Purpose:To evaluate the physical dose distributions and tolerance in locally recurrent nasopharyngeal carcinoma treated with intensity modulated radiation therapy (IMRT).Methods:From June 1999 to August 2000, 10 patients with locally recurrent nasopharyngeal carcinoma proven by histology, without neck lymphoid node recurrence and distant metastasis, were treated by IMRT. The interval from the time of initial radiotherapy to recurrence ranged from 14—50(median 25) months. The initial radiotherapy in all patients were treated with external radiotherapy alone to a median dose of 69Gy/35fractions/52days. IMRT method was performed by means of the MIMiC collimator and inverse treatment planning system (NOMOS company, USA). The prescribed dose of re radiotherapy to target volume was 57Gy/19fractions/4weeks.Results:All patients, except for some patients with mild weight loss and reaction in the oral cavity during radiotherapy, tolerated this IMRT well. IMRT achieved the better immediate responses in 10 patients with complete response in 6 cases and partial response in 4 cases. Recurrence was found in two patients, lung distant metastasis in one patient. The median survival in the 10 patients was 14 months. The median planning target volume (PTV) was 95.8(60—134) cm 3. The dose distributions in the PTV were as follows: median average dose 59.65?2.47Gy, median homogeneity index (HI): 1.18(?0.06); median dose to 95% PTV ≥ 53.2 (?1.36)Gy. The median dose to the organs at risk (OAR) were as follows: spinal cord 9.46?5.23Gy; brain stem 20.24?3.55Gy; left parotid gland 18.53?5.30Gy, right parotid gland 19.68?6.21Gy; left lens 2.11?0.65Gy, right lens 2.94?0.57Gy;Chiasm12.34Gy; left optical nerve13.14 Gy, right optical nerve17.65 Gy.Conclusions:Better dose distributions treated with IMRT was achieved in the patients with locally recurrent nasopharyngeal carcinoma and the patients tolerated this radiotherapy well. Objective To develop a new continent cutaneous diversion using a tapered ileum wrapped by rectus abdominis muscular flap. Methods Ten dogs underwent a procedure in which two 6 cm ileal segments were tapered and connected to the U-shaped reservoir which was detubularized by a 30-40 cm ileal segment with end-to-side anastomosis. A continent tube was created by a 6 cm length and 3 cm width rectus abdominis muscular flap with its blood supplied which was penetrated be-tween the mesenteric vessels of one tapered ileum (study tube) and wrapped around its periphery. The other tapered ileum (control tube) was brought out into the abdominal skin directly without any extra-mural support. Urodynamic studies were carried out at 1, 3 and 6 months postoperatively. The data of maximum inner pressure (MIP) and functional pressure length (FPL) of every study and control tubes at every study phase were recorded and statistics analysis were taken. Retrograde radiograms of the ef-ferent tubes were performed prior to sacrifice and histological examinations of study group were carried out after sacrificed. Results The average MIP in experimental group was significantly higher than that in control group at 1, 3 and 6 months post-operatively (P<0.01). However, no significant differences in MIP or FPL were found in study group between empty and full reservoir. In the control group, MIP was found rising significantly (P<0.001) and FPL (P<0.05) decreased sharply com-pared with empty and full reservoir. Retrograde radiograms confirmed that all efferent tubes were pa-tent and positioned straightly beneath the abdominal wall. Histological examination of study group at 6 months postoperatively demonstrated a layer of striated muscle around the serosa of the ileum. Conclusions The continent mechanism of tapered ileum could be enhanced by extramural support from wrapped rectus abdominis muscular flap. This technique could setup a reliable, easy catheterization continent cutaneous efferent tube.

Objective To compare the operative cooperation of da Vinci robotic surgery and traditional laparoscopic surgery, so as to provide evidence to qualify the operative cooperation of minimally invasive operation. Methods Comparison of the operative cooperation with related experiences and literature was made between the two kinds of surgeries. Results More requirements were put forward during the da Vinci robotic surgery on nurses′ service quality and training methods, ethics & law knowledge and humanistic care, equipment installation and preparation, complicated nursing performance content, system error handling and postoperative management, etc.. Conclusions It is suggested that the operative cooperation of the precise minimally invasive operation should be improved as follows:strengthen the nurses′ special training and continuing education; establish related operation process and management standards;do patients′explanation work and surgical team cooperation.

