1.Targeted treatment strategies for patients with trastuzumab-resistant HER2-overexpressing breast cancer

Trastuzumab combined with chemotherapy improves overall response rate in women with HER2-overexpressing breast cancer.However,therapeutic resistance to trastuzumab typically occurs after several months of starting therapy and overall survival does not improve significantly.Studies have reported that the potential mechanisms of resistance to trastuzumab are mainly related to the signal pathway activation from receptor tyrosine protein kinases outside of the HER family and the amplification of the PI3K-AKT pathway.Novel target therapies such as tyrosine kinase inhibitors,PI3K inhibitors are approved as substitutes for the treatment of HER2-overexpressing breast cancer,but the response to each single-agent tends to be short-lived.Several large randomized adjuvant trials have showed that novel molecular targeted therapies combinations will markedly limit or eventually abrogate acquired resistance to primary trastuzumab therapy.
2.Study on the correlation between short,long term efficacy with cut width in primary angle closure glaucoma iris weeks after cutting holes

Objective To investigate the primary angle-closure glaucoma iris weeks after cutting holes cut the width of the near future, long-term effect relationship, selected after close to long-term efficacy of the forecasting methods to improve iridectomy success rate. Methods 90 cases (116 eyes) with primary angle-closure glaucoma were randomly divided into three groups:A group(the width of the iris-week cut-hole cut 1.5mm) ,30 cases 42 eyes;B group(removal of the width of the iris weeks Ceccon 2 mm) ,30 cases 40 eyes;C group(the width of the iris-week cut-lole cut 3 mm) ,30 cases 34 eyes,after over 1 month,they were followed up to observe the intraocular pressure,anterior chamber angle width of the depth, depending on the features, the iris in recent weeks, as well as cutting holes,long-term concurrency disease. Results A:recent surgery success rate of 85.79% ,long-term surgical success rate of 76. 19% ;B group of recent surgery success rate of 87.5% ,long-term surgical success rate of 82.5% ;C group of recent surgery success rate of94.11% ,long-term surgical the success rate of 88.23%, X2 test the difference was significant(P< 0. 05) ;3 postoperative IOP decreased in varying degrees compared with the preoperative differences were significant(P<0.01) ;peripheral angle after deep Random mean :A group of ≥ 1/3CT,compared the difference between before and after surgery were significantly( P < 0.01 ) ;B group and C group > 1/2CT,2 groups were compared before and after surgery the difference was highly significant( P < 0.05 ). Conclusion Iridectomy was the treatment of primary angle-closure glaucoma safe and effective method of cutting holes. The width of the iris weeks to 2~3 mmwas appropriate, beneficial to control the iris weeks before the adhesion and angle-cut hole shallow narrowing.
3.Effect of the HMGB1、CEA and SCC-Ag by paclitaxel combined with cisplatin chemotherapy in advanced esophageal cancer

Objective To investigate the effect of the high mobility group protein 1 (HMGB1), cancer embryonic antigen (CEA) and squamous cell carcinoma antigen ( SCC-Ag) by paclitaxel combined with cisplatin chemotherapy in the treatment of advanced esophageal cancer patients .Methods 43 cases advanced esophageal cancer patients from our hospital were selected and randomly divided into the control group and the experiment group.19 cases in the control group were treated by surgery combined with postoperative chemotherapy , 24 cases in the experimental group were treated with surgery and chemotherapy.The clinical efficacy and high mobility group protein 1 ( HMGB1 ) , cancer embryonic antigen ( CEA ) and squamous cell carcinoma antigen ( SCC-Ag ) levels were compared between the two groups before and after treatment.Results The total effective rate of the experimental group was (91.7%) higher than that of the control group (57.9%), the difference was statistically significant (P <0.05).After treatment, the serum levels of SCC-Ag, CEA and HMGB1 were decreased in the two groups, compared with the control group, the experimental group SCC-Ag, CEA and HMGB1 levels were lower, the difference was statistically significant ( P <0.05 ) .There was no significant difference in adverse reactions between the two groups.Conclusion Paclitaxel combined with cisplatin in the treatment of advanced esophageal cancer patients with good results, presumably with the decrease of serum SCC-Ag, CEA and HMGB1 levels in patients with.
4.A flexion loading system for knee biomechanics research.

