1.Effect of Fluoxetine on Depression Neurologic Impairment and Activity of Daily Living after Cerebral Infarction

Objective To observe the effect of fluoxetine on the depression neurologic impairment and activity of daily living after cerebral infarction.Methods 80 patients with acute cerebral infarction following depression were randomly divided into treatment group and control group, 40 cases in each group. Two groups were assessed with Modified Edinburgh-Scandinavia Stroke Scale(MESSS), Hamilton Depression Scale(HAMD) and Modified Barthel Index(MBI) before and after treatment.Results There were no significant difference between the two groups before treatment in HAMD, MESSS and MBI(P>0.05), while there were significantly different between the two groups in HAMD, MESSS and MBI 6 weeks after treatment(P<0.01). Conclusion Fluoxetine can alleviate depression of patients with acute cerebral infarction and improve the symptom of neurological function and the activity of daily living.
2.Accuracy of ultrasonography in determining laryngeal mask airway position: evaluation using 3D-CTR

Objective:To evaluate the accuracy of ultrasonography in determining the laryngeal mask airway position using three-dimensional CT reconstruction (3D-CTR) images as the standard.Methods:One hundred and thirty-four patients, aged 18-64 yr, weighing 30-100 kg, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ, scheduled for elective cerebral vascular interventional stent surgery in our hospital, were selected.Cervical ultrasonography was performed before and after laryngeal mask airway placement, and digital subtraction angiography was performed during operation to obtain 3D-CTR images.Neck ultrasound and 3D-CTR images were analyzed and graded.Results:A total of 125 patients completed the study.At the vertical position of the laryngeal mask airway, ultrasonic grading was positively correlated to 3D-CTR grading ( r=0.742, P<0.05). The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of ultrasound in determining the rotation of the laryngeal mask airway was 73.0% (95% CI: 62.0%-84.4%), 92.8% (95% CI: 86.9%-98.7%), 89.1% (95% CI: 80.5%-97.7%), 81.0% (95% CI: 72.6%-89.4%), and 74.2% (95% CI 65.2%-81.1%), respectively.In terms of laryngeal mask airway depth grading, the correlation between ultrasonic grading was positively correlated to 3D-CT grading ( r=0.332, P<0.05). The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of ultrasound in determining the depth of the laryngeal mask airway was 58.5% (95% CI: 46.9%-70.1%), 93.3% (95% CI: 87.2%-99.4%), and 90.5% (95% CI: 82.0%-99.0%), 67.5% (95% CI: 57.7%-77.3%), and 33.2% (95% CI: 16.8%-47.8%), respectively.The positive predictive value of ultrasound in determining the optimal placement of the laryngeal mask airway was 61.1% (95% CI: 48.6%-73.6%). Conclusion:Although ultrasound cannot measure the depth of the tip of the laryngeal mask airway into the esophagus, it has higher accuracy in determining the rotation and can be used as a reliable tool for clinical detection of the position of the laryngeal mask airway, especially the rotation.
3.Effect of dexmedetomidine on cell apoptosis during acute lung injury induced by blunt chest trauma in rats

