1.Effects of loop diuretic on renal function in patients with chronic heart failure

Objective To describe the effect of loop diuretic on renal function in patients with chronic heart failure.Methods Data were of 211 impatients diagnosed as HF collected in a nested case-control study.The association of application of loop diuretic with renal function was assessed to identify whether loop diuretic was an independent risk factor of worsening renal function(WRF).To identify WRF,we defined serum creatinine level as a rise ≥26.5 μmol/L compared to the level at admission.Therefore,the subjects were divided into case group(serum creatinine level ≥ 26.5 μmol/L,n =66)and control group(serum creatinine level < 26.5μmol/L,n =145).Results The doses of loop diuretic were higher in the case group([385.17 ± 49.37]mg)than the control group([244.50 ± 34.82]ag)(P < 0.05),but it was not independent risk factor of WRF (P>0.05).Creatinine level and NYHA class at admission were independent risk factor for WRF,with OR of 2.248(95 % CI:1.088-4.647)and 2.485(95 % CI:1.385-4.459)respectively(Ps < 0.05).Conclusion The doses ofloop diuretic were not independent risk factor of WRF,creatinine level and severity of HF at admission are the most important predictors of the occurrence of WRF.

3.Service of Institutional Care for People with Disabilities in Shanghai, China

Objective To analyze the current situation of institutional care for people with disabilities in Shanghai, China. Methods Sec-ondary data were collected from the synthetical information platform of Shanghai Disabled Persons' Federation, including data about the res-idential institutions and people with disabilities living in the institutions from 2004 to 2015. Results The number of residential institutions and people with disabilities showed an increasing tendency from 2004 to 2015. At the end of 2015, there was 368 residential institutions in Shanghai, providing institutional care for 5821 people with disabilities. People in the institutions were mainly with intelligence disability and mental disability, and mainly in 35 to 59 years old. Conclusion The institutional care for people with disabilities was generally in a good condition in Shanghai and met people's needs.
4.Clinic study of serum product advanced oxidation protein changes of patients with chronic obstructive pulmonary disease

Objective To investigate the serum levels of advanced oxidation protein products (AOPP) in patients with chronic obstructive pulmonary disease (COPD) as well as explore the relevance of its clinical significance AOPP and COPD.Methods Fifty-four patients with mild/moderate COPD (COPD group) were enrolled in this study,who were treated in the No.100th Hospital of the Chinese People's Liberation Army from Apr.2011 to Nov.2013.Thirty healthy volunteers (control group) at same period were selected as control group and the general condition of two groups were matched.AOPP,superoxide dismutase(SOD) and malondialdehyde (MDA) levels were measured.Results The serum AOPP,MDA and SOD in control group were (45.78 ± 12.54) μmol/L,(2.96 ± 0.55) μmol/L and (78.40 ± 8.37) kU/L respectively.And its were (68.93 ± 10.62) μmol/L,(6.07 ± 2.44) μmol/L and (53.66 ± 5.99) kU/L respectively in COPD group.The differences were statistically significant (t =-8.57,-9.14,14.38 ; All P values were less than 0.01).The serum AOPP,MDA and SOD in mild COPD group were (65.56 ±9.65) μmol/L,(4.21 ± 1.83) μmol/L and (62.97 ± 6.28) kU/L respectively,and (71.79 ± 11.37) μmol/L,(7.43 ± 3.12) μmol/L and (41.25 ± 5.89) kU/L respectively in moderate CODP group.The differences were statistically significant (t =-2.17,-4.80,13.00; P < 0.05 or P < 0.01).Conclusion The increased levels of serum AOPP is important pathological changes in patients with COPD,which may be involved in the occurrence and development of COPD,and is the reaction of oxidative stress injury in patients with COPD early sensitive indicator.

