1.Surveillance on allergic rash associated with vaccination in Huzhou City

Objective:
To investigate the incidence of allergic rash associated with vaccination in Huzhou City from 2017 to 2021, so as to provide insights into the prevention and control of allergic rash.
Methods:
Data pertaining to cases with vaccination-related allergic rash in Huzhou City from 2017 to 2021 were captured from the National Adverse Events Following Immunization Surveillance System of Chinese Disease Control and Prevention Information System, and epidemiological characteristics, vaccination and clinical features of allergic rash were analyzed.
Results:
Totally 324 allergic rash cases were reported in Huzhou City from 2017 to 2021, with an annual reported incidence of 6.12/105 doses. The incidence of allergic rash was 6.40/105 doses among men and 5.82/105 doses among women, and allergic rash predominantly occurred among cases under 1 year, with a reported incidence of 8.32/105 doses. The largest number of cases was reported in the second quarter (108 cases, 33.33%), and the largest number of cases was reported in Deqing County (92 cases, 28.40%) with a reported incidence of 11.52/105 doses. The highest reported incidence of allergic rash was seen following immunization with combined live-attenuated measles-mumps-rubella vaccines (41.23/105 doses). There were 203 cases (62.65%) diagnosed as allergic rash, 55 cases (16.98%) diagnosed as urticaria, 51 cases (15.74%) diagnosed as measles scarlet fever-like rash, and 15 cases (4.63%) diagnosed as maculopapule. There were 94 (29.01%) cases with fever, and 78.70% allergic rash occurred within 24 hours of vaccination.
Conclusion
The incidence of allergic rash associated with vaccination was high in Huzhou City from 2017 to 2021, and vaccination-related allergic rash predominantly occurred within 24 hours following immunization with the first dose of vaccines. Allergic rash associated with vaccination was highly prevalent among children under 1 year.
2.Inclusion of hepatitis A vaccine in Expanded Program on Immunization on hepatitis A incidence by interrupted time-series analysis

Objective :
To assess the impact of inclusion of hepatitis A vaccine in Expanded Program on Immunization(EPI)on hepatitis A incidence.
Methods :
The data of patients with hepatitis A from 1998 to 2017 in Huzhou were collected through Huzhou Infectious Diseases Yearbook and China Disease Prevention and Control Information System. Interrupted time-series models were constructed using the year 2008 when hepatitis A vaccine was included in EPI as the intervention time point. The influence of hepatitis A vaccine into EPI on the hepatitis A incidence was evaluated by analyzing the changes in the level and slope of incidence before and after hepatitis A vaccine into EPI.
Results :
Before the inclusion of hepatitis A vaccine in EPI,the average annual incidence rate of hepatitis A was 10.228/100 000 and the incidence of hepatitis A showed a significant descending trend with an annual decrement of 1.558/100 000. After the inclusion of hepatitis A vaccine in EPI,the average annual incidence rate of hepatitis A was 1.721/100 000 and the incidence of hepatitis A showed a significant descending trend with an annual decrement of 0.263/100 000. The average annual rate of decline in the hepatitis A incidence decreased by 1.295/100 000 after the inclusion of hepatitis A vaccine in EPI.
Conclusion
The incidence of hepatitis A significantly reduced after inclusion of hepatitis A vaccine in EPI,and the EPI strategy had a continuous effect on the decreasing trend of hepatitis A incidence.
3.Surveillance of adverse events following immunization with combined attenuated live measles, mumps and rubella vaccines in Huzhou City from 2015 to 2021

Objective:
To monitor the adverse events following immunization (AEFI) with combined attenuated live measles, mumps and rubella vaccines (MMR) in Huzhou City from 2015 to 2021, so as to provide insights into the implementation of the MMR immunization strategy.
Methods:
All AEFI caused by MMR immunization in Huzhou City from 2015 to 2021 were captured from the AEFI Monitoring Information Management System of the Immunization Planning System of Chinese Disease Control and Prevention Information System, and the incidence, clinical features and epidemiological features of AEFI were analyzed descriptively.
Results:
The reported incidence of AEFI caused by MMR immunization appeared a tendency towards a rise in Huzhou City from 2015 to 2021 (χ2trend=124.126, P<0.001). Totally 324 386 doses of MMR vaccines were immunized, and 317 cases with AEFI were reported, with an reported incidence rate of 9.77/104 doses. Following two-dose immunization, the reported incidence of AEFI caused by two-dose MMR immunization was significantly lower than by one-dose immunization (6.01/104 doses vs. 25.43/104 doses; χ2=113.692, P<0.001). The incidence rates of general reactions, abnormal reactions and coincidental events were 6.20/104 doses, 3.42/104 dose and 0.15/104 doses, respectively. Fever and allergic rash were predominant clinical manifestations of AEFI, and no vaccine quality accident, inoculation accident or psychogenic reaction were reported. There were 246 (77.60%) cases with AEFI within 24 hours following vaccination, and among children with AEFI, there were 173 boys (54.57%), and 200 children (63.09%) age ages of less than one year (63.09%). AEFI was reported in each quarter, and 99 cases (31.23%) were reported in the fourth quarter. The largest number of children with AEFI was reported in Wuxing District (78 cases, 24.61%).
Conclusions
The safety of MMR vaccination is high in Huzhou City. General reaction is the predominant AEFI, which mainly occurs within 24 hours following vaccination. Two-dose MMR vaccination does not increase the risk of AEFI.
4.Surveillance of adverse events following immunization with inactivated influenza vaccines among the elderly in Huzhou City

