Objective: To explore the brain damage effects of lysophasphatidyl choline(LPC) in rats with pancreatitis and explore the pathogenesis of pancreatic encephalopathy(PE).Methods: SD rats were divided into test group,control group one and control group two at random.Acute edematous pancreatits rat model was induced following Aho HJ method for test group and control group one,then test group rats were venously applied with LPC and control group one was injected saline through tail vein.Control group two was treated with venous injection LPC through the rat tails without operation.Horseradish peroxidase(HRP) was used as a tracer to detemine if the blood-brain barrier(BBB)was open 7～10 days after successive application of LPC.The extravasated tracer was showed by diaminobenzidine(DAB).Rat brain tissue sections were examined by ponceau stain and Luxol Fast Blue stain to determine whether the rat brains were demyelination at the same time.Results: The BBB permeability of the test groupLTU rats increased greatly and obvious demyelination was observed in test group rat brains while both control groups had basically intact BBB and scarce demyelination was observed in both control groups.A statistical difference existed between the test group and control groups.Conclusion:LPC definitely open the BBB of the rat pancreatitis and demyelinates the brain of pancreatitis rats thus LPC plays an important role in the pathogenesis of PE.

Objective To evaluate the effects of rHGF on liver regeneration after partial transplantation.Methods The partial orthotopic liver transplanted model was established in SD rats with two-cuff technique. In experimental group, immediate after operation, 500 ?g/kg rHGF was administered intravenously twice a day. Same vehicle of saline was administered as control. The rats were (killed) randomly on the day 1, 2, 4 and 7 after operation. The serum levels of albumin and alanine (aminotransferase) were detected. Each liver was removed and weighed. The expression of Ki-67 antigen was detected by using immunohistochemistry assay. The regeneration response of hepatocytes was evaluated by flow cytometry.Results The wet weigh in rHGF group was significantly larger than that in control group 1, 2 days after transplantation. The serum level of alanine aminotransferase was (lower) and that of albumin was higher in rHGF group than in control group 4, 7 days after operation. The proliferating index and Ki-67 labeling index of graft in rHGF group were higher than in control group.Conclusion rHGF can promote the regeneration of hepatocytes after partial transplantation.

Objective To observe the hemodynamic variations of both the mother and fetus when the pregnancy was complicated with early-onset of severe preeclampsia, and to evaluate the role of monitoring the blood flow with Doppler in the management and prognostic value in the same condition. Methods A prospective case-control study was conducted in 36 pregnant women admitted for early-onset of severe preeclampsia (S-PE). The control group included 72 healthy pregnant women matched with the S-PE group. All had color Doppler sonography monitored during this study period. Data were collected for the umbilical artery (UA) pulsatility index (PI) and systolic/diastolic ratio (S/D) , the middle cerebral artery (MCA) PI and S/D and the maternal mean uterine artery (UtA) PI and S/D. Those subjects with adverse perinatal outcomes were followed up after birth. Results The mean Doppler indices(S/D and PI) of fetal UA, MCA and maternal UtA were all declined gradually with the advancing gestational ages in both groups. OBJECTIVE To investigate current status of ventilator-associated pneumonia(VAP) and to provide prevention measures associated with nursing.METHODS Clinical data,such as incidence,categories of pathogenic bacteria and prevention measures associated with nursing,were retrospectively analyzed in 106 patients who received ventilator treatment in our hospital during 2004-2006.RESULTS The positive rate of microorganism culture in these patients was 95.3%.Totally 178 strains of microorganisms were cultured,including 63 strains of fungi,48 strains of nonfermanters,34 strains of Staphylococcus,33 strains of Enterobacteriaceae.There were many risk factors contributed to pathogenesis of VAP.Among these factors,underlying disease was one of the important internal causes and invasive medical procedure was one of the important external causes.The incidence of VAP was reducing yearly as improving of prevention measures associated with nursing.CONCLUSIONS Nurse acts an important role in prevention of VAP.Enhancing nursing management,improving nursing procedure and adopting appropriate intensive care measurement will reduce incidence of VAP.

