Objective To evaluate the penumbra of a new multileaf collimator equipped with Elekta Synergy accelerator. Methods The penumbra were derived from beam profiles measured in air and water using PinPoint ion chamber with PTW MP3 water phantom. Variations of penumbra with X-ray beam energy, depth in water, and leaf position were investigated. Results The penumbra in air for 6 MV X-ray was 2 mm less than that at depth of maximal dose in water. The penumbra of leaf side was 1 mm less than that of the leaf end. The penumbra had close relationship with beam energy, depth in water and leaf position. Penumbra was increased with beam quality and water depth. The leaf position had great influence on the penumbra. Conclusions The penumbra of the multileaf collimator is related to its original design and radiation delivery technique. Special considerations should be taken into during treatment planning.Regular measurement should be performed to guarantee the delivery quality.

Objective To determine whether pulmonary arterial hypertension(PAH) and pulmonary venous hypertension (PVH) can be differentiated noninvasively by echocardiography. Methods Fifty-six patients with pulmonary arterial systolic pressure(PASP) ≥40 mmHg by echocardiography were involved, and cardiac catheterization performed within 7 days of each other. Based on left ventricular end-diastolic pressure or pulmonary capillary wedge pressure(PCWP) ,30 patients were classified as PAH group and 26 patients as PVH group. The early(E) and late(A) diastolic mitral inflow velocities,E/A ratios, deceleration time(DT),early dastlic mitrial annular velocity(E') and E/E' ratios were measured by conventional and Doppler tissue imaging echocardiography in the two groups. Results Compared with PVH group, the PAH group had significantly higher A,DT,PASP and E',and significantly lower E,E/A ratio and E/E' ratio (P < 0. 01 or P <0. 001). E/E' and E/A ratio was optimal indexes for differentiation of PAH and PVH,the area under receiver operating characteristic curve was 97% and 91 %, respectively. Optimal cutoff for diagnosing PVH was E/E'>9.2 (sensitivity 95%, specificity 97%) and E/A> 1. 7 (sensitivity 75%,specificity 92%). Conclusions PAH and PVH imaging can be reliably differentiated by echocardiography.

A novel series of sorafenib analogs containing 2-picolinyl hydrazide moiety were designed and synthesized. In vitro, most of synthesized compounds have antiproliferation activity on MDA-MB-231, ACHN, HepG2, Mia-PaCa-2 and SW1990 cell lines tested by MTT assay. It is worth noting that the antitumor activities of compounds 2c, 2d and 2f are more potent than that of sorafenib on pancreatic cancer cells Mia-PaCa-2 and SW1990, and the activities of compounds 3f and 3g are 2-3 times than that of sorafenib on human hepatocellular carcinoma HepG2 cell line.

Objective To explore the potential relationship between 6 single nucleotide polymorphisms (SNPs) of mitochondrial DNA (mtDNA) coding region and Parkinson's disease (PD) in northern Chinese population. Methods Samples from 353 patients with PD and 389 control participants, collected in our hospital from April 2010 to November 2015,were genotyped by multiplex polymerase chain reaction (PCR)-restriction fragment length polymorphism (RFLP) techniques.Six SNPs (T4216C within the ND1 gene,A4917G within the ND2 gene,A11084G within the ND4 gene,A12308G within the tRNA gene,A13966G within the ND5 gene, and G15928A within the tRNA gene) in mtDNA were amplified in multiplex PCR systems and subsequently genotyped by digestion with endonucleases. Results An efficient, rapid, and economical way of multiplex genotype SNPs was successfully established. No significant differences were observed in the 6 loci and corresponding haplotypes between PD and control cohorts (P>0.05).After stratification by gender, however, frequencies of allele 11084G and the haplotype T-A-G-A-A-G in female group with PD were all higher than those in control ones, with significant differences (P<0.05).Conclusions The 6 SNPs may not be susceptible factors in the northern Chinese population. The A11084G missense mutation might be related to PD in female patients.

