1.Laparoscopic ovarian cystectomy 96 patients with benign clinical experience

Objective To evaluate the laparoscopic ovarian cystectomy of benign lesions of security,superiority.Methods Laparoscopic ovarian cystectomy of benign lesions of security,superiority.Results The operation time of laparoscopic group (40.5 ± 5.3) min,significantly shorter than the control group (60.7 ± 6.8) min (t =-14.815,P =0.000) ; Bleeding (55.1 ± 9.8)ml was significantly less than that in the control group (90.0 ±15.0) ml (t =-18.275,P =0.001) ; Anus exhaust time (13.5 ± 8.3) hour was significantly shorter than that in control group (24.5 ± 9.6) hour (t =-4.351,P =0.000).The average postoperative hospital stay was (3.5 ±1.3) days was significantly shorter than that in control group (5 ± 2) days (t =-2.158,P =0.000).Postoperative morbidity was not statistically different from controls (t =0,5,P =0.053).Postoperative pain was significantly lower than that in control group (x2 =8.525,P =0.002).Conversion to open operation in laparoscopic group:2 cases,postoperative analgesic drugs were not needed,the postoperative 12h after catheter removal started eating and ambulation,postoperative shoulder pain in 2 cases ; open operation group after all need to use analgesics,postoperative 24h after catheter removal started eating and ambulation,abdominal incision fat liquefaction and delayed healing in 4 cases.The two groups had no visceral injury,postoperative bleeding,and chemical peritonitis.Conclusion Laparoscopy in the treatment of benign ovarian tumor is safe,less bleeding,rapid recovery,fewer complications and other characteristics.
2.Influence factors of depression in patients with diabetic foot

Objective To investigate the prevalence of depression among the patients with diabetic foot and analyze the influence factors. Methods One hundred and ten patients with diabetic foot were inquired and assessed with patient health questionnaire for self-administered measurement (PHQ-9), meanwhile, the demo-graphic data, metabolic data and diabetes behaviors were also investigated. Results Prevalence of depression was 47.3%. Logistic regression analysis showed that alternation of diarrhea and astriction (OR = 6.901, P =0.017) and formication (OR = 23.401, P = 0.009) were risk factors, and medical insurance (OR = 0.217, P =0.007) was a protective factor. Conclusions Depression is a frequent mental disorder in patients with diabet-ic foot and its influence factors include alternation of diarrhea and astriction , formication and medical insur-ance .
3.Barium-based Colonic Transit Detection and Individualized Treatment for Chronic Constipation

Background:Chronic constipation is a major cause of impaired quality of life in modern society. Reasonable and effective management of chronic constipation could be achieved based on the principle of evidence-based medicine and the modern concept of constipation,and this is a challenge facing the clinicians. Aims:To investigate the role of barium-based colonic transit detection in diagnosis and treatment of chronic constipation. Methods:Fifty patients with chronic constipation from Apr. 2013 to Oct. 2014 at the First Hospital of Shanxi Medical University were recruited and randomly allocated into two groups,control group and individualized treatment group. Patients in individualized treatment group received 20 barium markers orally and abdominal plain radiography was performed 48 and 72 hours later,respectively for calculating the colonic transit index. According to the type of colonic transition and the characteristics of colonic motility estimated by colonic transit index and clinical manifestations,an individualized therapeutic regimen was formulated and the efficacy was evaluated. Patients in control group were treated empirically according to the clinical manifestations. Results:Mosapride and lactulose or polyethylene glycol were administered orally in control group;when abdominal pain or abdominal distension was predominant,pinaverium bromide or trimebutine was used respectively instead of mosapride. Barium-based colonic transit detection revealed that 9 patients in individualized treatment group were slow transit constipation,6 were outlet obstructive constipation and 8 were the mixed type. After 2 weeks of empirical or individualized treatment,the defecation rates of the two groups were 24. 0%(6 / 25)and 52. 2%(12 / 23)within 24 hours and 64. 0%(16 / 25)and 87. 0%(20 / 23)within 48 hours,respectively(P all < 0. 05). Conclusions:Barium-based colonic transit detection is a simple,economical and practical modality for guiding the individualized treatment in patients with chronic constipation.
4.Effects of isobavachalcone on proliferation and apoptosis of tongue squamous cell carcinoma Tca-8113 cells and their mechanisms

