Objective To investigate the expression of lysyl oxidase like protein?2(LOXL?2)in the sera of patients with active systemic scleroder?ma(SSc)and mixed connective tissue disease(MCTD). Methods An enzyme?linked immunosorbent assay was adopted to measure LOXL?2 in the serum of 20 patients with active SSc,20 patients with active MCTD,and 20 healthy controls. The measurements among different groups was com?pared, and correlations between LOXL?2 levels and clinical manifestations of SSc and MCTD were examined. Results The levels of LOXL?2 ex?pression in MCTD and SSc groups were significantly higher than those in control group(P<0.05 for all groups). LOXL?2 expression is also related to the presence of skin lesions in SSc(r=0.982 P=0.001). Conclusion High serum level of LOXL?2 in these patients with active SSc and active MCTD suggests that LOXL?2 may be involved in the process of fibrosis and the resulting vasculitis in multiple organs.

BACKGROUND: Non-specific protein adsorption resistance is the most important factor for biocompatibility; pre-adsorption of hydrophilic polymer on artificial material surface is one of the effective methods to inhibit protein adsorption. OBJECTIVE: To study pre-adsorption and protein adsorption resistance of poly (isopropylacrylamide) (PNIPAM)-based amphiphilic comb-block copolymers on polystyrene (PS) surface, and to understand the effect of the copolymer structure, i.e. the chain length and the number of hydrophilic polyvinylpyrrolidone (PVP) or polyethylene oxide (PEO) comb-branches, on protein adsorption resistance. DESIGN, TIME AND SETTING: An observational study was performed at College of Materials Science and Engineering, Beijing University of Chemical Technology from November 2007 to November 2008. MATERIALS: Monodisperse PS microsphere was employed to simulate surface of hydrophobic materials, and lysozyme was used as protein model. METHODS:① Qualitative analysis: Aqueous suspension of PS microspheres (0.1 g/L) was treated with PNIPAM-based copolymer (0.1 g/L) at room temperature for 24 hours to allow pre-adsorption of the copolymer on PS surface to build up a hydrophilic layer. Lysozyme (0.1 g/L) was mixed with the PS suspension at 37 ℃ and the mixture was kept for 24 hours. Apparent particle size and turbidity of the suspension were measured at 37C to observe coagulation or flocculation of PS microspheres, which related to the extent of protein adsorption on PS surface. ②Quantitative analysis of protein adsorption: The PS suspension containing lysozyme was subjected to ultracentrifuge (15 000 r/min) to collect clear aqueous solution. The lysozyme concentration in the clear solution was measured by spectrophotometry at 280 nm. The amount of protein adsorbed on PS surface was calculated based on the decrease in the protein concentration in the supernatant solution. MAIN OUTCOME MEASURES: Apparent particle diameter, turbidity, mass concentration of aqueous phase residual protein. RESULTS: ① Suspension of bare PS microspheres flocculated in the presence of lysozyme at 37℃ due to protein adsorption, which caused significant decrease in the turbidity of the suspension. Bare PS surface could adsorb 25.5 mg lysozyme per square meter.② PS microsphere surface pre-adsorbed by PNIPAM-based amphiphilic comb-block copolymer adsorbed much less protein. No flocculation and only limit coagulation was observed when the suspension of the surface modified PS microspheres was mixed with lysozyme, due to less protein adsorption on the PS surface. CONCLUSIONS: ① Pre-adsorption of water-soluble PNIPAM-based comb-block copolymer on PS microsphere surface can inhibit protein adhesion on the surface. ② The structure of the copolymer strongly affects the performance of protein resistance. A proper amount of hydrophilic branch unit, VP or EO, is required to exhibit good protein resistance of the copolymer. However, excess hydrophilic branches in the copolymer results in worse protein resistance probably due to less stability of the pre-adlayer of the copolymer on PS surface.