Objective To investigate the value of peripheral blood soluble interleukin-2 receptor(sIL-2R),CD4+lymphocyte percentage/CD8+lymphocyte percentage ratio(hereinafter referred to as CD4+/CD8+)and tumor necrosis factor-α(TNF-α)in evaluating the efficacy of chemotherapy in elderly patients with newly treated active pulmonary tuberculosis. Methods A total of 102 elderly patients with newly treated active tu-berculosis admitted to the hospital from December 2019 to December 2022 were enrolled in the study as the observation group, and 102 healthy people aged 60 and older who underwent physical examination in the hos-pital during the same period were enrolled as the control group. The levels of sIL-2R,TNF-α and CD4+/CD8+in peripheral blood were compared between the two groups, and the correlations between sIL-2R,TNF-α and CD4+/CD8+were analyzed. The observation group was treated with 2HRZE/4HR anti-tuberculosis treatment regimen. The levels of sIL-2R,TNF-α and CD4+/CD8+in peripheral blood of patients with different efficacy before treatment,1 month and 6 months after treatment in the observation group were compared. The correla-tion between sIL-2R,CD4+/CD8+,TNF-α levels and therapeutic effect was analyzed. The receiver operating characteristic(ROC)curve was used to analyze the efficacy of indicators in evaluating the efficacy of chemo-therapy in elderly patients. Results The levels of sIL-2R and TNF-α in the observation group were higher than those in the control group, while CD4+/CD8+was lower than that in the control group, and the differ-ences were statistically significant(P<0.05).In the observation group,sIL-2R and TNF-α were negatively correlated with CD4+/CD8+(P<0.05),sIL-2R was positively correlated with TNF-α(P<0.05).After 1 month and 6 months of treatment, the levels of sIL-2R and TNF-α in patients with apparent efficacy were low-er than those in patients with efficacy, and the latter were lower than those in patients with no effect, while the CD4+/CD8+in patients with apparent efficacy was higher than that in patients with efficacy, and the latter was higher than that in patients with no efficacy, and the differences were statistically significant(P<0.05).The levels of sIL-2R and TNF-α were negatively correlated with the efficacy(P<0.05),and CD4+/CD8+was positively correlated with the efficacy(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of sIL-2R,CD4+/CD8+,and TNF-α used in combination to assess efficacy was significantly greater than the AUCs of the single indicators used in the assessment at each time point of treatment(P<0.05),and the AUC of the combination of the indicators was greater after 6 months of treatment than after 1 month of treatment(P<0.05).Conclusion The levels of sIL-2R,CD4+/CD8+and TNF-α are closely related to the ef-ficacy of chemotherapy in elderly patients with newly treated active pulmonary tuberculosis, and the combina-tion of the above indicators has certain reference value in evaluating the efficacy of chemotherapy in patients.

Objective:To explore the risk factors of poor renal prognosis in patients with coronary artery bypass surgery (CABG)-associated acute kidney injury (AKI), and establish a preliminary clinical risk prediction model for chronic kidney disease (CKD) progression in CABG-associated AKI patients, and evaluate its predictive efficacy. Methods:It was a retrospective, observational cohort study. The study subjects were patients who underwent CABG at Central China Fuwai Hospital from January 1, 2018 to December 31, 2020, with a baseline estimated glomerular filtration rate (eGFR)>60 ml·min -1·(1.73 m 2) -1 and postoperative complication of AKI. The patients were followed up for 90 days after discharge from hospital. The endpoint event was defined as progression to CKD after 90 days of the occurrence of CABG-associated AKI. The patients were divided into CKD group and non-CKD group based on whether they experienced endpoint events. The baseline clinical data were compared between the two groups. The logistic regression model was used to analyze the risk factors of endpoint event. The receiver-operating characteristic curve was drawn to evaluate the performance of the clinical risk prediction model for predicting poor renal prognosis in CABG-associated AKI patients. Results:A total of 124 CABG-associated AKI patients were enrolled in the study, including 95 males and 29 females, aged (62.57±9.61) years old. Thirty-eight patients (30.8%) progressed to new-onset CKD 90 days after CABG-associated AKI. Compared with non-CKD group, CKD group had lower preoperative hemoglobin ( t=2.778, P=0.006) and baseline eGFR ( t=3.603, P<0.001), higher proportion of women ( χ2=10.714, P=0.001), and higher preoperative blood NT-proBNP ( Z=-2.150, P=0.030) and discharged serum creatinine ( Z=-5.290, P<0.001). The multivariate logistic regression analysis results revealed that female ( OR=5.179, 95% CI 1.535-17.477, P=0.008), high preoperative blood NT-proBNP ( OR=1.049, 95% CI 1.004-1.095, P=0.032), low baseline eGFR ( OR=0.928, 95% CI 0.889-0.968, P=0.001), and high serum creatinine at discharge ( OR=1.019, 95% CI 1.009-1.029, P<0.001) were independent influencing factors of CABG-associated AKI to CKD. The clinical risk prediction model including female, preoperative blood NT-proBNP, preoperative baseline eGFR, and serum creatinine at discharge produced a moderate performance for predicting CABG-associated AKI to CKD ( AUC=0.872, 95% CI 0.806-0.939, P<0.001). Conclusion:Patients with CABG-associated AKI are at high risks for new-onset CKD. Female, preoperative high NT-proBNP, preoperative low baseline eGFR and high serum creatinine at discharge can help identify patients with a high risk of CABG-associated AKI to CKD.