Objective:To observe the effects of isobavachalcone (IBC)on the proliferation and apoptosis of tongue squamous cell carcinoma Tca-8113 cells,and to explore their mechanisms. Methods:The Tca8113 cells cultured in vitro were divided into control group and different doses (0, 10, 20,40, 80 μmol · L-1 )of IBC groups.The inhibitory rates of cell proliferation were detected by MTT method.The apoptosis was detected by flow cytometry.Western blotting method was used to detect the expressions of Akt,p-Akt,Erk,p-Erk,Bax, Bcl-2 and Caspase-3 proteins in the Tca8113 cells in various groups.Results:The MTT results showed that the inhibitory rates of proliferation of Tca8113 cells were increased in a concentration-and time-dependent manner;the IC50 at 12,24 and 48 h were (285.13±8.97), (132.40±7.76),and (58.56±5.93)μmol·L-1 ,respectively;and there were significant differences between different time points (P <0.05).The FCM results showed that the apoptotic rates of Tca8113 cells in 20 and 40 μmol·L-1 IBC groups at 48 h were (8.21 ±2.32)% and (22.45 ± 1.18)%, respectively; compared with control group (1.69% ± 0.65%), the differences were statistically significant (P <0.05).The Western blotting results showed that the expression levels of Bcl-2,p-Akt,and p-Erk, in 20 and 40 μmol·L-1 IBC groups were decreased and the expression levels of Bax were increased compared with control group (P < 0.05);the expression levels of Akt and Erk had no significant changes (P > 0.05 ).The expression levels of Caspase-3 in Tca8113 cells in 40 μmol·L-1 IBC group at 48 h were increased compared with control group (P <0.05).Conclusion:IBC could inhibit the proliferation and induce the apoptosis of Tca8113 cells;Akt and Erk signaling pathway may be the pathway of IBC to induce the apoptosis of tumor cells.
5.Exosomes secreted from IFN-γprestimulated hUC-MSCs induce regulatory T cells

Aim To investigate whether human umbili-cal cord mesenchymal stem cells(hUC-MSCs)exposed to inflammatory conditions could release large amounts of exosomes to induce regulatory T cells(Treg).Meth-ods hUC-MSCs were isolated by enzyme digestion method.(In vitro)interferonγ(IFN-γ)was added in-to hUC-MSCs to mimic inflammatory microenviron-ments,then exosomes were extracted from the superna-tant of normal conditional medium or IFN-γpretreated hUC-MSCs.Both sources of exosomes,Nor-hUC-exo and IFN-γ-stimulated hUC-exo, were identified by Nanoparticle Trafficking Analysis (NTA )and Western blot for the exosome-enriched protein CD63 .Next,hu-man peripheral blood mononuclear cells (PBMCs ) stimulated with PHA were respectively co-cultured with hUC-MSCs,IFN-γ-pretreated-hUC-MSCs,hUC-MSCs exosomes or IFN-γ-stimulated-hUC-MSCs exosomes for 5 days to assess the exosomes-T cells communication. The proliferation rate of PBMCs and frequency of CD4 +/CD25 +/Foxp3 + Treg were measured by flow cytometry.Results The isolated cells from human um-bilical cord tissue,which were positive for CD73, CD44,CD29,CD90 and HLA-ABC,but were nega-tive for CD31 and CD34,were mesenchymal stem cells indeed.After IFN-γtreatment,hUC-MSCs secreted nu-merous exosomes(P<0.05 ).Morerover,there was a significantly higher level of CD63 ,but no difference in diameter between Nor-hUC-exo and IFN-γ-stimulated hUC-exo.IFN-γ-stimulated hUC-exo had a superior a-bility compared with Nor-hUC-exo to suppress the pro-liferation of PHA stimulated PBMCs due to their upreg-ulation of the percentage of Treg (1 1.53 ±0.88% vs 6.60 ±0.56%,P <0.01 ).Conclusion hUC-MSCs could promote the expression of Treg to modulate im-munosuppression through exosomes,especially for IFN-γ-licenced exosomes,which might carry much immu-notherapeutic potential.
6.Study of the mechanism underlying the effect of SOCS3 rs4969170 A/G polymorphism on the occurrence of insulin resistance in chronic hepatitis C patients.

OBJECTIVETo study the mechanism underlying the effect of the SOCS3 rs4969170 A/G alleles on the occurrence of insulin resistance (IR) in patients with chronic hepatitis C.
METHODSThe promoter region of the SOCS3 gene was amplified by PCR,and luciferase expression vectors were constructed and transfected into HepG2,Huh7 cell lines.The relative luciferase activity of each expression vector was assessed by the dual luciferase reporter gene assay system.Western blotting was used to detect SOCS3 protein expression in PBMCs from groups of patients with the rs4969170 AA and AG genotypes.The state of IR in eight patients was evaluated by determining their HOMA-IR.
RESULTSThe pGL3-A, PGL3-G and pGL3-control vectors showed significantly different luciferase expression in the HepG2 cells (0.121 00 ± 0.022 07,0.027 00+/-0.012 49 and 0.043 33 ± 0.005 51; F =48.068, P=0.001) and in the Huh7 cell lines (0.164 70 ± 0.007 10,0.027 33 ± 0.017 04 and 0.033 67 ± 0.014 98; F =115.137, P=0.001). The expression of SOCS3 protein was significantly higher in the rs4969170 AA genotype group than in the AG genotype group (1.22 ± 0.40 vs. 0.30 ± 0.19; t =4.149, P=0.006).The IR index of patients with the rs4969170 AA genotype and the AG genotype was 4.11 ± 2.62 and 1.47 ± 1.01 respectively.There were three patients with IR in the rs4969170 AA genotype group and one in the rs4969170 AG group. There was no statistically significant difference between the two genotype groups (t=1.881, P=0.109).
CONCLUSIONSThe SOCS3 rs4969170 A haplotype may enhance transcriptional activity of the gene promoter to regulate gene expression, thereby increasing intracellular SOCS3 protein level and ultimately interfering with insulin signaling and causing IR in patients with chronic hepatitis C.

