Objective To discuss the clinical effect of intravenous thrombolysis combined with psychological intervention in the treatment of acute cerebral infarction. Methods 84 patients with acute cerebral infarction were randomly selected at the June 2015 to September 2016 in our hospital were divided into experimental group 42 cases, 42 cases in the control group, the control group was given aspirin and Plavix treatment, experimental group received alteplase intravenous thrombolytic therapy combined with psychological intervention, each stage of the NISS scores of the two groups were compared, the two groups of patients with SDS self rating scale score, Barthel index and NDS score were compared. Results The experimental group of stage NISS was significantly better than the control group, the differences between groups was statistically significant (P<0.05); the experimental group with SDS self rating scale score, Barthel index and NDS score was significantly better than the control group, the differences between groups was statistically significant (P< 0.05). Conclusion In the treatment of acute cerebral infarction, the effect of intravenous thrombolysis combined with psychological intervention is obvious, and it is worthy of clinical promotion and application.

Objective To explore the changes of serum CRP and SAA in patients with comprehensive assessment COPD group,and correlation with lung function.Methods We selected 139 cases patients with COPD.According to the guide of GOLD 2015,the COPD patients were divided into group A (23 cases),group B (19 cases),group C (51 cases) and group D (46 cases).Analysis of the correlation between serum CRP and SAA level and injury severity of pulmonary function was perforined.Results Compared with group A,C,the group D had significance and had the highest level of serum CRP,SAA (P ＜ 0.05).Group B was higher than group C.The Lung function damage degree was negatively correlated with the level of CRP and SAA in patients with COPD (P ＜ 0.05).Conclusion The change of the concentration of serum CRP,SAA will contribute to the evaluation of patients with COPD and provide objective basis for the treatment and management of patients with COPD.Detection of CRP,SAA is helpful to evaluate the damage degree of lung function.

BACKGROUND:The resin infiltration technique for masking white spot lesions has obtained a result superior to the conventional remineralization. Up to now, few studies have evaluated the color of carious incipient lesions treated with the resin infiltration.
OBJECTIVE:To assess the color change of caries incipient lesions in the enamel treated with resin infiltration.
METHODS: Forty-eight extracted teeth were exposed to demineralizing solution for 4 weeks to establish artificial caries models. Then artificial caries models were treated with resin infiltration and randomized into four groups: these specimens were immersed in red wine, coffee, tea and artificial saliva for 2 and 4 weeks, respectively. The base line color was evaluated by spectrophotometer. The variation in color (ΔE) was analyzed by one-way analysis of variance and Tukey’s tests.
RESULTS AND CONCLUSION:Al specimens underwent color and lightness changes, irrespective of immersion media. After 2 weeks of immersion, ΔE values were ranked as folows: red wine > coffee > tea > artificial saliva, and there were significant differences between four groups (P < 0.05). After 4 weeks of immersion,ΔE values were ranked as folows: red wine≈ coffee > tea > artificial saliva, and there were significant differences between four groups (P < 0.05) but not between the red wine and coffee groups (P > 0.05). Significant changes in the color were found in the red wine and coffee groups at different time after immersion (P < 0.05). Immersed in red wine, coffee and tea, the?E value was > 3.7, which is not acceptable in clinic. These findings indicate that the color stability of carious incipient lesions located in the enamel treated with resin infiltration is associated with the soak solution and soak time. In addition, immersion in red wine and coffee results in significantly increased color alteration (ΔE) compared with the artificial saliva.

Objective To investigate the effects of Simo decoction on ghrelin and calcitonin gene-related peptide (CGRP) in diabetic rats.Methods A total of 48 SD rats were divided into healthy control group,diabetes group (DM),DM treated with low dose Simo decoction group (DMTL),DM treated with medium dose Simo decoction group (DM-TM),DM treated with high dose Simo decoction group (DM-TH) and DM prevented with medium dose Simo decoction group (DMPM) ; there were eight rats in each group.The rats of DM group and other intervention groups were intraperitoneally injected with 1％ streptozotoein (STZ) to establish the model.The rats of healthy control group were injected with solvent.At eight weeks after the model successfully established,the rats of each intervention group were gavaged with low dose of Simo decoction (0.15 ml · kg-1 · d-1 ),medium dose of Simo decoction (1.5 ml · kg-1 · d-1 ) and high dose of Simo decoction (3.0 ml · kg-1 ·d-1 ) for two weeks; the rats of control group and DM groups were gavaged with equal volume of 0.9％NaCl.The rats of DM-PM group were gavaged with Simo decoction at 1.5 ml · kg-1 · d-1 for 10 weeks after the model successfully established.Rats were sacrificed at the end of drug intervention and gastric antrum tissues were dissociated for detecting the expression of ghrelin and CGRP by real time polymerase chain reaction (PCR).The data were analyzed by one way ANOVA and t test.Results There was no significant difference in ghrelin expression at mRNA level in gastric antrum tissues between diabetic rats and control group.However after intervened by low dose and medium dose of Simo decoction,the ghrelin expression increased (1.45 ± 0.34,1.87 ± 0.68 compared with 0.87 ±0.28,Dunnett's T3 test,P＜0.05).There was no effects of high dose Simo decoction on the expression of ghrelin in diabetic rats.There was no significant difference in ghrelin between DM-TM and DM-PM.The expression of CGRP at mRNA level in gastric antrum tissues of diabetic rats was significantly higher than that of control group (4.61 ± 1.67 compared with 1.06 ± 0.40,t =5.843,P＜0.01).There was no effects of low dose Simo decoction on CGRP expression of diabetic rats.The expression of CGRP of diabetic rats was down-regulated by medium and high dose of Simo decoction intervention.There was no significant difference in CGRP expression between DM-TM and DM-PM.Conclusions Simo decoction can promote ghrelin expression in gastric antrum of diabetic rats.The expression of CGRP in gastric antrum of diabetic rats significantly increased,but Simo decoction may down-regulate the expression of CGRP in gastric antrum of diabetic rats.