Objective To evaluate the effect of dexmedetomidine on the cell apoptosis during the acute lung injury induced by blunt chest trauma in rats.Methods Thirty pathogen-free male SpragueDawley rats,weighing 240-270 g,aged 8 weeks,were divided into 3 groups (n=10 each) using a random number table:shan operation group (group Sham),blunt chest trauma group (group T) and blunt chest trauma plus dexmedetomidine group (group TD).The model of acute lung injury was induced by dropping a 300 g weight onto a precordium in anesthetized rats.After the model was established,dexmedetomidine was intravenously infused at a rate of 5 μg · kg 1 · h-1 for 6 h in group TD.At 6 h after the model was established,blood samples were collected from the femoral artery for determination of concentrations of serum tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6).The rats were sacrificed after blood sampling,and lungs were removed for examination of the pathological changes (with light microscope) and for determination of Bax and Bcl-2 expression in lung tissues (by immuno-histochemistry) and cell apoptosis (by TUNEL).Bcl-2/Bax ratio and apoptosis index (AI) were calculated.Results Compared with group C,the serum TNF-α and IL-6 concentrations and AI were significantly increased,the expression of Bax and Bcl-2 was up-regulated,and Bcl-2/Bax ratio was decreased in T and TD groups (P< 0.01).Compared with group T,the serum TNF-α and IL-6 concentrations and AI were significantly decreased,the expression of Bax in lung tissues was down-regulated,the expression of Bcl-2 in lung tissues was up-regulated,Bcl-2/Bax ratio was increased (P<0.05),and the pathological changes of lung tissues were significantly attenuated in group TD.Conclusion The mechanism by which dexmedetomidine mitigates the acute lung injury induced by blunt chest trauma is related to inhibition of cell apoptosis in rats.
4.Influence research on alcohol to genotype disposition of HCV and change of cytokines in HCV patients

Objective:To analyze the genotypes of HCV RNA in alcoholic and nonalcoholic HCV patients and contrast the distribution of the genotypes between those,identifying whether alcoholic patients have a higher susceptibility to certain genotype than nonalcoholic patients.Meanwhile,to detect the level of IFN-? and TNF-? for gaining the mechanism of hepatic lesion in alcoholic HCV patients.Method:114 HCV patients,according to the history of alcohol abuse,were divided into the alcohol group and nonalcohol group.The genotypes of HCV RNA in the two groups were performed by gene chip technique.Liver biopsy was practiced in 30 patients to diagnose.Ficoll gradient centrifugation and ELISA were used for extraction of PBMC and detection of IFN-?and TNF-?,respectively.Result:The genotype 1b was greater than other genotypes in the two groups,and there was no statistical significance(Alcohol group was 60% and nonalcohol group was 43.75%,P=0.22).The serum titers of HCV RNA of alcohol group were(1 1100?14 174.3)?103copies/ml and the nonalcohol group were(3 927.8?4 549.6)?103copies/ml,and there was statistical significance between the two groups(P=0.025).Among 18 alcoholic patients of 30 patients practiced liver biopsy,their liver inflammation was higher than nonalcoholic patients.The level of TNF-?of the alcohol was higher than that of nonalcohol,there was statistical significance between the two groups(P=0.048) and no statistical significance between those of IFN-?.Conclusion:Alcohol can promote the copies of HCV RNA in HCV patients and aggravate liver injury,but can't add susceptibility to certain genotypes of HCV RNA.The liver inflammation of alcoholic patients is higher than that of nonalcoholic patients.TNF-?,which builds an important effect in alcoholic HCV patients,is one of the important mechanisms in the liver injury.

Objective To evaluate the effect of combination of growth hormone(GH),somatostatin(ST) with hypocaloric parenteral nutrition(HPN) on SAP patients with MOD. Methods forty one cases of SAP complicated with MOD were randomedly divided into control group (20 cases) and experiment group (21 cases). Objective: To establish HPLC method for the determination of loureirin A and loureirin B in Sanguis Draxonis Capsules.Methods: HPLC system included C 18 reverse phase column and acetonitrile 1% acetic acid (34∶66) as mobile phase, detection at 280nm and external standard method.Results:The standard curves of loureirin A was linear in the concentration range of 98~490ng, r =0.9996.The average recovery was 97.45%, RSD was 1.82%.The standard curves of loureirin B was linear in the concentration range of 43.2~259.2, r = 0.9993 .The average recovery was 96.92%, RSD was 1.57%.The contents of loureirin A and loureirin B in Sanguis Draxonis from heating free technology were higher than traditional technology.Conclusion: This heating free technology is a new technique that is a creation with higher extract rate,this method is worth populariging.
7.Effects of dexmedetomidine on hemodynamics and myocardial injury in patients with pulmonary hypertension undergoing mitral valve replacement

