Neonatal hypoxic-ischemic brain damage (HIBD) is a complex disease process in which injury severity,duration and timing of the antenatal injury are difficult to discern.HIBD triggers a cascade of pathophysiological events (blood-brain barrier damage,oxidative stress,inflammation,glutamate excitotoxicity,energy depletion and cell death) leading to brain damage and neonatal death.Under this background,the changing of biomarkers in different types of biological fluids able to be detected and prognosed HIBD.
3.Efficacy of nerve stimulator-guided thoracic paravertebral block in patients undergoing segmental mastectomy

ObjectiveTo evaluate the efficacy of nerve stimulator-guided thoracic paravertebral block in patients undergoing segmental mastectomy.MethodsFifty ASA Ⅰ or Ⅱ female patients,aged 18-64 yr,with body mass index < 24 kg/m2,undergoing segmental mastectomy,were randomly divided into 2 groups ( n =25 each):thoracic epidural block group (group A) and nerve stimulator-guided thoracic paravertebral block group (group B).In group A,an epidural catheter was placed at T3,4 interspace and 0.5% ropivacaine 7-10 ml was administrated via the epidural catheter.In group B,thoracic paravertebral block was performed at T4 under the guide of the nerve stimulator and 0.5% ropivacaine 25 ml was administrated.The perioperative adverse cardiovascular events (hypotension and bradycardia) were recorded.Shivering,chest distress,and dyspnea during operation and nausea and vomiting after operation were also recorded.The range of block on the operated side was determined by pinprick test.The efficacy of anesthesia was evaluated.ResultsThe incidence of shivering,chest distress,and dyspnea during operation and nausea and vomiting after operation was significantly lower in group B than in group A (P < 0.05).Adverse cardiovascular events and respiratory depression were not found in the two groups.There was no significant difference in the range of block on the operated side in the two groups ( P > 0.05).The satisfactory anesthesia was found in 100% of cases.ConclusionNerve stimulator-guided thoracic paravertebral block can be safely and effectively used in patients undergoing segmental mastectomy and the efficacy is better than that of thoracic epidural block.
4.Changes in intrapulmonary shunting during orthotopic liver transplantation without veno-venous bypass

Objective To investigate the changes in pulmonary gas exchange and intrapulmonary shunting during orthotopic liver transplantation (OLT) without veno-venous bypass. Methods Sixteen ASA III-IV patients with end-stage liver diseases were studied. Anesthesia was induced with midazolam 0.1 mg ? kg-1 , fentanyl 10 ?g kg -1 , scopolamine 0.6 mg, and vecuronium 0.1 mg ? kg-1 and maintained with isoflurane and propofol infusion (2-3 mg? kg-1h-1 ) supplemented with intermittent iv boluses of midazolam, fentanyl and vecuronium. The patients were intubated and mechanically ventilated with 100% O2 Swan-Ganz catheter was inserted via right internal jugular vein. Radial artery was cannulated. MAP, HR, ECG, CVP, SpO2 and PET CO2 were continuously monitored. Body temperature was maintained at≥35.5℃ during operation. Blood samples were taken from radial and pulmonary artery for blood gas analysis. P(A-a) O2 and Qs/Qt were calculated immediately before operation (T1 ), cross-clamping (T2) and unclamping of inferior vena cava and portal vein (T3 ) , 5 min (T4) and 90 min (T5 ) after graft reperfusion and at the end of operation (T6). Results P(A-a) O2 and Qs/Qt were both higher than normal value at T1 . PaCO2, P(A-a) O2 and Qs/Qt increased significantly at T3 and T4 as compared with those at T2 P(A-a)O2 and Qs/Qt were significantly decreased at T5 and T6 as compared with those at T3 and T4 Qs/Qt was positively correlated with P(a-A) O2 and PAP. Conclusion There is a certain degree of intrapulmonary shunt before and during orthotopic liver transplantation without veno-venous bypass. The amount of fluid infused should be strictly controlled during anhepatic phase.
5.Analysis in effect of nursing intervention in prevention of premature rupture of the membrane infection

Objective To investigate the role of nursing intervention in the prevention of premature rupture of the membrane infection. Methods 131 patients with premature rupture of the membrane were divided into the control group(65 cases) and the experimental group(66 cases), they adopted routine nursing method and nursing intervention respectively. The duration of pregnancy, fetal condition and laboratory tests for infection were compared between two groups. Results No significant differences were found between the two groups in terms of age, duration of pregnancy, newborn weight. Significant differences were observed in Apgar score, newborn death between two groups. Conclusions Monitoring for signs of premature rupture of the membrane such as infection and laboratory tests are important for the prevention of infeclion, if infection occurs, delivery may be necessary to prevent further complications.
6.Serum Dickkopf-1 levels in patients with ankylosing spondylitis not affected by using tumor necrosis factor-α inhibitor

