Objective To study and analyze the rehabilitation effect of self prepared wind relieving antispasmodic Decoction and psychological intervention in the treatment of sequelae of stroke. Methods 100 patients with sequelae of stroke treated in our hospital from February 2015 to August 2016 were selected and randomly divided into the control group and the experimental group, with 50 patients in each group. Patients in the control group were given conventional treatment, given the corresponding blood pressure, blood sugar control drugs, and brain cell protective agents. On the basis of this experiment, the experimental group was given self prepared wind relieving antispasmodic decoction, and psychological intervention was carried out to strengthen communication with patients.The therapeutic effects of the two groups were compared. Results After the corresponding treatment, the effective rate of the experimental group (92.0％) was significantly higher than that of the control group (74.0％),the difference was statistically significant (P<0.05). Before treatment, there was no significant difference in the CRP levels between the two groups. After treatment, the CRP level was(3.1±0.3)μg/mL in the experimental group was significantly lower than that in the control group,the difference was statistically significant (P<0.05). Conclusion The self-made wind relief soup and psychological intervention treatment of stroke sequelae can significantly improve the rehabilitation effect of patients, to a great extent, reduce the level of CRP, with clinical significance of further promotion.

Objective To observe the analgesic effect of Qilong Toutong Granule and explore its underlying mechanisms of action. Methods Kunming mice were randomly divided into blank group, Zhengtian pill group, ibuprofen group, and high-, medium- and low-dose of Qilong Toutong Granule groups. After completing the prophylactic treatment for 3 days, hot-plate test and acetic acid-induced writhing test were used to observe the pain reaction of mice. SD rats were randomly divided into blank group, Zhengtian pill group, model group, rizatriptam group, and high-, medium- and low-dose of Qilong Toutong Granule groups. After gavaged for continuous 7 days, rats were intraperitoneally injected nitroglycerin (10 mg/kg), and 4 hours later, blood samples were collected from postcava for measuring the levels of plasma CGRP and β-EP by radioimmunoassay. Subsequently, rats were perfused transcardially and the brain tissues containing the trigeminal nucleus caudalis (Sp5C) were achieved for detecting the expression of Fos protein by immunohistochemical method. Results Qilong Toutong Granule significantly raised the pain threshold, reduced the number of writhing response of mice, remarkably down-regulated the levels of plasma CGRP, up-regulated the levels of plasmaβ-EP and inhibited the expression of Fos protein in Sp5C of rats compared with the model group. Conclusion Qilong Toutong Granule has good analgesic effect and its mechanisms on relieving migraine may be through regulating the levels of neurotransmitters or neuropeptides, and inhibiting the activation of Fos pathway.

Objective To investigate health care professionals′attitude of patient involvement in patient safety and analyze its influencing factors. Methods A convenience sample of 946 health care professionals recruited from twelve hospitals in Shanghai was employed for a cross-sectional survey. Results The general attitudes of patient involvement in patient safety scored 3.86 ±0.56, the perception of the content of patient involvement scored 3.97±0.54, the average score of caring involvement (4.12±0.59) was larger than the average score of decisive involvement (3.88±0.58), the average score of complaining involvement was the least (3.39±0.81), the difference had statistical significance (F=583.797, P<0.01). The regression analysis showed that identity, professional title, hospital grade influenced health care professionals′general attitude of patient involvement in patient safety and that identity, hospital grade influenced health care professionals′attitude of the content of patient involvement. Conclusions In health care professionals′opinion, patient involvement in patient safety has both advantages and disadvantages. They preferred to caring involvement, followed by decisive involvement, then complaining involvement. So, hospital managers should view patient involvement in patient safety discriminately and construct rational involvement model ofpatients willing, medical support, promote interaction and harmony build.

Objective To establish an evaluation index system for the quality and safety of the specialized nursing care in cardiovascular medicine. Methods To screen and evaluate the safety quality evaluation index via a group discussion with Delphi method and analytic hierarchy process referenced to the literatures from the evaluation index of domestic nursing safety quality (NSQ)and on the base ofthe factor quality- quality of links-final quality3-dimensional quality structure theory by Donabedian. Results The first and second round of expert consultation and the recovery rate were 97.05% (33/34) and 94.12% (32/34) respectibely, the effective rate was 100%;The authoritative coefficient of expert of the safety quality evaluation index of the specialized nursing care is 0.867 and 0.879 respectively. In the first round, the average of the importance rating is 3.50-5.00, the standard deviation is 0-0.042 and full mark rate is 34.10%-100%. In the second round, the average of the importance rating is 3.80-4.70, the standard deviation is 0~0.047 and full mark rate is 31.60%-100%. The fluctuation degree of the first round is more than 0.10 with a low coordination degree. However, the fluctuation degree of the second round result is less than 0.10. The coordination coefficients from all qustionnair scores by experts in the two roudns are 0.169 and 0.286. The difference was statistically significant according to the chi-square analysis (X2=6.842, P=0.002). The NSQ evaluatin index included 3 first degree indexes (weight coefficient including 0.2684,link qualing 0.6144,and quality 0.1172),16 second degree indicators (weight coefficient 0.034~0.0859) and 61 third degree indexes ( weight coefficient 0.034~0.0859) and 61 third degree indexes (weight coefficient 0.0052~0.0412). Conclusions This paper preliminary established a specialied NSQ evaluation index with factor quality- quality of links-final quality 3-dimensional quality structure in the field of cardiovascular medicine, which is based on Delphi method and analytic hierarchy process. NSQ provides a measurement tool for nurses in CME.

