Objective　To determine serum true insulin (TI) and proinsulin (PI) levels in patients with coronary heart disease (CHD), and to explore the relationships of them to other cardiovascular risk factors. Methods　Highly specific ELISA assays for TI and PI and RIA for immunoreactive insulin (IRI) were used. The fasting serum IRI, TI, PI levels in 30 nondiabetic patients with CHD and 30 healthy, age-matched control subjects were investigated. And the fasting and 2h postprandial plasma glucose, serum lipids and blood pressure were also determined in each individuals. Results　The levels of serum IRI, TI and PI were significantly higher in patients with CHD than those in controls (P＜0.05), while the value of insulin sensitivity index (ISI) was lower in patients with CHD than in controls (P＜0.05). There were significant independent correlations between TI, PI and age, BMI, plasma glucose, serum lipids. Conclusion Nondiabetic patients with CHD have hyperinsulinemia and insulin resistance and there is an independent association between proinsulin and dyslipidemia.

Objectives To investigate the effect of different physical activities on the blood pressure in elderly patients with hypertension,and to provide evidence for the prevention and treatment of hypertension.Methods 256 elderly patients with grade 1,2 hypertension and no other complications,and 256 healthy elderly people were selected as the control subjects.According to the blood pressure and the level of physical activity,the patients were divided into hypertension combined with low physical activity group(HL group),hypertension combined with middle physical activity group(HM group),normal blood pressure combined with low physical activity group(NL group)and normal blood pressure combined with middle physical activity group (NM group).The changes of blood pressure were compared in quiet,during and after physical activity.Results The diastolic blood pressure(DBP) and systolic blood pressure(SBP) measured at rest,peak exercise and 5 minutes after exercise were significantly higher in group HL and HM group than in NL group and NM group,and the differences between groups were statistically significant(SBP:F=11.846、18.337、10.976,all P＜ 0.001 for all;DBP:F=13.182、11.694、11.453,all P＜0.05).The DBP and SBP in HL group at rest and 5 minutes after exercise were significantly higher than those in the HM group,and the difference was statistically significant(P＜0.05).The change magnitudes of DBP and SBP were significantly lower in HL group than in the HM group,and the differences were statistically significant (P ＜ 0.05).Conclusions Taking part in an appropriate exercise can improve blood pressure level and a recovery capability of post-exercise blood pressure in elderly patients with hypertension.

Objective To compare the effects of three artificial abortion methods, Mifepriston with Misopro-fil,Rivanol with Mifepristone and Rivanol,in mid-term pregnancy. Methods 235 women who were pregnant for 12 to 28 weeks and volunteered to abort with drugs were selected and observed. The total subjects were randomly divided into three groups,study group Ⅰ (n=80),study group Ⅱ (n=78),and control group (n=77). In study group Ⅰ, each was given Mifepristone 50 mg, po, bid, for 3 days. On the fourth day, Misoprofil 200-1000 μg was administered though vagina. In study group Ⅱ, Rivanol(70-100 mg) was injected into amniotic cavity and at the same tame, Mife-pfistone,75 mg was given bid,po,for each case for 1 day. In study group Ⅲ,each case was injected with Rivanol (70-100 nag) only into amniotic cavity. Results The complete abortion rate in study group Ⅰ, Ⅱ and Ⅲ was 88.75% (71/80), 65.38% (51/78), 38.96% (30/77) respectively. The success rate was 98.75% (79/80), 71.79% (56/78),76.62% (59/77) respectively. The mean time from the initial uterine contraction to the expulsion of the fetus and placenta was (6.85±3.68) h, (8.87±3.58) h, (14.67±3.50) h respectively. The volume of in-trapartum and postpartum vaginal hemorrhage within 2 hours was (80.86±40.28) ml, (94.70±42.35) ml, (120. 68±43.30)ml respectively. The complete abortion rate and success rate in study group Ⅰ was significantly greater than that in study group Ⅱ and study group Ⅲ. In addition, the birth process was shorter and the vaginal hemorrhage was less significantly in study group Ⅰ than those in the other two groups (P<0.01). The complete abortion rate in study group Ⅱ was significantly greater than that in study group Ⅲ (P<0.01). Conclusions Mffepristone admin-istered with Misoprpfil has many advantages usages, such as high complete abortion rate and success rate, short birth process, and less volume of vaginal hemorrhage, when used for artificial abortion in mid-term pregnancy. They are es-pecially suitable for the mid-term artificial abortion under 20-week pregnancy. Rivanol administered with Mifepristone can raise complete abortion rate,shorten labor process and decrease vaginal hemorrhage when they are used in artifi-cial abortion. They are suitable for the mid-term artificial abortion above 16-week pregnancy. The stage of labor lasts long when using only Rivanol for artificial abortion. The pain is relatively heavier and the rate of incomplete abortion is higher.