6.Comparison for Manipulation and Bed Rest in Treatment of Acute Nonspecific Low Back Pain

This study was aimed to investigate the efficacy, acceptance, complications / adverse events treated with traditional manipulation and bed rest for patients with acute nonspecific low back pain (ANLBP). A total of 60 ANLBP patients were distributed into the Group A/B randomly and equally. Patients in Group A were treated by bed rest absolutely for one week; meanwhile patients in Group B were treated by traditional manipulation for one week. IBM SPSS20.0 was used to analyze the Visual Analogue Scale (VAS), Chinese Oswestry Disability Index (ODI), acceptance, complications / adverse events and others. The results showed that VAS and ODI reduced after one-week treatment in Group A and B (t = 14.67, 11.55, allP < 0.001 andt = 24.80, 15.35, allP <0.001). Differences of VAS and ODI were with significant difference between Group A and B (t = 3.24, 2.75,P =0.002, 0.009). Scores of acceptance and complications / adverse events were with significant difference between Group A and B (t = 2.65,P = 0.01 andχ2= 10.00,P = 0.002). It was concluded that both manipulation and bed rest can alleviatepain due to ANLBP, promote functional recovery. However, traditional manipulation can better improve symptoms, easier to be accepted by patients with less complications / adverse events.
7.Expression of FGFR3 in acute lymphoblastic leukemia patients and its contribution to proliferation of circulating endothelial cells

AIM: To evaluate the expression of fibroblast growth factor receptor 3 (FGFR3) in acute lymphocytic leukemia (ALL) patients and its contribution to the proliferation of circulating endothelial cells (CECs).METHODS: The mRNA expression levels of FGFR3 in 44 patients with ALL were assayed by RT-PCR.Overall survival (OS) rates of the patients in FGFR3+ group and FGFR3-group were estimated by Kaplan-Meier analysis.The CECs were sorted from peripheral blood by magnetic-activated cell sorting and then counted by 3-color flow cytometry.The cell counts, antigen expression, growth curve and colony forming rate of the CECs in the 2 groups were determined.The FGFR3 expression of CECs was identified by immunofluorescence staining.RESULTS: The positive rate of FGFR3 mRNA expression was 43.2% in 44 ALL patients with normal karyotype.T-ALL expressed higher level of FGFR3 than B-ALL (P<0.05).FGFR3 was over-expressed in ALL patients with bone marrow blast proportion ≥5% (P<0.05).The probability of OS was significantly lower in FGFR3+ group than that in FGFR3-group (P<0.05).The sorted CECs highly expressed CD31, CD144, VEGFR-2 and CD146, and rarely expressed CD45.The counts of CECs and expression level of CD133 significantly increased in FGFR3+ group compared with FGFR3-group.The same result of the amount of colony formation was observed (P<0.05).There was significant difference at 3 time points of cultured CECs count in vitro between FGFR3+ group and FGFR3-group (P<0.05).The positive rate of FGFR3 expression of CECs from 19 ALL-FGFR3+ patients was (29.00±15.71)%.CONCLUSION: The over-expression of FGFR3 gene in ALL may be helpful to evaluate the prolife-ration of CECs, and become a double target with anti-tumor and anti-angiogenesis effects to offer more choice for molecular therapy in the future.
8. Analysis of related factors of anemia in ten malignant tumors in a hospital

Objective:
To explore the occurrence of anemia in patients with ten common malignant tumors and the related risk factors.
Methods:
A retrospective analysis was performed in 15 612 patients with ten common malignant tumors admitted in Zhejiang Cancer Hospital from January 2016 to September 2016 by single factor analysis.
Results:
Among the 15 612 cases, there were 7 468 males (47.83%), 8 144 females (52.17%), and anemia occurred in 5 541 cases, accounted for 35.5% of all the cases with malignancy.Ovarian cancer, gastric cancer and esophageal cancer ranked top three in all types with occurrence of 57.9%, 53.1% and 49% respectively.According to the grading, mild to moderate (Grade 1-2) anemia accounted for 91.2% of the total cancer related anemia, while severe (Grade 3-4) consisted only a small proportion of 8.8%.The mean age of malignant tumor patients was (56.1±11.6)years old, and that showed a significantly difference between the anemia group and non-anemia group in patients' ages among lung cancer, colorectal cancer, gastric cancer and nasopharyngeal carcinoma(
9.Analysis of immunogenicity and protective effect of a bivalent DNA vaccine expressing interleukin-2 and an outer membrane protein of Treponema pallidum(Gpd)

