1.Mechanism of long non-coding RNA in prostate cancer

Long non-coding RNA (lncRNA) is a type of RNA molecule whose transcript is longer than 200 nucleotides and cannot encode protein. Studies in recent years have shown that lncRNA plays an important role in the physiological and pathological processes of the body. It participates in the occurrence, development and metastasis of prostate cancer. Further study on the role and mechanism of lncRNA in prostate cancer is expected to provide new ideas for the diagnosis and treatment of prostate cancer.
2.Therapeutic Effect of Shenling Baizhu Powder Combined with Erchentang in Treatment of Senile Pneumonia and Influence on Procalcitonin

Objective To observe the efficacy of Shenling Baizhu powder combined with Erchentang in treatment of senile pneumonia, and its influence on serum procalcitonin. Methods With randomized controlled trial, 100 patients were randomly divided into two groups. The control group (49 cases) was treated by routine western medicine, and the treatment group (51 cases) was treated by Shenling Baizhu powder combined with Erchentang additionally. The treatment course was 1 week, and the therapeutic effect was observed after two courses. The improvement time of clinical symptoms and signs, and the change of serum procalcitonin were detected. Results Clinical efficacy of the two groups had no significant difference (P>0.05). The improvement time of signs and symptoms in treatment group was better than that in control group (P<0.05). The serum procalcitonin improved in treatment group was better than that in control group (P<0.05). Conclusion Shenling Baizhu powder combined with Erchentang has obvious anti-infection effect, and can improve symptoms and signs of senile pneumonia rapidly.
3.Effect of Four CT Scan Modes on Image Quality and Radiation Dose of Pulmonary Angiography

Purpose To compare radiation dose and image quality of different scan modes for CT pulmonary angiography (CTPA) including high-pitch flash mode, dual energy CT (DECT) mode, 128-slice mode of dual source CT and 64-slice CT mode. Materials and Methods One hundred and sixty-seven patients with suspected pulmonary embolism were retrospectively studied. All the patients underwent CTPA. Twenty patients were selected randomly from the patients scanned by high-pitch flash mode. Patients who were scanned by the other three modes were selected with body mass index and age matched those in high-pitch flash mode, with 20 patients in each group. Two radiologists assessed signal-to-noise ratio (SNR) and image quality with 5-piont scale. Dose parameters of volume CT dose index (CTDIvol), dose length product (DLP), and effective dose (ED) were compared among the four groups. Results Mean CTDIvol, DLP and ED were (3.72±0.74) mGy, (137.5±28.7) mGy · cm, and (2.34±0.41) mSv for Flash mode;(5.31±1.21) mGy, (181.6±34.5) mGy· cm and (3.24±0.57) mSv for DECT mode;(5.66±1.47) mGy, (198.7±42.1) mGy·cm and (3.58±0.63) mSv for 128-CT mode;and (6.75±1.68) mGy, (231.5±54.2) mGy·cm and (4.21±0.89) mSv for 64-CT mode. There was no significant difference of SNR and image quality among the four modes (P>0.05). Conclusion There are no significant difference of image quality among the four groups. Flash mode allows for minimum radiation dose compared to other modes. DECT mode and 128-CT mode get higher radiation dose with no difference between them. 64-CT mode gets the highest radiation dose.
4.Significance of determination for binding ratio of L-PHA with serum alkaline phosphatase in diagnosis of liver cancer

Objective To investigate the significance of determination for binding ratio of L-PHA(leukoagglutinating phytohemagglutinin) with serum alkaline phosphatase in the diagnosis of primary hepatic cancer.Methods The binding ratios of L-PHA with serum alkaline phosphatase in 50 healthy individuals,103 cases of primary hepatic cancer,22 cases of hepatocirrhosis,36 cases of chronic hepatitis,18 cases of acute hepatitis and 13 cases of digestive tract tumor were determined by lectin affinity chromatography.Results The binding ratio of L-PHA with serum alkaline phosphatase in primary hepatic cancer(16.01% 5.42%) were significantly increased as compared with the healthy control(7.50% 2.53%)(P 0.01).The binding ratios were positively correlated with the tumor size and AFP level.Conclusion The binding ratio of L-PHA with serum alkaline phosphatase is a potential parameter for the laboratory diagnosis of primary hepatic cancer.
5.Decision-making under risk condition in benign epilepsy children with centrotemporal spikes

