1.The Role of HIV Replicative Fitness in Perinatal Transmission of HIV

Perinatal transmission of Human immunodeficiency virus(HIV),also called mother-to-child transmission(MTCT),accounts for 90% of infections in infants worldwide and occurs in 30%-45% of children born to untreated HIV-1 infected mothers.Among HIV-1 infected mothers,some viruses are transmitted from mothers to their infants while others are not.The relationship between virologic properties and the pathogenesis caused by HIV-1 remains unclear.Previous studies have demonstrated that one obvious source of selective pressure in the perinatal transmission of HIV-1 is maternal neutralizing antibodies.Recent studies have shown that viruses which are successfully transmitted to the child have growth advantages over those not transmitted,when those two viruses are grown together.Furthermore,the higher fitness is determined by the gp120 protein of the virus envelope.This suggests that the selective transmission of viruses with higher fitness occurred exclusively,regardless of transmission routes.There are many factors contributing to the selective transmission and HIV replicative fitness is an important one that should not be neglected.This review summarizes current knowledge of the role of HIV replicative fitness in HIV MTCT transmission and the determinants of viral fitness upon MTCT.

OBJECTIVE:To discuss the problem lies in the orientation of clinical pharmacy of medical university.METHODS:The differences between traditional pharmacy education and clinical pharmacy in orientation were discussed,and reasonable proposals based on the practical situation of our country are given.RESULTS & CONCLUSION:Renovations of clinical pharmacy education should be conducted from aspects of orientation of subject,teaching model,curriculum arrangement,mechanism of practice etc.The orientation of clinical pharmacy is to train applied talents who can offer patient-centered pharmaceutical care and appropriately direct rational clinical drug administration.
3.Experimental study on treatment of acute spinal cord injury by autologous activated Schwann cells transplantation in the rat

Objective To observe the effect of treating acute spinal cord injury in rats by transplantation of autologous activated Schwann cells(AASCs). Methods Unilateral saphenous nerves were ligated directly, free it and culture Schwann cells 1 week later using the tissue clot method. Nerve growth factor(NGF) and brain-derived neurotrophic factor(BDNF) in medium were detected in different periods. 90 female Wistar rats[(200?30) g] were randomly assigned to 3 different study groups as follows: control group A(n=30): 20% DMEM injection; research group B(n=30): autologous Schwann cells(ASCs) transplantation; research group C (n=30): AASCs transplantation. The cells were purified before transplantion to the injuried T10 spinal cord site of rats (New York University type weight drop apparatus, NYU). The recoveries of the lower extremity were observed using Basso-Beattie-Bresnahan(BBB) locomotor scoring system and somatosensory evoked potential and motor evoked potentials(SEP & MEP). And then observe the coticospinal tract(CST) using the biotinylated dextran amine(BDA) tracing. 4.The Clinical Study of the H1N1 Influenza Virus with Fanggan Decoction

[Objective]To discuss the clinical effect of Fanggan decoction on H1N1 Influenza virus.[Method]Randomly divided 95 cases into 2 groups,treatment group 48 and control one 47.Treatment group takes Fanggan decoction,the control one uses reducing fever tablet or anti-virus drugs if needed.[Result]The effective rate was 89.6% for treatment group and 85.1% for the control one.The treatment group was better than the control one on the reducing fever time,cough duration and cold symptom time.[Conclusion]Fanggan decoction has a good effect on H1N1 influenza virus.It also can be one of the therapies to relieve influenza patients in Zhejiang Province.
5.Bovine Herpesvirus 1 Protein bICP0 Represses the Transcription of bISG15 in Fetal Bovine Lung Cells

The ubiquitin-like modifier bISG15 is an antiviral protein found in fetal bovine lung (FBL) cells.Bovine Herpesvirus 1(BHV-1),which is a viral pathogen of cattle,can infect FBL cells and induce cytopathic effects.Real-time PCR assays showed that BHV- 1 's infection could repress the basal or inducible transcription of bISG15 in FBL cells.It demonstrates that this repression effect depends on BHV-1 viral infection and new protein synthesis.Our previous work showed that bIRF-3 was the key factor in the stimulation of bISG 15 in FBL cells,so the effect of BHV-1 viral protein on bIRF-3 activating the promoter of bISG15 was confirmed.The luciferase assay showed the BHV-1 viral protein bICP0 inhibited the activation of bISG15 promoter stimulated by bIRF-3.Taken together,our work suggested that BHV-I had some molecular mechanism to resist the cellular bISG15'santiviral functions.

Brain natriuretic peptide (BNP) is one of the most sensitive and specific laboratory indicators during cardiac dysfunction.N-Terminal pro-brain natriuretic peptide (NT-proBNP) is the N-terminal precursor of BNP.The functions of both are not consistent,but its physicochemical properties are superior to BNP.Now it has been widely used in the diagnosis and treatment monitoring of cardiovascular diseases.In the field of stroke,the testing and application of BNP,especially NT-proBNP is less.This article reviews the roles of NT-proBNP in cardio-cerebrovascular diseases,especially in ischemic cerebrovascular disease.
7.Treatment results of different radiotherapy for 763 patients with advanced cervical cancer