7.Analysis of abdominal internal hernia with main manifestation of metabolic acidosis

Abdominal internal hernia is relatively rare in clinics, which is easily to be ignored and occur serious complications, mainly the obstruction of gastrointestinal tract, such as gastrointestinal ischemia and necrosis, that could be life-threatening. This article introduced one case with atypical abdominal pain as the first manifestation of the disease, then the main symptom was metabolic acidosis and treatment was carried out, and finally by surgery the diagnosis of abdominal internal hernia was confirmed and proper treatment given.

9.Quality of life and its correlation between patients with malignant tumor of middle or terminal stages and their family caregivers

Objective To study the quality of life and the relationship between patients with malignant tumor of middle or terminal stages and their family caregivers.Methods Totals of 136 patients, diagnosed as malignant tumor of middle or terminal stages in Shaanxi Provincial People′s Hospital, and their family caregivers were selected as research object, and were surveyed and analyzed by quality of life scales respectively for patients with malignant tumor of middle or terminal stages and for their family caregivers.ResultsTotal scores of quality of life for family caregivers were positively correlated with scores of various dimensions and total scores in the scale for the patients (r=0.21~0.48,P<0.05); total scores of quality of life for patients with malignant tumor of middle or terminal states were positively correlated with scores of various dimensions (except the dimension of "relationship") and total scores in the scale for the family caregivers (r=0.14~0.47, P<0.05); severity of the patients′ disease were negatively correlated with their relationship with their caregivers (r= -0.16,P<0.05).Conclusions Quality of life of patients with malignant tumor of middle or terminal stages may influence quality of life of their caregivers. Scheme of family nursing helps to improve quality of life of the patients.
10.Characteristics of total cerebral small vessel disease burden in octogenarians with cerebral small vessel disease and its relation with serum uric acid levels

Objective:To analyze the characteristics of total cerebral small vessel disease (CSVD) burden in octogenarians with CSVD and to investigate its correlation with serum uric acid.Methods:From January 2019 to December 2020, patients with CSVD diagnosed by 3.0 T magnetic resonance imaging (MRI) in Departments of Neurology and Departments of Geriatrics, Liaoning Jinqiu Hospital were enrolled retrospectively. The patients were scored and grouped according to the total CSVD burden assessment standard. Ordered logistic regression analysis was used to investigate the independent influencing factors of the total CSVD burden score, and the Spearman rank correlation method was used to conduct the correlation analysis between serum uric acid and the total CSVD burden score. Results:A total of 217 octogenarians with CSVD were enrolled in the study. Their age was 87.3±4.5 years, 150 were males (69.1%), and their median serum uric acid was 313.0 μmol/L. The total CSVD burden score: 50 patients (23.1%) in 1-point group, 79 (36.4%) in 2-point group, 68 (31.3%) in 3-point group, and 20 (9.2%) in 4-point group. The imaging subtypes of CSVD were mainly asymptomatic lacunar infarcts (ALIs; 98.2%) and white matter hyperintensities (WMHs; 77.0%). The CSVD subtype combination was different between different total CSVD burden score groups. There were only two subtypes in the 1-point group, mainly ALIs (92.0%), and WMHs accounted for only 8.0%. Four subtypes could be observed in other groups, among which ALIS+ WMHs mixed subtype (100% in the 3-point group and the 4-point group) accounts for the highest proportion. The detection rates of cerebral microbleeds (CMBs) and enlarged perivascular spaces (EPVS) increased significantly with the increase of the total CSVD burden score. The 3-point group was 44.1% and 55.9% respectively. Univariate analysis showed that there were significant differences in age, male, hypertension, coronary heart disease, serum uric acid level, high-density lipoprotein cholesterol and the highest quartile of serum uric acid between total CSVD burden score groups ( P<0.05). Multinomial ordinal logistic regression analysis showed that there was significant independent correlation between age (odds ratio 1.074, 95% confidence interval 1.013-1.140; P=0.017), serum uric acid (odds ratio 1.005, 95% confidence interval 1.000-1.009; P=0.032) and the total CSVD burden score. Spearman rank correlation analysis showed that there was a positive correlation between serum uric acid and the total CSVD burden score ( r=0.153, P=0.024). Conclusions:ALIs and WMHs are most common in octogenarians with CSVD. Four subtypes were observed in each group ≥2, and the proportion of EPVS and CMBs also increases with the increase of the total CSVD burden score. There is an independent correlation between serum uric acid and the total CSVD burden in the octogenarians.