Objective To investigate the ability of decision-making under risk condition in benign epilepsy children with centrotemporal spikes (BECT).Method Seventy-eight BECT patients and seventyfive healthycontrols were studied by Iowa Gambling Task (IGT) with inexplicit probability and Game of Dice Test (GDT) with explicit probability.Results (1) There was a significant difference in IGT task scores between the two group (t=-2.56,P<0.05).The total number of advantageous choices in IGT were lower in BECT group (49.35±5.38) than that in healthy control group (52.24±6.23).BECT group (-1.41 ± 10.60) performed worse on task than healthy control group (4.43±11.88),and the difference was statistically significant (t=-3.21,P<0.05).The control group scored higher on Blockl,Block4 and Block5 than that of BECT group (P<0.05).(2) There was no significant difference between the BECT group (4.42±7.74) and the control group(5.49±6.81) in the GDT test scores (t=-0.91,P=0.37).(3) Pearson correlation analysis showed that there was no significant correlation between the age,age of onset,education,seizure frequency and onset duration and the score of IGT and GDT (r =-0.091-0.274,all P > 0.05).Conclusion There is no significant decision-making disability in children with BECT under the condition of certain risk probability,while there is significant impairment under the condition of uncertain risk probability.The decision-making disability may be associated with impaired frontal lobe function.
6.Maintenance effects of acupoint catgut embedding at early time on gastrointestinal function in patients with craniocerebral injury.

OBJECTIVETo explore the maintenance effects of acupoint catgut embedding at early time on gastrointestinal function in patients with craniocerebral injury.
METHODSSixty craniocerebral injury patients with 5 to 12 points of Glasgow coma scale (GCS), according to treatment order, were alternately divided into an observation group and a control group, 30 cases in each one. Patients in the control group were treated with regular treatment and nursing care. Based on this, patients in the observation group, according to different pathogenesis and symptoms presented within 24 h into hospitalization, were additionally treated with acupoint catgut embedding. The recovery time of borborygmus, time of first anal aerofluxus, time of first defecation, abdominal pressure at different time points, the occurrence rate of complications (upper gastrointestinal hemorrhage, diarrhea, vomiting), time of enteral nutrition tolerance rate reaching 30 kcal/kg x d were observed and recorded.
RESULTSThe recovery time of borborygmus, time of first anal aerofluxus, time of first defecation and time of enteral nutrition tolerance rate reaching 30 kcal/kg x d in the observation group were all earlier to those in the control group (all P<0.01). At 48 h, 4 d and 7 d into hospitalization, the abdominal pressures in the observation group were all lower than those in the control group [(11.10 +/- 1.47) mmHg vs. (13.50 +/- 1.43) mmHg, (8.40 +/- 1.25) mmHg vs. (11.90 +/- 1.56) mmHg, (6.73 +/- 0.74) mmHg vs. (10.80 +/- 1.30) mmHg, all P<0.01]. There were 8 cases with complications of gastrointestinal hemorrhage, diarrhea and vomiting in the observation group with the occurrence rate o 27% (8/30), which was lower than those in the control group (70.0% (21/30), P<0.01.
CONCLUSIONThe acupoint catgut embedding at early time in craniocerebral injury patients could improve the recovery of gastrointestinal function, reduce intolerance of enteral nutrition and occurrence rate of various complications.