Objective To explore the efficacy of different bulbourethral sling procedures in the treatment of male acquired urinary incontinence.Methods A retrospective study of 105 patients with acquired urinary incontinence was performed.The patients underwent 3 different bulbourethral sling procedures under urodynamic monitoring between October 2000 and June 2013.Mean age was 54 years (range 15-81).Urinary incontinence was secondary to post-prostatectomy in 70 patients and posterior urethroplasty in 35.Preoperatively,10 patients were completely urinary incontinence and 95 patients were stress urinary incontinence.Mean duration of urinary incontinence was 3 years (1-12).The surgical techniques were composite device suspension in 54 patients,pedicled rectus abdominalis muscle and fascial flaps suspension in 10 and transobturator sling in 41.Results The patients were followed up for 3-128 months (mean 54 months).The urethral catheter was left in situ for 5-7 days in 103 patients.Of the 103 patients,5 patients were difficulty in voiding but corrected by indwelling of urethral tube for another 1 week in 4 patients and transurethral bladder neck resecting in 1.In the remainder 2 patients,the maximum urethral pressure was 110 and 158 cm H2O (1 cm H2O=0.098 kPa) at the end of surgery and both patients were able to void on day 14 and 21 respectively and achieved complete continence.In this study,complete continence was achieved with good voiding in 74 patients (70.5％),completed control of urination rate was 81,4％ (57/70) in group of prostate and 48.6％ (17/35) in group of posterior urethroplasty.Twenty-six patients were improved and 5 patients were failed.Conclusion Bulbourethral sling procedure under urodynamic monitoring is an effective option in the treatment of male acquired urinary incontinence,especially for patients of incontinence of post-prostatectomy.

OBJECTIVETo observe the difference in acupuncture for pain threshold at different time points among the groups of 9 TCM constitutions.

METHODSThe cross-sectional survey was adopted to investigate TCM constitutions among 600 subjects and determine 9 TCM constitution types (neutral constitution, qi-deficiency constitution, yang-deficiency constitution, yin-deficiency constitution, phlegm-damp constitution, damp-heat constitution, blood-stagnation constitution, qi-stagnation constitution, special diathesis constitution). The same acupuncture manipulation was applied to Zusanli (ST 36) on the left side in the subjects and the needle was retained for 30 min. The tenderness threshold was detected with 2390 type Von Frey apparatus at different time points, named before acupuncture, at the moment after qi arrival, in 10 min of needle retaining, in 30 min of needle retaining and in 15 min after needle withdrawal in the subjects of 9 TCM constitutions.

RESULTSThe interactive effect happened between the constitution type and time point (P < 0.05). Among the groups of 9 TCM constitutions, the pain threshold values at the moment after qi arrival (except blood-stagnation constitution, qi-stagnation constitution, special diathesis constitution) in 10 min of needle retaining and in 30 min of needle retaining were increased as compared with those before acupuncture separately (P < 0.01), among which, the value increase was the most significant in 30 min of needle retaining. The differences in the pain thresholds were significant in 15 min after needle withdrawal in the groups of neutral constitution and damp-heat constitution as compared with those before acupuncture (both P < 0.01). In 10 min of needle retaining and in 30 min of needle retaining, as compared with the group of neutral constitution, the changes in pain thresholds of the rest abnormal constitutions were apparently lower (all P < 0.05).

CONCLUSIONAcupuncture at Zusanli (ST 36) presents different effects among the groups of different constitution types. The effect maintaining durations are different.

Objective To investigate the serum IL-33 level and its association with TH1,TH2,TH17 and Treg cells in patients with unexplained recurrent spontaneous abortion(URSA).Methods Forty-six URSA patients and 40 healthy controls were enrolled.The proportions of TH1,TH2,TH17 and Treg cells in peripheral blood samples were determined by flow cytometry,and serum IL-33 levels by ELISA.Results The levels of serum IL-33 in URSA patients were significantly lower than that in healthy controls.The proportions of TH2 and Treg cells in URSA patients were significantly lower than that in healthy controls (P ＜ 0.05),while the proportions of TH 1 and TH 17 cells in URSA patients were significantly higher than that in healthy controls.Serum IL-33 levels were negatively correlated with the proportions of TH 1 and TH17 cells,and positively with that of TH2 cells,while no correlation with Treg cells.Conclusion Serum IL-33 levels decrease significantly in URSA patients,and are correlated with the proportions of TH1,TH2 and TH17 cells,indicating that IL-33 may be associated with TH1,TH2 and TH17 cells in URSA patients.

