1.Changes of bone mineral density and risk factors analysis of OP in typeⅡdiabetes old female patients

Objective To explore the changes of bone mineral density and risk factors of OP in old female patients with type Ⅱ diabetes.Methods 737 cases of old female patients with type Ⅱ diabetes in Department of Endocrinology were analyzed.All patients were first divided into two groups(Group A,with OP;Group B,without OP)according to OP.Then,the changes of bone mineral density and related risk factors were analyzed with Logistic regression approach.Results In the comparison of Group A and Group B,the results are shown as follows.In Group A:ICTP(7.34±1.84)μg/L,FBG(7.12±1.14)mmol/L,BMI(22.45±2.37)kg/m2,BMD,A(62.45±17.56);InGroup B,the corresponding levels were(4.13±1.07)μg/L,(8.48±1.28)mmol/L,(15.46±6.78)mmol/L,(89.75±14.56)respectively.The resuhs presented statistical differences.(t=29.958、-14.405、16.262、-22.665,both P<0.05).In Group A,HbAle(7.33±0.32)%,serum calcium(2.35±0.72)mmol/L,serum phosphorus(1.03±0.23)mmol/L,the corresponding values in Group B are(7.21±0.24)%,(2.42±0.34)mmol/L,(1.02±0.28)mmol/L.There sxe no statistical differences in two groups(both P>0.05).In Group A,FN(52.50±18.40)pmol/L,UALB was(8.03 ±1.86)mg/L,Leptin was(5.14 ±1.19)μg/L;the Values in Group B were (40.70±16.34)pmol/L,(5.91±1.24)mg/L,(5.63±1.23)μg/L,The values of FN,UALB in both groups had statistical differences(t=7.469、18.515,both P<0.05);the values of Loptin in two groups had no statistical difference(P>0.05).Based on the incidence of OP among old female patients,the leveh of HbAlc,serum calcium,serum phosphorus,ICTP,FBG,BMI、BMD,FN,UALB,Leptin,course of disease,losing body mass,were taken as independent variables and to analyze use Logistic Regression approach.Conclusion FN,UALB,course of disease,BMI and losing body mass were the independent risk factors of OP in old female patients with type Ⅱ diabetes.
2.Observation of Specific Body Positions on Blood Pressure and Heart Rate

Objective To explore the impact of different specific body positions on blood pressure and heart rate.Methods The blood pressure and heart rate of 178 healthy teachers and students was tested with BIOLIGHT M69 in different body positions.Results ①Blood pressure and heart rate were significantly higher in squatting position than in other three specific positions(sitting position, supine position, and standing position) (P<0.01). ②The blood pressure and heart rate were going up from standing position, and standing position with right arm lifting, to standing position with arm weight lifting positions (P<0.01). Conclusion The change of special body positions resulted in change of blood pressure and heart rate.
3.Differentiation of helper T lymphocyte in type Ⅲ prostatitis

Objective To investigate the differentiation of CD4+T helper cell in expressed prostatic secretions(EPS)in type Ⅲ prostatitis. Methods Seventy-six patients were studied,aged from 18 to 47(average 31.8).All patients presented with typical clinical symptoms for over 3 months.Cases were classified as type Ⅲ A(47 cases),type Ⅲ B(29 cases)and control group(16 cases)according to NIH classification system.Type Ⅲ A was also divided into group Ⅲ A1 with 26 cases(mild inflammation)and group Ⅲ A2 with 21 cases(severe inflammation).Th1 cytokine IFN-γ,Th2 cytokines IL-4 in EPS were detected by double antibody sandwich ELISA,and IFN-γ/IL-4 was determined.Results Compared with group control,IFN-γ in group Ⅲ A and Ⅲ B were significantly up regulated (134.78±43.67 pg/ml,109.82±30.09 pg/ml,P<0.05),while IFN-γ level in group Ⅲ A was higher than in group Ⅲ B(P<0.05).IL-4 in groupⅢ A was not changed(51.99±20.59 pg/ml,P>0.05).IL-4 in group ⅢB was higher(76.40±17.99 pg/ml,P<0.05).IFN-γ/IL-4 in group ⅢA was elevated significantly(2.94±1.12,P<0.05),while IFN-γ/IL-4 in group ⅢB was not changed(1.49±0.48,P>0.05).IFN-γ/IL-4 in group Ⅲ A2 was higher than in groupⅢA1 significantly(3.67±0.82vs 2.34±0.97,P<0.05). Conclusions Th1 eell differentiation took the dominance in type Ⅲ A prostatitis.Th1/Th2 equilibrium was shifted to Th1.It is probably one of the pathogenies that Th1 dominant differentiation leads to local inflammation in prostate of type Ⅲ A prostatitis.
4.Blood-letting and herbal-cupping therapy for lumbar spinal stenosis: prospective case series study

