1.ADR Reports:Analysis of 1451 Cases in Our City in 2007

OBJECTIVE:To discuss the characteristics and regularities of adverse drug reactions(ADR)occurring in our hospital.METHODS:A total of 1 451 ADR cases collected by ADR monitoring center in 2007 were analyzed statistically in respect of patients' age,routes of administration,drug category,ADR-involved organs or systems and clinical manifestations etc.RESULTS:Of the 1 451 ADR cases,72.16% of the ADR-inducing drugs were administered by intravenous drip,and most were associated with anti-infective drugs,predominantly penicillins,cephalosporins and quinolones.The ADR manifested predominantly as lesions of skin and its appendants.CONCLUSION:Rational drug use and ADR monitoring should be strengthened to reduce or avoid the occurrence of ADR.
2.Analysis on501Cases of Adverse Drug Reaction Reports in Lishui City of Zhejiang Province

OBJECTIVES:To fathom situation and the cause about adverse drug reactions(ADR)from each drug manu?facturing enterprise and drug handling enterprise in our city.METHODS:501cases of ADR reports in our city collected by ADR monitoring center in2004were classified and analyzed statistically.RESULTS:Of the501ADR cases,226cases were resulted from antibiotic drugs which accounted for45.1%,80cases were resulted from by traditional Chinese drugs prepara?tions which accounted for16.0%;ADR most commonly manifested itself in the injury of skin and appendants,which accounted for48.9%(245cases),then the damage of digestive system and circulation system,there were21(4.2%)comparative serious ADR cases among which;most cases were resulted from iv which accounted for65.9%(330cases)of the total.CONCLU?SION:Cultivation of professional quality and service quality of the professional staff should be strengthened and the om?nibearing ADR monitoring work should be deployed so as to lessen and avoid the occurrence of ADR.
3.Cause analysis and countermeasures of the comfort degree of PICC patients in the process of placing tube

Objective To investigate the reasons and nursing countermeasures of the discomfort of peripher-ally inserted central catheter(PICC)intubation patients in the process of placing tube.Methods 290 patients with PICC who were treated with PICC for the first time were summarized.Results 290 patients with PICC in catheter had not comfortable experience.The main reason of causing discomfort was psychological factors,accounted for 90.0%, catheter when the cold experience leads to the discomfort of 82.8%,catheter environmental factors accounted for 6 3 .8 % ,pain caused by discomfort accounted for 15.5% ,puncture posture factors leading to 13.1% .Conclusion The causes of discomfort in the process of PICC is more,strengthen preoperative health education and the operation of effective communication,to create a relaxed comfortable environment to help patients take comfortable posture,improve a catheter success rate,preventing catheter during cold and other measures can improve the patients'comfort.
4.Clinical Study on Hot Compress with Medicinal Salt Pack in Preventing PICC-associated Mechanical Phlebitis

Objective To observe the effect of hot compress with medicinal salt pack in preventing PICC-associated mechanical phlebitis.Method Totally 200 inpatients undergone PICC intubation were randomized into a treatment group and a control group, 100 cases in each group. After PICC intubation, the treatment group was intervened by hot compress with medicinal salt pack, while the control group was by hot compress with warm wet towel. The occurrence of mechanical phlebitis was observed at different time points and compared between the two groups. Result The occurrence rate of mechanical phlebitis was 9.0% in the treatment group versus 26.0% in the control group, and the difference was statistically significant (P<0.05). In the treatment group, the occurrence rate of mechanical phlebitis was respectively 6.0% and 3.0% when treatment≤72 h and>72 h, versus 16.0% and 10.0% in the control group, and the differences were statistically significant (P<0.05). Conclusion Hot compress with medicinal salt pack can effectively reduce the occurrence of PICC-associated mechanical phlebitis.
5.The Role of CNQX in the Different Types of Synaptic Release in Mice

