1.The protective effects of dexmedetomidine on myocardial injury of severely burned patients

Objective To investigate the protective effects of dexmedetomidine on myocardial injury of severe burn patients. Methods Seventy-eight cases of severe burn patients were enrolled in this study in our hospital from May 2014 to May 2016. According to the clinical characteristics of treatment, the patients were divided into the control group and the observation group, with 39 cases in each group. Patients in the control group were treated with midazolam sedation on the basis of conventional burns, and patients in the observation group were treated with dexmedetomidine combined with midazolam sedation. The myocardial function-related indicators, heart rate (HR) and mean arterial pressure (MAP) of patients were compared between the 2 groups after treatment. Results (1) The SOD, MDA, CK-MB, cTnI and TNF-αof the two groups after treatment were significantly lower than those before treatment, and the degree of reduction in the observation group was significantly higher than that in the control group (P<0.05);(2) There were significant changes in MAP and HR before and after treatment in the both 2 groups. The decrease s of HR and the degree of MAP elevation in the observation group were significantly higher than those in the control group (P < 0.05, respectively). Conclusions Dexmedetomidine can improve the SOD activity of patients with severe burns, and the decreased expressions of CK-MB, cTnI and TNF-αcan reduce the degree of damage to myocardial tissue microstructure, and can protect the myocardium of patients with severe burns.
2.Reliability and characteristics of ultrasonography in patients with severe proliferative diabetic retinopathy

Objective To evaluate the percentage of agreement between the ultrasonographic data and the surgical findings,and investigate the characteristics of the ultrasonographic imaging of proliferative diabetic retinopathy.Methods B-scan ultrasonography was performed in 48 patients (62 eyes ) affected by proliferative diabetic retinopathy,before vitreoretinal surgery,by observing and recording the position,shape,range,to evaluate the percentage of agreement between the ultrasonographic data and surgical findings.Results Ultrasonography can show kinds of vitreoretinal changes clearly,and is not affected by the opacity of refractive media.The agreement was quite good for the pathological changes of proliferative diabetic retinopathy.The accurate rates of diagnosis of vitreous hemorrhage,posterior vitreous detachment,retinal detachment,vitreal membranes,vitreoretinal adhesions and preretinal membranes were 100%, 91.9%, 94.9%, 96.4%, 80.0% and 87.9%,respectively.Conclusions B-scan ultrasonography is valuable in proliferative diabetic retinopathy and favorable in the ophthalmic clinic practice.
3.The balanced scorecard and behavioral medicine discipline development in general hospitals

This study run through the process of developing behavioral medicine discipline in a general hospital with action research method,based on the balanced scorecard (BSC) framework.The four perspectives of the behavioral medicine BSC covered the main areas in hospital discipline development,including patients and community,healthy financial disciplines,clinical service process and efficiency,and research,innovation and growth.By the implementation of the discipline BSC,this hospital clarified its discipline development strategy,provided a communication platform,determined the objective and monitored the discipline effectively,and provided a mode which can integrate the discipline development and providing clinical services.The hospital set up the first neurology behavioral medicine clinic in Jinan Region though the exploration of the path and methods for developing behavioral medicine discipline,which had practical guiding significance for behavioral medicine development and services.
4.Reliability and Validity of Chinese Version of the Reflective Function Questionnaire for Youth

Objective To evaluate the reliability and validity of the Reflective Function Questionnaire for Youth(RFQY). Methods School sample:a total of 1 813 students from the fifth grade primary school to the second grade high school were selected for the study(aged 10 to 17 years). Clinical patients:another 52 adolescents(aged under 17 years)with borderline trait who attended Department of Psychiatry in Shengjing Hospital were recruited. All subjects completed the RFQY,basic empathy scale( BES )and emotional intelligence scale(EIS). After 3 weeks,85 students were randomly selected and filled in the questionaires again. Results The internal consistency of the RFQY was 0.73,and the test?retest reliability was 0.686(P<0.001). The RFQY scores of the school students were significantly higher than those in clinical patients(8.71±0.65 vs 8.10±0.89,P<0.001). The RFQY scores of male students were significantly lower than those in female students(8.61±0.67 vs 8.78±0.63,P<0.001). The score increased with the age increased(P<0.001). Conclusion The Chinese version of RFQY shows good reliability and validity , which can be used to measure the reflective function of adolescents in China .
5.Reliability and Validity Study of Chinese Version of Reflective Functioning Questionary-54

Objective To evaluate the reliability and validity of reflective functioning questionary-54 (RFQ-54) in China.Methods School sample:a total of 635 students were selected for the study.Clinical patients:another 75 patients with borderline personality trait who visited Department of Psychiatry in Shengjing Hospital of China Medical University were recruited.All subjects filled the RFQ-54,MIS-BPD,TAS,FFMQ.44 students accepted the retest after three weeks.Results The internal consistency for RFQ-C and RFQ-U were both 0.628.The test-retest reliability was 0.835,0.683,respectively.The correlations with TAS,FFMQ,MIS-BPD supported its validity.The RFQ subscales scores were significant1y different between only-child and non-only child,urban and country,school group and clinical group (22.11 ± 1.34 vs 18.97 ± 1.22,22.59 ± 1.36 vs 17.61 ± 1.12,21.39 ± 1.32 vs 15.34 ± 1.08,10.95 ± 8.43 vs 13.58 ± 8.19,10.95 ± 8.36 vs 13.88 ± 8.26,11.47 ± 8.02 vs 22.43 ± 1.24,respectively.P < 0.01).Conclusion The Chinese version of RFQ-54 shows good reliability and validity,which can be used to assess the reflective function of adults in China.
6.Bridging fixation with locking plate for the treatment of subtrochanteric femoral fractures of Seinsheimer type Ⅴ

Objective:To investigate the clinical effect of bridging fixation with locking plate for the Seinsheimer type V subtrochanteric femoral fracture. Methods:From March 2009 to September 2014,18 cases of Seinsheimer type V sub-trochanteric femoral fracture were treated by open reduction and bridging fixation with locking plate through proximal and distal approach including 16 males and 2 females with an average age of 41 years old ranging from 22 to 67 years old. Among them , 12 cases caused by traffic accident,5 cases by falling,1 case by heavy aboving. All cases were fresh and closed fractures. Time between injury and operation was from 4 to 9 days with an average of 6.2 days. Of them ,11 cases were fixed with reverse LISS and the other 7 cases were fixed with anatomical locking plates of proximal femur. Results:The mean time of operation was 110 min (ranged from 90 to 155 min). The mean blood loss during operation was 425 ml (ranged from 350 to 650 ml) and 16 cases got blood transfusion which was meanly 300 ml. The mean hospital time was 14 days (ranged from 12 to 18 days). The mean duration of followed up was 11.8 months (ranged from 8 to 22 months). The mean time of bone union was 6.6 months (ranged from 5 to 8 months). There was not any complication such as infection,implant failure,hip varus,external rotation deformity of low limb or fat embolism. The Sanders hip scores were 53.22±6.48,the result was excellent in 12 cases and good in 6 cases at the last follow up. Conclusion:Under the principle of biological osteosynthesis ,treatment of Seinsheimer type V sub-trochanteric femoral fracture with bridging locking plate fixation has such advantages as high mechanism ,less interference of blood supply,stable fixation and little complication. It is a safe and idea way for the treatment of the Seinsheimer type V sub-trochanteric femoral fracture.