Objective The purpose of study was to evaluate the safety and effectiveness of theblood-letting and herbal-cupping therapy for lumbar spinal stenosis.Methods A multi-center prospective case series was performed.The LSS patients meeting the inclusion criteria received 8 treatments as a course and 4 courses in total.The primary outcomes were the symptom severity and physical function scale ofthe Swiss Spinal Stenosis Measurement (SSM,total score 0-5 for each domain).The secondary outcomes were thethe 12-item short form health survey (SF-12,total score 0-100),and Oswestry disability index (ODI,total score 0-100) at time of baseline,completion of last treatment of each course.The minimal clinically important differences (MCIDs) were calculated for estimating the percentage of improvement in the population.The adverse events were reported at any time of the intra-and post-operation.This was a phrase analysis of the studyat seven months.Results Forty-eight patientswere included,with 64.6% (31/48) of LSS showing neurogenic claudication (walking distance ≤200 m).The average age was 63.1 ± 11.7 years,19 (39.6%) female,and the average BMI was 25.3 ± 3.3 kg/m2.The scores of symptom severity scale of SSM were 2.8 ± 0.6,2.6 ± 0.7,2.3 ± 0.6,1.9 ± 0.2 at baseline,1st,2nd,3rd course,and the scores of physical function scale were 2.5 ± 0.8,2.4 ± 0.7,2.1 ± 0.5,1.8 ± 0.3,and all the changes between baseline and each course showed significant improvement.The patient satisfaction of SSM,ODI and SF-12 showed significantimprovements after the 1st,2nd,3rd course (P<0.05).The SF-12 subgroup physical composite scores after 3rd course and mental composite score after 1st showed no significant improvement.The minimal clinically important difference for the “SymptomSeverity scale” in the SSM was achieved withimprovement of 18.8%,40.6%,83.3% in the LSS patient population after 1st,2nd,3rd course;and the "physical function scale" in SSM was achieved withimprovement of 22.9%,31.3%,50.0%.A total of 15 patients felt pain when they were micro-punctured with little blood at first time,but the symptom wereimmediately relieved without any treatment.Conelusions The Blood-letting and herbal-cupping therapy could benefit patients with lumbar spinal stenosis after third course of treatment in the fields of symptom relief and quality of life with no severe adverse event.However,this was a phrase analysis,so more evidence of this study and large comparative researches should be warranted in future.
5.Clinical features of different isoforms of PML鄄RARα fusion gene in patients with acute promyelocytic leukemia