The liver injury induced by Polygonum multiflorum Thunb. (PM) was investigated based on idiosyncratic hepatotoxicity model co-treated with lipopolysaccharide (LPS) at a non-hepatotoxic dose. Sprague-Dawley (SD) rats were intragastrically administered with three doses (18.9, 37.8, 75.6 g crude drug per kg body weight) of 50% alcohol extracts of PM alone or co-treated with non-toxic dose of LPS (2.8 mg·kg(-1)) via tail vein injection. The plasma alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activities were assayed and the isolated livers were evaluated for histopathological changes. The dose-toxicity relationships of single treatment of PM or co-treatment of LPS were investigated comparatively to elucidate the idiosyncratic hepatotoxicity of PM. The results showed that no significant alterations of plasma ALT and AST activities were observed in the groups of solo-administration of LPS (2.8 mg·kg(-1), i.v.) or different dosage (18.9, 37.8 and 75.6 g·kg(-1), i.g.) of PM, compared to normal control group (P > 0.05); while significant elevations were observed in the co-administration groups of PM and LPS. Treatment with LPS alone caused slight infiltration of inflammatory cells in portal area but no evident hepatocytes injury. Co-treatment with LPS and PM (75.6 g·kg(-1), i.g.) caused hepatocyte focal necrosis, loss of central vein intima and a large number of inflammatory cell infiltration in portal areas. When further reduce the dosage of PM, significant increases of plasma ALT and AST activities (P < 0.05) were still observed in co-administration groups of LPS and PM (1.08 or 2.16 g·kg(-1)), but not in LPS or PM solo-administration groups. Nevertheless, the co-treatment of low dosage of PM (0.54 g·kg(-1)) with LPS did not induce any alteration of plasma ALT and AST. In conclusion, intragastric administration with 75.6 g·kg(-1) of PM did not induce liver injury in normal rats model; while the 2 folds of clinical equivalent dose of PM (1.08 g·kg(-1)) could result in liver injury in the LPS-based idiosyncratic hepatotoxicity model, which could be used to evaluate the idiosyncratic hepatotoxicity of PM.

Objective:To investigate the clinical effect of liver transplantation from organ donors in elderly patients. Methods:The clinical data of 346 patients who underwent liver transplantation in the general surgery center of Beijing You′an Hospital Affiliated to Capital Medical University from January 2018 to November 2020 were retrospectively analyzed. After screening, they were divided into the elderly donor group (30 cases) aged 60-70 years and the non elderly donor group (60 cases) aged < 60 years. The observation indexes were operation time, anhepatic phase time and operation bleeding. The prognosis indexes were: postoperative ICU stay, length of hospital stay, primary graft nonfunction (PNF), delayed graft function recovery (DGF) and in-hospital mortality. The measurement data in accordance with normal distribution are expressed as ( Mean± SD). The comparison between the two groups was analyzed by t-test. The measurement data that did not conform to the normal distribution were expressed as median (range). Mann Whitney U test was used for comparison between the two groups. The counting data were collected by the computer χ2 test or Fisher′s exact test analysis. Results:The operation time, anhepatic period and blood loss in the elderly group were (444.33±72.44) min, 56.0 (30-170) min, 1 992.1(400-9 000) mL, respectively and the non elderly group were (460.88±91.32) min, 58.45 (35-140) min, 1 885.8(400-12 000) mL, respectively, there were no significant difference between the two groups ( P>0.05); Among the prognostic indicators, the length of stay in ICU, the length of stay in hospital and the in-hospital mortality of the elderly group were 4.9 (2-21) d, 20.4 (3-40) d and 10% respectively, while the length of stay in ICU, the length of stay in hospital and the in-hospital mortality of the non elderly group were 5.3(1-32) d, 22.1(3-61) d and 10% respectively, with no significant difference between the two groups ( P>0.05); No PNF occurred in the two groups, but the incidence rate of DGF in the elderly group was 13.33%(4/30), and the non elderly group was 1.67%(1/60). There was a significant difference between the two groups ( P<0.05). Conclusion:Under strict preoperative donor evaluation and accurate recipient selection, 60-70 years old donors can achieve similar short-term effect as non elderly donors, but the long-term effect needs further observation.

Chinese Journal of Orthopaedics 2022;42(8):482-491
Objective:To study the method of intravascular angiography in vivo, analyze the clinical significance, and supply the basis of diagnosis and treatment of related orthopaedic diseases. Methods:The development was realized by improving the developer to increase the local concentration. Based on the study of Lijianmin-Chengkun Complexes and using the theory of magnetic microspheres, Fe 3O 4 magnetic microspheres with amino (negatively charged) shell are used to adsorb the aggregated ionic developer meglumine diatrizoate (positively charged diatrizoate). That is, by improving the method of developer, the magnetic microspheres can carry the developer to make new nanoparticles magnetic imaging composite particles. Under the action of external magnetic field, the magnetic imaging composite particles brought by blood circulation continue to stay and gather in the blood vessels in the magnetic field area, and the developer carried by the magnetic microspheres in the blood vessels in the magnetic field area is concentrated to reach the imaging concentration, so as to realize in vivo intravascular vascular imaging. By adjusting the ratio of the two reagents, the charge can be neutralized and condensed into small groups to improve the development efficiency. Thus, the electron microscope experiment, CT in vivo experiment, rabbit imaging experiment, experimental rabbit tissue picture confirmation, CT in vivo human body (the author is a volunteer) imaging experiment were carried out step by step. Results:Electron microscope experiment: meglumine diatrizoate, scanning electron microscope, the particle diameter is about 20 nm. Scanning electron microscope showed that the diameter of the magnetic microspheres was about 100 nm and the distribution was uniform. After the two reagents are mixed in a certain proportion, the neutralizing charge condenses into small groups, but it still has magnetohydrodynamic properties and strong paramagnetism. In vivo rabbit imaging experiment: the ideal intraosseous vascular imaging of the proximal tibia was captured. The tissue pictures of experimental rabbits confirmed that the distribution of Fe 3O 4 was obviously visible in the blood vessels in the proximal tibia on the side with magnetic field, but not on the side without magnetic field. In vivo human imaging experiment: the ideal intraosseous vascular imaging of the proximal fibula was captured. Conclusion:Through the preparation of new reagent of magnetic imaging composite particles (magnetic microspheres + meglumine diatrizoate), the concentration of in vivo bone developer can be achieved under the action of external magnetic field, and the in vivo external diameter ≥ 0.5mm can be achieved under CT thin-layer scanning.

