1.Surveillance on the quality of disinfection and sterilization among medical institutes in Xi’an in 201 1-2013

Objective To investigate the status of disinfection and sterilization quality of different levels of medical institu-tes in Xi’an,and improve the management level of disinfection and sterilization of medical institutes. Methods Disinfection and sterilization efficacy of 69 medical institutes were monitored and evaluated according to Standardization for Disinfec-tionTechniques (2002 edition)and Sanitary Standards for Hospital Disinfection (GB15982-1995). Results From 2011 to 2013,2 224 specimens were tested,1 766 (79.41% )were qualified.The qualified rate of tertiary medical institutes was higher than second and below medical institutes (83.67% vs 77.50% ,P= 0.001),the qualified rate between public medical institutes and private medical institutes was not statistically different (79 .64% vs 78 .20% , P= 0.532),the qualified rate of comprehensive medical institutes was higher than specialized medical institutes (80.18% vs 74.92% ,P= 0.030). Of different detected specimens,the qualified rates of disinfectant in use and pressure steam sterilizers (including test pack and process challenge device)were higher (98.46% ,100.00% ,and 98.06% respectively),while the hands of health care workers (HCWs)and glutaraldehyde were lower(58.48% and 43 .28% respectively).Conclusion Disinfection and sterilization quality in different medical institutes in Xi’an is dif-ferent,and different disinfected and sterilized objects are also varied .It is necessary to intensify the management of hand hygiene of HCWs and concentration of glutaraldehyde.
3.Insulin induced LPL mRNA expression in THP-1 macrophage and acceleration of transferring the macrophage to foam cell

AIM: To investigate the effect of insulin on ox-LDL transferring the THP-1 cells to foam cells and influencing the LPL mRNA expression in THP-1 cells. METHODS: THP-1 cells were incubated with 50 mg/L ox-LDL and insulin at concentrations of 10 mU/L,100 mU/L,1 000 mU/L and 10 000 mU/L, respectively. The expression of LPL mRNA in cells was detected by RT-PCR. Lipoprotein lipase of THP-1 cells was presented by no-specific lipase staining. THP-1 cells were stained with oil red O. Accumulation of total cholesterol (TC) in THP-1 cells was determined with oxidase assay. RESULTS: In 100 mU/L?1 000 mU/L?10 000 mU/L insulin groups, LPL mRNA expression increased 2 times, the average cell perilength was longer, the percentage of positive oil red O staining cells was significant higher, the content of cholesterol in THP-1 cells was higher than in ox-LDL control ( P
4.Effect of tea-polyphenols on the activation of NF-?B an d the expression of TGF-?_1 mRNA in THP-1 cells incubated with VLDL, LDL or ox-LDL

AIM: To investigate the effect of tea-polyphe nols (TP) on the activation of NF-?B and the expression of TGF-? 1 mRNA in TH P -1 cells (a human acute monocytic leukemia cell line). METHODS: TH P-1 cells were incubated with the different concentrations of TP, VLDL, LDL or o x-LDL. In the THP-1 cellls, the nuclear malposition rate of NF-?B was detected with immunohistochemistry technique, the positive index of the TGF-? 1 mRNA ex p ression was detected by hybridization in situ, and accumulation of total cholest erol (TC) in cells incubated with 0 4-40 ?g/L TP was determined with oxidase a ssay. RESULTS: The nuclear malposition rate of NF-?B, the posit ive index of the TGF-? 1 mRNA expression and TC in THP-1 cells incubated with 0 4-40 ?g/L of TP were lower than those with 0 ?g/L of TP in TP-V group, TP-L group and TP-O ( P 0.05). CONCLUSION: TP inhibited the activation of NF-?B, the expressio n of TGF-? 1 mRNA and the foam cell formation in the mono-macrophage.

ObjectiveTo investigate the protective effect of erythropoietin (EPO) on human renal tubular epithelial-mesenchymal transition (EMT) induced by high glucose. MethodsCultured HK-2 cells were divided into 5 groups: normal control group, osmolarity control group, high glucose group, high glucose with EPO (5 U/ml)group and high glucose with EPO (10 U/ml)group. The mRNA expression of α-SMA, TGF-β1, Smad2, ILK of cells were measured by RT-PCR. The levels of intracellular α-SMA and TGF-β1 protein were measured by immunocytochemistry. ResultsCompared with the control groups, mRNA expression of α-SMA, TGF-β1, Smad2, ILK and protein expression of α-SMA and TGF-β1 were increased after high glucose treatment (P<0.01). Compared with the high glucose group, EPO 5 U/ml or EPO 10 U/ml remarkably down-regulated the expression of α-SMA, TGF-β1, Smad2 and ILK (P<0.01). ConclusionEPO can inhibit the progression of EMT and the up-regulation of TGF-β1, Smad2 and ILK induced by high glucose.
9.The effectiveness and safety of 125I seed implantation for treatment of gastric cancer

Objective To explore the effectiveness of 125I seed implantation for gastric cancer and to determine whether the therapy could increase the survival rate.Methods Seventy-six gastric cancer patients in stage Ⅱ or Ⅲ were involved and randomly divided into treatment group (n =42) and control group (n =34)by simple random sampling method.The patients in the control group underwent D2 or D3 surgery and the patients in treatment group underwent D2 or D3 surgery plus interstitial implantation of 125I seeds.All patients signed the informed consents.Treatment results were evaluated as CR,PR,NC and PD.CR and PR were considered as effective and the effective rate was calculated.All patients were followed up and the three-or five-year survival rate was calculated,the complications were examined.x2 test was used to compare the significant difference between the two groups.Results The total effective rate in control group was 50.00% (17/34),lower than that of treatment group (73.81%,31/42; x2 =4.578,P<0.05).In the treatment group,the three-year and five-year survival rates were 61.90%(26/42) and 42.86%(18/42) respectively,and the corresponding rates in the control group were 11.76%(4/34) and 0(0/34) respectively (x2=19.771,19.094,both P<0.001).Both of the two groups had few severe side effects.Conclusion Radical surgery plus 1~Iseed implantation is effective and safe for the treatment of stage Ⅱ or Ⅲ gastric cancer and can further improvelong-term survival.
10.A Safety Study on Honghua Injection Based on Literature

Objective To conduct a comprehensive evaluation on safety of Honghua Injection through adopting the method of the evidence-based method;To provide reference for clinical reasonable application of Honghua Injection. Methods Computers were used to retrieve some Chinese databases, such as China Biology Medicine, China National Knowledge Infrastructure, Wangfang Database and VIP database. At the same time, other search methods were employed, up to July 2013, including all research types about Honghua Injection. The adverse reactions in the reports of published literature were analyzed by description and statistical analysis. Results Sixty-nine researches on Honghua Injection were included. The total cases of adverse drug reaction (ADR) were 1111, among which male cases were 568 (51%), and female cases were 543 (49%). Thirty-six (52%) papers described ADR of Honghua Injection in detail, and thirty-three (48%) papers just mentioned ADR or did not describe ADR in detail. Skin, skin accessories damage and pathological changes in circulatory system were main contents of ADR. In terms of original diseases, diseases of circulatory system play an important role. Solvent medium was largely in line with its product specification requirements. Most ADR appeared when the drug was used for the first time, from 5 minutes to 5 days. Conclusion The current published literature data show that severe ADR does not happen after the intervention of Honghua Injection.