Objective To evaluate the clinical value of different radiotherapy protocols in the treatment of advanced cervical cancer. Methods From 1976 to 2006,763 patients with stage Ⅲ cervical cancer(722 with squamous cell carcinoma and 41 with adenocarcinoma)were treated by radiotherapy in our hospital. 113 patients were treated by two-field whole pelvic irradiation in conventional fractionation plus brachytherapy (CF group), 44 by four-field whole pelvic irradiation in accelerated hyperfractionation plus brachytherapy( AHF group), and 606 by concomitant four-field unconventional fractionation radiotherapy and brachytherapy(FRT group). Sixty-one patients were treated by radiotherapy and chemotherapy. Among 350 patients who had complete data, the short-term efficacy and toxicities were compared. Results For patients in CF,AHF and FRT groups, the 3-year overall survival rates (OS) were 65.7%, 66.8% and 44.3%, respectively (P=0.000), and the 5-year OS were 65.7 % ,66.8 % and 36.3 %, respectively (P=0.000). The 10-year OS were 43.3% and 31.9% in CF and FRT groups(P=0.200). For squamous cell carcinoma,the OS was higher of patients with chemotherapy than those without. In 350 patients who had complete data,the local control rates of CF, AHF and FRT groups were 83.0% ,93.2% and 86.1%, respectively(X2=2.70, P=0.259);AHF group had the lowest side effect rate, especially skin reaction (9.1%,X2=20.25,P=0.002) ;CF group had the lowest acute bone marrow suppression rate(X2=25.95,P=0.000);for squamous cell carcinoma, the OS was higher in patients with chemotherapy than those without;the acute bone marrow and intestinal toxicities were more in patients with chemotherapy than those without. Conclusions CF and AHF groups have similar 5-year OS of patients with advanced cervical cancer. AHF group has less toxicities, shorter treatment course and a trend of better local control. Concurrent chemoradiation could improve survival and local control of the patients with advanced squamous cell cervical cancer while increase the side effects. The individual condition should be considered to choose the treatment protocol.
8.The value of Tei index for evaluation on chronic heart failure

Objective To explor the value of Tei index for evaluation the cardiac function of patients with chronic heart failure(CHF).Methods Tei index in 60 patients with CHF(CHF group) and 30 normal controls(control group) were evaluated and compared with the levels of NT-ProBNP and the NYHA class,and the sensitivity and specificity of Tei index for CHF diagnosis were appraised.Results (1) Tei index in CHF group was significantly higher than that in control group.When Tei index was more than and equal to 0.45,the sensitivity,specificity,positive predictive value,and negative predictive value of Tei index for diagnosing CHF were 85.6%,90.4%,89.8% and 78.0%.(2)There was a remarkable positive correlation between Tei and Log NT-ProBNP(r =0.84,P < 0.01).(3)There were significant differences in Tei index among NYHA Ⅱ,Ⅲ,Ⅳ class [(0.47 ± 0.06),(0.56 ± 0.08),(0.64 ±0.13)].Conclusion Tei index can be used to diagnose CHF and evaluate the degree of it.

Objective To provide clinical experience in treatment of cook′s fungus infection of evidence‐based medical evidence , separation of medical environment of cook′s bacteria identification and antimicrobial susceptibility test .Methods Collected in the hospital infection of hygiene monitoring ,collection and medical staff hands ,skin and mucous membrane ,air and object surface sam‐ples from 86 strains of cook ,application ATB Expression semi‐automatic bacteria identification of susceptibility analyzer supporting article try ID32 Staph identification and drug susceptibility test Staph 5 ,strain identification and drug sensitive test .Results 86 strains bacteria identification cook 62 strains bacteria ,mutation detection rose cook bacteria strains of 16 ,cook eight strains of bac‐teria .Cook bacterium of norfloxacin(16 .3% ) ,and with nitrofurantoin low because of the sensitive rate(20 .9% ) ,of erythromycin (69 .3% ) and gentamycin(67 .4% ) ,the sensitive rate at nearly 70 .0% ,sensitivity to penicillin and other 12 kinds of antimicrobial a‐gents more than 80 .0% ,even 100 .0% .Conclusion Cook for most still sensitive ,clinicians and microorganism inspection personnel should strengthen the cook .
10.EFFECT OF BERBAMINE ON METABLIC ENZYME IN THE PROCESS OF INCREASING LEUKOCYTE

Berbamine ( Ber. ) is a bis-benzyl-isoquinoline alkaloid isolated from the traditional Chinese medical herb, Berberis poiretis, and has Vide pharmacological effects. Clinical studies have demonstrated that Ber. had an effect of ingreasing the number of leukocytes in various leukocytopenia. But, up till now it has not been reported that Ber. take effect on enzymic metablism of inner cell and isoenzyme in the process of increasing leukocyte. The experiment observed the mice dynamic change of LDH in leukocyte and LDH isoenzyme of serum by cytochemistry and plane polyacrylamide gel electrophoresis tech-nigue. The result indicates that LDH enzyme reaction of granulocyte have increased in Ber. group and intensity of enzyme reaction has a positive interralation with the time of administrations. All specimen show 5 zones after LDH isoenzyme pattern analyse. The proportion of LDH-5 was changed and LDH-5 sub-band was detected in control and combination group. The experiment indicated that Ber. can affeet enzymic metablism of inner cell in the process of increasing leukocyte. There was simultaneously with a change in ratio of LDH isoenzyme pattern.