Journal of Apoplexy and Nervous Diseases 2021;38(1):62-64
Objective To explore the clinical features, causes of misdiagnosis and preventive measures of hepatic myelopathy (HM). Methods The clinical data of 24 cases of HM admitted to Xijing Hospital from November 2009 to June 2019 were analyzed retrospectively, and relevant literatures were reviewed. Results All the 24 patients had a history of cirrhosis, and the clinical manifestations were decreased muscle strength of both lower limbs. Laboratory and imaging examinations were abnormal in some patients. Currently, there is no unified diagnostic standard for HM. According to the patient’s history, clinical manifestations, laboratory examination and imaging examination results, combined with literatures and expert opinions, hepatic myelopathy was diagnosed. One patient was treated with liver transplantation, and all patients were given symptomatic treatment and rehabilitation exercise. Scores of Modified Rankin Scale (mRS) improved in 3 patients at discharge. Follow-up of 3 months after discharge showed scores of mRS improved in 6 patients. The prognosis may be correlated with the course of disease. Conclusion HM has a low incidence and the clinical manifestations lack specificity.Detailed medical history, careful physical examination, timely examination of liver function and abdominal ultrasound can help early diagnosis and improve the clinical symptoms and prognosis of patients.

Chinese Journal of Ultrasonography 2022;31(4):304-311
Objective:To explore the diagnostic value of ultrasound in posterior fossa anomalies (PFA) at 11-13 + 6 gestational weeks by measuring brainstem (BS), brainstem-to-occipital bone (BSOB) diameter and BS/BSOB ratio. Methods:A total of 209 normal fetuses (control group) were randomly selected from Beijing Obstetrics and Gynecology Hospital, Capital Medical University, between March 2018 and November 2021. Reference ranges for BS, BSOB diameter and BS/BSOB ratio were obtained on the mid-sagittal view of the fetal profile and the relationship of three parameters and crown-rump length (CRL) was investigated. The intra- and inter-observer reliabilities were determined by intraclass correlation coefficient (ICC) in 30 normal fetuses. Fourteen fetuses diagnosed with PFA in the same period including 10 cases of cystic posterior fossa malformations (cPFM) and 4 cases of open spine bifida (OSB) were retrospectively selected to compare BS, BSOB diameter and the BS/BSOB ratio with control group. Results:BS and BSOB diameters were successfully obtained in all control fetuses (100%), and the intra- and inter-observer reliabilities for BS and BSOB diameters were good (ICC=0.877, 0.846 and 0.939, 0.895). In the control group, BS and BSOB diameter linearly correlated with CRL ( r=0.867, 0.794; all P<0.001), while the BS/BSOB ratio was 0.75 (0.71, 0.79). There were significant differences of BSOB diameter and BS/BSOB ratio between control group and PFA group (all P<0.05). Except for one isolated vermian hypoplasia (VH), the BSOB diameters in 9 (90%) cases of cPFM were above the 95th percentile of the calculated normal range and were below the 5th percentile in 4(100%) cases of OSB.Except for one isolated VH, the BS/BSOB ratio in 9 (90%) cases of cPFM was below the 5th percentile of the calculated normal range. The BS/BSOB ratio in 4 (100%) cases of OSB was above the 95th percentile of the calculated normal range. Conclusions:The measurements of BS and BSOB diameter are feasible with good repeatability. Abnormal BSOB diameter and BS/BSOB ratio are suggestive for PFA. The posterior fossa of isolated VH can be normal in the first trimester.