Objective:
To investigate the incidence of adverse events following immunization (AEFI) with inactivated influenza vaccine among the elderly in Huzhou City, Zhejiang Province, so as to provide insights into safety monitoring and evaluation of inactivated influenza vaccines.
Methods:
Data pertaining to surveillance on AEFI with inactivated influenza vaccines among the elderly at ages of 60 years and older in Huzhou City from 2020 to 2022 were collected from the AEFI Monitoring Information Management System of the Immunization Planning System of Chinese Disease Control and Prevention Information System, including demographics, time of AEFI occurrence, classification of AEFI and clinical syndromes, and the reported incidence and epidemiological features of AEFI with inactivated influenza vaccines were analyzed using a descriptive epidemiological method.
Results:
Totally 84 elderly cases at ages of 60 years and older were reported with AEFI with inactivated influenza vaccines in Huzhou City from 2020 to 2022, with a reported incidence rate of 9.83/105 doses, and the reported incidence rates of AEFI with trivalent and quadrivalent inactivated influenza vaccines were 9.74/105 doses and 48.71/105 doses, respectively. The reported incidence rates of general, abnormal, coincidence and psychogenic reactions were 7.96/105 doses, 1.52/105 doses, 0.23/105 doses and 0.12/105 doses, respectively, and no vaccine quality accidents or wrong vaccine administered were reported. The cases with AEFI included 52 women and 32 men, and most cases were aged from 60 to 69 years (44 cases, 52.38%). The highest incidence of AEFI was reported in Nanxun District (17.94/105 doses), and there were 79 cases (94.05%) with AEFI within 24 hours following vaccination. The clinical symptoms mainly included fever, local redness and swelling, and local induration, with reported incidence rates of 2.22/105 doses, 3.74/105 doses, and 1.99/105 doses, respectively.
Conclusions
The reported incidence of AEFI with inactivated influenza vaccines is low among the elderly at ages of 60 years and older in Huzhou City, with general reactions as predominant AEFI, and most AEFI occurs within 24 hours following vaccination.
5.Transurethral frontfiring photoselective vaporization over transurethral bipolar plasmakinetic resection for the treatment of cystitis glandularis

Objective To evaluate the safety and efficacy of transurethral frontfiring photoselective vaporesection for the treatment of cystitis glandularis,by comparing with the transurethral bipolar plasmakinetic resection.Methods From January 2014 to July 2016,41 patients with pathologically diagnosed cystitis glandularis in our hospital,were divided into two groups,including 22 cases underwent transurethral frontfiring photoselective vaporesection(the observation group),and the other 19 cases underwent transurethral bipolar plasmakinetic resection(the control group).All patients were regularly treated with postoperative intravesical instillation chemotherapy with pirarubicin.The clinical data of two groups were statistically analyzed to compare the differences of the safety and efficacy.Results All the surgeries were performed successfully.There were no statistical significances in perioperative data,the operation time,the decreased concentration of hemoglobin(Hb)and Na+,operative related complications, indwelling catheter duration and hospitalization duration between the two groups.The first-time cure rate and the effective rate of transurethral frontfiring photoselective vaporesection showed significantly better than those of transurethral bipolar plasmakinetic resection(P<0.05 for each).Conclusion Comparing the traditional transurethral bipolar plasmakinetic resection for the treatment of cystitis glandularis,tansurethral frontfiring photoselective vaporesection with postoperative intravesical instillation chemotherapy with pirarubicin,is a safer,simpler,and more effective method,which could be a new optional method in the conditional hospitals,deserving the worthy of clinical popularization.
6.Clinical efficacy of tip-flexible ureterorenoscope with holmium laser for one-stage management of parapelvic cyst

Objective To evaluate the efficacy and safety of tip-flexible ureterorenoscope (tf-URS) with holmium laser for one-stage management of parapelvic cyst.Methods The clinical data of 26 patients treated with tf-URS incision and drainage using holmium laser from February 2016 to August 2018 were reviewed.The study were including 15 male and 11 female patients,ranging from 32 to 68 years old,with an average of 53.5 years old.There were 24 cases of unilateral single renal parapelvic cyst and 2 cases of bilateral single renal parapelvic cyst.There were 4 cases in 26 cases with unilateral single renal parapelvic cyst and contralateral single renal cyst,2 cases with unilateral single renal parapelvic cyst and ipsilateral kidney of stones.The diameter of parapelvic cyst was 3.4-5.6 cm,average 4.8 cm.All patients had undergone holmium laser endo-decortication of parapelvic cyst by tf-URS.With general anesthesia,tf-URS accessed pelvis retrogradely and decorticated parapelvic cyst with 200 μm Holium laser to drainage the cyst to pelvis.If the tf-URS was not placed successfully for the first time,Double-J tubes were retained for 1-2 weeks before treatment.The operative time,hospitalization time,blood loss,postoperative complications and clinical symptoms were collected and analyzed.Results The one-time access success rate of insertion of ff-URS was 88.5% (23/26).All operations were successful without severe complications.The average time of operation was 17.2 min,ranging from 11 to 25 min.In 3-30 months follow-up,the cysts disappeared in 22 patients and reduced by more than one half in 1 patients.Flank pain relieved in 19 patients.Conclusions Holmium laser endo-decortication of parapelvic cyst by tf-URS could be a simple,minimally invasive,safe and effective method for parapelvic cyst,which is worthy of further promotion and application in clinical practice.