Objective To investigate the effects of dexmedetomdine on hemodynamics and myocardial injury in patients with pulmonary hypertension undergoing mitral valve replacement.Methods Forty ASA Ⅱ or Ⅲ patients with pulmonary hypertension,aged 18-64 yr,undergoing mitral valve replacement,were randomized into 2 groups (n =20 each):control group (group C) and dexmedetomidine group (group D).Anesthesia was induced with iv midazolam 0.05 mg/kg,sufentanil 1 μg/kg,etomidate 0.3 mg/kg and rocuronium 0.6 mg/kg and maintained with combined intravenous-inhalational anesthesia.Narcotrend index values were maintained at 40-50.The patients were tracheal intubated and mechanically ventilated.A loading dose of dexmedetomidine 1 μg/kg was injected intravenously over 10 min after intubation followed by infusion at 0.5 μg· kg-1· h-1 until the end of operation in group D.While the equal volume of normal saline was given in group C.Venous blood samples were taken immediately before dexmedetomidine administration,at 10 min after termination of cardiopulmonary bypass (CPB),at the end of operation,and at 6 and 24 h after operation for determination of plasma levels of creatine kinase MB (CK-MB) and cardiac troponin Ⅰ (cTnI) by ELISA.Before and after administration of dexmedetomidine,at skin incision,at sternum splitting,before and after CPB,and at the end of operation,HR,MAP,mean pulmonary arterial pressure (MPAP),cardiac output (CO),systemic vascular resistance (SVR) and pulmonary vascular resistance (PVR) were detected by using Swan-Ganz catheter and Vigilance Ⅱ monitor.The requirement for vasoactive drugs and condition of spontaneous heart beats were recorded.Results In group D,each hemodynamic parameter was maintained stable at skin incision and sternum splitting,and MPAP was decreased after CPB and maintained at a lower level.The plasma levels of CK-MB and cTnI,consumption of phenylephrine and dopamine,and requirement for epinephrine were significantly lower in group D than in group C (P < 0.05).There was no significant difference in the consumption of milrinone and rate of spontaneous heart beat between the two groups (P > 0.05).Conclusion During mitral valve replacement for patients with pulmonary hypertension,dexmedetomidine infused at 0.05 μg·kg-1· h-1 after a loading dose of 1 μg/kg is helpful in maintaining systemic circulation and pulmonary circulation stable and provides myocardial protection to some extent.
8.Epidemiology investigation of chronic kidney disease in 1031 consecutive cases undergoing coronary angiography

Objective To investigate the prevalence of CKD in patients underging coronary angiography with suspected coronary heart disease(CHD). Methods A total of 1031patients with suspected CHD undergoing coronary angiography in Zhongda Hospital from December 2008 to October 2009 were enrolled in the study.The prevalence of CKD and associated risk factors were analyzed.GFR Wag estimated with MDRD equation.CKD was deftned as eGFR<60branches of coronary artery was considered as CHD. Results The mean age of patients were (64.37±11.02)years.There were 543 males and 488 females,including 551 patients with CHD and 134 patients with CKD(13%).Patients with CHD had a significantly higher prevalence of CKD compared with patients without CHD(18.33%vs 6.88%,P<0.01).With the increasing number of stenosis coronary vessels(n=0,1,2,3),eGFR was declined[(84.25±19.00),(81.61±23.92),increased(0.42%,0.82%,1.96%,3.25%,P=0.006),and the prevalence of CKD increased (6.88%,13.11%,21.57%,23.38%,P<0.01).Logistic regression analysis indicated that increasing age(OR 1.094,95%C/1.068 to 1.120),increasing number of stenosis coronary vessels(OR 1.288,95%CI 1.074 to 1.543).hypertension(OR 1.974,95%CI 1.082 to 3.603),cardiac systolic insufficiency(OR 3.183,95%CI 1.696 to 5.972),and hyperuricemia(OR 5.366,95%CI 3.224 to 8.9311 were risk factors for CKD. Conclusions The prevalence of CKD in patients with CHD diagnosed by coronary angiography is quite high.Aging,elevated number of stenosis coronary vessels,hypertension,cardiac systolic insufficiency and hyperuricemia are important risk factors for angiographic patients with CKD.
9.The association of hepatitis B virus DNA levels in the peripheral blood mononuclear cell with antiviral efficacy in chronic hepatitis B

