1.A Clinical Study on Gut Barrier Function Change after Acute Intraperitoneal Hemorrhage

12h interval from injury to operation had significantly higher concentration of plasma D-lactate and endotoxin at above metioned 4 time points. Plasma concentration of D-lactate and endotoxin in the last two groups declined to normal range at 72h after operation, while in the first two groups reduced to normal range before leaving hospital. Conclusion Acute intraperitoneal hemorrhage could damage gut barrier function, and injury degree was related with interval from injury to operation and blood loss amount.
2.Synthesis and biological evaluation of diarylamines with antitumor activity.

By structural modifications of our previous leads 1-3, 18 diarylamines were designed, synthesized and evaluated with a human tumor cell line panel, including A549, DU145, KB, and KB-vin cell lines, resulting in the discovery of new antitumor agents A6 and B2 with low micromolar G50 values ranging from 1.55-2.10 micromol x L(-1) for above cell lines, and A9 with GI50 values ranging from 1.55-2.10 micromol x L(-1) specially for DU145, KB, and KB-vin cells. Current structure-activity relationships are helpful for further lead optimization.
3.The Practice of Tubular Anastomate in Treatment of Rectal Space Occupying Lesion and Design of Endoscopic Cutter Used for Proctopolypus

Objective To evaluate the clinical value of anastomate in treatment of rectal lesion. Methods The anastomate was used to resect space occupying lesion which was 5~12cm from anal.Then thwe operation procedure,operation time, recurrence complications and correlated problems were summarized.Results The average operation time was 34 minutes. Exceot for one case of anastomtic bleeding and one case of recurrence 14 months post operation.there were No other complications.Conclusions Stapling-resecting the rectal lesion was feasible,but some problems were solved from the design of anastomate.
4.Left renal vein entrapment syndrome (report of 10 cases)

Objective To evaluate the diagnosis and treatment of left renal vein entrapment syndrome. Methods The clinical data of 10 cases of left renal vein entrapment syndrome were retrospectively analyzed.Their mean age was 24 years (range,14-37 years).Of them 7 cases presented with non-glomerular origin hematuria;2,with orthostatic proteinuria;1 with hematuria accompanied by proteinuria.Color Doppler sonography showed that the dilated segment of the left vein was three-fold than the strictured segment in diameter,and magnetic resonance angiography (MRA) showed that the left renal vein was compressed in 9 cases. Results Stent placement across the left renal vein was performed in 7 cases.Superior mesenteric arteriectomy and replantation were performed in 3 cases.The operations were all successful.Follow-up of 1-5 years showed that the clinical symptoms were improved and urine tests were normal. Conclusions Non-glomerular hematuria or orthostatic proteinuria is frequently found in left renal vein entrapment syndrome.Left renal vein entrapment syndrome can be diagnosed by Doppler sonography,MRA and cystoscopy in the presence of hematuria or proteinuria.Generally,conservative treatment and careful follow-up are conducted in these patients.Surgical and radiologic interventional procedures are effective alternatives for those with glomerular dysfunction and complications.
5.Correlationship between methylation status of cholangiocarcinoma-related genes in bile and cholan-giocarcinoma

Objective To determine the aberrant methylation status of TFPI-2,FOXE1 and NPTX2 gene promoter region in bile from cholangiocarcinoma patients and the diagnostic value for cholangiocarcinoma. Methods Methylation—specific PCR(MSP)was used to detect the promoter methylation status of TFPI-2, FOXE1 and NPTX2 genes in bile from 29 patients with cholangiocarcinoma and 27 patients with choledocholi-thiasis.The sensitivity and specificity of combined methylation of 3 promotors and brush cytology were analyzed. Results There was significant difference between the methylation rate of NPTX2,FOXE1 in choledocholithiasis group and cholangiocarcinoma group(75. 86% VS 25. 93%,χ2 = 13. 964,P<0. 05;44. 82%VS 14. 81%,χ2 = 4. 622,P<0. 05).There was no significant difference between the methylation rate of TFPI-2 in choledocholithiasis group and cholangiocarcinoma group(17. 24%VS 18. 52%,χ2 = 0. 016,P>0. 05).Positive rate was 34. 48% with endoscopic retrograde brush cytolog,and 86. 20% with promoter methylation of FOXE1 combined with NPTX2 genes,and there was significance difference in the positive rates between the two methods(χ2 = 14. 122,P< 0. 05). Conclusion The methylation rates of NPTX2,FOXE1 were significantly higher in choledocholithiasis group than those in the cholangiocarcinoma group. Detection of comethylation rate of the FOXE1 and NPTX2 genes could improve diagnostic sensitivity of cholangiocarcinoma.
6.Effects of PI3-K/AKT signal transduction pathway in rat neural stem cells