Objective To analyze the clinical application value of endometrial collector combined with liquid based cytology and histological examination for screening of endometrial cancer.Methods One hundred and eighty-two cases high-risk people with endometrial cancer in Songgang People's Hospital of Bao'an district of Shenzhen from June 2014 to December 2015 were collected,among them 71 patients with endometrial cancer were found in the patients who were diagnosed by pathological diagnosis,and the remaining 111 were not diagnosed with endometrial cancer.The material satisfaction,sample collection time of sample collection and endometrial curettage device were comparatively analyzed and also the cervical dilatation production rate,the amount of bleeding,pain score(VAS) were analyzed.The application value of endometrial collector combined with liquid based cytology and histological examination for screening of endometrial cancer was judged according to the results obtained.Results There was no significant difference between the two methods based on satisfaction(χ2=1.953,P=0.089).A methods of cervical endometrial expansion rate(3.85% vs.93.96%) and acquisition time((16.35±2.19) min vs.(11.23±4.26) min),bleeding volume((1.64±0.63) ml vs.(15.73±5.41) ml),VAS score((0.82±0.24) points vs.(4.34±2.43) points) were significantly lower than that of curettage sample collection method(χ2=16.473,P=0.000;t=5.733,P=0.034;t=11.183,P=0.000;t=3.618,P=0.043).With curettage pathological diagnosis as the gold standard,the specificity,sensitivity and diagnostic coincidence rate of endometrial biopsy combined with liquid based cytology and histological examination for screening endometrial carcinoma were respectively 94.51%,95.05% and 94.96%.Conclusion Compared with curettage sampling method,endometrial curettage collector has features of high success rate,short time,low pain and high security and the cervical dilation rate is low.Endometrial curettage collector can be combined with liquid based cytology and histology to examine and screen endometrial cancer.This methods is worth promoting.

Objective To investigate the biological function of SPA-PEI conjugate(staphylococcal protein A-polyethyleneimine cross-linker),which is one key component for construction of a novel antibody-targeted DNA delivery system.Methods The binding capacity of SPA-PEI conjugate with multiple sources of IgG was determined by enzyme-linked immunosorbent assay and neutralization inhibition assay.The binding capacity of SPA-PEI conjugate with DNA fragment was determined by DNA gel retardation assay,and its DNA condensing ability was measured by Ethidium bromide exclusion assay.Results SPA-PEI conjugate could bind well to many species-derived IgGs.SPA-PEI conjugate had no significant effect on the binding properties of SPA.SPAPEI conjugate could neutralize negative charges of the plasmid DNA or DzTi.Its DNA condensing ability was nearly same to that of free PEI,which suggested a excellent DNA condensing ability of the SPA-PEI conjugate.Conclusion SPA-PEI cross-linkers prepared by this project group maintained the biological activity of SPA and PEI.SPA-PEI cross-linkers could be used for the construction of a novel antibody-targeted non-virus DNA delivery system.

Objective:To analyse the curative effect and immune indices of tripterygium glycosides on glomerulonephritis rats . Methods:Aged Wistar rats were randomly divided into 3 groups , Group A:glomerulonephritis model rats received tripterygium glyco-sides treatment;Group B:healthy Wistar rats received no treatment; Group C: glomerulonephritis model rats received no treatment . Results:12 weeks after experiment , Group A and Group C had higher levels of 24-hour urine protein than Group B and these of pre-experiment(P<0.05).Group A had a lower level of 24-hour urine protein than Group C(P<0.05).Group A and Group C had high-er levels of TG, TC than Group B, with lower levels of ALB than Group B (P<0.05).Group A had lower levels of TG, TC than Group C, but a higher level of ALB than Group C (P<0.05).Group A had a higher rate of 0-1 grades in pathology grade than group C(χ2 =6.667,P=0.010).Group C had higher levels of TNF-αand IL-6 than Group B and Group A (P<0.05).Conclusion:Tripterygium glycosides can reduce the urine protein excretion of rats that with mesangial proliferative glomerulonephritis , rectify dyslip-idosis, raise plasma-albumin, decrease the protein expression of TNF-αand IL-6 in glomerulus.