Objective:To explore the role of 6-CYANO-2,3-DIHYDROXY-7-NITROQUIN OXALINE (CNQX) in different types of synapse secretion.Methods:The spontaneous mEPSCs and eEPSCs at different extracellular concentrations of CNQX in cultured cortical or hippocampal neurons were recorded respectively.Results:The half inhibitory concentration (IC50) of CNQX in evoked neurotransmitter release was significantly higher than that of spontaneous release,indicating that the spontaneous neurotransmitter release was more sensitive to CNQX.No apparent difference was observed between cortical and hippocampal cells,suggesting that the blocking effect of CNQX was similar in different brain regions.Conclusion:CNQX might have differential regulating mechanisms between excitatory spontaneous and evoked neurotransmitter release,but without brain regions specificity.

In 1970′s a prevailing hypothesis postulated herpes simplex virus in cervical cancer aetiology,but Hausen has made seminal observations that identify novel human papilloma viruses as key contributors to cervical cancer and hybridization technique of nucleic acid has proven it.After the discovery of human immunodeficiency virus(HIV) by Sinossi and Montagnier,they held the evolutional history of HIV.The article objectively evaluates the role in the discovery of HIV,played by Gallo.
7.Membrane protein proteomic analysis of in vivo induced carbapenemases resistance Acinetobacter baumannii

Objective To investigate the role of outer membrane protein in clinical isolated car-bapenem resistance Acinetobacter baumannii. Methods Carbapenem resistance and sensitive strains were collected from the same patient. After MIST and REP-PCR analysis, carbapenemases were detected by isoe-lectric focusing. Different expressed membrane proteins were identified by two-dimension electrophoresis and mass spectrometry analysis. We also used efflux pump inhibitor PAβN(Phe-Arg-β-naphthylamide) to con-firm the phenotype. Results Carbapenem resistance and sensitive strains were attributed to the same pat-tern. At positions of P17.6 and P19.0, two β-lactamases were expressed in two investigated strains, no cabapenemases were detected. Six differential expressed membrane proteins were identified, a 34 × 10~3 membrane protein that was confirmed by efflux pump inhibitor PAβN experiment (imiponem MIC decreased from far above 32 μg/ml to 8μ/ml) and OprD and CarO. Conclusion Up-regulation of exported protein accompanied with down-regulation of OprD and CarO other than carbaponemases are responsible for carbap-enem resistance in A. baumannii.
8.Establishment and application of hnman platelet antigen genotyping with PCR sequencing-basod typing method

Objective To establish a PCR sequencing-based typing (PCR-SBT) method for simultaneous genotyping of human platelet antigen HPA-1 to HPA-16w.Methods All DNA polymorphism sites of HPA-1 to HPA-16w were obtained from the immuno polymorphism database.The specific primers were designed using Primer Premier 5.0 software to amplify nucleotide acid fragments encompassing each HPA polymorphism site.The primer sequence and PCR condition were optimized to obtain specific and single amplification product.The PCR product was purified and then sequenced to determine the HPA genotypes.Two standard DNA samples were detected using the HPA PCR-SBT method to examine the accuracy d this method.Sixteen reference samples (including 6 interference samples with HPA gene mutations) provided by 14th platelet immunology workshop of international society of blood transfusion (ISBT) in 2008 were also tested by this home-brew HPA PCR-SBT method.Results Total eleven pairs of primers were designed to amplify and sequence the sixteen HPA systems.The HPA genotypes of two standard samples were 1aa/2aa/3ab/4aa/5ab/6aa/7aa/8aa/9aa/10aa/11aa/12aa/13aa/14aa/15aa/16aa and 1aa/ 2aa/3aa/4aa/5aa/6aa/7aa/8aa/9aa/10aa/11aa/12aa/13aa/14aa/15aa/16aa,respectively.The 256 HPA genotypes of 16 reference samples were clear.128 genotypes among them were completely accordance with the results provided by ISBT report.Conclusions The PCR-SBT assay combining high-throughput DNA sequencer established in the study provides a simple,rapid and accurate method for HPA-1 to HPA-16w systems genotyping.The assay is suitable for routine clinical HPA genotyping and shows a broad prospect in further applications.
9.Cellular uptake study of CAP/GPC-MPEG nanoparticle in breast cancer cells