Objective Dickkopf-1 (DKK-1) is an antagonist of the Wnt signaling pathway of bone formation and it is the target gene of TNF.When DKK-1 is inhibited,osteoprotegerin (OPG) would increase,OPG can antagonize bone erosion caused by receptor activator of NF-κB ligand (RANKL).Detection the levelsof serum DKK-1,OPG,RANKL and TNF-α of patients with AS before and after applying TNF-α antagonist,the effects of anti-TNF-α treatment on signaling pathway of bone formation was explored.Methods Medical records and serum samples of 43 AS patients receiving TNF-α receptor-Ⅱ IgG Fc fusion protein for 12 weeks were collected and the levels of serum DKK-1,OPG,RANKL and TNF-α were detected by enzyme-linked immunosorbent assay.Paired t-test and Person's correlation analysis were used for data analysis.Results Disease activity indexes in AS patients with anti-TNF-α treatment,including VAS score,time of morning stiffness,BASDAI,BASFI,ESR and CRP,which were [(2.7±2.0) cm,(7±4) min,1.1±1.1,0.8±1.2,(10±9) mm/1 h and (16±26) mg/L] respectively,were reduced when compared with those before treatment,[(7.6±1.9) cm,(46±33) min,6.0±1.3,4.4±2.0,(39±29) mm/1 h and (76±43) mg/L] respectively,and the differences were statistically significant (P<0.05).Serum levels of DKK-1,(3.3±1.5) μg/L vs (3.2±1.3) μg/L,OPG,(144±71) pg/ml vs (136±62) pg/ml,and RANKL,(71±37) pg/ml vs (68±30) pg/ml,were not changed (t=14.043,8.031,20.166,13.521,7.679,9.563,all P>0.05).The levels of TNF-α increased from (1.9±1.3) pg/ml to (55.0±50.6) pg/ml (t=6.951,P<0.05).There were no relations between DKK-1 and TNF-α,RANKUOPG,patient VAS,duration of morning stiffness,BASDAI,BASFI,ESR and CRP,allP>0.05.Conclusion Clinical indicators are improved significantly by anti-TNF-α therapy; however,it should not be withdrawal suddenly.There is no relationship between serum levels of DKK-1 and level of inflammation in AS patients.
7.Family History, Psychosocial Factors of Parkinson's Disease--a Case-control Study in Linxian County

Objective:To investigate the relationship of family history , psychosocial factors of PD (Parkinson's Disease) in a rural population in north China. Methods: A case control study was conducted in rural population in China. Cases (n=77) have been diagnosed by a two-phase screening by neurologists and interviewers in May, 2000. Controls (n=154), randomly selected from the same cohort, who were matched with patients (control/cases ra-tio=2) for sex, age (?5 years). Information of participants' characteristics came from the interview in 2001. Data was analyzed by using conditional logistic regression by SAS 6.12. Results: Family history (OR=13.54) .negative life events (OR=3.02), interpersonal communication (OR=2.10) were associated with a significantly elevated risk for the onset of PD. Conclusion: These results indicate that family history, psychosocial factors were risk factors of PD.
8.Formula Optimization of Piribedil Hydrochloride Oral-Disintegrating Tablets

OBJECTIVE:To optimize the formula of piribedil hydrochloride orally disintegrating tablets and to investigate their related indexes.METHODS:Taking the contents of crospovidone(PVP XL)(disintegrating agent),amylum pregelatinisatum(loading agent) and Gum Acacia(glidant) as factors,the disintegrating time(td),the wet time(t) and the suspend stability(?A) as the evaluate indexes to carry on the orthogonal experimental to optimize the formula.The dissolution curve of the optimized formula was drawn.RESULTS:The optimized formula for the piribedil hydrochloride orally disintegrating tablets were as follows:the contents of crospovidone(PVP XL),amylum pregelatinisatum and Gum Acacia were 15%,20%,and 15% respectively.The prepared orally disintegrating tablets disintegrated completely within 30s and dissolved basically within 2min.CONCLUSION:The prepared preparation can meet the related standards specified of China Pharmacopeia.
9.Analysis of correlation of AMH expression and insulin resistance degree in PCOS rats

Objective:To study the correlation between the expression of anti-mullerian hormone( AMH) and insulin resistance (IR)degree in serum and local ovary and single follicle induced by polycystic ovary syndrome(PCOS)model. Methods: PCOS rat model was constructed by DHEA method. and then calculate the homeostasis model assessment of insulin resistance( HOMA-IR) val-ues. according to HOMA-IR value ,the object is simple screening PCOS group,PCOS merger IR group and control group were detected by immunohistochemistry model mice, the expression of AMH and insulin receptor in serum and single follicle and ovary were checked. Results:The serum FINS level and HOMA-IR was significantly higher in PCOS with IR group compared with that in control group and PCOS without IR group(P<0. 05);while the combined IR PCOS group T,Flns and HOMA-IR was significantly higher in the control group(P <0. 05);Pearson correlation analysis showed that local ovarian AMH expression of insulin receptor expression was no significant correlation(r=0. 015,P>0. 05);ovary AMH expression and serum insulin significantly was correlated(r=0. 671,P=0. 003),a single follicle AMH expression of insulin receptor expression was no significant correlation(r=0. 011,P>0. 05);HOMA-IR and serum AMH had significant positive correlation(r=0. 683,P=0. 001). Conclusion: AMH were positively correlated with serum insulin levels in PCOS rat. The expression of AMH in partial ovarian was positively correlated with the insulin receptor, while the expression of AMH in single follicle was no significant correlation with the insulin receptor. Howerer,whether the expression of AMH is correlation with hyperandrogenism has need further study.
10.Ultrasound-guided microwave intervention in the treatment of solid carcinoma

5cm hepatic cancer, the effective rate (CR+PR) was 60 8% (62/102), no change (NC) rate 39 2% (40/102), and the 1-, 2-, 3-year survival rates 67 6% (69/102), 49 1% (50/102) and 39 2% (40/102), respectively. The effective rates (CR+PR) in patients with bone tumor and lung cancer were 100%(12/12) and 60%(3/5), respectively. There were no other severe complications found. Conclusions Ultrasound-guided microwave intervention is a safe, effective and feasible method for hepatic cancer, lung cancer and bone tumor.