Objective To incubate rat neural stem cells (NSCs) with selection-specific inhibitor ( LY294002) of PI3-K, and explore the effects of PI3-K/AKT signal transduction pathway in NSCs. Methods NSCs derived from E15-16 rats were isolated and cultured. After incubation of NSCs with different concentrations of LY294002 (0 to 40 μmol/L), cell survival ( WTS-8 assay), proliferation ( BrdU immunohistochemical identification), differentiation (β Tubulin-III immunohistochemical identification) and AKP phosphorylated protein (Western blotting) were detected. Results The effects of LY294002 on the survival, proliferation and differentiation of NSCs was concentration-dependent. The survival of the NSCs decreased significantly with a higher concentrations of LY294002 (25, 30, 35, 40 μmol/L) (P < 0. 05). The numbers of BrdU positive cells with LY294002 of 30, 35 and 40 μmol/L were significantly smaller than that of the control (LY294002 0 μmol/L) (P < 0.05). The numbers of β Tubulin-III positive cells with LY294002 of 35 and 40 μmol/L were significantly smaller than that of the control (P < 0. 05). The expression of AKT was blocked by LY294002 in a concentration-dependent manner. Conclusion PI3-K/AKT signal transduction pathway plays a vital role in the survival, proliferation and differentiation of NSCs.
7.The value of antinucleosome antibody in diagnosis of juvenile systemic lupus erythematosus and the association with disease activity

ObjectiveTo evaluate the frequency of antinucleosome antibody(AnuA) in juvenile systemic lupus erythematosus(JSLE),comparing it to that observed for anti-dsDNA,anti-Sm antibodies,and explore the correlation of these antibodies with clinical manifestations and disease activity.MethodsWe included 80 children with JSLE and 56 children with other rheumatic diseases into this study.Clinical records were reviewed.AnuA,antinuclear antibody (ANA) and anti-Sm antibody were detected by ELISA,ⅡF and Western-blot respectively.Anti-DNA were detected by ELISA and ⅡF.Disease activity was assessed by SLEDAI score.ResultsAnuA showed sensitivity of 76.25% and specificity of 98.21%.AnuA combine with anti-dsDNA antibody or anti-Sm antibody was detected,the sensitivities and specificities were 83.05%,86.44% and 96.43%,98.21%,respectively.It showed that the sensitivity was higher than any one of the three.The presence of AnuA was associated with red blood cell count,hemoglobin level,ESR,hematuria,low complement levels and anti-dsDNA antibody (r=-0.499,-0.503,0.388,0.227,0.303,0.531,P=0.000,0.000,0.000,0.042,0.006,0.000).The presence of AnuA was associated with the SLEDAI score(P=0.000). Conclusion AunA hasthe highest sensitivity and specificityamongthese autoabtibodies,particularly,when combined with anti-dsDNA antibody or anti-Sm antibody.The level of AnuA is associated with the disease activity of JSLE.AnuA is not only useful for the diagnosis of JSLE but also for evaluation of the disease activity.
8.Safety and stability of posterior chamber phakic intraocular lens for correction of extreme myopia: A follow-up of 11 cases

BACKGROUND: The phakic intraocular lens implantation has advantages of reversibility and accommodative capability, which is a promising refractive operation. Posterior chamber phakic intraocular lens implantation is a developing method for the correction of extreme myopia. OBJECTIVE: To evaluate the efficacy, safety and stability of posterior chamber phakic intraocular lens implantation for the correction of extreme myopia. METHODS: This study included 18 eyes of 11 patients whose received posterior chamber phakic intraocular lens implantation at Shierming Eye Hospital from March 2006 to January 2009, including 4 males (7 eyes) and 7 females (11 eyes). All patients were followed-up 3-6 months, mean 5.3 months. The refraction, intraocular pressure, contrast sensitivity, glare sensitivity, endothelial cell morphometry, as well as lens opacities were observed. RESULTS AND CONCLUSION: After operation, naked visions of the 18 eyes were better than or equal to preoperative corrective vision, and the astigmatism was relieved obviously. The naked contrast sensitivity and glare sensitivity was better than that before the operation. The anterior chamber depth, intraocular pressure and corneal endothelium counting before and after the operation had no significant difference. No serious complication was detected during the follow-up. The vision decent at night and dazzling sensation occurred in 2 cases. Shot-term observation shows that implanting intraocular lens in posterior chamber to correct high myopia is safe, stable and predictable. This operation is a promising method for correcting high myopia.
9.Practice of hospital cost control by way of diseases payment scoring of global budget in Huaian,Jiangsu province

By means of global budget,the disease-specific scoring method was put in place for both settlement and payment in Huaian,Jiangsu province,for the purpose of cost control by the hospitals.This reform consists of total cost accounting,rational performance-based distribution system,multi-approached regulation of medical behaviors,and individual disease management to regulate medical behavior.These efforts aim at aligning with medical insurance payment reform,so as to improve hospital management,operation quality and socio-economic outcomes.Problems found in practice were also reviewed.
10.Analysis of 63 patients with laryngeal carcinoma

Objective To investigate the survival rate and its prognostic factors of laryngeal carcinoma. Methods We retrospectively analyzed 63 cases of laryngeal carcinoma. A total of 7 clinicopathologic factors were studied by univariate analysis and Cox multivariate model. Results The overall cumulative survival rate was 69.8% at 3 years,54.0%at 5 years.In univariate analysis, the survival was related to location of the tumor(P< 0.01), T status (P < 0.01), N status (P < 0.01), clinical stages (P < 0.01), pathologic grade (P < 0.01) and types of treatment (P < 0.01), while age was not correlated to prognosis (P > 0.05). In Cox multivariate model, T status (P < 0.01) and N status (P < 0.01) were independent prognostic factors. Conclusions T and N status were independent prognostic factors for patients with laryngeal carcinoma. Early detection and treatment should be given to improve the survival of patients.