Objective To explore the relationship between the antiviral effect and peripheral blood mononuclear cell (PBMC) hepatitis B virus (HBV) DNA when the patients reach the standard of withdrawal of antiviral therapy in chronic hepatitis B (CHB).Methods Ninety CHB patients treated with interferon(n=44) or nucleot (s) ide(n=46) who reached the standard of withdrawal of antiviral therapy were recruited.HBV DNA levels in PBMCs were tested at the end of treatment,and its relationship with serum HBV DNA level before treatment in PBMC HBV DNA positive group and negative group were compared.The correlation between HBV DNA in PBMCs at the end of treatment and relapse were explored.Measurement data were analyzed by student t test and enumeration data were analyzed by X2 test.Results Among 90 patients,67(74.4%) were PBMC HBV DNA negative at the end of treatment,and 23(25.6%) were positive.The serum HBV DNA positive conversion rate in PBMC HBV DNA negative patients was 13.4%,which were significantly lower than that in positive group (73.9%) (X2=30. 4873, P<0.01 ). There were no significant differences of alanine aminotransferase (ALT) levels when hepatitis flare (t=0. 8729, P=0. 3913) and relapse time (t=1. 9222, P=0. 0665) between PBMC HBV DNA negative group and positive group after withdrawal of therapy, while the serum HBV DNA rebound was greater in positive group than that in negative group (t=2. 7493, P=0. 0112). There were five patients who achieved hepatitis B surface antigen (HBsAg) seroconversion, whose PBMC HBV DNA were all undetectable, and none relapsed during follow-up for 6-12 months. The pretreatment HBV DNA as level in PBMC HBV DNA positive was (7.2±1.1) lg copy/mL, which was much higher than that in negative group[(5.2±2.1) lg copy/mL] (t=4. 3557, P<0.01). Conclusions In patients who reach the standard of drug withdrawal,PBMC HBV DNA at the end of treatment is an important predictor for durability of antiviral therapy in CHB.
10.Association between dietary patterns during pregnancy and gestational diabetes mellitus

Objective:
To explore the association between dietary patterns during pregnancy and the development of gestational diabetes mellitus (GDM), so as to provide the guidance of diet balance and GDM prevention for pregnant women.
Methods:
Pregnant women who underwent prenatal examination at the First Affiliated Hospital of Xinjiang Medical University from December 2021 to July 2022 were selected as the subjects. The oral glucose tolerance test (OGTT) was employed at 24 to 28 weeks of gestation to diagnose GDM. The questionnaire of "The Survey on Nutrition and Health of Pregnant Women in China" was used to collect general information, food intake frequency and amount during a month before OGTT. Dietary patterns were determined by factor analysis, and the association between dietary patterns and the development of GDM was identified using a multivariable logistic regression model.
Results:
Totally 449 pregnant women were included, with a median (interquartile range) age of 31.00 (5.00) years and a median (interquartile range) gestational age of 35.00 (3.00) weeks. There were 7 dietary patterns named potato-cereal pattern, poultry-meat-dessert pattern, milk-egg-aquatic product pattern, mushroom-vegetable-offal pattern, fruit-bean pattern, pickle-wheaten food pattern, and beverage-nut pattern according to the food types with high factor loads. There were 89 cases diagnosed as GDM, with a prevalence rate of 19.82%. Multivariable logistic regression analysis showed that the development of GDM was associated with the poultry-meat-dessert pattern (OR=0.242, 95%CI: 0.086-0.678) and the fruit-bean pattern (OR=0.093, 95%CI: 0.025-0.342), compared with the potato-cereal pattern.
Conclusion
Compared to the potato-cereal pattern, the poultry-meat-dessert pattern and the fruit-bean pattern may reduce the risk of GDM.