Objective To study the clinical application of Compass (R) system,a novel 3D quality assurance system for the verification of esophageal carcinoma intensity-modulated radiotherapy (IMRT) plan.Methods 12 esophageal carcinoma IMRT plans were optimized with Eclipse 8.6 treatment planning system (TPS),and then Compass (R) reconstructed 3D dose distributions with the patient anatomy.Comparison was performed among the reconstructed and calculated with TPS,Dose-volume parameters (γ pass rate、average dose deviation) to the planning target volume (PTV) and critical structures were quantitative valuated.Furthermore two-dimensional dose verification were performed use MatriXX,γ pass rate were evaluated with 3％/3 mm criteria.Results The γ pass rate of actual gantry angle was found generally declined seemingly compared with 0 degree gantry angle in two dimensional verification,difference was statistically significant (P =0.018-0.001).In 3D dose verification,the γvolume of PTV were exceed 93％,the deviation of D95,D50,D2 were less than 3％;The γvolume of lungs and heart were exceed 95％,the average dose deviation were less than 3％;The γ pass rate of spinal cord and trachea were exceed 98％.The independent check verified more conformed with the TPS calculated.Dose deviations appeared in the radiation field edge area.Conclusions 3D dose verification can provide more information to comprehensively evaluate the plan which is benefit for evaluating the clinical value of verification.

Objective To investigate the effects of parecoxib and morphine on remifentanil-induced postoperative hyperalgesia in patients undergoing orthopedic operation. Methods Sixty ASA Ⅰ or Ⅱ patients,aged 20-62 yr, weighing 45-100 kg, undergoing orthopedic surgery, were randomly divided into 3 groups ( n = 20 each). Anesthesia was induced with midazolam, propofol, remifentanil and rocuronium. The patients were mechanically ventilated after tracheal intubation. Group Ⅰ received iv injection of morphine 0.15 mg/kg, group Ⅱ received iv injection of parecoxib 20 mg and morphine 0.075 mg/kg and group Ⅲ received iv injection of parecoxib 40 mg and morphine 0.075 mg/kg. Anesthesia was maintained with infusion of propofol and remifentanil and intermittent iv boluses of vecuronium. The emergence time, consciousness recovery time, extubation time,incidence of agitation and shivering, and VRS score at 5 min after recovery of consciousness were recorded. Pain at rest and at movement was evaluated using VAS score at 1,2, 4, 8, 12 and 24 h (T1-6) after surgery and MAP andHR were recorded simultaneously. The incidence of nausea and vomiting during 24 h after surgery was also recorded. Blood samples were taken before induction of anesthesia, at the end of operation and 24 h after operation for determination of plasma concentrations of PGE2 and TNF-α. Results There was no significant difference in emergence time, consciousness recovery time, extubation time, VRS scores, MAP, HR, incidence of agitation,shivering, nausea and vomiting among the 3 groups. Compared with group Ⅰ , VAS scores at rest at T1-2 and at movement at T1-6 were significantly increased in group Ⅱ , while VAS scores at rest and at movement decreased at T1-5 in group Ⅲ (P＜0.05). VAS scores at rest at T1-6 and at movement at T1-5 were significantly lower in group Ⅲ than in group Ⅱ (P＜ 0.05). There was no significant difference in the plasma concentrations of PGE2 and TNF-α at different time points between group Ⅰ and Ⅱ (P＞0.05). The plasma concentrations of PGE2 and TNF-α were significantly lower at the end of surgery in group Ⅲ than in group Ⅰ and Ⅱ (P＜0.05). Conclusion Preoperative iv parecoxib 40 mg and morphine 0.075 mg/kg can reduce remifentanil-induced postoperative hyperalgesia in patients undergoing orthopedic operation, and the efficacy is better than that of morphine alone.