Background and purpose:A pressing obstacle in clinical chemotherapy is drug resistance in breast cancer.A nano-delivery system,which has many advantages as a drug carrier,such as carrying anticancer drugs,can be used effectively to overcome drug resistance in tumors.This paper examined a new nano-delivery system,called calcium phosphate and glycerophosphocholine-mPEG(CAP/GPC-MPEG)composite nanoparticle and its influence on the cellular drug uptake of BCRP-over expressing mitoxantrone(MIT)-resistant breast cancer cell MCF-7/MIT.This paper will also examine its effect on overcoming drug resistance in the MCF-7/MIT cells.Methods:After the calcium phosphate and GPC-mPEG composite nanoparticles were designed and prepared,the entrapment efficiency and in vitro drug release of mitoxantrone-loaded nanoparticles were investigated.Quantitative comparisons were made between cellular uptake of drug-loaded nanoparticles and free drugs.Finally,a confocal laser scanning microscopy Was used to compare the subcellular distribution of drug-loaded nanoparticles and the free drugs.Results:Calcium phosphate and GPC-mPEG composite nanoparticles were nanoporous spherical particles with diameters between 50-100 mn.The MIT-loaded nanoparticles have an entrapment efficiency of(89.45±0.05)%.Although the drug-loaded nanoparticles showed an initial burst of drug release,it was followed by a more sustained release.The concentration of mitoxantrone was 1.89 times treated with MIT-loaded nanoparticles for 1 h compared to that treated with free mitoxantrone for 1 h in MCF-7/MIT cells.and which was 2.33 times in MCF-7 cells.Fluorescent red mitoxantrone appeared in the cytoplasm and nucleus of the MCF-7 and MCF-7,MIT cells treated with MlT-loaded nanoparticles whereas it is almost undetected in both cells treated with free mitoxantrone.Conclusion:Calcium phosphate and GPC-mPEG composite nanoparticles Can promote the cellular uptake and entering of mitoxantrone to the nucleus in MCF-7 and its corresponding BCRP-over expressing MIT-resistant MCF-7/MIT breast cancer cell lines.This nanoparticle is a potential nano-carrier for overcoming drug resistance in tumors.
10.Catheter-directed thrombolytic therapy for acute superior mesenteric vein thrombus: comparison between via venous route and via arterial route

Objective To compare the clinical effects of catheter-directed thrombolytic therapy for acute superior mesenteric vein thrombus (SMVT) between via superior mesenteric vein (SMV) route and via superior mesenteric artery (SMA) route. Methods The clinical data of 32 patients with acute SMVT, who were treated with catheter-directed thrombolytic therapy at the Central Hospital of Shengli Oil Field, were retrospectively analyzed. Among the 32 patients, percutaneous transhepatic catheter-directed thrombolytic therapy via SMV route was performed in 23 (SMV group), and percutaneous transhepatic catheter-directed thrombolytic therapy via SMA route was carried out in 9 (SMA group). Results After the treatment, the clinical symptoms were significantly improved in 28 patients, including 22 of SMV group (95.7%) and 6 of SMA group (66.7%). CT angiography showed that the blood flow became almost complete patency in 17 patients of SMV group (73.9%) and in only 3 patients of SMA group (33.3%). Both the thrombolysis procedure time and X-ray exposure time of SMV group were obviously longer than those of SMA group. Conclusion Both via SMV and via SMA catheter-directed thrombolytic therapies are effective treatment for acute SMV thrombosis. The former is more effective, while the manipulation of the latter is technically simpler.