Objective To explore the clinical features and prognosis of different PML_RARα fusion gene isoforms in acute promyelocytic leukemia (APL). Methods The clinical data of 78 patients initially diagnosed with APL in Fujian Medical University Union Hospital from February 2013 to July 2016 were collected. The clinical features and prognosis of different PML_RARα fusion gene isoforms were analyzed. Results There were 32 females (41%) and 46 males (59%) in 78 patients, with a median age of 40 years old (13-68 years old). The most common PML_RARα fusion gene was L type (48.7%, 38/78), followed by S type (46.2%, 36/78) and V type (5.1%, 4/78). The patients with white blood cell count more than 10×109/L (high_risk) occurred mostly in S type (61.1%, 22/36), compared with V type and L type, and there were statistically different (χ 2 = 7.683, P < 0.05). A total of 78 patients included 8 cases (10.2%) of combined CD34 positive, 17 cases (21.8%) of combined FLT3_ITD mutation, 12 cases (15.4%) of combined DNMT3A mutation and 9 cases (11.5%) of additional chromosomal abnormalities. There were no significant differences in CD34 positive, FLT3_ITD, DNMT3A, and the incidence of additional chromosomal abnormalities among the three different isoforms (P>0.05). The most common occurrence of retinoic acid syndrome (RAS) during treatment was S type (21/36), while rare for L type and V type (χ2= 7.633, P< 0.05). There were no statistical differences in the complete remission (CR) rate and disease_free survival rate among the patients with different PML_RARα isoforms (P>0.05). Conclusions The clinical characteristics of different PML_RARα fusion gene isoforms are different, including most_common L type, more_common V type and S type in high risk groups; complicated RAS is commonly found in S type during the treatment. And different isoforms have no effect on the CR and DFS rate.
6.Study on transcriptome characteristics of respiratory syncytial virus bronchiolitis in children by RNA sequencing

To explore the biological characteristics related to the pathogenesis and severity of respiratory syncytial virus (RSV) bronchiolitis by RNA sequencing of white blood cells in children with RSV bronchiolitis. This study is a case-control study. A total of 87 children diagnosed with bronchiolitis and RSV antigen positive and/or RSV nucleic acid positive in the pediatric respiratory department of the Second Affiliated Hospital of Wenzhou Medical University from October 2019 to April 2022 were selected as the case group. The case group was divided into three groups based on the condition: mild, moderate, and severe, and there were two groups according to the presence or absence of atopic symptoms: the atopic group and the non -atopic group, forty healthy children in the same period were selected as the control group. The whole blood leukocyte RNA of the children in the case group and the control group was extracted for RNA sequencing, and the data were analyzed to obtain differentially expressed genes (DEGs). Then, the immunobiological pathways and genes related to the pathogenesis, disease condition, and atopy were screened through Gene Ontology (GO) annotation, Kyoto Gene and Genome Encyclopedia (KEGG) annotation, and protein interaction network (PPI) construction methods. Construct the weighted gene co-expression network analysis (WGCNA) module to identify potential biological indicators related to disease severity.Compared with the control group, the case group had a total of 1 782 DEGs, including 1 586 upregulated genes and 196 downregulated genes. The GO pathway enrichment of DEGs is mainly enriched in molecular functions such as peroxidase activity and oxidoreductase activity. In the cytological components, it is mainly enriched in cytoplasmic vesicle lumen and secretory granule lumen. In biological processes, it is mainly enriched in processes such as neutrophil activation involved in immune responses, neutrophil degranulation, and neutrophil activation. KEGG analysis is mainly concentrated in the signal pathway of the viral protein interaction with cytokine and cytokine receptor. A PPI network was constructed to screen four genes at the core position, including CCL2, IL-10, MMP9 and JUN. The DEGs obtained by comparing different disease groups with the control group are mainly enriched in retrograde endocannabinoid signaling and cell apoptosis pathways. WGCNA analysis showed that the brown module related to oxygen saturation was most closely related to the disease, and its gene was mainly enriched in the RNA helicase retinoic acid inducible gene-I (RIG-I) like receptor signal pathway. There are 230 specific DEGs in the atopic group and 444 in the non -atopic group. KEGG enrichment analysis results show that both groups are enriched to NF-κB signaling pathway, the characteristic does not cause significant changes in immune response and transcriptome characteristics in children with RSV bronchiolitis. In conclusion, neutrophil activation, degranulation pathway and signal pathway of interaction between viral protein and cytokine and cytokine receptor are involved in the immune response of RSV bronchiolitis host. CCL2, IL-10, MMP9 and JUN genes may be associated with the pathogenesis. They might be potential biomarkers related to disease severity in RIG-I like receptors, cell apoptosis, and endogenous cannabinoid related signaling pathways.