Objective To assess the changes of the velocity,strain and strain rate on the corresponding vascular of plaque and without plaque,bard plaque and soft plaque,the cupular part of fiber cowl and shoulder of plaque.Methods Eighty-six patients with coronary artery disease(CAD)according to the results of coronary arteriography and 50 normal subjects underwent the high frequency ultrasound,and carotid artery intima media thickness(IMT)was detected.The velocity,strain and stain rate of carotid were obtained by velocity vector imaging(VVI)speckle tracking technique.Results The level of IMT in CAD patients was higher than that in normal control(P＜0.05).The systolic maximum velocity of carotid with normal contract was higher significantly than that of carotid without plaque with CAD(P＜0.05).The systolic maximum velocity and strain rate of soft plaque in carotid atherosclerosis were higher significantly than those of hard plaque(P＜0.001,P＜0.05).The maximum velocity and strain rate of fabric cowl in carotid atheroscIerosis plaque were lower significantly than those of plaque shoulder(P＜0.05,P＜0.00 1).Conclusions VVI technique could early detect the elasticity degree of blood vessel wall,artherosclerosis and the mechanism asynchrony of the plaque intima.The value difference could be used as the predictive parameter and quantization for artherosclerosis and plaque instability.

BACKGROUND:Sodium alginate and chitosan are the polycation and polyanion natural polymer materials respectively, and they can be crosslinking agents complementing each other to form composite gel and avoid the cytotoxicity resulting from some common crosslinking agents .
OBJECTIVE:To prepare the chitosan-sodium alginate composite gel and evaluate its cytotoxicity in vitro. METHODS:Chitosan was dissolved in 0.25 mol/L acetic acid to make a 30 g/L mass concentration solution, and 0.1 mol/L NaOH solution was added to neutralize its acidity. Neutralization of the chitosan solutions leads to the formation of a precipitate in ultrasmal particles. Then the chitosan and 3%sodium alginate solution in deionized water were mixed in 1:1 volume ratio by high frequency oscil ating to produce composite gel. The composite gel were detected by scanning electron microscopy and Fourier transform infrared spectrometry after freeze-drying. The 24-hour and 72-hour leaching solutions of composite gel, 24-hour and 72-hour leaching solutions of polyethylene and phenol solution were added to the L-929 cells’ culture medium respectively in order to evaluate the cytotoxicity of composite gel in vitro.
RESULTS AND CONCLUSION:The results of Fourier transform infrared spectrometry showed the variation of characteristic peak values of composite gel which were different from sodium alginate and chitosan;and under scanning electron microscope, a spatial network structure formed with abundant intervals. Result of the cytotoxicity valuation was qualified for the chitosan-sodium alginate composite gel. These findings indicate that the chitosan-sodium alginate composite gel can be used as tissue engineering scaffold materials.

By means of a demand characteristic analysis of high-value consumables in surgical operations,this article established the agile supply chain model of those consumables,and studied its management model as well as the running mechanism based on a series of advanced information technologies,such as case knowledge base,virtual storage,dynamic monitoring,etc.With such a system in place,the hospital realized on-demand and zero-inventory management of high-value consumables to satisfy the dynamic demand for surgical operations.

Objective To establish the proper review rules for the microscopic screening of urine samples tested by automatic urinalysis work station.Methods A total of 3 154 random urine samples were enrolled to establish and validate review rules .All the samples were collected from the inpatients and outpatients of Shanghai First People′s Hospital from March to May 2013 and tested by urinalysis work station.Three thousands one hundred and fifty four urine samples were firstly tested by urinalysis work station,including both urine dry chemical analyzer and urine sediments analyzer .Then each urine sample was examined microscopically by two technicians-in-charge using double-blind method.The average results from the two technicians were used as review results .Compared with review results ,the review rules were set up.According to different test methods by automatic urinalysis work station , four microscopic review protocols were defined:(1)Protocol 1:based on chemistry results only ,microscopy review was performed when any of WBC,RBC,PRO and NIT was positive;(2)Protocol 2:based on urine sedimental analysis only ,microscopy review was performed when any of WBC ,RBC and CAST count was over upper limit of the reference range;(3)Protocol 3:if any of BLD ,RBC,LEU,WBC was different between two systems ,or quantitative results had two or more than two gradient differences ,microscopy review was performed;(4) Protoco1 4:if any of BLD, RBC,LEU ,WBC was different between two systems , or CAST was over upper limit of the reference range , or alarm appeared , microscopic review was performed .400 randomly selected urine samples were tested to validate the review rules .Omission diagnostic rate and review rate were used to evaluate the rules .Results According to the review rules,the positive samples rate was 43.47%(1 371/3 154) and the negative rate was 56.53%( 1 783/3 154 );Positive samples were composed of RBC ( 55.58%) , WBC ( 59.66%) and CAST(6.42%).The review rates of four protocols were 44.48%(1 403/3 154),45.69%(1 441/3 154), 26.09%(823/3 154),28.95%(913/3 154),respectively.The false negative rates (omission diagnostic rates)were 7.13%(225/3 154),4.53%(143/3 154),2.73%(86/3 154) and 1.02%(32/3 154), respectively .Protocol 4 was selected as an ideal plan.Additional 400 urine samples were tested using protocol 4 in order to confirm the review rule.The review rate, false negative rate were 26.25%(105/400), 0.75%( 3/400 ), respectively.After image review revised, the review rate was 14.50%(58/400).Conclusion This study formulates that the automatic urine analysis workstation review rules have clinical maneuverability and validity.