8.Drug resistance surveillance in Mycobacterium tuberculosis in Jiaxing City

Objective:
To investigate the resistance of Mycobacterium tuberculosis to first-line anti-tuberculosis drugs in Jiaxing City, Zhejiang Province from 2017 to 2019, so as to provide insights into improvements of the therapeutic effect of pulmonary tuberculosis.
Methods:
Data pertaining to pulmonary tuberculosis in Jiaxing City from 2017 to 2019 were collected from the Tuberculosis Surveillance System of Chinese Disease Prevention and Control Information System, including demographics, treatment classification, sputum culture and drug resistance. The spectrum, types and prevalence of drug resistance in M. tuberculosis to four first-line tuberculosis drugs, including isoniazid (INH), rifampicin (RFP), streptomycin (SM) and ethambutol (EMB), was analyzed using a descriptive epidemiological method.
Results:
A total of 1 310 M. tuberculosis isolates were cultured from pulmonary tuberculosis patients in Jiaxing City from 2017 to 2019, and there were 259 M. tuberculosis isolates that were resistant to anti-tuberculosis drugs, with an overall drug resistance rate of 19.77%. The prevalence rates of drug resistance to INH, SM, RFP and EMB were 13.36%, 11.83%, 5.50% and 3.59%, respectively. The prevalence of drug resistance was lower in M. tuberculosis isolates from treatment-naïve patients than from retreated patients (18.45% vs. 34.58%, P<0.05). M. tuberculosis isolates presented high resistance to SM (4.50%) and INH alone (4.35%), the highest resistance to INH-SM combinations (3.28%), and the highest resistance to INH+RFP+SM combinations (1.83%). Sixteen isolates were resistant to all the four drugs, with a drug resistance rate of 1.22%. The proportions of resistance to a single drug, RFP resistance, multidrug resistance and resistance to two and more drugs were 10.31%, 5.50%, 4.73% and 4.73%, respectively. In addition, the prevalence of RFP resistance among all patients and treatment-naïve patients both showed a tendency towards a rise from 2017 to 2019 (P<0.05). The prevalence of RFP resistance (7.01% vs. 3.76%) and resistance to two and more drugs (6.01% vs. 3.25%) was both higher among interprovincial mobile tuberculosis patients than among local non-mobile patients (P<0.05).
Conclusions
The overall prevalence of drug resistance was lower in M. tuberculosis isolates in Jiaxing City from 2017 to 2019 than in Zhejiang Province, with INH and RFP resistance as predominant types.
9.Ciprofloxacin non-susceptible Streptococcus pyogenes due to mutations in parC gene and its homology analysis

Objective To investigate the resistant mechanism of Streptococcus pyogenes to ciprofloxacin and its homology.Methods Forty-eight isolates of Streptococcus pyogenes were collected from patients diagnosed with scarflet fever in districts of Beijing in March,2012 and MIC to ciprofloxacin and other 7 common antibiotics in clinic were detected by using blood M-H agar dilution method.Thirteen isolates,which have MICs≥4 mg/L against ciprofloxacin,were detected for mutations of Fluoroquinolone resistance genes gyrA,gyrB,parC,parE.At the same time,4 isolates,with MIC ≤ 0.25 mg/L against ciprofloxacin,were used for comparison.Homology analysis of 17 isolates from different areas of Beijing was performed by using the method of pulsed field gel electrophoresis.Results Sensitive rates of Streptococcus pyogenes to levofloxacin,ampicillin and penicillin were all 100%.The resistance rates to tetracycline,erythromycin and clindamycin were 91.7% (44/48),91.7% (44/48) and 89.6% (43/48),respectively.MIC50 of ciprofloxacin,levofloxacin and moxifloxacin was 2 mg/L,1 mg/L and ≤ 0.25 mg/L,respectively ; MIC90 was 4 mg/L,2 mg/L and 0.5 mg/L,respectively.Of the 48 isolates of Streptococcus pyogenes,12 isolates showed the MIC at 4 mg/L,while one isolate has a MIC against ciprofloxacin at 8 mg/L,which isolated from Chaoyang district.Analysis of sequence of chromosome mediated fluoroquinolone resistance genes in those 13 ciprofloxacin non-susceptible isolates exhibited that there were 12 isolates that harbored Ser79Phe/Tyr mutation and 10 isolates harbored Ala121Val in parC gene.It is shown that one isolate contained Ser79Phe mutation in parC gene in the occurring of Ser371Leu mutation in parE gene for the first time,but there was no marked increase in ciprofloxacin MIC (MIC =4 mg/L).There were no mutations in gyrA and gyrB genes.The PFGE results demonstrated that the 17 tested isolates could be divided into 7 clones.The clone A isolates from Chaoyang,Daxing,Fengtai,Shunyi and Shijingshan district have a MIC ≥ 4 mg/L against ciprofloxacin,which covered 69.2% of all MIC ≥4 mg/L isolates.The clone C isolates from Huairou district were MIC ≥4 mg/L isolates.B,D,E,F and G clones isolates come from different districts.Conclusions The mutation of parC gene was the main reason that contribute to the slightly increase of ciprofloxacin MIC in Streptococcus pyogenes isolated from Beijing.The PFGE analysis showed that there was a small scale prevalence caused by the infection of Streptococcus pyogenes in some districts.
10.Computed tomography and magnetic resonance imaging features of the myomatous hepatic angiomyolipoma

