic complications in the laparoscopic radical gastrectomy

Intraoperative hemorrhage is the most common complication of laparoscopic radical gastrectomy,which is also the main cause of conversion to open surgery.With the popularization of laparoscopic surgery technology worldwide,how to complete high-quality surgery and effectively avoid intraoperative or postoperative complications is the goal pursued by all surgeons.Intraoperative hemorrhage in the laparoscopic radical gastrectomy is mainly divided into parenchymal hemorrhage,perigastric vascular injury-induced hemorrhage,lymph node hemorrhage and anastomotic bleeding.Compression,electrocoagulation,vascular clipping and vascular suture repair are the most common treatments for intraoperative hemorrhage.It is necessary to use above methods in clinical work.Proficiency in laparoscopic operation skills,familiarity with perigastric vascular anatomy and variation,and improved teamwork to reduce complications are the key to laparoscopic surgery for gastric cancer.Combined with literature reports,the authors summarize experience in laparoscopic surgery for gastric cancer,discuss the prevention and treatment of hemorrhagic complications during laparoscopic radical gastrectomy.
9.Effect of LPS-induced M1 polarization of macrophages on tumor growth

Objective To study the effect of lipopolysaccharide(LPS)-induced M1 polarization of macrophages on tumor growth.Methods Female Balb/c mice were randomly divided into control group and experimental group.Renca cells were used to establish subcutaneous tumor model.NaCl(100μl/mice,once every two days)or LPS(100μg/mice,once every two days)was injected to the tumor side when the tumor grew to 50mm3.The M1 polarization level of tumor-associated macrophages was detected by flow cytometry.Mouse tumor cell lines(ML-1,MC38,Renca)were divided into three groups:blank group(complete medium with 50%DMEM basal medium),control group(complete medium with 50%medium supernatant of cultured macrophages)and experimental group(complete medium with 50%medium supernatant of LPS pretreated cultured macrophages).The proliferation of tumor cells was detected by cell counting kit-8(CCK-8).The cell cycle and apoptosis of tumor cells were detected by flow cytometry.Results The proportion of tumor-associated macrophages M1 increased after LPS treatment(P<0.001),thus enhancing its anti-tumor function.LPS-induced M1 polarization of macrophages can significantly inhibit the proliferation of tumor cells(Renca:P=0.023,ML-1:P=0.045).LPS-induced M1 polarization of macrophages blocked G0/G1 phase(MC38:P=0.011,ML-1:P=0.022)or S phase(Renca:P=0.022)of tumor cell cycle,and then cell division was inhibited.LPS-induced M1 polarization of macrophages significantly induced apoptosis of tumor cells(Renca:P=0.04,ML-1:P=0.007).Conclusion LPS can play an anti-tumor role by inducing M1 polarization of macrophages.
10.Research progress on myeloid-derived suppressor cells in multiple myeloma

Multiple myeloma(MM)is a malignant proliferative disease of plasma cells,ranking as the second most common hematologic tu-mor.Although the use of proteasome inhibitors and immunotherapeutic regimens has improved the prognosis of patients with MM,it re-mains incurable in most patients,mainly because of the eventual development of drug resistance in MM cells.Myeloid-derived suppressor cells(MDSCs)are a heterogeneous group of cells causing significant suppression of the T-cell immune response.They arise from bone mar-row myeloid progenitor cells that are blocked from differentiation and promote MM development by resisting immune destruction.Recent studies indicate that MDSCs stimulate MM cell proliferation,inducing drug resistance and metastasis.In this paper,we review multiple mechanisms exhibited by MDSCs in MM pathogenesis and discuss the feasibility and challenges of current therapeutic strategies targeting MDSCs,aiming to provide pertinent references regarding MM treatment.