Objective To investigate anterior migration of spinal cord after anterior cervical decompression and reconstruction for cervical spondylosis and its mechanism.Methods Eighty-one patients with cervical spondylosis who underwent anterior decompression and reconstruction were reviewed and evaluated retrospectively.There were 44 males and 37 females,with an average age of 52.3 years (range,37-72 years).The distance from anterior margin of the spinal cord to posterior wall of the spinal canal (Da),and medium of the spinal cord to posterior wall of the spinal canal (Dm) in magnetic resonance imaging were measured preoperatively and postoperatively.The recovery rate was calculated comparing the Japanese Orthopaedic Association (JOA) scores before and after operation.Results The average following up was 20.7 months (9-48 months).According to the measure on MRI,preoperative Da and Dm were 11.11±1.59 mm and 6.30±0.84 mm respectively,which were significant different from postoperative Da 11.63±-1.47 mm and Dm 6.62±0.80 mm.The postoperative Da and Dm at C3-4,C4-5,C5-6,C6-7 levels had statistically significant differences compared to preoperative Da and Dm,except for ones at C2-3 level.The significant migration rate was 43.2％(35/81),which reached △Da ＞2 mm or △Dm ＞1.3 mm.Preoperative cervical curvature index (CCI) was 10.0％±6.3％,which was significant different from postoperative CCI 13.5％±6.5％.JOA score increased from 9.7±1.5 preoperatively to 13.7±1.1 at one week postoperatively,and 13.9±1.3 at 6 months postoperatively.But the JOA score decreased at 6 months postoperatively compared with that at 1 week postoperatively in 15 cases.Among them,14 cases got the standard of spinal cord migration based on MRI measurement.No other severe complications were found in these 81 cases.Conclusion Anterior migration of the spinal cord commonly occurred after cervical decompression and reconstruction via anterior approach,and some patients might suffer from the secondary compression of the cervical spinal cord.

Objective We reported 16 eosinophilic fasciitis (EF) patients with eosinophilic fasciitis and performed a systematic review of the literature to improve the disease awareness.Methods The clinical course of 16 patients with eosinophilic fasciitis at the Peking Union Medical College Hospital were described,inclu-ding demographic data,clinical manifest-ations,laboratory tests,pathology and treatment.Results The mean age at diagnosis was (47±8) years,with 13 female and 3 male patients.Three cases had exertion or strenuous sports before the onset of EF.Positive ANA was noted in 6 of 12,positive RF was noted in 3 of 10,hyper-gammaglobulinemia was noted in 6 of 7,elevated IgG was noted in 8 of 13,peripheral blood eosinophilia was noted in 10 of 16,while thrombocytopenia was found in one patient.Conclusion Based on this and other reported cases in the literature,EF may be a kind of autoimmune disease.Genetic influence and environ-mental factors are involved in the development of this disease.Systemic involvement is rare.In general,corticosteroids and immunosuppressive are effective in EF.

Objective To evaluate the influence of rectal washout on the prognosis of rectal cancer after anterior resection.Methods Data of 144 patients with rectal cancer undergoing anterior resection at Chinese Academy of Medical Science Cancer Hospital between May 2006 and November 2007 were reviewed.69 patients received rectal washout and 75 patients did not.The incidences of recurrence and 5-year disease-free survival rates were compared.Data were analyzed by SPSS 13.0 software packet,using x2 and t inspection.Kaplan-Meier method was used to calculate postoperative survival rate.Results Washing sample was positive for malignant cells in 55 patients (79.7％).The incidences of recurrence were 4.6％ and 6.7％ in washout group and no-washout group respectively (x2 =0.368,P =0.721).The five-year disease-free survival rate was 79.7％ in washout group and 74.7％ in no-washout group(x2 =0.517,P =0.553).Conclusions Free malignant cells were ubiquitous in rectal lumen during anterior resection,and there was a high positive rate of cytological examine for washout sample.Rectal washout may reduce local recurrence and prolong patients' survival.