Objective To investigate the computed tomography (CT) and magnetic resonance imaging (MRI) features of the myomatous hepatic angiomyolipoma (MHAML).Methods The retrospective cross-setional study was conducted.The clinicopathological data of 22 patients with MHAML who were admitted to the Southwest Hospital of the Third Military Medical University between January 2010 and June 2016 were collected.Patients underwent plain and enhanced scans of CT and MRI,and then received pathological examination after surgical resection or liver puncture and immunohistochemical staining.Observation indicators:(1) findings of CT and MRI,2 radiologists independently read films;(2) diagnostic consistency of 2 radiologists;(3) results of pathological examination.The Kappa test was used for evaluating the consistency,κ ≥0.75 as a good consistency,0.40<κ<0.75 as a normal consistency and κ ≤0.40 as a poor consistency.Results (1) Findings of CT and MRI:of 22 patients,16 received CT scans and 6 received CT and MRI scans.Tumors of 22 patients were single lesion,showing similar-circular type.Tumors located in the right liver lobe,left liver lobe and caudate lobe were respectively detected in 14,7 and 1 patients.① Plain and enhanced scans of CT:tumors of 22 patients showed low density.Twenty patients had clear boundary of tumor and 2 had an unclear boundary.Tumors of 22 patients demonstrated obvious enhancement in arterial phase by enhanced scans of CT,including fast-in and slow-out enhancement in 10 patients and fast-in and fast-out enhancement in 12 patients.The draining veins inside tumors were detected in 12 patients in early arterial phase by enhanced scans of CT.The dilated blood vessels inside tumors were found in 12 patients.The ring enhancement of tumor margin was detected in 16 patients,with formation of small blood vessels involving tumor blood supply.② MRI scan:tumors of 6 patients presented as low signal on T1WI and high signal on T2WI.Of 6 patients,5 had clear boundary of tumor and 1 had an unclear boundary.Tumors of 6 patients demonstrated obvious enhancement in arterial phase by enhanced scans of MRI,with a fast-in and fast-out enhancement.The draining veins inside tumors were detected in 3 patients in early arterial phase by enhanced scans of MRI.The dilated blood vessels inside tumors were found in 1 patient.The persistent ring enhancement of tumor margin was detected in 5 patients,with formation of small blood vessels.All the lesions of 6 patients using GD-EOB-DTPA MR contrast-enhanced scan demonstrated restricted diffusion with a high b value (b=800 s/mm2),an average apparent diffusion coefficient of 1.549× 10-3 mm2/s (1.209× 10-3-1.796× 10-3 mm2/s) and low a signal in liver phase.(2) Diagnostic consistency of 2 radiologists:there were good diagnostic consistencies of 2 radiologists in tumor location,density,T1WI,T2WI,bleeding,enhancement method and dilated blood vessels (κ=1.00,1.00,1.00,1.00,0.82,0.82,P<0.05).There were normal diagnostic consistencies of 2 radiologists in tumor fat,calcification,component of cystolization,boundary,draining veins and enhancement of tumor margin (κ =0.46,0.45,0.64,0.54,P<0.05).(3) Results of pathological examination:results of pathological examination of tumors from surgical resection of 17 patients and liver puncture of 5 patients showed that smooth muscle cells were the major components,and thick-walled vessels were found in the tumor of 12 patients.Results of immunohistochemical staining showed that anti melanoma specific monoclonal antibody (HMB-45) was positive.Conclusion The persistent enhancement of tumor margin,draining veins in early arterial phase by enhanced scans and dilated blood vessels might play roles in diagnosis of MHAML.