Objective To investigate the immune response to and protective effect of a bivalent DNA vaccine expressing interleukin-2(IL-2)and Gpd proteins in New Zealand rabbits.Methods Seventy-two male New Zealand white rabbits were equally and randomly divided into 4 groups to be immunized with recombinant plasmids pcDNA3.1(+)/Gpd-IL-2(pcD/Gpd-IL-2),pcDNA3.1(+)/Gpd(pcD/Gpd),empty plasmid pcDNA3.1(+)(pcD)and phosphate buffered saline(PBS),respectively.Immunization was carried out by intramuscular injection at multiple sites with a 2-week interval for 3 times.On week 10 after the initial immunization,the rabbits were challenged intradermally with T.pallidum(Nichols strain).Enzyme-linked immunosorbent assay(ELISA)was used to quantify the serum level of anti-Gpd antibodies in the rabbits and the level of IL-2 and interferon(IFN-γ)in the supernatant of Gpd protein-stimulated spleen cells from the rabbits at different time pionts.MTT assay was conducted to detect the proliferation response of spleen cells collected from the rabbits on day 0,14,28,140 and 168 after the challenge.Results Compared with pcD and PBS,both the vaccines pcD/Gpd and pcD/Gpd-IL-2 elicited significantly higher levels of anti-Gpd IgG antibodies in rabbits at different time points during the vaccination and infection period,with the titers peaking at 1 ∶ 1024 and 1∶4096,respectively(both P < 0.01).There were also significant differences in the serum levels of anti-Gpd IgG antibodies between the pcD/Gpd-and pcD/Gpd-IL-2-immunized rabbits at different time points(all P <0.01).The levels of IL-2 in the supematant of spleen cells from pcD/Gpd-and pcD/Gpd-IL-2-immunized rabbits on week 8 after the immunization were 110 ± 12.6 and 167 ± 15.7 μg/L respectively,and those of IFN-γwere 225 ± 17.6 and 447 ± 22.4 μg/L respectively,significantly higher than those in that from the other two groups of rabbits(all P < 0.01).Furthermore,an apparent proliferation response was observed in spleen cells from pcD/Gpd-and pcD/Gpd-IL-2-immunized rabbits with a higher stimulation index compared with pcD-and PBS-immunized rabbits(all P < 0.01).Dark-field microscopic examination of early-stage infected lesions revealed that pcD/Gpd-IL-2-immunized rabbits had a lower detection rate(17.5%)of Tp from lesions,occurrence of ulcerative lesions(15%)and shorter curing time compared with pcD/Gpd-immunized rabbits.Conclusion The recombinant plasmid pcDNA3.1(+)/Gpd-IL-2 could induce protective humoral and cellular immune response more efficiently in rabbits.
10.Application of contrast-enhanced ultrasound in needle biopsy of tuberculous cervical lymph node.

OBJECTIVETo study the value of contrast-enhanced ultrasound in tuberculous cervical lymph node biopsy.
METHODSSeventy-nine patients with cervical lymph node were divided into two groups. The lymph nodes of group A were used as real-time ultrasound guided biopsy; the lymph nodes of group B were contrast-enhanced ultrasound firstly to determine the target lymph node and the puncture point, then were detected with needle biopsy.
RESULTSGroup A: for the 23 lymph nodes of 23 patients, the rate of intact tissue was 52.1% and the positive rate of pathological diagnosis was 73.9%. Group B: for the 56 lymph nodes of 56 patients, the rate of intact tissue was 98.2% and the positive rate of pathological diagnosis was 100% (Chi square value was 23.37 and 12.31, P < 0.05).
CONCLUSIONThe application of contrast-enhanced ultrasound before cervical lymph node biopsy can obviously improve the rate of intact tissue and the positive rate of pathological diagnosis.