Objective To investigate nerve growth factor (β-NGF) and its receptors expression in human pancreatic ductal adenocarcinoma. Methods Expression and distribution of β-NGF, tyrosine kinase A (TrKA) and P75NGFR were detected in operation tissue specimens of pancreatic ductal adenocarcinoma with immunohistochemistry and real-time PCR. Relations of β-NGF and its receptors with clinicalpathological characters, especially nerve invasion were analyzed. Results β-NGF and TrKA expression are higher in pancreatic adenocarcinoma than normal pancreas, and the differences are significant (P < 0. 01). Β-NGF and TrKA expression are associated with the differentiation grades(DG), lymphatic node metastasis, nerve invasion and surgical pathological stages. Poorer of DG and later stages, more expression of β-NGF and TrKA. Β-NGF and TrKA expression have positive correlations. Β-NGF, TrKA and P75NGFR mRNA expression have significantly increased 3.84,4. 23 and 2. 41 times than normal tissues by real-time PCR, respectively. Conclusions β-NGF and TrKA might play potential rules in carcinogenesis for pancreatic cancer,have affinity with clinicopathological characters of pancreatic cancer. Β-NGF and TrKA may have mutual effect in signal transduction leading to perineural invasion of pancreatic carcinoma.

Objective To investigate artemin and its receptors GFRα3 expression in human pancreatic ductal adenocarcinoma and its significance. Methods Expression and distribution of artemin and GFRα3 in 100 cases of human pancreatic ductal adenocarcinoma(PDAC) and 40 cases of normal pancreatic tissue were detected by immunohistochemistry and RT-PCR, quantitative RT PCR. Relations of artemin and GFRα3 with clinicopathological characteristics, especially nerve invasion were analyzed. Results Nerve invasion was detected in 64 out of 100 cases of PDAC, and the rate of nerve invasion was 64%. The ratio of expression of Artemin, GFRα3 protein in PDAC/normal pancreatic tissue was 2.697 ± 0.231 and 2.599 ± 0.588; the ratio of expression of Artemin, GFRα3 mRNA was 7.01 and 4.63. Artemin and GFRα3 expression were associated with tumor location, differentiation degree, TNM staging, nerve invasion node metastasis, but it was not associated with age, sex and tumor size. Artemin and GFRα3 expression was more positively expressed in cancer cells close to nerve tissue than cells far from nerve tissue. Conclusions Artemin and GFRα3 were involved in the development of pancreatic cancer and their high expression was closely related to perineural invasion.

Objective To compare the influences of different dilution titers on the ANA detection by the indirect immunofluores‐cence assay(IIF) in children for investigating the necessity of reducing serum initial dilution titer .Methods Serum ANA was detec‐ted by using the indirect immunofluorescence assay at a serial of dilution titer in 110 healthy controls and the results were compared with the results of specific ANAs by the linear immunoassay (LIA);meanwhile the ANA‐LIA results in clinical children patients with ANA‐IIF negative were also analyzed .Results With the dilution titers gradual decrease from 1∶80 ,1∶40 and 1∶20 in the samples of the health group ,the positive detection rates of ANA‐IIF were risen ,which were 7 .3% ,9 .1% and 10 .9% respectively , but the differences were not statistically significant (P>0 .05) ,the weak‐positive rates were 7 .3% ,15 .5% and 31 .8% respective‐ly ,the differences were statistically significant (P<0 .01) .Among 110 healthy children under going the physical examination ,the specific ANA was detected out in 8 samples ,the positive rate was 7 .3% .Among 8 positive cases at the dilution titer of 1∶80 by the IIF method ,specific ANA was in 2 cases;in 4 added cases of fluorescence ANA positive samples at the dilution titers of 1∶40 and 1∶20 ,specific ANA was in 1 case .If with any positive of ANA‐IIF(1∶80) or ANA‐LIA as the ANA positive ,the ANA positive rate was risen from 7 .3% to 12 .7% .In the clinical samples among 29 cases of ANA‐IIF(1∶80) negative autoimmune liver disease related autoantibody detection ,the specific ANA‐LIA positive was detected in 5 cases (17 .2% ) .Conclusion Reducing the initial ti‐ter of children serum is unable to obviously increase the ANA‐IIF positive detection rate ,on the contrary increases the non‐specific weak positive .Therefore ,clinical laboratory does not change the dilution titer of children routine ANA sample .The detection by combining with the specific ANA‐LIA spectrum is conducive to find ANA .