It is important to design and build a kinetic loading system for flexing movement of knee joint to study knee biomechanics. The system reported here includes driving device, control device, and flexion angle determination imaging system. The driving device was constructed with a stepper motor and a mechanical transmission with a serried of clamps, shanks and so on, and the driving device was controlled by the control device with micro-control unit, a computer and the serial 232. While the knee joint was driven to move by the stepper motor, the flexion angle of the knee was determined using imaging-based techniques. The system achieved accurate loading and control of speed, extent and duration of knee flexion, as well as fast and non-contract determination of flexion angle during knee flexing movement. The system is simple to build, easy to operate, highly accurate and reliable and it provides an important tool for the study of knee biomechanics, and potentially provides a tool for helping patients of knee surgery during their post operation recovery training.

5.Status quo and influencing factors of quality of life in neurological patients with cognitive disorder

Objective? To explore the quality of life in patients with cognitive disorder and to discuss its influencing factors so as to provide a reference for improving the quality of life in patients with cognitive disorder. Methods? Totally 1 398 neurological patients from a neuropsychiatric hospital in Nanjing were screened using cluster sampling and 125 patients with cognitive disorder were selected as subjects between January and December 2017. The subjects received a cross-sectional study with Quality of Life in Alzheimer's Disease (QOL-AD), Montreal Cognitive Assessment Scale (MoCA), Activities of Daily Living Scale (ADL), Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS) and the general information questionnaire. Variance analysis, Pearson correlation analysis and multivariate linear regression analysis were used for statistical analysis. Results? The morbidity of cognitive disorder in neurological patients from our hospital was 9.3%. Their QOL-AD score was (31.88±6.54). Sex, educational background, nature of work, household income, sleep and bad social relations are factors affecting the quality of life of the subjects (P<0.05). According to multivariate linear regression analysis, MoCA, SAS and SDS were main factors affecting the quality of life of the subjects (P<0.05). Conclusions? Male patients with lower education, physical work and bad social relations are high-risk patients with lower quality of life. Cognitive disorder, anxiety and depression are main risk factors affecting the quality of life in patients with cognitive disorder. Nursing workers should improve and maintain the patients'cognitive ability, enrich their cultural life and social activities, and enhance their mental health, thereby improving their quality of life.
6.Construction and validation of a depression risk prediction model for patients with cognitive impairment

Objective:To explore the risk factors for depression in patients with cognitive impairment and construct a prediction model to preliminarily validate the predictive performance of the model, aiming to provide medical and nursing staff with a screening tool for high-risk groups.Methods:From January 2020 to December 2021, convenience sampling was used to select 1 130 patients with cognitive impairment admitted to the Affiliated Brain Hospital of Nanjing Medical University as the research subject. The research subjects were divided into a modeling group ( n=791) and a validation group ( n=339) at a ratio of 7∶3. The influencing factors of depression in patients with cognitive impairment were determined using binomial Logistic regression and a risk prediction model was established. The predictive performance of the prediction model was tested using the receiver operating characteristic (ROC) curve. Results:The incidence of depression in 1 130 patients with cognitive impairment was 51.3% (580/1 130). Binomial Logistic regression analysis showed that the influencing factors for depression in patients included age, Activities of Daily Living Scale score, Hamilton Anxiety Scale score, Pittsburgh Sleep Quality Index score, and Lewy Body Composite Risk score ( P<0.05). In the modeling group, the area under the ROC curve was 0.921, the Youden index was 0.716, the sensitivity was 0.834, the specificity was 0.882, and the prediction accuracy was 0.858. In the validation group, the area under the ROC curve was 0.896, the Youden index was 0.651, the sensitivity was 0.824, the specificity was 0.827, and the prediction accuracy was 0.825. Conclusions:The depression risk prediction model can effectively predict the risk of depression in patients with cognitive impairment, and can provide a screening tool for high-risk groups for medical and nursing staff.