Objective To investigate related factors of hypersensitivity reactions in patients undergoing paclitaxel chemotherapy for cervical cancer and provide measures for prevention of the side effects.Methods A ret-rospective survey was taken to analyze the clinical data from 186 cervical cancer patients who accepted paclitaxel chemotherapy from January 2013 to December 2014.SPSS 17.0 was used for single factor analysis and multi -factor logistic regression analysis.Results Among the 186 cervical cancer patients enrolled in this study,34 cases occurred hypersensitivity reactions.The incidence rate of anaphylaxis was 18.3%.Both eosinophil count of more than 0.2 × 109 /L(χ2 =4.171,P <0.05)and history of drug allergy(χ2 =10.667,P <0.01)were not merely risk factors of anaphylaxis,but also independent risk factors(OR =4.691,2.257,all P <0.01 ).Conclusion The incidence of hypersensitivity after paclitaxel chemotherapy in cervical cancer patients is high.Eosinophil count ≥ 0.2 ×109 /L before chemotherapy and history of drug allergy are not merely risk factors of anaphylaxis,but also independent risk factors,which require more attention.

Objective To investigate the prevalence rate of nosocomial infections and the use of antibacterial agents in Zhejiang Cancer Hospital.Methods The cross-sectional survey on nosocomial infections was conducted among inpatients in Zhejiang Cancer Hospital on August 11,2011 and June 19,2012.The differences in prevalence rates of nosocomial infections,purposes of antibacterial agents use (for treatment,for prevention,or for both treatment and prevention),methods of medication (single or combined medication),and pathogen detections between 2011 and 2012 were analyzed with x2 test.Results There were 854 and 886 patients enrolled in the study in 2011 and 2012,and the prevalence rates of nosocomial infections were 5.27％ (45/854) and 5.08％ (45/886),respectively (x2 =0.186,P ＞ 0.05).Lower respiratory tract infection was the most common nosocomial infections both in 2011 and 2012,which accounted for 28.89％ and 36.96％ of all infections,respectively; and there was no significant difference in infection sites between 2011 and 2012 (x2 =1.415,P＞0.05).Totally 154 out of 854 patients (18.03％)in 2011 and 186 out of 886 patients (20.99％) in 2012 received antibacterial agents,and single medication was the most common.There were no significant differences in the purposes and methods of medication between 2011 and 2012 (x2 =1.994 and 3.042,P ＞ 0.05).Pathogen detections were performed in 56 out of 59 patients (94.92％) and 57 out of 65 patients (87.69％) who received antibacterial agents for treatment in 2011 and 2012,respectively.The most common pathogens were Escherichia coli,Pseudomonas aeruginosa,and Acinetobacter baumanni.Conclusion The prevalence rate of nosocomial infection in Zhejiang Cancer Hospital is of the satisfactory level,which is due to the good management of antibacterial agents use.

Objective To investigate the neuroprotective effect of miR-181c on hypoxia-preconditioned ischemia in rats and its mechanism.Methods Thirty-nine male SD rats were randomly divided into 5 groups of control group,sham-operated group,middle cerebral artery occlusion (MCAO)group,hypoxia-preconditioned group,hypoxia-preconditioned and MCAO group.Infarct volume and behavioral deficits were quantified.Real-time PCR was applied to detect the expression levels of miR-181c and Western blotting was used to verify the target protein of mt-cox1.Results Under the treatment of hypoxia-preconditioned,the neurological impairment was alleviated and the infarct volume was reduced significantly from 22.50％ ±2.96％ to 16.40％ ±3.13 ％ (t =5.26,P ＜0.01).The expression of miR-181c was decreased significantly in hypoxia-preconditioned and MCAO group than that in MCAO group (1.89 ± 0.14 vs 3.05 ± 0.26,t =6.10,P ＜ 0.01),and the expression of mt-cox1 protein was also significantly decreased (0.54 ± 0.07 vs 0.93 ± 0.04,t =8.01,P ＜ 0.01).Conclusion Hypoxia-preconditioned may attenuate the ischemic injury in SD rats,which may be related to the down-regulation of the expression of miR-181c,therefore increasing the expression of its targeted protein mt-cox1.

Objective To research the early effects and side-effects of the local advanced nasopharyngeal carcinoma patients using intensity-modulated radiotherapy(IMRT)combined with concurrent chemotherapy.Methods From January 2005 to January 2007,60 patients with nasopharyngeal carcinoma of stage m-IV b were received IMRT combined with concurrent chemotherapy in our center.Sixty patients were divided into paclitaxel concurrent group(32 patients)and cisplatin concurrent group(28 patients).The prescribing doses of the primary tumor were 68-72 Gy for each group.The patients of paclitaxel concurrent group patients of the cisplatin concurrent group got earlier radiodennatitis and radiation-induced mucositis but also got significantly higher rate of radiodermatitis,radiation-induced mucositis,radiation-induced leucopenia and gastrointestinal toxicity,as well as the loss of weight.No significant difference was found on liver and renal funcfons between two groups.Four patients(12.5%)of the paclitaxel concurrent group were broken-off,which was much better than the cisplatin concurrent group.There was no significant difference on the specific length of break-off time,the 2-year overall survival rate and the 2-year diseaee-free survival rate between two groups.Conclusions IMRT combined with concurrent chemotherapy of paclitaxel liposome for local advanced nasopharyngeal carcinoma results in less side-effects and better tolerance than IMRT combined with concurrent cisplatin chemotherapy.

With a low incidence and a non-specific clinical manifestation, medial medulary infarction (MMI) can often be misdiagnosed or missed diagnosis. Magnetic resonance imaging(MRI) played a major role in diagnosing the MMI. This article reviews the epidemiology, etiology, pathogenesis, clinical and imaging manifestations, diagnosis and differential diagnosis, and prognosis of MMI.