Dyslipidemia is an important risk factor of atherosclerotic cardiovascular disease (ASCVD). Statins delay the occurrence and development of ASCVD, and reduce the risk of cardiovascular events and death. Due to safety concerns, there exist insufficient use of lipid-lowering agents and a high withdrawal rate of the agents in the elderly. To promote the prevention and treatment of ASCVD, this expert consensus is issued and focuses on the management of dyslipidemia of Chinese elderly basing on the clinical evidence of the use of lipid-lowering drugs by the elderly, and the lipid management guidelines and expert consensus recommendations at home and abroad.
8.Maternal hepatic venous hemodynamics and cardiac output in normal and fetal growth restricted pregnancies.

To evaluate relationship of maternal hepatic vein Doppler flow parameters and cardiac output (CO) with neonatal birth weight in uncomplicated pregnancies (UP) and pregnancies complicated by fetal growth restriction (FGR) .
Methods: Hepatic vein impedance index (HVI), venous pulse transit time (VPTT), and CO were measured in women with UP at the 14th-37th weeks and complicated by FGR at the 26th-37th weeks who underwent maternal hepatic hemodynamic and echocardiographic examination during the ultrasonography. After delivery, the birth weight and the birth weight percentile of each neonate in this study were recorded. Correlations among HVI, VPTT, and CO were analyzed.
Results: In the UP group, HVI, VPTT, and CO changed with the increase of gestation. In the FGR group, HVI was higher, VPTT was shorter, CO and neonatal birth weight were obviously lower than those in the UP at the 26th-37th weeks (P<0.05).
Conclusion: There is a series of adaptive changes in hepatic venous hemodynamics and CO in UP with the increase of gestation to meet the demand of fetal growth, while the maladaptive changes in hepatic venous hemodynamics and CO in pregnant woman may contribute to FGR.

9.Effects of Sushen Jiubao decoction on Th1/Th2 factors in patients with acute exacerbation of bronchial asthma

Objective To observe the clinical efficacy of Sushen Jiubao decoction in the treatment of acute exacerbation of bronchial asthma and its effect on Th1/Th2 factors.Methods Ninety-eight patients with acute exacerbation of bronchial asthma treated in Sishengbianji Hospital of Traditional Chinese Medicine Affiliated to Zhejiang Chinese Medical University from February 2022 to May 2023 were selected and divided into treatment group and control group according to random number table method,with 49 cases in each group.The control group was given conventional treatment such as oxygen inhalation and atomization,and the treatment group was additionally treated with Sushen Jiubao decoctionon based on the treatment of control group.The clinical efficacy,TCM syndrome score,pulmonary function indicators[forced expiratory volume in first second(FEV1),peak expiratory flow(PEF)],and laboratory indicators[interleukin(IL)-4,IL-13,interferon-γ(IFN-γ),immunoglobulin E(IgE)].Results The total effective rate of treatment group was significantly higher than that of control group(χ2=9.287,P=0.010).After treatment,the TCM syndrome scores,serum IL-4,IL-13 and IgE levels of two groups were significantly lower than before treatment,and FEV1,PEF and IFN-γ were significantly higher than before treatment(P<0.05).The TCM syndrome score,serum IL-4,IL-13 and IgE levels in treatment group were significantly lower than those in control group,and FEV1,PEF and IFN-γ were significantly higher than those in control group(P<0.05).Conclusion Sushen Jiubao decoction can significantly improve clinical efficacy,reduce the level of inflammatory factors,and improve pulmonary function in the treatment of acute exacerbation of bronchial asthma.

"Biochemical Engineering Experiment" is a compulsory curriculum for the concentrated practical teaching of biotechnology majors in Hunan University of Science and Engineering. It is also an experimental curriculum for improving the overall quality of bioengineering students under the context of "Emerging Engineering Education". The course includes comprehensive experiments and designable experiments, and the contents of which are designed by combining the local characteristic resources of Yongzhou, the research platform and the characteristics of the talents with engineering background. In the teaching practice, methods such as heuristic teaching, research cases-embedded teaching and interactive teaching are comprehensively used to boost students' interest in learning and stimulate their innovative thinking and application capability. Through curriculum examination and post-class investigation, it was found that the students' abilities of knowledge transfer and application were significantly improved, and they achieved excellent performances in discipline competitions and approved project proposals. The practice and continuous improvement of this course may facilitate fostering high-level innovative and application-oriented talents of biotechnology majors.
